ABSTRACT

OBJECTIVE: Evaluate the effect of combined training on body image (BI), body composition and functional capacity in patients with breast cancer. As also the relationship of BI with body composition and functional capacity. METHODS: This was a Controlled Clinical Trial study, this study including 26 patients with breast cancer (30 to 59 years). The training group (n = 13) underwent 12 weeks of training, including three 60-min sessions of aerobic exercise and resistance training, and two sessions of flexibility training per week; each flexibility exercise lasted 20s. The Control Group (n = 13) received only the standard hospital treatment. Participants were evaluated at baseline and after 12 weeks. BI (primary outcomes) was assessed using the Body Image After Breast Cancer Questionnaire; Body composition was estimated with the indicators: Body mass index; Weight, Waist hip Ratio; Waist height ratio; Conicity index; Reciprocal ponderal index; Percentage of fat; Circumference of the abdomen and waist; Functional capacity by cardiorespiratory fitness (cycle ergometer) and strength (manual dynamometer). The statistic was performed in the Biostatistics and Stata 14.0 (α = 5%). RESULTS: The patients in the training group showed a reduction in the limitation dimension (p = 0.036) on BI, However, an increase in waist circumference was observed in both groups. In addition an increase in VO2max (p < 0.001) and strength in the right (p = 0.005) and left arms (p = 0.033). CONCLUSION: Combined training demonstrates to be an effective and non-pharmacological strategy to patients with breast cancer, with improvement on BI and functional capacity, changing related variables negatively when there is no physical training.


OBJETIVO: Avaliar o efeito do treinamento combinado na imagem corporal (IB), composição corporal e capacidade funcional em pacientes com câncer de mama. Assim como a relação do IB com a composição corporal e capacidade funcional. MéTODOS:: Este foi um estudo de Ensaio Clínico Controlado, este estudo incluiu 26 pacientes com câncer de mama (30 a 59 anos). O grupo de treinamento (n = 13) foi submetido a 12 semanas de treinamento, incluindo três sessões de 60 min de exercício aeróbio e treinamento de resistência, e duas sessões de treinamento de flexibilidade por semana; cada exercício de flexibilidade durou 20s. O Grupo Controle (n = 13) recebeu apenas o tratamento hospitalar padrão. Os participantes foram avaliados no início e após 12 semanas. O IB (desfechos primários) foi avaliado por meio do Body Image After Breast Cancer Questionnaire; A composição corporal foi estimada com os indicadores: índice de massa corporal; Peso, relação cintura-quadril; Relação da altura da cintura; Índice de conicidade; Índice ponderal recíproco; Porcentagem de gordura; Circunferência do abdômen e cintura; Capacidade funcional por aptidão cardiorrespiratória (cicloergômetro) e força (dinamômetro manual). A estatística foi realizada na Bioestatística e no Stata 14.0 (α = 5%). RESULTADOS: Os pacientes do grupo de treinamento apresentaram redução da dimensão da limitação (p = 0,036) no IB, porém, foi observado aumento da circunferência da cintura em ambos os grupos. Além disso, um aumento do VO2máx (p <0,001) e da força nos braços direito (p = 0,005) e esquerdo (p = 0,033). CONCLUSãO:: O treinamento combinado demonstra ser uma estratégia eficaz e não farmacológica para pacientes com câncer de mama, com melhora do IB e da capacidade funcional, alterando variáveis relacionadas negativamente quando não há treinamento físico.

Introdução: A citometria de fluxo é uma metodologia importante para o diagnóstico das doenças linfoproliferativas crônicas de células B (DLPCB), contudo, por vezes, o citometrista não encontra subsídios suficientes para a definição exata da entidade patológica envolvida. Objetivo: Analisar os laudos emitidos a pacientes com doenças linfoproliferativas crônicas (DLPC) atendidos em um laboratório particular de Belém-PA, segundo os critérios de classificação estabelecidos pelos estudos de Matutes et al. e Craig e Foon. Método: Estudo retrospectivo com laudos de pacientes que realizaram imunofenotipagem por citometria de fluxo para diagnóstico de DLPCB no período entre setembro de 2015 a dezembro de 2019. Resultados: Depois de aplicados os critérios de Matutes et al. e Craig e Foon para os laudos analisados, observou-se concordância em: 45,24% casos de leucemia linfoide crônica de células B/linfoma linfocítico de pequenas células B; 14,29% casos de linfoma folicular; 4,76% casos de leucemia de células pilosas; e 21,43% de casos definidos como "outras DLPCB não classificáveis por citometria de fluxo". Entretanto, o teste de hipóteses de Hotelling (p=0,0409) mostrou haver diferença estatística para a definição das DLPCB segundo os critérios aplicados. Conclusão: Os resultados ressaltam que, mesmo sendo a citometria de fluxo importante para a caracterização das DLPCB, por vezes, o citometrista necessita incluir no laudo a categoria "outras doenças linfoproliferativas crônicas de células B não classificadas por citometria de fluxo" para induzir o prescritor a solicitar mais exames complementares.


