ABSTRACT

Objective: multidisciplinary treatment has become increasingly common in dental offices and this treatment strategy is based on the achievement of a healthy, harmonious, and pleasant smile. In addition, the development of new ceramic systems and the possibility of making indirect restorations with a low thickness, such as laminates veneers, allowed combining highly satisfactory aesthetic properties, preserving the dental structure through minimally Invasive technique. Case report: this paper describes a multidisciplinary approach involving periodontics and restorative dentistry for the successful, functional, and aesthetic treatment in an adult patient. The digital smile design was performed as a guide to plan the clinical case considering factors such as tooth inclination and position, golden proportion, tooth size and shape. Discussion: based on these concepts, the present work reports, through a clinical case, the restoration of aesthetics and function with ceramic laminates veneers in lithium disilicate, in the case of anatomical correction and diastema closure. Conclusion: overall, the multidisciplinary treatment plan demonstrated to have greater advantages both for the aesthetics and function of the patient, as well as for the work team. During the 24-month follow-up, this case presented great functional and aesthetics results. (AU)


Introdução: o tratamento multidisciplinar tornou-se cada vez mais comum nos consultórios odontológicos, sendo uma estratégia baseada no tratamento da obtenção de um sorriso saudável, harmonioso e agradável. Além disso, o desenvolvimento de novos sistemas cerâmicos e a possibilidade de fazer restaurações indiretas com baixa espessura, como facetas laminadas, permitiram combinar propriedades estéticas altamente satisfatórias, preservando a estrutura dentária com o mínimo desgaste. Caso clínico: este artigo descreve uma abordagem multidisciplinar envolvendo Periodontia e Odontologia restauradora para um tratamento funcional e estético em um paciente adulto. O desenho digital do sorriso foi realizado como um guia para planejar o caso clínico, considerando fatores como inclinação e posição do dente, proporção áurea, tamanho e formato do dente. Discussão: com base nesses conceitos, o presente trabalho relata, através de um caso clínico, a restauração estética e funcional com laminados cerâmicos em dissilicato de lítio para correção anatômica e fechamento de diastemas. Conclusão: em geral, o plano de tratamento multidisciplinar demonstrou ter maiores vantagens tanto para a estética e função do paciente, quanto para a equipe de trabalho. Durante os 24 meses de acompanhamento, esse caso apresentou ótimos resultados funcionais e estéticos. (AU)

ABSTRACT

OBJECTIVE: This study aims to evaluate the influence of the light activation of simplified adhesives on the shear bond strength of resin cements to a glass-ceramic. Three factors were evaluated: (1) cement in two levels (light cured and dual cured); (2) adhesive in two levels (Single Bond 2 and Single Bond Universal), and (3) light activation in two levels (yes or no). MATERIALS AND METHODS: Thirty-two 1-mm thick slices of a leucite-reinforced glass-ceramic (IPS Empress CAD) were divided into eight groups according to adhesive (Single Bond 2 or Single Bond Universal), cement (AllCem Veneer or AllCem), and light activation of the adhesive before application of the cement (yes or no). Ceramic surfaces were etched for 60 s with 5% hydrofluoric acid, and adhesives were applied. Four cement cylinders were made over each ceramic slice (n = 16) and then submitted to shear bond strength tests. STATISTICAL ANALYSIS: Data were analyzed with three-way ANOVA and Tukey (α = 0.05). RESULTS: There were significant differences between adhesives (P < 0.0001) and no differences between cements (P = 0.0763) and light activation (P = 0.4385). No interaction effect occurred (P = 0.05). Single Bond 2 showed higher bond strength than Single Bond Universal. CONCLUSIONS: The light activation of the adhesive before the application of the resin cement did not influence the bond strength.

A composição de adesivos dentários pode influenciar o seu desempenho quando utilizado para união às estruturas dentárias ou cerâmica. O objetivo do presente foi avaliar a influência de diferentes concentrações de MDP na resistência ao cisalhamento de um cimento resinoso à zircônia. Seis adesivos experimentais foram preparados com a seguinte composição: 0,50% em peso de CQ, 1,0% em peso de DABE, 0,20% em peso de BHT, 0,45% em peso de DPHIF, 10% em peso de HEMA, 15% em peso de TEGDMA, 25% em peso de Bis-EMA, 10% em peso de etanol, 25% em peso de UDMA, e 12,85% em peso de BIS-GMA. O monômero MDP foi adicionado em seis porcentagens em peso: 0%, 3%, 6%, 9%, 12%, ou 15%. Como grupo controle, quatro adesivos disponíveis comercialmente foram avaliados: Single Bond Universal, Single Bond 2, Ambar e Signum Zirconia Bond. A resistência de união à zircônia foi avaliada em uma máquina de universal de ensaios através de teste de cisalhamento. Os modos de falha foram analisados com lupa estereoscópica. A análise estatística foi realizada com Anova a um critério e Tukey (α = 0,05). Foram encontradas diferenças significativas entre os grupos (p < 0,00001). Os valores de resistência ao cisalhamento mais elevados foram obtidas com o Signum Zirconia Bond e o Single Bond Universal. O adesivo sem MDP Single Bond 2 apresentou os valores mais baixos. Não houve diferenças entre os adesivos experimentais. Todos os grupos mostraram falhas adesivas. Concluiu-se adesivos que contenham MDP são importantes para a união à zircônia, entretanto, a concentração de MDP não é o único fator a ser considerado.(AU)


The composition of dental adhesives may influence their performance when used for adhesion to dental structures or ceramics. The purpose of the present study was to evaluate influence of different MDP concentrations on the shear bond strength of a resin cement to zirconia. Six experimental adhesives were prepared with the following composition: 0.50 % CQ by wt, 1.0 % DABE by wt, 0.20% BHT by wt, 0.45% DPHIF by wt, 10% HEMA by wt, 15% TEGDMA by wt, 25% BIS-EMA by wt, 10% ethanol by wt, 25% UDMA by wt, and 12.85% BIS-GMA by wt . The MDP monomer was added at 0% by wt, 3% by wt, 6% by wt, 9% by wt, 12% by wt, or 15% by wt. As a control, four commercially available adhesives were evaluated: Single Bond Universal, Single Bond 2, Ambar and Signum Zirconia Bond. The shear bond strength to zirconia was evaluated in a universal testing machine. Failure modes were analyzed with a stereoscopic loupe. Statistical analyses were performed with one-way Anova and Tukey's HSD test (α = 0.05). There were significant differences between groups (p < 0.00001). The highest shear bond strength values were obtained with Signum Zirconia Bond and Single Bond Universal. Singlebond 2 showed the lowest values. There were no differences between experimental adhesives. All groups showed adhesive failures. MDP-containing adhesives are important for bonding resin cements to zirconia, however, the concentration of MDP is not the only factor to be considered.(AU)
