ABSTRACT

Background: Stroke is the main disease that causes the burden of neurological disease, leading to upper limb dysfunction and affecting their self-care abilities. Robot-assisted rehabilitation therapy has been gradually used in the rehabilitation of upper limb function after stroke. However, it would be beneficial to explore auxiliary interventions such as reminiscent music therapy, a combination of music and reminiscent, to relieve negative emotions and post-stroke fatigue and improve rehabilitation outcomes. This protocol aims to evaluate the effectiveness of reminiscent music therapy combined with robot-assisted rehabilitation in older stroke patients. Methods: This trial is a single-blind, three-arm randomized controlled trial. Older stroke patients with upper limb dysfunction will be recruited. Participants will be randomly assigned to receive usual rehabilitation treatment and care, usual rehabilitation treatment and care plus robot-assisted rehabilitation and reminiscent music therapy, or usual rehabilitation treatment and care plus robot-assisted rehabilitation. Robot-assisted rehabilitation will be conducted by rehabilitation doctors five times per week for 3 weeks. In experimental group 1, a reminiscent song list will be played for patients. The primary outcome is activities of daily living. All outcomes will be evaluated at baseline and in the week immediately post-intervention. Discussion: We are conducting the first randomized controlled trial on the effects of reminiscent music therapy combined with robot-assisted rehabilitation in older stroke patients. It is expected that this study, if proven effective in improving the activities of daily living in older stroke patients with upper limb dysfunction, will provide evidence-based rehabilitation strategies for medical staff.Clinical Trial Registration: ChiCTR2200063738.

ABSTRACT

Objective: To summarize the status and characteristics of the available evidence, research gaps, and future research priorities for preventing falls in stroke patients through balance training. Methods: We used a scoping review framework. A systematic search of PUBMED, Embase, and Cochrane databases for main articles was conducted. Our study only included articles that on balance training and fall-related indicators in stroke patients. Two researchers independently screened the literature according to the inclusion and exclusion criteria. The data of demographic, clinical characteristics, intervention, sample, and outcome indicators were extracted. The characteristics and limitations of the included literature were comprehensively analyzed. Results: Of the 1,058 studies, 31 were included. The methods of balance training include regular balance training, Tai Chi, Yoga, task balance training, visual balance training, multisensory training, aquatic balance training, perturbation-based balance training, cognitive balance training, system-based balance training, and robot-assisted balance training. The commonly used outcome measures include clinical balance test, such as Berg balance scale (BBS), Timed Up-and-Go Test (TUG), Fall Risk Index assessment (FRI), Fall Efficacy Scale score (FES), and instrumented balance tests. Conclusion: This scoping review summarizes the existing primary research on preventing falls in stroke patients by balance training. Based on the summary of the existing evidence, the characteristics of balance training and their relation to falls in stroke patients were found. The future researches should explore how to develop personalized training program, the sound combination of various balance training, to more effectively prevent falls.

