ABSTRACT

Accumulated in 2004-2014 yrs in Department of laparoscopic surgery and choledocholithiasis experience of treatment in patients, suffering parasitic and nonparasitic hepatic cysts, was analyzed. Own improved classification of hepatic cystic lesions was proposed. Peculiarities of performance of open operations, of the puncture miniinvasive operations and interventions for hepatic cysts, using laparoscopic approach, were enlighten. Main indications for surgical treatment of hepatic cysts were adduced, depending on their kind, size, localization and clinical signs.

ABSTRACT

The results of conduction of endoscopic transpapillary interventions in 976 patients, suffering stenosing papillitis in 2000 - 2014 yrs, were summarized. In 48 (4.9%) patients complications have had occurr, all the patients are alive.

ABSTRACT

The own experience on surgical treatment of more than 400 patients, suffering parasytic and nonparasitic hepatic cysts, benign and malignant hepatic tumors, using laparoscopic technologies, was summarized. Indications for laparoscopic operations, the main technical aspects, immediate and late follow-up results were analyzed. Advantages of laparoscopic operations in comparison to open operative interventions in thoroughly selected patients were noted.

ABSTRACT

The comparative analsis of the effectiveness of surgical interventions performed by the standard method and using electric welding technology in endocrine surgery was hold. Compared duration of surgery, amount of intraoperatve blood loss, frequency of intra- and early postoperative complications. Found that the use of weding technology ensures shorter duration of surgery, on average by 30%, amount of blood loss--by 20-50%, the frequency of intra- and early postoperative complications.

ABSTRACT

Changes in the neutrophils system of peripheral blood in 26 patients, suffering different forms of an acute pancreatitis, were studied. Concordance between results of the acute pancreatitis prognostication in accordance with the Ranson and APACHE II scales and structural-functional state of the neutrophils system were established. Technological quality, speed of performance, sensitivity, objectiveness constitute advantages of a new method of estimation. The proposed method is a perspective one for the course prognostication and monitoring of complicated forms of an acute pancreatitis. Its application would facilitate the patients stratification in accordance to severity of their state and choice of the treatment tactics.

ABSTRACT

Liver protein synthesis was estimated comparing the levels of prothrombin and its inactive form PIVKA-prothrombin. The latter indicates liver dysfunction. These diagnostic tests allow monitoring the effectiveness of the commonly applied preparation "Essentiale Forte" and that of the liposomal form of the biologically active additive (BAA) FLP-MD based on phospholipids of various origin.

ABSTRACT

The changes in a system of the blood neutrophils in 65 patients, suffering different forms of an acute pancreatitis, were studied. The trustworthy differences in a neutrophils system structure were determined in patients while their effective conservative treatment, effective surgical treatment, complicated course of postoperative period and exitus lethalis. This data permit to adjust a base for algorithm creation on prognostication, diagnosis and monitoring in patients, suffering an acute pancreatitis.

Retrospective analysis of postoperative complications, occurred in patients, to whom laparoscopic interventions were performed in the Department of Laparoscopic Surgery and Cholelithiasis, was conducted. The rate of inflammatory and thromboembolic complications have constituted 2.2 and 0.1%, accordingly.

ABSTRACT

The experience of a method of electric welding of biological tissues in laparoscopic surgery is submitted. 1494 operations with use of electrowelding procedure were conducted (cholecystectomy - 1434 (95.9%), echinococcectomy - 32 (2.1%), pericystectomy of echinococcosis cyst - 19 (1.27%), regional resection of a liver with a cyst - 1 (0.06%), enucleation of a hemangioma - 5 (0.33%). On the estrablishment of the received results it is possible to make a conclusion, that procedure of electric welding of biological tissues is a reliable, simple method of a hemostasis and tissue bonding method in laparoscopic surgery.

ABSTRACT

Medicamentous hepatitis in mice is accompanied by infringement of synthetic, conjugative and eliminative liver functions. It is experimentally proved, that chemical composition of bile and duodenal contents may be corrected by including drugs with reparation actions in the rehabilitation circuit, in particular those made on the basis of phospholipids of various origin: biologically active additive FLP-MD and preparation "Essentiale".

ABSTRACT

Results of examination and treatment of 609 patients with chronic biliary pancreatitis (CHBP), caused by various forms of cholelithic disease, papillostenosis, cystic transformation of biliary ducts, were analyzed. There were applied open, laparoscopic and endoscopic methods of treatment. Diagnostic algorithm was elaborated, which proved its trustworthiness in 92% of observations. There were established possibilities and advantages of miniinvasive technologies in the treatment of CHBP, expediency of their usage to reduce the hospital stay duration and frequency of complications, to improve the quality of life of the patients. Application of minitraumatical methods had permitted to avoid mortality.

ABSTRACT

The results of studying of the treatment effectiveness in 30 patients with residual choledocholithiasis (RC) were generalized. In 131 (23.8%) of patients simultaneously with endoscopic papillosphincterotomy (EPST) various endobiliary manipulations on biliary ducts were conducted, in 21 (16.2%) of them--nasobiliary drainage, in 14 (10.8%)--mechanical lithotripsy, in 6 (4.5%)--lithotripsy plus nasobiliary drainage. Great significance in RC diagnosis have the methods of direct contrasting of biliary ducts. The authors characterize the partial, subtotal and total EPST according to the length of major duodenal papilla incision. After papillosphincterotomy in 97 (75%) of patients the concrements were mechanically removed with maximal diameter 2.5 cm. In 10.8% of patients complications occurred. All of the patients are alive.
