ABSTRACT

Importance: Understanding acupuncture point microenvironments is vital for optimizing treatment efficacy. Evaluating changes in water content at these points can provide further insights into the effects of acupuncture on tissues. Objective: This study aimed to measure tissue dielectric constant (TDC) and assess changes in water content, specifically at stomach 36 (ST36, Zusanli) and spleen 6 (SP6, Sanyinjiao) acupuncture points. Methods: In a controlled, blinded, randomized trial, 113 healthy volunteers were divided into six groups based on TDC sensor diameters (XS, M, and L): three control groups and three acupuncture groups. They were assessed at three time points: T1, baseline; T2, 20 min post-needle withdrawal; and T3, 40 min post-needle withdrawal. Electrical impedance (EI) was also analyzed. Significance level was set at p < 0.001. Results: TDC at ST36 and SP6 significantly decreased with the XS probe at T2 and T3 compared with that at T1 (F8, 452: 54.61). TDC did not significantly vary between T2 and T3 with M and L probes. EI data indicated that the current passage increased in the SP (F2, 226: 39.32) and ST (F2, 226: 37.32) groups during T2 and T3 compared with that during T1 within their respective groups and controls. Conclusions: and Relevance: This study demonstrated the efficacy of TDC measurements in detecting water content fluctuations at acupuncture points and their responses to needles. TDC measurements, which were validated against EI, provide valuable insights into acupuncture point microenvironments and thus help optimize treatments.

ABSTRACT

Fear and anxiety are considered concerns for animal welfare as they are associated with negative affective states. This study aimed to pharmacologically validate an attention bias test (ABT) for broiler chickens (Gallus gallus domesticus) as a cognitive bias test to determine anxiety. Two-hundred-and-four male Ross 708 broiler chickens were arbitrarily allocated to either the anxiogenic or control treatment at 25 days of age (n = 102/treatment). Birds from the anxiogenic group were administered with 2.5 mg of ß-CCM (ß-carboline-3-carboxylic acid-N-methylamide [FG 7142]) per kg of body weight through an intraperitoneal injection (0.1 ml/100 g of body weight). Birds from the control group were administered with 9 mg of a saline solution per kg of body weight. During ABT, birds were tested in groups of three (n = 34 groups of three birds/treatment) with commercial feed and mealworms as positive stimuli and a conspecific alarm call as a negative stimulus. Control birds were 45 s faster to begin feeding than anxiogenic birds. Birds from the control group vocalized 40 s later and stepped 57 s later than birds from the anxiogenic group. The occurrence of vigilance behaviors did not differ between treatments. This study was successful in pharmacologically validating an attention bias test for fast-growing broiler chickens, testing three birds simultaneously. Our findings showed that latencies to begin feeding, first vocalization, and first step were valid measures to quantify anxiety.

ABSTRACT

INTRODUCTION: Timor-Leste has one of the world's highest estimated tuberculosis (TB) incidences, yet the data which informs this estimate is limited and the true burden of TB disease is not known. TB prevalence surveys offer the best means of determining robust estimates of disease burden. This study aims to provide an estimate of the prevalence of bacteriologically confirmed pulmonary TB in Timor-Leste and provide additional insights into diagnostic coverage and health-seeking behaviour of persons with symptoms suggestive of TB. METHODS AND ANALYSIS: A national population-based cross-sectional cluster survey will be conducted in which participants aged 15 years and older will be screened for pulmonary TB using an algorithm consisting of symptom screening and digital X-ray of the chest with computer-aided detection software for X-ray interpretation. Xpert Ultra and liquid culture methods will be used to confirm survey TB cases. Additional data will be collected from persons reporting symptoms suggestive of TB to assess health-seeking behaviour and access to TB diagnosis and care. The survey aims to screen a target sample population of 20 068 people, living within 50 clusters, representing every municipality of Timor-Leste. Bacteriologically confirmed pulmonary TB prevalence will be estimated using WHO-recommended methods. ETHICS AND DISSEMINATION: Research ethics approval has been granted by the human research ethics committee of the Northern Territory, Australia, and the Instituto Nacional da Saúde, Timor-Leste. The results will be published in a peer-reviewed scientific journal and disseminated with relevant stakeholders. TRIAL REGISTRATION NUMBER: ACTRN12623000718640.