ABSTRACT

BACKGROUND: Although some studies suggest that robot-assisted technology can significantly improve upper limb function in stroke patients compared to traditional rehabilitation training, it is still necessary to incorporate an auxiliary intervention to alleviate negative emotions, thereby alleviating the post-stroke fatigue and encouraging patients to actively respond to rehabilitation. However, the effect of the auxiliary intervention is unknown. OBJECTIVE: To evaluate the effect of reminiscent music therapy combined with robot-assisted rehabilitation in elderly patients with upper limb dysfunction. METHODS: From November 2022 to March 2023, elderly patients with upper limb dysfunction after stroke were assigned to one of three groups, with group A receiving usual rehabilitation treatment and care plus robot-assisted rehabilitation and reminiscent music therapy, group B receiving usual rehabilitation treatment and care plus robot-assisted rehabilitation, and group C receiving only usual rehabilitation treatment and care. Thirty patients completed this study, with 10 participants in each group. Activities of daily living, self-esteem, rehabilitation self-efficacy, positive emotion and upper limb function were measured before and after the intervention. One-way analysis of variance, paired-sample t-test, Kruskal-Wallis H test, Wilcoxon signed rank sum test and Chi-square test were used to analyze the data. RESULTS: According to the intragroup comparisons, in the three groups, all outcome measurements were significantly higher than those at baseline (all P < 0.05). After the intervention, the differences in the self-management effectiveness, rehabilitation self-efficacy, and positive emotion score were statistically significant among the three groups (all P < 0.05). In accordance with the results of Bonferroni analysis, the self-management effectiveness score of group A was significantly higher than that of Group B and Group C (all P < 0.05). The rehabilitation self-efficacy score of group A was significantly higher than that of Group B and Group C (P < 0.05). The positive emotion score of group A was significantly higher than that of Group B and Group C (P < 0.05). CONCLUSION: Reminiscent music therapy combined with robot-assisted rehabilitation is a promising approach to improve rehabilitation self-efficacy and positive emotion, which is evidence that reminiscent music therapy may be an effective auxiliary intervention to improve rehabilitation outcomes.

ABSTRACT

This study aims to explore the intellectual landscape and research hotspots in the central venous catheter-related thrombosis (CVC-RT) research field. Studies discussing CVC-RT published from 1973 to 2022 in the Web of Science Core Collection database were retrieved on February 24th, 2022. Citespace was used to perform a scientometric analysis to identify the intellectual landscape and research hotspots in the research fields of CVC-RT. A total of 4358 studies were retrieved, with an ascending trend in publication numbers. The United States of America was the most influential country. The Journal of Vascular Access published the most studies, and McMaster University was the most prolific institution. The results showed that the focus population of CVC-RT research has changed from pediatric patients to cancer patients, the management of CVC-RT has become more formal and standardized, and the focused CVC type has shifted to port and peripherally inserted central catheters. In addition, seventeen active burst keywords were detected, such as patient safety, clinical practice guidelines, and postthrombotic syndrome. This study comprehensively reviewed publications related to CVC-RT. The research topics on patient safety, clinical practice guidelines, and postthrombotic syndrome related to CVC-RT may be future hotspots.

ABSTRACT

Background: In China, the risk of stroke is higher than that in developed countries such as Europe and North America. Informal caregivers play a major role in providing support to stroke survivors. Currently, only limited studies on changes in psychological state of the caregivers at different stages of stroke have been published. Purpose: To investigate the stress and psychological state of informal caregivers of stroke patients in different periods and to explore the factors that affect their states. Methods: 202 informal caregivers of stroke patients were selected in a 3a-grade hospital in Chengdu, Sichuan. Follow-up was conducted by face-to-face interviews, telephone calls, or home visits on days 3, 2 months, and 1 year after onset. We investigated the basic information about the caregivers, including their anxiety, depression and social support conditions. We analyzed the pressure and psychological conditions of informal caregivers at different stages of stroke and analyzed its influencing factors. The data were displayed by the number and percentage of the cases; the continuous variables were described by means and standard deviation. In addition, the data were compared by Pearson correlation analysis and logistic regression analysis. Results: (1) Within 3 days after the onset of stroke, the informal caregivers had the highest stress, the most severe anxiety and depression, the heaviest burden, and the lowest score of medical-social support. Over time, the pressure and burden of the caregivers are gradually decreasing, anxiety and depression are increasing, and social support is also increasing. (2) The stress and psychological status of informal stroke caregivers are affected by multiple factors, including the caregiver's age, relationship with the patient, patient's age, and patient's physical conditions. Conclusion: The stress and psychological status of informal caregivers varied in the different stages of stroke, and they were affected by several factors. Medical staff should pay attention to informal caregivers while providing care for patients. Relevant interventions may be developed based on the results to improve the health of informal caregivers and thus to promote the health of patients.

