ABSTRACT

Surgery to expose the anterior occiptocervical junction (OCJ) is exacting, and optimal approaches are debatable. The close proximity of vital structures and difficult surgical access present a unique challenge to treat lesions in this area. Routine access to the upper anterior cervical spine remains limited. The authors present a modified retropharyngeal approach and instrumentation in order to resect an exceptionally rare atypical rhabdoid teratoid tumor involving the craniovertebral junction. The technical aspects of this approach in anatomical perspectives are discussed in this article.

ABSTRACT

OBJECTIVE: This article describes the case report of a prostate adenocarcinoma in the mandible. BACKGROUND: Prostate adenocarcinoma is a malignant tumor common in men from the fourth decade of life. The occurrence of oral metastatic lesions is rare. CASE REPORT: A 78-year-old male patient was referred to the oral and maxillofacial surgery service of the Pontifical Catholic University of Rio Grande do Sul for complaints of painless volume increase in the mandible. The diagnosis through the association of clinical, radiographic, and histopathological examination with the patient's health history determined that the lesion was prostatic adenocarcinoma metastasis. CONCLUSION: Despite the rare occurrence of metastases in the oral region, the dental surgeon should be aware of the possibility for correct diagnostic conduction and, subsequently, the institution of treatment in the early stages of disease.

Abstract Introduction: Solitary plasmacytoma is a rare malignant tumor of plasma cells with no evidence of systemic proliferation. There are two known subtypes: extramedullary solitary plasmacytoma and solitary bone plasmacytoma. The etiology is still unknown. Both lesions present a risk of progression to multiple myeloma. A number of approaches have been used for treatment of solitary plasmacytoma. Objective: To carry out a systematic review of the case reports described in the literature, focusing on therapeutic and prognostic aspects. Methods: A search of clinical case reports was performed in the PubMed database using Mesh Terms related to "plasmacytoma" under the following criteria: type of study (case report), articles in English language, conducted in humans, with no publication date limits. Results: Of the 216 articles found, only 21 articles met the pre-established inclusion criteria. Conclusion: The occurrence of solitary bone plasmacytoma in the bones of the face is a rare condition prevalent between the 4th and 6th decades of life, located in the posterior region of the mandible in most cases. Histopathological examination and systemic investigation are mandatory for confirmation of diagnosis.


Resumo: Introdução: O plasmocitoma solitário é um tumor maligno raro de células plasmáticas sem evidência de proliferação sistêmica e engloba dois subtipos: plasmocitoma solitário extramedular e plasmocitoma solitário ósseo. A etiologia ainda é desconhecida. Ambas as lesões apresentam risco de progressão para mieloma múltiplo. Uma série de abordagens tem sido usada para seu tratamento. Objetivo: Realizar uma revisão sistemática da literatura com enfoque nos aspectos terapêuticos e prognósticos. Método: Realizou-se uma busca de relatos de caso clínico na base de dados PubMed com termos de busca relacionados com "plasmocitoma" sob os seguintes critérios: tipo de estudo (relato de caso), artigos na língua inglesa, estudos realizados apenas em humanos, sem limites de data de publicação. Resultados: Dos 216 artigos encontrados, apenas 21 preencheram os critérios de inclusão pré-estabelecidos. Conclusão: A ocorrência de plasmocitoma solitário ósseo nos ossos da face é uma condição rara prevalente entre a 4a e a 6a décadas de vida, localizada na região posterior de mandíbula na maioria dos casos. O exame histopatológico e a investigação sistêmica são mandatórios para confirmação do diagnóstico.

ABSTRACT

The basis of craniofacial tumor surgery is complete surgical extirpation, preferably en bloc or with free borders. Craniofacial techniques are the gold standard and primary strategies in the treatment of anterior skull base tumors. In the last decade, the reports favoring results of endoscopic techniques have increased. One could conclude that the classical transfacial approaches might become useless, but, in fact, there is little research comparing these techniques.In this article, the authors present their experience with combined craniofacial resection of anterior craniofacial skull base tumors in 9 consecutive patients between January 2013 and July 2015. This article aims to review some of the traditional transfacial approaches, illustrating them with this series of surgical patients. Benefits and drawbacks of these approaches are discussed.

ABSTRACT

The objective of this study was to perform a review of the pathological features and treatment of intraosseous myofi broma of the jaw. Electronic searches were performed of MEDLINE, via Pubmed (from 1950 to November 2012), EMBASE and Cochrane Wiley, on November 27th, 2012, with no language restrictions. The electronic search yielded 41 hits, of which 27 articles were considered relevant after evaluation of titles and abstracts and underwent eligibility assessment according to the eligibility criteria Of these, 10 did not meet one or more inclusion criteria and were rejected. The 17 articles selected included three retrospective studies, 13 case reports and one case series. These articles reported on a total of 24 cases of solitary myofi broma in the jaw. Intraosseous myofi bromas of the jaw are rare lesions and are part of the wide group of spindle-cell neoplasms and are generally observed in the posterior mandible. It is important to establish the appropriate treatment to avoid unnecessary aggressive therapy. Conservative surgery is the preferred option. Keywords: Myofi broma; Mandibular Neoplasm; Treatment.