Introduction: Flow cytometry is an important methodology for the diagnosis of chronic B-cell lymphoproliferative diseases (CBCLPD), however, sometimes the cytometrist does not find sufficient elements for the exact definition of the pathological entity involved. Objective: To analyze the reports issued to patients with chronic lymphoproliferative diseases (CLPD) tested at a private laboratory in Belém-PA, according to the classification criteria established by the studies by Matutes et al. and Craig and Foon. Method: Retrospective study with reports of patients who underwent immunophenotyping by flow cytometry for the diagnosis of CBCLPD from September 2015 to December 2019. Results: After applying the criteria by Matutes et al. and Craig and Foon to the reports analyzed, agreement was reached for 45.24% of the cases of chronic b-cell lymphocytic leukemia/small B-cell lymphocytic lymphoma, 14.29% of the cases of follicular lymphoma, 4.76% of the cases of hairy cell leukemia and 21.43% of the cases defined as "other CBCLPDs not classifiable by flow cytometry". However, Hotelling's hypothesis test (p=0.0409) showed a statistical difference for the definition of CBCLPD according to the criteria adopted. Conclusion: The results emphasize that even though flow cytometry is important for the characterization of CBCLPD, sometimes the cytometrist needs to include the category "other chronic B-cell lymphoproliferative diseases not classified by flow cytometry" in the report to induce the prescriber to request additional complementary exams.


Introducción. La citometría de flujo es una metodología importante para el diagnóstico de enfermedades linfoproliferativas crónicas de células B (ELPCB), sin embargo, en ocasiones el citometrista no encuentra suficientes subsidios para la definición exacta de la entidad patológica involucrada. Objetivo: Analizar los informes emitidos a pacientes con enfermedades linfoproliferativas crónicas (ELPC) tratados en un laboratorio privado en Belém-PA, de acuerdo con los criterios de clasificación establecidos por los estudios de Matutes et al. y Craig y Foon. Método: Retrospectivo con relatos de pacientes que se sometieron a inmunofenotipificación por citometría de flujo para el diagnóstico de ELPC de septiembre de 2015 a diciembre de 2019. Resultados: Tras aplicar los criterios de Matutes et al. y Craig y Foon a los informes analizados, se observó concordancia en: 45,24% de los casos de leucemia linfocítica crónica de células B/linfoma linfocítico de células B pequeñas; 14,29% casos de linfoma folicular; 4,76% casos de leucemia de células peludas; y 21,43% de los casos definidos como "otros ELPCB no clasificables por citometría de flujo". Sin embargo, la prueba de hipótesis de Hotelling (p=0,0409) mostró diferencia estadística para la definición de ELPCB según los criterios aplicados. Conclusión: Nuestros resultados enfatizan que si bien la citometría de flujo es importante para la caracterización de ELPCB, en ocasiones el citometrista necesita incluir en el informe la categoría "otras enfermedades linfoproliferativas crónicas de células B no clasificadas por citometría de flujo" para inducir al prescriptor a solicitar más exámenes complementarios.