ABSTRACT

This research delves into the consequences of consistent pinprick stimulation on preterm offspring to ascertain its long-term implications for pain sensitivity. The primary objective of this protocol was to investigate the impact of neonatal pinprick stimuli on the pain threshold in the later stages of life using a preterm rat model. By establishing this model, we aim to advance the research on understanding and managing early postnatal pain associated with prematurity. The findings of this study indicate that while the baseline thresholds to mechanical stimuli remained unaffected, there was a notable increase in mechanical hypersensitivity following complete Freund's adjuvant (CFA) injection in adult rats. Interestingly, compared with male rats, female rats demonstrated heightened inflammatory hypersensitivity. Notably, maternal behavior, the weight of the litters, and the growth trajectory of the offspring remained unchanged by the stimulation. The manifestation of altered nociceptive responses in adulthood after neonatal painful stimuli could be indicative of changes in sensory processing and the functioning of glucocorticoid receptors. However, further research is needed to understand the underlying mechanisms involved and to develop interventions for the consequences of prematurity and neonatal pain in adults.

ABSTRACT

A disciplina Integração-Ensino-Serviço-Comunidade III (IESC III) faz parte do itinerário curricular da Universidade do Grande Rio ­ Professor José de Souza Herdy (UNIGRANRIO-AFYA) e, por ter caráter obrigatório, deve ser cursada por todos os graduandos ao decorrer do curso. Como parte da metodologia de ensino-aprendizagem, acompanhados pelos professores atribuídos ao curso, os discentes, como parte das atividades da disciplina, realizam, em sua prática, um Projeto Terapêutico Singular (PTS), seguindo as normas da Equipe de Saúde da Família (ESF), tendo a oportunidade de acompanhar uma família por meio dessa ação. Este artigo tem como objetivo relatar as experiências vivenciadas pelos discentes durante os quatro meses de IESC III em que foi posto em prática o desenvolvimento do PTS. Na percepção dos estudantes, tal ação formativa foi substancial para a construção dos futuros médicos, haja vista o contato mais próximo com a família em questão, que proporcionou a familiarização com o trabalho que é desenvolvido diariamente pelos Agentes Comunitários de Saúde. Ademais, atesta-se pelo próprio sucesso obtido ao final do trabalho, que ele oportunizou o conhecimento de realidades completamente distintas daquelas vivenciadas por cada integrante do grupo.

Objetivo: Analisar a mortalidade de profissionais de saúde pelo COVID-19 no Brasil nos anos de 2020 à 2022. Metodologia: Estudo ecológico, exploratório, descritivo, comparativo e de abordagem quantitativa. Os dados foram adquiridos junto ao Sistema de Informação da Vigilância Epidemiológica da Gripe (SIVEP-Gripe) do Departamento de Informática do Sistema Único de Saúde (DATASUS) daSecretaria de Vigilância em Saúde e Ambiente (SVS) do Ministério da Saúde (MS). Resultados: Foi possível identificar o universo de 886 registros, com média e desvio-padrão (295,3±379). Foi verificado que a maior preponderância se constituiu de profissionais técnicos ou auxiliares de enfermagem com 26% (n=230), médicos 15% (n=133) e enfermeiros com 10% (n=89). O nível de formação educacional com maior preponderância foi o superior com 50% (n=443). Considerações finais: Foi possível verificar aumento na frequência de registros de mortalidade de profissionais de saúde pelo COVID-19 no recorte geográfico e histórico analisado.

Methodology: Ecological, exploratory, descriptive, comparative study with a quantitative approach. The data were acquired from the Influenza Epidemiological Surveillance Information System (SIVEP-Gripe) of the Department of Informatics of the Unified Health System (DATASUS) of the Secretariat of Health and Environmental Surveillance (SVS) of the Ministry of Health (MS). Results: It was possible to identify the universe of 886 records, with mean and standard deviation (295.3±379). It was found that the greatest preponderance was made up of technical professionals or nursing assistants with 26% (n=230), doctors with 15% (n=133) and nurses with 10% (n=89). The level of educational training with the greatest preponderance was higher education with 50% (n=443). Final considerations: It was possible to verify an increase in the frequency of mortality records of health professionals due to COVID-19 in the geographical and historical area analyzed.