O objetivo deste estudo foi realizar uma revisão de literatura sobre miofi broma intraósseo da mandíbula, suas características patológicas e tratamento. Foi realizada uma busca eletrônica nas base de dados MEDLINE via Pubmed (1950 a novembro de 2012), EMBASE e Wiley Cochrane, no dia 27 de novembro de 2012 sem restrição de linguagem. A busca eletrônica resultou em 41 citações, e 27 artigos foram considerados relevantes pela leitura do título e pela avaliação do resumo. Desses 27 artigos, 10 não atenderam um ou mais critérios de inclusão e foram excluídos. Entre os 17 artigos selecionados, havia três estudos retrospectivos, 13 relatos de caso e uma série de casos. Vinte e quatro casos de miofi broma solitário intraósseo da mandíbula foram identifi cados. Miofi bromas intraósseos dos maxilares são lesões raras que fazem parte do grupo de lesões neoplásicas fusiformes e são comumente encontrados na região posterior da mandíbula. É importante estabelecer o tratamento apropriado para evitar terapia agressiva desnecessária. Cirurgia conservadora é a opção desejada.

The scope of this study was to verify the possible reasons to justify the delay in the referral of oral cancer patients to specialized health care sectors from the standpoint of the dental surgeons involved in Primary Health Care (PHC) in Porto Alegre, state of Rio Grande do Sul. This target public of this study was dental surgeons working in five different labor environments in PHC in Porto Alegre. Criteria for inclusion was dental surgeons graduated in and working in PHC. The data collection instrument was the focus group technique and application of a structured questionnaire. After data collection, interpretative analysis was conducted with classification of the information under four main headings: professional, team, community and services network. The results suggest that there are several factors associated with delayed in referral of patients with oral cancer to specialized health care sectors. These include omission of early diagnosis, lack of multidisciplinary work and insufficient attention to the need for joint community responsibility with patients for their own health status. It is also due to deficiencies in the health service network, specifically regarding the quality of communication between professionals at different levels of the health services.

ABSTRACT

O objetivo deste estudo é verificar, na percepção dos cirurgiões-dentistas atuantes em Atenção Primária em Saúde (APS) em Porto Alegre (RS), quais as possíveis razões que justifiquem o atraso da chegada do paciente com câncer bucal aos setores de atenção em saúde de maior complexidade. O presente estudo teve como público-alvo cirurgiões-dentistas atuantes em cinco diferentes realidades laborais em APS em Porto Alegre. Foram utilizados como critérios de inclusão profissionais com formação em APS e que estivessem atuando na Atenção Básica. O instrumento para coleta de dados foi a técnica de grupo focal com aplicação de questionário estruturado. Uma vez coletados os dados, foi realizada análise interpretativa e categorização das informações em quatro eixos centrais: profissional, equipe, comunidade e rede de serviços. Os resultados sugerem que os seguintes fatores estão associados ao atraso da chegada do paciente com câncer bucal aos setores de atenção em saúde de maior complexidade: falha na identificação precoce, ausência de trabalho multidisciplinar, desvalorização da necessidade de corresponsabilização pela própria saúde por parte da comunidade e a deficiência da rede de atenção no que tange a qualidade da comunicação entre profissionais de diferentes níveis de atenção.

Objetivos: Verificar quais as possíveis razões que justifiquem o atraso da chegada do paciente com câncer bucal aos setores de maior complexidade de atenção em saúde. Materiais e métodos: O estudo teve como público-alvo cirurgiões-dentistas atuantes em Atenção Primária à Saúde formalmente vincula¬dos ao Projeto TelessaúdeRS (276 potenciais participantes). Foi utilizado um questionário hospedado em software de questionários online, com o link para acesso disponibilizado por correspondência eletrônica e através da homepage do TelessaúdeRS. As respostas foram tabuladas em uma planilha de Excel (Microsoft®). Resultados: Setenta e quatro questionários respondidos (26,8% da amostra inicial); 52,7% relatam não haver Centro de Especialidades Odontológicas para referência na sua Coordenadoria Region¬al de Saúde; 58,1% consideram como possíveis causas do atraso no diagnóstico de câncer bucal a asso¬ciação dos fatores: falha do profissional, do paciente, da rede de saúde e falta de informação; em relação à autopercepção de aptidão para rastreamento e diagnóstico de câncer bucal, 71,6% responderam que seriam importantes capacitações. Conclusões: O presente estudo sugere como possíveis razões que justi¬fiquem o atraso da chegada do paciente com câncer bucal aos setores de maior complexidade de atenção em saúde a deficiência da rede de atenção e a necessidade de mais capacitações relacionadas à patologia.


Aims: To verify the possible reasons to justify the delay in the arrival of the patient with oral cancer to the most complex sectors of health care. Materials and Methods: The study had as target audience dentists working in Primary Health Care formally linked to the Project TelessaúdeRS (276 potential participants). It was utilized a questionnaire hosted in a software of online questionnaires, with the link to access available by electronic mail and homepage of TelessaúdeRS. The responses were tabulated in an Excel spreadsheet (Microsoft ®). Results: Seventy-four answered questionnaires (26.8% of the initial sample); 52.7% reported that there is no Dental Specialty Center for reference in its Regional Health’s Coordination; 58.1% considered as the possible causes of the delay in diagnosis of oral cancer the association of factors: failure from professional, from patient, from the health network and lack of information; related to the self-perception of suitability for screening and diagnosis of oral cancer, 71.6% replied that it would be important the offer of qualification courses. Conclusion: The present study suggests that the possible reasons to justify the delay of the arrival of the patient with oral cancer to the most complex sectors of health care are: the deficiencies in the network care and the need for more qualifications courses related to the pathology.
