ABSTRACT

Massive intracavitary hemorrhage in case if severe concomitant injury is the most frequent cause of death. Necessity of blood loss replacement requires effective and safe methods. One of them is blood reinfusion - technically less complicated method. The article shows advantages of using the hardware and filtration methods of blood sampling and reinfusion, developed scale rapid diagnosis of the level of hemolysis of autologous blood. Using of the method of filtration blood sampling provides 2X minimal amount of free hemoglobin than using an aspirator and reinfusion of blood with the contents therein to free hemoglobin 2.0g/ L prevents the risk of developing the syndrome and massive blood transfusion reduces the incidence of deaths at 11%. In patients with massive intracavitary hemorrhage it is advisable to use the method of semi-quantitative rapid analysis of free hemoglobin color scale.

ABSTRACT

The examination was applied to 598 patients with polytrauma and patients after surgery on bones and joints on the subject of various diseases of locomotor apparatus. All patients were tested on fat embolism in blood serum and cerebrospinal fluid. The modificated Guard's technique was applied. The test data permitted to develop the method of express diagnostics of oil embolism. It is established that fat liquorglobulia is a pathognomonic symptom of cerebral form of fat embolism.

ABSTRACT

On the basis of an analysis of the tactics of transfusion therapy in 272 wounded persons and victims with acute massive blood loss, the authors conclude that it is necessary to strictly follow the principle of individual directionality during blood transfusions. By taking into account the high risk of development of iatrogenic complications in the postshock periods of traumatic disease, it is expedient to do away with hemotransfusions in the advancing mode.

ABSTRACT

The article deals with diagnosis and treatment of fat embolism concomitant with trauma. A new term is suggested, i.e. liqoroglobulism, which is concomitant with the cerebral form of fat embolism. The results of investigations of patients, who endured fat embolism, are presented. A new method is suggested for the treatment of fat embolism, i.e. a combination of hemosorption and liquosorption. A thesis on that the blood-brain barrier is damaged in the cerebral form of fat embolism is formulated.

ABSTRACT

The authors discuss evaluation of the severity of fatty embolism. Three criteria are taken into consideration: clinical symptoms, presence of fat in the serum or liquor, and presence of fat on the surface of serum or liquor. The broth symptom is defined. The studies indicate that after detection of this symptom microscopy of the serum for detecting fatty globulemia is inefficient.

ABSTRACT

The article is devoted to the problem of diagnosis and treatment of 28 patients with cranio-cerebral injury with posttraumatic fat embolism. In 15 of them fat liquoroglobulia was diagnosed which is a reliable symptom of the cerebral form of fat embolism. The method of detection of fat liquoroglobulia is described. A new method--liquorosorption--was used in treatment of patients with the cerebral form of fat embolism. A disturbance of the hematoencephalic barrier is thought by the authors to be possible in such embolism.

ABSTRACT

The article is devoted to the problem of fat embolism. In 63 patients with combined and isolated traumas the maximum frequency of fat embolism was diagnosed in the investigation of blood serum from the femoral artery. It can be supposed that no isolated injury of the lesser circulation circuit can occur in fat embolism. An investigation of the venous-arterial difference of fat globulemia has shown the main role of the so-called theory of shunting for fat embolism to be doubtful.
