ABSTRACT

The article presents information about results of implementation of program of additional education of general practitioners within the framework of cognitive-behavioral prevention of iron deficiency in women. The prospective analysis of level of corresponding awareness and skills of physicians from sample (n=120) and control (n=120) groups before and after training established statistically significant differences in favor of physicians who were trained within the framework of experimental work program that confirms the effectiveness of the last one.

ABSTRACT

The research in the field of regional trends and risk factors related to population morbidity is considered as background of management decision-making in health care. The article presents the results of analysis of medical demographic indices and population health status in the Jewish Autonomous Oblast in 2000-2019. It is established that in in the Jewish Autonomous Oblast population size reduced by 35 100 people to 2019. The negative migration dispositions facilitate losses of population size from 0.5% to 1.23% annually. The gradual increase of number of the elderly brought to ratio working/non-working people 100 to 128. The infant mortality rate decreased from 20.2 in 2000 to 9.2 per 1000 newborns in 2019 and continues to decrease. The total morbidity increase (diagnosed for the first time) of cardiovascular diseases and neoplasms is probably related to preventive measures efficiency. In the structure of malignant neoplasms leading positions are taken by tumors of bronchopulmonary system, skin affections and breast cancer. In the Jewish Autonomous Oblast, mortality of cardiovascular diseases comprised 807.32±22.87 per 100 000 of population. In the last decade, this most important indicator decreased up to 2.12%. The mortality of malignant neoplasms made up to 217.31±15.25 per 100 000 of population. The mortality due to consolidated causes "Traumas, poisonings and some other consequences of external causes exposure" made up to 16.19±3.28 per 100 000 of population. In 2019, the average life interval made up to 68.8 years, having increased by 6.2 years since 2000. The evaluation of medical demographic indices and population health status demonstrate necessity of adjusting medical organizational measures targeting to improve medical care of population considering regional characteristics.

ABSTRACT

Due to the introduction of new medical technologies, there is an increase in the number of people, including the elderly and senile age, who have extended their lives, but who need constant care from medical and social services at home. In the study of age differences of biological needs in home care, it was found that for middle-aged people the greatest difficulties and contribution to the reduction of quality of life brought insufficient nutrition, limited independence in the performance of household manipulations due to joint damage, pain and other biological causes, as well as hypothermia syndrome compared with elderly and senile age. For people of older age groups was characterized by the need to bridge the lack of communication; enhancing the role of the family in the implementation of care; increased confidence in caregivers; increased availability of medicines; and increased availability of religious services. Developed on the basis of identified age-based determinants of quality of life, the original model of the bio-psychosocial activating care for the geriatric contingent of patients at home, the difference from traditional approaches is the use of activating kinesitherapy and cognitive exercises, contributes to the achievement of a higher level of functionality and independence of the geriatric patient, which is accompanied by improving the quality of life.

ABSTRACT

AIM: The aim of this article is to establish a weighted average costs of the workflow of the doctor-the therapist of the district, providing primary health care to the population in the outpatient setting to determine the costs when you visit one patient. MATERIALS AND METHODS: Held fotohronometra working process research 39 physicians in the 17th precinct pilot areas from all Federal districts. Within fotohronometra research conducted 6474 measurement. RESULTS: The average time a physician of the district while visiting a patient in a medical organization made up of 15.30±2.0 min, while visiting at home - 32.5±2.0 min the Largest proportion of the working time of the doctor-the therapist of the district while visiting a patient in the outpatient setting is necessary to work with medical records (47.31%). On core activities while visiting a patient in the practitioner, the district spends to 45.54% of the time. For other activities - 7.15%. CONCLUSION: As a result fotohronometra research workflow of doctors-therapists of district, providing primary health care in outpatient settings, set the average time a physician of the district while visiting one patient (15.30±2.0 min), while visiting at home (32.5±2.0 min).

ABSTRACT

The sexually transmitted infections represent an actual medical social problem. The purpose of study is to analyze characteristics of their prevalence in various social groups of population. The data from annual reports on morbidity in the Republic of Udmurtia in 2006-2015 is analyzed. The analysis of morbidity of sexually transmitted infections during the mentioned period demonstrated the highest morbidity of chlamydia infection (from 135 in 2006 to 194,8 in 2015 per 100 000 of population) and trichomoniasis (from 63.5 in 2006 to 183.4 in 2015 per 100 000 of population). In the structure of morbidity in 2006 trichomoniasis went to 25.6±1.2%, chlamydia infection -- 19±0.9%, syphilis -- 17.6±0.8%. In the end of analyzed period, chlamydia infection was in the lead in overall structure of morbidity. The analysis of morbidity subject to social belonging demonstrated that in relation to syphilis and trichomoniasis preponderance the diseased was in the direction of non-working population while in case of other infections in the direction of workers of middle and minor business. The results of study demonstrated the necessity of development of activities in area of primary prevention among working population, workers of middle and minor business and student youth.

ABSTRACT

Benign hyperplasia of prostate is a very important not only medical but also social problem. The clinical and neuroimmunoendocrine status of elderly patients with benign hyperplasia of prostate treated by 1-blocker doxasosine is described in this article. Intercellular interactions were examined for tumor necrosis factor a (TNF-alpha), proinflammative interleukine-6 (IL-6), soluble intercellular molecule of adhesion (sICAM). The positive role of doxasosine is demonstrated.

ABSTRACT

Arterial hypertension accompanied by prostate benign hyperplasia is very important not medical but social problem in elderly people. The clinical and neuroimmunoendocrine status of elderly patients with arterial hypertension accompanied by prostate benign hyperplasia treated by alpha1-blocker doxasosine is described in the article. Intercellular interactions were examined for tumor necrosis factor alpha, proinflammative interleukine-6, soluble intersellular molecule of adhesion. The positive role of doxasosine is demonstrated.