ABSTRACT

ABSTRACT BACKGROUND AND OBJECTIVES: Chronic Pelvic Pain (CPP) is characterized by persistent pain in the pelvic region for more than six months, affecting both men and women and causing significant impairment in quality of life (QoL). Two of the main non-invasive approaches are Transcranial Magnetic Stimulation (TMS) and Transcranial Direct Current Stimulation (tDCS). These techniques aim to modulate neural activity and promote pain relief. In this context, this research conducted an integrative literature review to summarize the results of relevant studies, aiming to identify the key parameters used in TMS and tDCS for CPP treatment. The objective was to assess the effect and efficacy of non-invasive neuromodulation as a therapeutic intervention for CPP. CONTENTS: For this integrative review, electronic searches were conducted in Pubmed, Scielo, PEDro, Medline, Cochrane, and Scopus databases, examining studies in Portuguese, English, or Spanish. The keywords "pelvic pain," "transcranial direct current stimulation," and "transcranial magnetic stimulation" and their derivatives were searched in the three languages in studies from 2013 to 2023. Seven studies were included for analysis. Both techniques showed positive effects in managing CPP, improving pain levels and quality of life to a relevant extent. However, there is still no consensus on the parameters applied in TMS and tDCS techniques for CPP. CONCLUSION: Non-invasive neuromodulation improves pain levels and quality of life in patients with CPP. Further studies are needed to establish more reliable parameter relationships, and the limited number of studies restricts definitive conclusions on the subject.


RESUMO JUSTIFICATIVA E OBJETIVOS: A dor pélvica crônica (DPC) é caracterizada pela persistência da dor na região pélvica por mais de seis meses, afetando tanto homens quanto mulheres e causando prejuízos significativos na qualidade de vida (QV). Duas das principais abordagens não invasivas são Estimulação Magnética Transcraniana (EMT) e a Estimulação Transcraniana por Corrente Contínua (ETCC). Nesse contexto, esta pesquisa realizou uma revisão integrativa da literatura com o intuito de resumir os resultados de estudos relevantes, buscando identificar os principais parâmetros utilizados no tratamento da DPC. O objetivo foi fornecer uma visão abrangente sobre essas técnicas de neuromodulação e suas aplicações específicas no controle da dor pélvica crônica. CONTEÚDO: Para esta revisão integrativa, as buscas eletrônicas ocorreram nas bases de dados Pubmed, Scielo, PEDro, Medline, Cochrane e Scopus, verificando estudos em português, inglês ou espanhol. "Dor pélvica", "estimulação transcraniana por corrente contínua" e "estimulação magnética transcraniana" e suas derivações foram pesquisadas nos três idiomas em estudos entre 2013 e 2023. Sete estudos foram incluídos para análise. Ambas as técnicas apresentaram efeitos positivos no manejo da DPC, melhorando os níveis de dor e a QV em proporções relevantes. Entretanto, ainda não há um consenso sobre os parâmetros aplicados nas técnicas de EMT e ETCC para DPC. CONCLUSÃO: A neuromodulação não invasiva melhora os níveis de dor e a QV em pacientes com DPC. São necessários mais estudos para que relações mais confiáveis de parâmetros possam ser preestabelecidas e a ausência de um maior número de estudos limita conclusões acerca do assunto.

The research aimed to assess the effects of auditory environmental enrichment on sows in mixed housing (caged until 35 days after insemination and then collective pens) or collective housing (caged until 72 h after insemination and then collective pens). Reproductive performance, body surface and eye temperature (ET) were evaluated as sows' welfare indicators. A sample of 56 sows between 2nd and 6th parity was submitted to the treatments from artificial insemination to weaning. The sows were assigned in a randomized block design with a 2 × 2 factorial scheme of treatments: mixed housing-control (MH-C), collective housing-control (CH-C), mixed housing-music (MH-M) and collective housing-music (CH-M). Auditory enrichment consisted of exposing sows daily throughout gestation and lactation to 6 h of classical music divided into 2 h periods. The ET of pregnant sows in collective housing was lower than that of sows in mixed housing (33.77 °C vs. 34.91 °C). Pregnant and lactating sows exposed to auditory environmental enrichment exhibited lower ET compared to those that had no access to the stimulus (pregnant 33.36 °C vs. 34.32 °C and lactating 34.21 °C vs. 34.83 °C). No housing type effect was found on the reproductive performance parameter; however, piglets from sows submitted to auditory environmental enrichment, regardless of the type of housing during gestation, were heavier at weaning (6.32 kg vs. 5.57 kg). Collective or mixed housing does not affect sows' reproductive performance; perhaps, auditory environmental enrichment reduced stress in the gestation and lactation phases and provided greater piglet weight gain at weaning.

