ABSTRACT

The authors report six cases of lens-induced inflammations that help to illustrate the wide variability and clinical overlap of these conditions. Using cytological and histopathological findings, they attempt to clarify the confusion surrounding the terminology used to label these inflammations, and outline the immune factors responsible for them. With the recent upsurge in extracapsular cataract surgery, it is important to consider lens-induced inflammations in the differential diagnosis of any postoperative uveitis or endophthalmitis.

ABSTRACT

At three university centres 66 patients presenting with herpetic dendritic or geographic ulcers participated in a double-blind comparative study of 3% acyclovir and 3% vidarabine ointment. There was healing in 31 (97%) of the 32 patients treated with acyclovir, in a mean time of 6.3 days, and in 30 (88%) of the 34 treated with vidarabine, in a mean time of 7.1 days. The two medications were statistically equally effective, no difference being demonstrated in the healing rate, in the frequency of punctate epithelial keratitis or stromal keratitis, or in the final visual acuity.

ABSTRACT

Twenty-five patients with abnormal blood vessels on the optic nerve head were examined both clinically and with fluorescein angiography to determine the diagnostic significance of these abnormal vessels. Six patients had congenital abnormal vessels--prepapillary or optociliary arteries or veins, or vessels associated with congenital anomalies of the disc. Nineteen patients had acquired abnormal vessels that were associated with retinal vein occlusion, chronic glaucoma, chronic papilledema, meningioma or vasculitis. The clinical and fluorescein angiographic features that differentiated these groups are discussed in detail.

ABSTRACT

Multiple endocrine adenomatosis type III (MEA--III) in a 32-year-old white woman is reported. The initial manifestation, a thyroid nodule, was detected at the age of 4 years. The characteristic ocular manifestation of MEA--III are discussed.

ABSTRACT

Epidermoid carcinomas of the lacrimal sac are rare. We report a case that illustrates the difficulties in their diagnosis. When the diagnosis was made in our patient the tumour had extended beyond the sac wall and treatment was unsuccessful. The prognosis of these tumours depends on early diagnosis and complete excision, which demands a high degree of suspicion in all cases of chronic tearing, chronic dacryocystitis or a nonreducible mass in the area of the lacrimal sac. Consultation with an otolaryngologist is esential in both diagnosis and management.

ABSTRACT

Five patients with retinal macroaneurysms are described. The aneurysms may be an incidental finding or they may cause visual loss from retinal, preretinal and vitreous hemorrhages or from macular edema or exudation. The prognosis is good in those cases presenting with hemorrhage but is poorer in those with macular problems. Acquired aneurysms are found in older patients who are hypertensive or have vascular disease. We believe that retinal emboli play an important part in their development.

ABSTRACT

We present a case of iris-nevus or Cogan-Reese syndrome with its characteristic pathological findings. Although a rare entity, it should be considered in the differential diagnosis of heterochromia associated with elevated intra-ocular pressure. To avoid unnecessary enucleation, it should not be confused with diffuse iris melanoma.