ABSTRACT

The Montado is a complex agroforestry-pastoral ecosystem due to the interactions between soil-pasture-trees-animals and climate. The typical Montado soil has an acidic pH and manganese toxicity, which affect the pasture's productivity and pasture floristic composition (PFC). The PFC, on the other hand, can also be influenced by the type and intensity of grazing, which can lead to significant decreases in the amount of biomass produced and the biodiversity of species in the pasture. The objective of this study was to evaluate the effect of grazing type, by sheep, and different stocking rates on the PFC throughout the vegetative pasture cycle in areas with and without dolomitic limestone application. Thus, four treatments (P1UC to P4TC) were constituted: P1UC-without limestone application (U) and continuous grazing (CG); P2UD-U and deferred grazing (DG); P3TD-with the application of limestone (T) and DG; P4TC-T and CG. In DG plots, the placement and removal of the animals were carried out as a function of the average height of the pasture (placement-10 cm; removal-3 to 5 cm). The PFC was characterized in winter, at the peak of spring and in late spring. The PFC data were subjected to a multilevel pattern analysis (ISA). The combination of rainfall and temperature influenced the pasture growth rates and consequently the height of the pasture at different times of the year. Therefore, with the different growth rates of the pasture throughout the year, the sheep remain for different periods of time in the deferred grazing treatments. In the four treatments, 103 plant species were identified. The most representative botanical families in the four treatments were Asteraceae, Fabaceae and Poaceae. ISA identified 14 bioindicator species: eight for the winter period, three for the late spring vegetative period and three for the TC treatment.

Abstract Introduction The tongue plays an important role in the development of craniofacial structures. At rest, the light and constant pressure of the tongue against the hard palate, counterbalanced by the pressure provided by proper lip sealing, serves as a guide for maxillary growth. Ankyloglossia makes tongue coupling against the hard palate difficult, impacting maxillary development, which may lead to breathing disorders. Objective To verify the effect of lingual frenotomy on the resting position of the tongue and lips in infants with ankyloglossia. Methods The sample consisted of 334 infants aged between 1 and 60 days old diagnosed with ankyloglossia. The groups were divided in: a) experimental group (EG), which consisted of infants whose mothers agreed with lingual frenotomy; b) control group (CG), which consisted of infants whose mothers either refused lingual frenotomy or were waiting for surgery. Both the position of the lips and of the tongue at rest were assessed while the infants were sleeping during the quiet sleep phase. For mothers who refused their infants to undergo the surgical procedure, a follow-up of the infants was proposed to verify possible interference of the frenulum with the resting position of the tongue and lips. Infants whose mothers agreed with surgery were referred for lingual frenotomy. Results Regarding the position of the tongue and lips at rest at the initial and final assessments, the statistical analysis demonstrated significant differences between both groups. Conclusion Lingual frenotomy enabled infants diagnosed with ankyloglossia to maintain both tongue coupling against the hard palate and closed lips at rest.

BACKGROUND: Refeeding syndrome (RS) is characterized by severe electrolyte derangement that may be associated with worse clinical outcomes in hospitalized patients. In this study, we aim to assess the incidence of RS in inpatients receiving parenteral nutrition (PN) and its possible association with the length of hospital stay (LHS) and mortality. METHODS: We conducted a historical cohort study involving adults receiving PN, monitored by a nutrition therapy team in a Brazilian public hospital. Data collection was made from electronic records. We assessed the presence of RS after PN therapy was initiated. The amount of energy delivered within 72 hours of PN, LHS, and mortality were compared between patients with and without RS. RESULTS: One-hundred ninety-seven patients (aged 57.7 ± 14.1 years; 55.7% men) were followed for a median of 51 days. The incidence of RS was equal to 25.4%. The adequacy of energy delivered in relation to energy requirements within 72 hours (69.4% ± 29.3% vs 61.8% ± 21.6%) was higher in patients with RS than in those without RS (P = .054). The incidence of death was 55.3% and did not differ between groups. The median of LHS was equal to 51.0 (32.0-83.0) days, and it also did not differ between patients with and without RS. CONCLUSION: RS was identified in 25% of patients receiving PN, but it was not associated to mortality and LHS, even when energy delivery was considered in combination to RS.