ABSTRACT

Three new complexes Na[Ln(pic)4]ּ⋅2.5H2O (Ln = Tb, Eu or Gd; pic = picolinate) were synthesized and characterized by infrared spectroscopy, powder X-ray diffraction and thermogravimetric analyses. The molecular structures of the complexes have been determined by single-crystal X-ray diffraction. The three isostructural lanthanide complexes crystalize in the hexagonal system with space group P6122 to Eu complex and Gd complex and space group P6522 to Tb complex. In each of the complexes, the picolinate ligands are bonded to Ln3+ and Na+ ions by different coordination modes promoting polymeric structures. The photoluminescent properties of complexes were studied and combined with theoretical studies using the density functional theory (DFT: B3LYP, PBE1PBE) and the semiempirical method AM1/Sparkle from the single crystal X-ray diffraction structures to assign a suitable model for describing the system. The B3LYP DFT functional was considered the most adequate for providing structural properties of the compounds and for describing luminescence properties. The excited triplet states (T1) and excited singlet states (S1) of the ligand were determined theoretically using Time-dependent DFT calculations (TD-DFT: B3LYP, CAM-B3LYP and LC-wPBE) and INDO/S-CIS, with the best agreement with experimental values obtained from the LC-wPBE DFT functional. The photoluminescent spectra of the complexes and their lifetime measurements were determined indicating that the Eu complex and Tb complex display different intramolecular energy transfer mechanisms with higher efficiency to ligand-to-terbium energy transfer. In addition, the experimental and theorical Judd-Ofelt intensity parameters and quantum yields of the complexes were also determined and discussed besides to a proposed 9-state diagram to describe the luminescence properties of the Eu complex. The low value of emission quantum efficiency of 5D0 emitting level of Eu(III) ion was explained by the presence of the ligand-to-metal charge transfer state (LMCT) evidenced experimentally and theoretically. A good agreement was obtained between the proposed kinetic model and experimental results showing the consistency of the set of rate equations assumed and the intramolecular pathways proposed.

Environmental enrichment can increase the occurrence of natural behavior and improve leg health and other animal welfare outcomes in broiler chickens. This study aimed to assess the effects of three environmental enrichments, specifically hay bales, step platforms, and laser lights, on subclinical spondylolisthesis prevalence, productivity, behavior, and gait of broiler chickens (Gallus gallus domesticus). Twenty-four hundred day-old male Ross® AP95 chicks from a commercial hatchery were used in a completely randomized design with four treatments and four replicate pens per treatment. Pens contained either a Control (C) treatment, an environment similar to a commercial broiler chicken system without environmental enrichments, or an environment with either additional hay bales (HB), additional step platforms (SP), or additional laser lights (LL). Performance, yield, behavior (frequencies), gait score, and subclinical spondylolisthesis prevalences were assessed. When raised with SP or LL access, fewer chickens had subclinical spondylolisthesis than chickens without enrichments (C) or with HB access. Chickens with access to SP exhibited higher wing yield and less abdominal fat than animals from the C group. Chickens from the LL and HB treatments explored more and rested less frequently than animals from the C and SP treatments. As chickens aged, they became less active, exploring less and increasing resting and comfort behaviors. Treatments did not affect gait. Gait was not associated with subclinical spondylolisthesis prevalence. Environmental enrichments benefitted chicken health (subclinical spondylolisthesis) and behavior (exploration) without negative consequences for performance and yield.

The present study aimed to evaluate the effects of partially fermentable insoluble dietary fiber supplementation on the behavior, surface temperature, and reproductive parameters of gestating and lactating sows, as well as on the performance of their litters. Four hundred hyper-prolific sows were assigned in a randomized block design with two treatment groups during the gestation phase: Control (C), corn-soy based diets, or corn-soy based diets with daily 55 g supplementation of eubiotic fiber (F) from the 85th day of gestation until the farrowing (late pregnancy). During the lactation phase, the sows were assigned in a 2 × 2 factorial design using the following treatment groups: (1) CC, no fiber included during gestation and lactation. (2) FC, daily inclusion of 55 g of fiber only during late pregnancy. (3) CF, daily inclusion of 55 g of fiber only during lactation. (4) FF, daily inclusion of 55 g of fiber during late pregnancy and lactation. Sows that received dietary fiber supplementation during the final third of gestation increased feed intake during lactation. Piglets from sows supplemented in both phases showed a significant increase in weight at weaning. The dietary supplementation of eubiotic fiber for sows in the end period of gestation and lactation improved performance and welfare, with positive consequences for developing their litters.