ABSTRACT

OBJECTIVE: To evaluate the effectiveness of mindfulness-based stress reduction (MBSR) and mindfulness-based cognitive therapy (MBCT) on depressive symptoms in people with stroke. METHODS: The PubMed, CINAL, Web of Science, Embase, The Cochrane Library, CNKI, and Wangfang databases were searched for relevant articles from inception to September 1st, 2022. Only randomized controlled trials (RCTs) examining the effect of MBSR and MBCT on depressive symptoms in poststroke patients were included. Data extraction and critical appraisal were conducted independently by two investigators. RESULTS: Seven trials with 502 participants were included. Using standardized mean differences, the meta-analysis showed evidence of a significant effect in depression (SMD = -0.93, 95% CI (-1.34 to -0.53), Z = 4.48, p < 0.001). MBSR and MBCT both affected depressive emotions in poststroke participants with depression (SMD = -1.27, 95% CI (-1.71 to -0.84), p < 0.001) and poststroke participants without clinically defined depression (SMD = -0.46, 95% CI (-0.75 to -0.17), p = 0.002). CONCLUSION: Although populations with stroke seem to potentially improve moods from MBSR/MBCT intervention, the impact on the physiological parameters of the disease has not been determined. Further studies with long-term follow-up and higher qualities are warranted for such interventions to determine the full effectiveness.

ABSTRACT

PURPOSE: The healthcare system and service capacity are overwhelmed by the Covid-19 pandemic. There is a substantial unmet need for rehabilitation service to all patients affected by Covid-19 directly or indirectly. This article aims to describe how to rapidly reconfigure and bring rehabilitation services back during the pandemic. METHOD: The Steer committee meeting was held for the development of a strategic preparedness plan and safety management based on principles and evidence of rehabilitation, which will lead to effective mitigation of consequences resulted from Covid-19. RESULTS: Five measures were taken in Southwest China during the pandemic, which includes the "First contact responsibility" and management system; the full-coverage system for nosocomial infection control; the "Closed-off management" system; the full-coverage system for body temperature monitoring; the adoption of 5G-based telerehabilitation. With the implementation of these measures, the capacity and capability were enhanced to safely reopen and operate rehabilitation facilities in Southwest China. CONCLUSION: Further measurement of quality of care and outcomes during and beyond the pandemic is needed in transforming the healthcare system and improving rehabilitation services. Hopefully, the positive message conveyed by this paper could encourage and support communities and the society of physical medicine and rehabilitation worldwide during this challenging time. IMPLICATIONS FOR REHABILITATION Rehabilitation services are essential and there is an unmet need posed by the Covid-19 pandemic. A feasible strategic plan and safety management measures are critical to reconfigure the capacity and capability of rehabilitation services suspended by Covid-19. The adoption of tele-rehabilitation technology has the potential to reshape public health emergency responses and the delivery of care. Measurement of quality and outcomes is of great importance to inform transformation and adaptation of rehabilitation services during and after the Covid-19 pandemic.

ABSTRACT

OBJECTIVE: The central venous catheter is used extensively worldwide. The purpose of this bibliometric analysis was to identify the 100 top-cited systematic reviews/meta-analyses in the literature on central venous catheters and to capture the most important trends in this area of research. RESEARCH METHODOLOGY: A search was performed in the Web of Science Core Collection on studies published prior to November 12th, 2019. The search terms included central venous catheter, systematic review and meta-analysis. Retrieved studies were ranked by citation number and selected by two of the authors. Information such as citation number, author, institution, country and year of publication was collected. RESULTS: The 100 top-cited studies published between 1992 and 2017 were reviewed, with the largest proportion published in 2008 (n = 17). The number of citations ranged from 14 to 660. The country with the largest number of studies was the United States of America (n = 36). Critical Care Medicine published the greatest number of these studies (n = 13). The largest number of these studies were focused on central venous catheter-related infection (n = 56) and thrombosis (n = 19). CONCLUSION: Developed countries were the most productive in the field of central venous catheters. Most meta-analyses focused on complications associated with central-venous catheters such as infection and thrombosis.

