ABSTRACT

Objective: To investigate the anxiety levels, sleep quality and potential risk factors of healthcare practitioners involved in the management of COVID-19 patients in a mobile cabin hospital, and further to assess the impact of progressive muscle relaxation (PMR) on their anxiety levels and sleep quality. Methods: We conducted a pre-post self-controlled trial. Healthcare practitioners meeting the inclusion criteria underwent daily 30-min PMR sessions for seven consecutive days. The Pittsburgh Sleep Quality Index (PSQI) and Hamilton Anxiety Scale (HAMA) were used to assess the anxiety and sleep quality of subjects pre- and post-intervention. Statistical analysis was performed using the Wilcoxon test, Mann-Whitney U test, Kruskal-Wallis H test, and Spearman rank correlation. Results: A total of 94 participants completed the study. No statistically significant differences in HAMA or PSQI total scores were observed between groups categorized based on demographic variables such as age, sex, and years of education (p > 0.05). The PSQI total score and its components (excluding sleep medication usage) exhibited a positive correlation with the HAMA total score and its psychological anxiety component (p < 0.05), and a correlation was observed between somatic anxiety manifestations and several components of the PSQI. The PSQI total scores before and after intervention were 10.0 (8.0, 13.0) and 8.0 (6.0, 9.0) respectively (p < 0.001); the HAMA total scores were 8.0 (5.0, 13.0) and 6.0 (4.0, 9.5) respectively (p < 0.001). The detection rates of poor sleep and anxiety states, along with their severity, significantly decreased post-intervention (p < 0.001). Conclusion: Healthcare practitioners experience prominent anxiety and sleep issues in the mobile cabin hospital. PMR can be an effective intervention for improving the anxiety and sleep quality of healthcare professionals during support periods in the mobile cabin hospital. However, trials with larger samples are necessitated to further affirm these preliminary findings.

ABSTRACT

BACKGROUND: This article describes a rare case of cervical spinal arteriovenous malformation (AVM) mimicking a neurogenic spinal tumor. OBSERVATIONS: A 22-year-old female presenting with a C6-7 AVM with a calcification nodule experienced new-onset acute right upper radiculopathy associated with extradural compression of the spinal cord. Note that spinal AVMs with a calcified nodule are rare. Endovascular embolization is generally used to relieve the symptoms of AVM; however, this procedure cannot relieve cord compression, particularly in cases complicated by calcified nodules. This article discusses treatment options. LESSONS: Decompression surgery is preferable to endovascular embolization because it alleviates symptoms while preventing cord compression and minimizing the risk of recurrence.

ABSTRACT

OBJECTIVE: Schizencephaly is a congenital cerebral malformation characterized by clefts in the hemispheres of the brain, where variations in semiology often make it difficult to localize epileptogenic focus. Here, we report on a series of patients who underwent stereo-encephalography (SEEG) for epileptogenic focus localization and subsequent SEEG-guided surgical intervention. METHODS: Four patients (ages 27, 33, 27, 25 years) with a mean seizure history of 16 years (range 8-22 years) were analyzed. Data pertaining to semiology, video encephalography (EEG), magnetic resonance imaging, positron emission tomography, and invasive EEG studies, surgical intervention and post-surgery outcome were collected and analyzed. RESULTS: All seizure onset zones were within the extent of schizencephaly; however, the limbic system (including the hippocampus, amygdala, cingulate gyrus, or insula) was involved in early spreading. Two patients underwent SEEG-guided radiofrequency thermo-ablation (RFTA) in the seizure onset zone, 1 patient underwent lesionectomy via craniotomy, and 1 underwent neither RFTA nor lesionectomy. At 2 years post-surgery, the outcomes were as follows: Engel grade Ia (n = 2), Ib (n = 1), and III (n = 1). CONCLUSIONS: This article reports on a precise approach to treating patients with schizencephaly dependent of seizure onset zone and functional cortex mapping. Subsequent SEEG-guided surgical interventions (radiofrequency thermo-ablation and lesionectomy) were shown to reduce seizure frequency, while preserving the neurologic functions in drug-resistant epilepsy patients with schizencephaly.

ABSTRACT

Treatment of a κ1-P-monodentate bicyclic diphosphane iridium(III) complex with a labile gold(I) precursor afforded an unusual IrIII/AuI complex in which the P-P single bond has been cleaved. This reaction was cleanly reversed upon addition of tertiary phosphine. Carbon-carbon bond activation, across neighbouring P2C2N rings of the coordinated bicyclic diphosphane, occurred upon thermolysis of the IrIII/AuI complex.

ABSTRACT

Over the last decade, unmanned aerial vehicles (UAVs) with antenna arrays have usually been employed for the enhancement of wireless communication in millimeter-wave bands. They are commonly used as aerial base stations and relay platforms in order to serve multiple users. Many beamforming methods for improving communication quality based on channel estimation have been proposed. However, these methods can be resource-intensive due to the complexity of channel estimation in practice. Thus, in this paper, we formulate an MIMO blind beamforming problem at the receivers for UAV-assisted communications in which channel estimation is omitted in order to save communication resources. We introduce one analytical method, which is called the analytical constant modulus algorithm (ACMA), in order to perform blind beamforming at the UAV base station; this relies only on data received by the antenna. The feature of the constant modulus (CM) is employed to restrict the target user signals. Algebraic operations, such as singular value decomposition (SVD), are applied to separate the user signal space from other interferences. The number of users in the region served by the UAV can be detected by exploring information in the measured data. We seek solutions that are expressible as one Kronecker product structure in the signal space; then, the beamformers that correspond to each user can be successfully estimated. The simulation results show that, by using this analytically derived blind method, the system can achieve good signal recovery accuracy, a reasonable system sum rate, and acceptable complexity.

ABSTRACT

BACKGROUND: Meningioma has the second highest incidence among primary intracranial tumors. There is no effective treatment or targeted therapy for meningioma except excision and radiotherapy. Sp1 (specificity protein 1) is a transcription factor that regulates tumor growth, but the literature describing the relationship between Sp1 and meningioma is scarce. The purpose of this study was to investigate the relationship between Sp1 expression and the clinicopathological parameters in meningioma by immunohistochemical staining. METHODS: We collected samples from 74 patients from 2008-2012 in Kaohsiung Medical University Hospital, with staging and prognosis of the pathological section of the meningioma. RESULTS: Our results show that Sp1 expression was significantly associated with World Health Organization grade, malignancy, and recurrence in patients with meningioma. CONCLUSIONS: High expression of Sp1 in patients with meningioma was linked to poor prognosis of recurrence-free time. Therefore Sp1 may be a candidate biomarker of prognosis and therapy target in meningioma.

ABSTRACT

In order to measure the low current in the quadrupole mass spectrometer (QMS), we design a novel wide band composite trans-impedance preamplifier. The noise filtering components, which built in the feedback loop of the preamplifier, are designed to reduce the noise of two-stage amplifiers. By using the package with low thermal resistance factor, reducing the power consumption of preamplifiers and reducing the feedback resistance, the temperature drift of baseline signal is reduced. Compared with the traditional composite preamplifier, the novel preamplifier reduces maximum temperature drift amplitude and reduces root mean square of noise. At last, the environmental reliability of QMS detection was improved.