ABSTRACT

We aimed to investigate the effects of chronic stress (CS) on experimental periodontitis (EP) in rats. For this, 28 Wistar rats were divided into four groups: control, ligature-induced experimental periodontitis (EP), chronic stress (CS; by physical restraint model) and CS+EP (association of chronic stress and ligature-induced periodontitis). The experimental period lasted 30 days, including exposure to CS every day and ligature was performed on the 15th experimental day. After 30 days, the animals were submitted to the behavioral test of the elevated plus maze (EPM). Next, rats were euthanized for blood and mandible collection in order to evaluate the oxidative biochemistry (by nitric oxide (NO), reduced-glutathione activity (GSH), and thiobarbituric acid reactive substance levels (TBARS)) and alveolar bone characterization (by morphometric, micro-CT, and immunohistochemistry), respectively. The behavioral parameters evaluated in EPM indicated higher anxiogenic activity in the CS and CS+EP, groups, which is a behavioral reflex of CS. The results showed that CS was able to change the blood oxidative biochemistry in CS and CS+EP groups, decrease GSH activity in the blood, and increase the NO and TBARS concentrations. Thus, CS induces oxidative blood imbalance, which can potentialize or generate morphological, structural, and metabolic damages to the alveolar bone.

ABSTRACT

Muscle electrical activity analysis can aid in the identification of oral motor dysfunctions, such as those resulting from an altered lingual frenulum, which consequently impairs feeding. Here, we aim to analyze the suprahyoid muscle electrical activity of infants via surface electromyography, based on lingual frenulum attachment to the sublingual aspect of the tongue and floor of the mouth during breastfeeding. In the present study, we have studied full-term infants of both genders, aged between 1 and 4 months old. The mean muscle activities were recorded in microvolts and converted into percent values of the reference value. Associations between the root mean square and independent variables were tested by one-way analysis of variance and Student's t-test, with a significance level of 5% and test power of 95%, respectively. We evaluated 235 infants. Lower mean muscle electrical activity was observed with the lingual frenulum attached to apex/lower alveolar ridge, followed by attachment to the middle third/lower alveolar ridge, and between the middle third and apex/lower alveolar ridge. Greater suprahyoid muscle activity was observed with lingual frenulum attachment to the middle third of the tongue/sublingual caruncles, showing a coordination between swallowing, sucking, and breathing. Surface electromyography is effective in diagnosing lingual frenulum alterations, the attachment points of which raises doubt concerning the restriction of tongue mobility. Thus, it is possible to identify oral motor dysfunctions.

ABSTRACT

Objetivo: Avaliar a qualidade da terapia nutricional parenteral (TNP) prestada aos pacientes internados em um hospital público de alta complexidade, através da utilização de indicadores de qualidade em terapia nutricional (IQTN) e suas possíveis associações com desfechos clínicos (tempo de uso da TNP, mortalidade intra hospitalar e tempo de internação hospitalar). Métodos: Estudo de coorte histórica realizado no Hospital Nossa Senhora da Conceição. Foi realizada coleta de dados de prontuário eletrônico e planilha de controle da TNP da Equipe Multidisciplinar de Terapia Nutricional (EMTN). A amostra foi de conveniência, sendo composta por pacientes maiores de 18 anos que tiveram prescrição de TNP e foram acompanhados pela equipe no período de outubro de 2011 até setembro de 2016. Para aplicação dos IQTN foram utilizadas as frequências relativas dos seguintes indicadores: pacientes com TNP central por menos de sete dias, medida ou estimativa do gasto energético e das necessidades proteicas, realização de avaliação e reavaliação nutricional periódica, recuperação de ingestão oral após término da TNP, medida de índice de massa corporal (IMC), acompanhamento nutricional do paciente pela EMTN, alterações hidroeletrolíticas e disfunções hepática, renal e glicêmicas. As frequências relativas observadas foram comparadas com as metas propostas na literatura. Foram coletadas variáveis sócio demográficas, clínicas, laboratoriais e antropométricas, calculadas as estatísticas descritivas e avaliada a associação entre os IQTN e os desfechos de interesse (tempo de internação hospitalar, tempo de TNP e mortalidade) a partir de análise univariada e multivariada... BACKGROUND AND AIMS: Histological healing has emerged as a promising therapeutic goal in ulcerative colitis. This is especially important in the context of biological therapies. The objectives of the present study were to investigate the ability of infliximab to induce histological remission in ulcerative colitis [UC] patients and to explore the utility of faecal calprotectin and lactoferrin in predicting histological activity. METHODS: Multi-centre, single-cohort, open-label, 52-week trial including moderately to severely biological-naïve UC patients receiving intravenous infliximab [5mg/kg]. The primary outcome was the proportion of patients with histological remission [Geboes index ≤ 3.0] after 8 weeks of treatment, scored by two independent pathologists. RESULTS: Twenty patients were included. The rate of histological remission increased from 5% at baseline to 15% and 35% at Week 8 and Week 52, respectively. At Week 8, 40% of patients were in clinical remission [Mayo ≤ 2] and 45% achieved mucosal healing [Mayo endoscopy subscore 0-1]. At Week 52, 25% of patients had clinical, endoscopic and histological remission. Faecal calprotectin and lactoferrin showed the highest correlation with histological activity at Week 8 (area under the curve [AUC] 94%, p = 0.017; and 96%, p = 0.013, respectively) and both markers revealed an excellent positive predictive value for this outcome at this time point [100%, p = 0.017; and 94%, p = 0.013, respectively]. CONCLUSIONS: Infliximab was able to induce histological remission. There was a good agreement between histology and faecal biomarkers. Faecal calprotectin and lactoferrin were good predictors of histological remission. Our data support inclusion of histology as a treatment target complementary to endoscopy in clinical trials when evaluating therapeutic response in UC.