ABSTRACT BACKGROUND AND OBJECTIVES: Health professionals attitudes and beliefs about musculoskeletal pain have a negative influence on patient beliefs and outcomes. However, there is no gold standard for assessing knowledge and attitudes toward pain among these professionals. Thus, the objective of the present study was to translate, adapt and validate the Knowledge and Attitudes of Pain (KNAP) questionnaire into Brazilian Portuguese (KNAP-Br), apply the questionnaire and analyze the correlation of its results with the Neurophysiological Pain Questionnaire (NPQ). METHODS: After being translated, back-translated and adapted, the NKAP-Br was applied to 60 physical therapy and medicine students for validation. The correlation between the results obtained by multiplying the scores of the questions of the NKAP-Br instrument and the NPQ was evaluated. To assess reliability, another 200 students responded to the initial KNAP-Br (T1), performed the Pain Education Program (PEP) in one week and one week after T1, the participants received access to answer the KNAP-Br end (T2). RESULTS: Concurrent validity was assessed by the correlation between NPQ and the final KNAP-Br score. A significant correlation was found between the NPQ result (0.3 and p-value=0.0001) and the KNAP-Br score. Intragroups, 84% improved in the KNAP-Br score after studying PEP, 43.50% improved at or above the minimal detectable difference (MDD) and at or above the minimal important difference (MID). CONCLUSION: The questionnaire was translated and adapted respecting the Brazilian population cultural aspects and presented satisfactory reliability and construct validity, being considered valid for the assessment of knowledge and the interpretation of pain by health professionals.


RESUMO JUSTIFICATIVA E OBJETIVOS: As atitudes e as crenças dos profissionais de saúde sobre dor musculoesquelética têm influência negativa nas crenças e resultados dos pacientes. Entretanto, não existe um padrão ouro para avaliar o conhecimento e as atitudes relativas à dor entre esses profissionais. Dessa forma, o objetivo deste estudo foi traduzir, adaptar e validar o questionário Knowledge and Attitudes of Pain (KNAP) para a língua portuguesa brasileira (KNAP-Br), aplicar o questionário e analisar a correlação dos seus resultados com o Questionário Neurofisiológico de Dor (QND). MÉTODOS: Após ser traduzido, retrotraduzido e adaptado, o KNAP-Br foi aplicado em 60 estudantes de fisioterapia ou medicina para validação. Foi avaliada a correlação entre os resultados obtidos pela multiplicação dos escores das questões dos instrumentos KNAP-Br e QND. Para avaliação da confiabilidade, outros 200 estudantes responderam o KNAP-Br inicial (T1), realizaram em uma semana o Programa de Educação em Dor (PED) e uma semana após T1 os participantes receberam acesso para responder o KNAP-Br final (T2). RESULTADOS: A validade concorrente foi avaliada pela correlação entre QND e o escore final do KNAP-Br. Foi encontrada uma correlação significativa entre o resultado do QND (0,3 e valor de p=0,0001) e o escore do KNAP-Br. Intragrupos, 84% melhoraram no escore do KNAP-Br após estudar PED, 43,50% melhoraram igual ou acima da menor diferença detectável (MDD) e igual ou acima da diferença mínima importante (DMI). CONCLUSÃO: O questionário foi traduzido e adaptado, respeitando os aspectos culturais da população brasileira, e apresentou satisfatória confiabilidade e validade de constructo, sendo considerado válido para avaliação do conhecimento e para a interpretação da dor por profissionais de saúde.

RESUMO: Objetivo: analisar fatores associados à não adesão terapêutica em pessoas com Hipertensão Arterial que procuraram serviços de pronto atendimento por emergência e/ou complicação hipertensiva. Método: estudo transversal realizado entre dezembro de 2019 a outubro de 2020, com 238 pessoas residentes em um município de médio porte do Sul do Brasil, mediante aplicação da Escala de Adesão Terapêutica de Oito Itens de Morisky. Na análise se utilizou Regressão Logística Múltipla. Resultados: 86 (36,1%) participantes foram considerados não aderentes. Foi observada maior chance de não adesão em pessoas com menos de 60 anos (Odds Ratio=2,04), que buscaram assistência nos serviços de pronto atendimento nos três anos em estudo (Odds Ratio=5,08) e que tinham vínculo com profissionais da Atenção Primária à Saúde (Odds Ratio=1,96). Conclusão: reconhecer os fatores associados com a não adesão à terapêutica possibilitará aos profissionais realizar intervenções educativas e assistir as pessoas com hipertensão de acordo com suas necessidades, prevenindo/postergando complicações.


