ABSTRACT

The decays of (214)Po into (210)Pb and of (212)Po into (208)Pb tagged by the previous decays from (214)Bi and (212)Bi have been studied inserting quartz vials inside the Counting Test Facility (CTF) at the underground laboratory in Gran Sasso (LNGS). We find that the mean lifetime of (214)Po is (236.00 ± 0.42(stat) ± 0.15(syst)) µs and that of (212)Po is (425.1 ± 0.9(stat) ± 1.2(syst)) ns. Our results are compatible with previous measurements, have a much better signal to background ratio, and reduce the overall uncertainties.

ABSTRACT

Bites from Loxosceles spiders can cause few symptoms to gangrenous skin necrosis or even death. To date, the treatment of the violin spider bite is largely unsatisfactory. Although no specific therapy exists, it has been suggested that heparin, steroids, dapsone, experimental antivenin and/or surgical excision may be beneficial. Three rare cases of suspected spider bite and their surgical treatment are reported. On the basis of geographical area, anamnesis and clinical symptoms, we suppose Loxoceles rufescens responsible for these bites.

ABSTRACT

A consistent generalization of statistical mechanics is obtained by applying the maximum entropy principle to a trace-form entropy and by requiring that physically motivated mathematical properties are preserved. The emerging differential-functional equation yields a two-parameter class of generalized logarithms, from which entropies and power-law distributions follow: these distributions could be relevant in many anomalous systems. Within the specified range of parameters, these entropies possess positivity, continuity, symmetry, expansibility, decisivity, maximality, concavity, and are Lesche stable. The Boltzmann-Shannon entropy and some one-parameter generalized entropies already known belong to this class. These entropies and their distribution functions are compared, and the corresponding deformed algebras are discussed.

ABSTRACT

Toxic epidermal necrolysis (TEN) is an acute drug-induced life-threatening disorder characterised by extensive epidermal exfoliation and high rate of mortality. Between October 2000 and April 2003, five severe TEN patients were evaluated using a specific TEN severity-of-illness scale (SCORTEN) and treated for the first time, with a combined therapy using Intravenous Human Immunoglobulins (IVIG) and plasmapheresis. The standardised mortality ratio (SMR) analysis ([Sigma observed deaths/Sigma expected deaths]x100) was applied to establish how IVIG and plasmapheresis treatment could reduce TEN patient mortality. The observed mortality was one out of five patients corresponding to 20%. The expected mortality based on SCORTEN was 3.319 corresponding to 66%. The SMR analysis revealed a 70% reduction in mortality (SMR=0.30; 95% confidence interval, 0.0-0.96). Our series show a low mortality rate (20%) related to the severity of the patients (66% expected mortality). The use of IVIG in association with plasmapheresis has a rational basis and may be effective in severe TEN patients.

ABSTRACT

UNLABELLED: Non-melanoma skin cancer (NMSC) is the most common type of skin cancer. Important controversial issues are the need for incisional biopsies, surgical margin, and timing of follow-up. METHODS: A retrospective study was undertaken on 2544 lesions. Accuracy of diagnosis and prevalence of incomplete excision were evaluated, comparing clinical and histological diagnosis using chi2 tests with Yates' correction. Kaplan-Meier recurrence graphs have been obtained. RESULTS: Lesions were correctly diagnosed in 94% of basal cell carcinomas (BCC) and in 69% of squamous cell carcinomas (SCC) (p < 0.001). Positive margins on pathological examination were 6.6% for BCC and 6.8% for SCC. A significant difference for incomplete excision has been found for BCC in the face (p < 0.001). Kaplan-Meier survival curves showed a different pattern for BCC and SCC. CONCLUSIONS: On the basis of our data, if clinical diagnosis is BCC, excision and reconstruction may be undertaken without an incisional biopsy. Alternatively, if clinical diagnosis is SCC, it is advisable to consider an incisional biopsy, before definitive surgical treatment.

ABSTRACT

Three patients developed erosive pustular dermatosis of the scalp (EPDS). Two of them, both males, had previously undergone surgical excision for squamous cell carcinoma and basal cell carcinoma, and a female experienced avulsive trauma of the scalp. The erosive lesions and crusts were located at the site of a skin graft; microbiological cultures were negative for bacterial and fungal growth. Histological examination ruled out pustular bullous disorders. Topical therapy with corticosteroids and antibiotics resulted in clinical remission in only 2 cases. The third case showed a tendency to recur despite numerous therapeutic attempts with oral dapsone and isotretinoin. We conclude that surgical trauma is a possible cause of EPDS. Our patients seem to be the first reported cases of EPDS in skin grafts following plastic surgical procedures.

ABSTRACT

This study describes the work carried out at the Burn Unit of the Neves Bendinha Hospital, Luanuda, Angola, during the 3-year period July 1991 to June 1994. During this period we admitted 2569 burned patients to our burn unit, and 4661 were treated on an outpatient basis. The data from the patients were analysed to indicate the distribution according to age, sex, TBSA, cause of the lesion and mortality. Our study gives some epidemiological data on burns in an undeveloped country undergoing a war, outlining the specific problems compared to the reality in civilized countries.

ABSTRACT

During recent years, sliding flaps have had great popularity owing to their advantages in terms of reliability, ease of execution, and good morphofunctional results. The limits of these flaps are related to the capacity of excursion in the advancement. To improve these limits, we describe in this article a personal modification of the flap that extends the possibilities of coverage in the distal areas, reducing the tensile distress. We report our experience with 19 patients, treated with this modified flap in various areas of the body, with good results and without noticeable complications.

ABSTRACT

Three clinical cases of mandibular giant follicular cyst were described. Personal experience with an initial conservative approach is described and some interesting conclusions about the short time of bone new generation of maxillary are presented. As soon as possible, the extraction of impacted tooth will be made without the risk of mandibular fracture or vascular-nervous lesions.

ABSTRACT

We report the clinical features of 62 consecutive patients with lichen planus observed in 18 months. The largest number of cases occurred between 50 and 70 years of age. Thirty-four patients had lichen planus only. In the remainder, lichen planus was associated with chronic liver diseases (16 cases), immune-related disorders (7 cases), and diabetes (5 cases). Mucous-erosive lichen planus was significantly more frequent in cases with lichen planus and other diseases. In all patients with liver diseases the histological features always showed a severe liver involvement. No relationship was observed between lichen planus and the etiology of the liver diseases. Females were more affected by immune-related disorders than males. The above data, together with the increased levels found of IgA, auto-antibodies, and cryoglobulins, even in cases with lichen planus only, suggest that lichen planus results from an immune imbalance, often associated with systemic involvement.