Aims: To assess the prevalence and epidemiological characteristics of hepatic trauma in victims referred to the Forensic Department of Palmas, State of Tocantins (TO), Brazil.Methods: A retrospective cross-sectional study was performed with all the victims referred to the Forensic Department of Palmas from January 2006 to December 2010, to investigate violent deaths with severe damage to the liver tissue. The data were collected from the Forensic Department records. The following data were evaluated: sex, age, mechanism of injury (in case of motor vehicle accidents, the type of vehicle involved), types of trauma associated with abdominal trauma, presence of abdominal lesions associated with hepatic injury, the American Association for the Surgery of Trauma Organ Injury Scale (AAST-OIS), and number of deaths caused directly by hepatic trauma. The statistical analysis included absolute and relative frequencies of the variables.Results: A total of 241 (13.4%) out of 1,796 victims had liver trauma, among whom 85.5% were male and 42.9% belonged to the 21-30 year age group. Motor vehicle accidents were the major cause of trauma in 143 cases, (59.3%), involving mainly motorcycles (53 cases or 37%). Blunt trauma was the most frequent mechanism of injury, having occurred in 179 cases (74.3%). The right lobe was injured in 98 cases (40.7%). A total of 147 victims (61%) had polytrauma. Splenic injury, present in 65 (27%) of the cases, was mostly associated with liver trauma. Liver injury of grade II was detected in 102 cases (42.4%) and was therefore the most prevalent type of damage. Twenty-one (8.7%) deaths were directly related to liver trauma.Conclusions: Blunt injury, caused mainly by motor vehicle accidents (especially with motorcycles), was the most common type of liver trauma observed in violent deaths, according to the records of the Forensic Department of Palmas. Young men of working age were the most frequent victims with liver trauma.


Objetivos: Analisar a prevalência e as características epidemiológicas relacionadas ao trauma hepático nas vítimas encaminhadas ao Instituto Médico Legal de Palmas, Tocantins.Métodos: Um estudo transversal retrospectivo foi realizado no Instituto Médico Legal (IML) de Palmas, Tocantins, incluindo todas as vítimas encaminhadas para esse serviço, no período de janeiro de 2006 a dezembro de 2010, consistindo em óbitos por causas violentas e que possuíssem lesões traumáticas de tecido hepático. A coleta de dados foi realizada mediante análise dos registros do IML. Foram anotados sexo, idade, mecanismo do trauma (quando acidentes de trânsito, o tipo de veículo envolvido), tipos de trauma associados ao trauma abdominal, presença de lesões abdominais associadas ao trauma hepático, índice de gravidade de lesões conforme a Associação Americana de Cirurgia do Trauma (AAST-OIS) e número de óbitos causados diretamente pelo trauma hepático. A análise estatística incluiu as frequências absolutas e relativas das variáveis.Resultados: Das 1.796 vitimas fatais, 241 (13,4%) delas apresentavam trauma hepático, entre os quais 85,5% eram do sexo masculino e 42,9% tinham entre 21-30 anos. A principal causa envolvida foram os acidentes de trânsito, em 143 (59,3%) dos casos, envolvendo mais comumente as motos, 53 (37%). O tipo de trauma mais encontrado foi o contuso, 179 (74,3%). O lobo hepático direito foi o mais acometido, 98 (40,7%). As vítimas eram politraumatizadas em 147 (61%) dos casos. A lesão esplênica foi a mais associada ao trauma hepático, observada em 65 (27%) dos avaliados. O grau de lesão hepática mais prevalente foi o grau II, presente em 102 (42,4%) casos. O número de óbitos que tiveram como causa direta o trauma hepático correspondeu a 21 (8,7%) de todos os casos.Conclusões: Entre os casos de trauma hepático observados em óbitos por causas violentas, registrados no IML de Palmas, o mais comum foi do tipo contuso, decorrente principalmente de acidentes de trânsito (especialmente com motocicletas). As vítimas mais frequentes com trauma hepático foram homens jovens na faixa etária laboral.