ABSTRACT Objective: to analyze factors associated with therapeutic non-adherence among individuals with Arterial Hypertension who seek emergency care and/or assistance due to hypertensive complications. Method: this is a cross-sectional study conducted from December 2019 to October 2020 with 238 people living in a medium-sized municipality from southern Brazil using Morisky's 8-Item Medication Adherence Scale. Multiple Logistic Regression was used in the analysis. Results: a total of 86 (36.1%) participants were considered as non-adherent. A higher change of non-adherence was observed in people younger than 60 years of age (Odds Ratio=2.04), who sought emergency services in the three years under study (Odds Ratio=5.08), and who had a bond with Primary Health Care professionals (Odds Ratio=1.96). Conclusion: acknowledging the factors associated with non-adherence to the therapy will allow professionals to conduct educational interventions and assist people with hypertension according to their needs, thus preventing/postponing complications.

Método: estudio transversal realizado entre diciembre de 2019 y octubre de 2020 con 238 residentes en un municipio de mediano porte del sur de Brasil, utilizando la Escala de Adherencia Terapéutica de 8 ítems de Morisky. En el análisis se utilizó Regresión Logística Múltiple. Resultados: se consideró que 86 (36,1%) participantes no cumplían con la terapia. Se observó una mayor probabilidad de no adhesión en personas menores de 60 años (Odds Ratio=2,04), que buscaron asistencia en los servicios de urgencia en los tres años en estudio (Odds Ratio=5,08) y que tenían una relación con profesionales de Atención Primaria de la Salud (Odds Ratio=1,96). Conclusión: reconocer los factores asociados a la no adhesión a la terapia permitirá que los profesionales realicen intervenciones educativas y asistan a las personas con hipertensión arterial según sus necesidades, previniendo/posponiendo complicaciones.

ABSTRACT

Background: Despite the widespread clinical use of acupuncture in painful situations, the use of this treatment should be further clarified. Nociception is mediated by the activation of nociceptors, such as transient receptor potentials (TRPs). The family of TRPs includes TRPV1, TRPM8, and TRPA1, which can be stimulated by substances such as capsaicin, menthol, and methyl salicylate, respectively. Objectives: This study aimed to investigate the role of TRPs in antinociception via the administration of agonists of these receptors in the Zusanli acupoint (ST36) in models of inflammatory, acute, and neuropathic pain. Methods: Male Wistar rats were used for this experiment. All rats received a subcutaneous injection of TRP agonists (capsaicin, menthol, or methyl salicylate) in ST36; saline was injected as control. Nociception was evaluated using the electronic mechanical threshold test and tail-flick test before the administration of complete Freund's adjunct or chronic constriction injury of the sciatic nerve and after the administration of TRP agonists. Results: Nociception was found to be attenuated after treatment with TRP agonists. The administration of different doses (0.03, 0.3, and 3.0 µg/20 µL) of capsaicin, menthol, and methyl salicylate in the different pain models (neuropathic, inflammatory, and nociceptive) induced antinociception in most of the evaluated time points. Conclusion: Based on the findings, we suggest that the activation of TRPV1, TRPM8, and TRPA1 receptors results in the antinociceptive effect of the stimulation of the ST36 acupoint. Thus, TRP receptors may present a new therapeutic opportunity for the control of inflammatory and neuropathic pain.