ABSTRACT

OBJECTIVE: To investigate the therapeutic effects of resolvin D1 (RvD1) on cerulein and lipopolysaccharide (LPS)-induced severe acute pancreatitis in mice. METHODS: The model of severe acute pancreatitis was induced by cerulein combined with LPS in mice. Mice were treated with RvD1 at a dose of 150 mg/kg for 4 h after the last injection of cerulein. Pathological changes and scores were assessed by HE staining, serum amylase and lipase levels were detected by ELISA, serum tumor necrosis factor-α(TNF-α) and interleukin-6 (IL-6) levels were determined by Luminex Assay. RESULTS: Cerulein combined with LPS successfully induced severe acute pancreatitis model in mice. RvD1 reduced the pathological changes of pancreas in severe acute pancreatitis mice, decreased the serum levels of amylase and lipase, as well as attenuated the serum levels of TNF-α and IL-6. CONCLUSION: RvD1 can reduce the severity of severe acute pancreatitis induced by cerulein and LPS in mice.

ABSTRACT

OBJECTIVE: To assess the physical,psychological and social functioning of stroke patients with home-based rehabilitation. METHODS: Convenient sampling strategy was adopted to select 234 stroke patients discharged from a tertiary hospital rehabilitation center. The patients were assessed using Barthel index (BI),instrumental activities of daily living (IADL) scale,modified rivermead mobility index (mRMI),self-perceived burden scale (SPBS),self-rating depression scale,and Zarit caregiver burden interview (ZBI) during their outpatient visits,household visits,or telephone interviews. RESULTS: The patients had an average BI score of 61.77±22.83,61.11% had mild dependence on daily life. Their IADL score was low at an average of 7.25±6.26. They had 28.90±10.22 in MRMI score and 29.26±7.07 in SPB score. More than 90% had a feeling of self-burden. The caregiver burden score reached 43.97±12.28. Male patients and those with cerebral infarction had better functioning than female patients and those with cerebral hemorrhage. Better physical functioning was demonstrated in those who were younger than 60 yr.,experienced more than one year home-based rehabilitation,were cared by relatives,and had a female caregiver. But such differences were not significant in SPBS and ZBI scores. CONCLUSION: Home-based rehabilitation of stroke has failed to achieve anticipated results. The outcomes vary across those with different age,gender,and type of stroke.

ABSTRACT

OBJECTIVE: To establish a comprehensive geriatric assessment scale which fit the situations of Chinese old people most, and to provide an evaluation tool for the proposal and intervention of functional health problems for the elderly. METHODS: The comprehensive geriatric assessment scale was produced by Delphi method combing with Chinese cultural background, some methods such as literature review and experts evaluation were conducted. The manuscript of this questionnaire was developed through Delphi and pre-experiment. Then the manuscript questionnaires were used among 300 old people, to analyze and assess the reliability, validity and reactivity. RESULTS: The experiment analysis demonstrated that the total Cronbach alpha coefficient was 0. 909 and that of each index ranged from 0.718-0.960, most of the dimension-related coefficients were over 0.5. The construct validity was validated by factor analysis and the cumulative contribution rate was up to 76.88%. Results of non-parametric test on reactivity showed that the differences of total scores and each dimension' scores and substitute scores between the elderly from different institutions were statistically significant (P < 0.01). CONCLUSION: The comprehensive geriatric assessment scale owns a reasonable reliability, validity and reactivity, and this scale can be used as an evaluation instrument for the finding and intervention of the elder's health problems.

ABSTRACT

Four diam(m)ineplatinum(II) complexes containing beta-phenylisosuccinate as the leaving groups were prepared, characterized, and evaluated for their cytotoxicity against A549/ATCC human lung cancer cell line and SGC-7901 human gastric cancer cell line. One of the complexes, (trans-1R,2R-diaminocyclohexane)-beta-phenylisosuccinatoplatinum(II) 4, was much more active than cisplatin and carboplatin.