ABSTRACT

Objetivos: Avaliar a prevalência e fatores agravantes do sintoma de pirose possivelmente associado a refluxo gastroesofágico, em estudantesde medicina da Universidade Federal do Tocantins (UFT).Métodos: Um estudo transversal avaliou questionários aplicados a estudantes do Curso de Medicina da Universidade Federal do Tocantinsentre 23/09/2013 e 04/10/2013. As variáveis analisadas foram faixa etária, sexo, índice de massa corporal, frequência de ocorrência dossintomas, percepção de fatores que agravam o sintoma de pirose, tabagismo e uso de medicamentos para aliviar sintomas de pirose. Paracalcular a prevalência, foi considerado como sintoma de refluxo gastroesofágico a ocorrência de pirose mais de uma vez por semana, mas osfatores agravantes foram pesquisados nos indivíduos que referiam ter tido o sintoma pelo menos uma vez na vida. A análise estatística incluiuas frequências absolutas e relativas das variáveis, bem como os intervalos de confiança de 95%.Resultados: Foram avaliados 186 estudantes, sendo que 160 (86,0%) referiram ter tido pirose em alguma ocasião. Naqueles que referiramtal sintoma, a faixa etária predominante foi de 21 a 25 anos (65,0%), sendo 51,3% do sexo masculino. O sintoma de pirose ocorrendo pelomenos uma vez por semana foi referido por 88 (47,3%) dos entrevistados, sendo uma vez por semana por 53 (28,5%) e mais de uma vez porsemana por 35 (18,8%). Considerando todos os 160 estudantes que referiam ter tido pirose em alguma ocasião, constatou-se piora do sintomana presença de alterações emocionais (67,5%), ingestão de alimentos gordurosos ou condimentados (63,7%) e bebidas alcoólicas (60,6%).Conclusões: Os estudantes de medicina em estudo apresentaram uma alta prevalência do sintoma pirose. O sintoma era agravado por ingestãode alimentos gordurosos ou condimentados, alterações emocionais e bebidas alcoólicas.


Aims: To evaluate the prevalence and aggravating factors of heartburn symptom possibly associated with gastroesophageal reflux in medicalstudents of the Federal University of Tocantins.Methods: A cross-sectional study evaluated questionnaires given to students of the Medical School of the Federal University of Tocantinsbetween 09/23/2013 and 10/04/2013. The analyzed variables were age, sex, body mass index, frequency of occurrence of symptoms, perceptionof factors that aggravate the symptom of heartburn, smoking, and use of drugs to relieve symptoms of heartburn. To calculate the prevalence,occurrence of heartburn more than once a week was considered as a symptom of gastroesophageal reflux, but the aggravating factors wereinvestigated in subjects who reported having had this symptom at least once in life. Statistical analysis included absolute and relative frequenciesof the variables as well as the confidence intervals of 95%.Results: At all 186 students were evaluated, of which 160 reported heartburn at some time. In those who reported such symptom, thepredominant age group was 21-25 years (65.0%), and 51.3% were male. The symptom of heartburn occurring at least once a week was reportedfor 88 (47.3%) of respondents, being 53 (28.5%) for once a week and 35 (18.8 %) for more than once a week. Considering all the 160 studentswho reported having had heartburn at some time, there was a worsening of symptoms in the presence of emotional disorders (67.5%), eatingfatty foods (63.7%) and alcoholic beverages (60.6% ).Conclusions: Medical students in the study showed a high prevalence of heartburn. This symptom was aggravated in most interviewees byintake of fatty or spicy foods, emotional changes, and alcoholic beverages.