RESUMO Objetivo: apreender como os profissionais atuantes na Atenção Primária à Saúde (APS) e no Serviço de Atenção Domiciliar (SAD) percebem as potencialidades e as limitações para o cuidado compartilhado. Método: estudo descritivo exploratório, de abordagem qualitativa, fundamentado nos pressupostos teóricos da Rede de Atenção à Saúde (RAS), realizado com 10 profissionais de saúde atuantes em uma capital brasileira. Os dados foram coletados em outubro de 2019 mediante a realização de uma única sessão de grupo focal, ocasião em que foram discutidas, a partir da construção da matriz SWOT (Strenghts, Weaknesses, Opportunities, e Threats), as forças, as fraquezas, as oportunidades e as ameaças que permeiam o cuidado compartilhado na atenção domiciliar. Os dados foram submetidos à análise de conteúdo, modalidade temática. Resultados: como potencialidades, foram destacadas: as ações de cuidado realizadas em conjunto pelos diferentes profissionais das equipes; a realização de reuniões objetivas e direcionadas; a divisão de responsabilidades e a definição de papéis; e os fluxos que podem melhorar a comunicação entre as equipes e potencializar a prática do cuidado compartilhado na atenção domiciliar. Como limitações, destacam-se: o conhecimento insuficiente dos critérios de elegibilidade para a atenção domiciliar; a deficiência de recursos materiais e de tecnologia da informação; e lacunas na formação profissional. Conclusão: os resultados podem contribuir para qualificação da assistência entre os diferentes serviços que compõem a Rede de Atenção à Saúde, especialmente pela identificação dos elementos relacionados ao próprio processo de trabalho que influenciam no cuidado compartilhado.


RESUMEN Objetivo: apreciar como los profesionales activos en la Atención Primaria a la Salud y el Servicio de Atención Domiciliaria perciben las potencialidades y limitaciones para el cuidado compartido. Método: estudio descriptivo exploratorio, de abordaje cualitativo, fundamentado en los presupuestos teóricos de la Red de Atención a la Salud, realizado con 10 profesionales de la salud activos en una capital brasileña. Los datos se recogieron en octubre de 2019 mediante la realización de una única sesión de grupo focal, en la que se discutieron las fortalezas, debilidades, oportunidades y amenazas que permean el cuidado compartido en la atención domiciliaria a partir de la construcción de la matriz SWOT (Strenghts, Weaknesses, Opportunities, e Threats). Los datos se sometieron a un análisis de contenido, modalidad temática. Resultados: como potencialidades se destacaron las acciones asistenciales realizadas de forma conjunta entre los diferentes profesionales de los equipos, la celebración de reuniones objetivas y focalizadas, el reparto de responsabilidades y la definición de roles y flujos que pueden mejorar la comunicación entre los equipos y potenciar la práctica de los cuidados compartidos en la atención domiciliaria. Como limitaciones destacaron el insuficiente conocimiento de los criterios de elegibilidad para la atención domiciliaria y la falta de recursos materiales y de tecnología de la información, así como las lagunas en la formación profesional. Conclusión: los resultados pueden contribuir a la cualificación de la asistencia, entre los diferentes servicios que componen la Red de Atención a la Salud, especialmente mediante la identificación de los elementos relacionados con el proceso de trabajo que influyen en el cuidado compartido.


ABSTRACT Objective: to apprehend how professionals working in Primary Health Care (PHC) and Home Care Service (HCS) perceive the potentialities and limitations of shared care. Method: descriptive exploratory study, with a qualitative approach, based on the theoretical assumptions of the Health Care Network (HCN), carried out with 10 healthcare professionals working in one of the capitals of a state in Brazil. Data were collected in October 2019 through a single focus group session, at which time, based on the construction of the SWOT matrix (Strengths, Weaknesses, Opportunities, and Threats), strengths, weaknesses, opportunities, and threats that permeate shared care in home care. Data were submitted to content analysis, thematic modality. Results: as potentials, the following were highlighted: the care actions carried out jointly by the different professionals of the teams; conducting objective and targeted meetings; the division of responsibilities and the definition of roles; and the flows that can improve communication between teams and enhance the practice of shared care in home care. As limitations, the following stand out: insufficient knowledge of the eligibility criteria for home care; the deficiency of material resources and information technology; and gaps in professional training. Conclusion: the results can contribute to the qualification of care among the different services that make up the Health Care Network, especially by identifying elements related to the work process itself that influence shared care.

ABSTRACT BACKGROUND AND OBJECTIVES: The aim of the present study was to evaluate the impact of the COVID-19 pandemic on psychological aspects and its relationship with bruxism from a representative sample of the Brazilian population. METHODS: A cross-sectional Internet-based survey was conducted in a nationally representative sample of Brazilian adults to estimate the sociodemographic correlates and characteristics of bruxism, the quality of life of individuals World Health Organization Quality of Life-BREF (WHOQOL) and Self-Compassion Scale (SCS) in the Brazilian population during the COVID-19 pandemic. Research was collected from May to August 2020. The data were analyzed using qualitative analysis. RESULTS: A total of 1476 Survey forms were completed, and 1265 (85.70%) respondents declared presenting daytime clenching; over half of respondents (843, 57.11%) reported that they have grinding of teeth; and 1054 (71.41%) reported both clenching and grinding of teeth. Most of the respondents (1128, 76.42%) reported a negative perception of oral symptoms on the last month and all (1476, 100.00%) were feeling nervous or stressed during the period of social withdrawal induced by the COVID-19 pandemic. Moreover, 289 (19.58%) started having symptoms of fatigue or pain in the muscles of the face upon awakening and 318 (21.54%) started having muscle fatigue and discomfort in the teeth upon awakening. CONCLUSION: Findings have shown that all respondents were feeling nervous or stressed during the period of social withdrawal induced by the COVID-19 pandemic and reported bruxism symptoms. They also had lower averages of WHOQOL and SCS, suggesting worse perception of quality of life and self-compassion.


RESUMO JUSTIFICATIVA E OBJETIVOS: O objetivo do presente estudo foi avaliar o impacto da pandemia de COVID-19 nos aspectos psicológicos e sua relação com o bruxismo a partir de amostra representativa da população brasileira. MÉTODOS: Foi conduzida uma pesquisa transversal baseada na internet em uma amostra representativa de adultos brasileiros para estimar os correlatos sociodemográficos e as características do bruxismo, a qualidade de vida (WHOQOL) dos indivíduos e a autocompaixão (SCS) na população brasileira durante a pandemia por COVID-19. A pesquisa foi coletada de maio a agosto de 2020. Os dados foram analisados por meio de análise qualitativa. RESULTADOS: Foram preenchidos 1.476 formulários da Pesquisa e 1.265 (85,70%) respondentes declararam apresentar apertamento diurno; mais da metade dos entrevistados (843, 57,11%) relatou ranger de dentes; e 1.054 (71,41%) relataram tanto apertamento quanto ranger de dentes. A maioria dos entrevistados (1.128, 76,42%) relatou percepção negativa dos sintomas de bruxismo no último mês e todos (1.476, 100,00%) estavam se sentindo nervosos ou estressados durante o período de afastamento social induzido pela pandemia de COVID-19. Além disso, 289 (19,58%) iniciaram sintomas de fadiga ou dor nos músculos da face ao acordar e 318 (21,54%) iniciaram com fadiga muscular e desconforto nos dentes ao acordar. CONCLUSÃO: Os resultados mostraram que todos os entrevistados estavam se sentindo nervosos ou estressados durante o período de afastamento social induzido pela pandemia de COVID-19 e relataram sintomas de bruxismo. Também apresentaram médias mais baixas no WHOQOL e SCS, sugerindo pior percepção de qualidade de vida e autocompaixão.

ABSTRACT

INTRODUCTION: Morphological reorganization in the neural networks of the medial prefrontal cortex (mPFC) may be involved in the development of chronic neuropathic pain (NP). OBJECTIVES: We investigated whether inactivation and neurostimulation of the infralimbic division (IFL) of the mPFC alter electroacupuncture-induced analgesia (EIA) at 2 Hz and 2/100 Hz in animals with chronic NP. METHODS: Wistar rats were submitted to chronic constrictor injury of the ischiadicus nerve (CCI). Von Frey and acetone tests were performed to evaluate mechanical or cold allodynia. Animals were submitted to electroacupuncture (EA) at 2 Hz and 2/100 Hz for 20 min. After EA, the IFL cortex synaptic contacts were inactivated by cobalt chloride (200 nL of 1.0 mM CoCl2). Neurostimulation of the IFL cortex was also performed at 20 µA for 15 s, after EA, using a deep brain stimulation device. RESULTS: EA at 2 Hz and 2/100 Hz attenuated mechanical or cold allodynia in CCI rats. Microinjection of CoCl2 into the IFL division of the mPFC blocked the EA effect. EA at 2 Hz and 2/100 Hz, in association with neurostimulation of the IFL cortex, attenuated mechanical and thermal allodynia. CONCLUSION: EA induces antinociception in CCI rats. The analgesia was potentiated in association with neurostimulation in the IFL division of the mPFC.
