ABSTRACT

The fatty acid composition of plasma triglycerides by gas chromatography, the dynamics of the segment ST, cardiac arrhythmia by daily monitoring of electrocardiogram in patients with unstable angina (progressive) and the effects of treatment with statins were studied. Revealed marked qualitative abnormalities of plasma triglycerides in patients with progressive angina manifest increase in the amount of saturated and reduction--of unsaturated fatty acids. High therapeutic effect of simvastatin and atorvastatin may be due to the identified strong correlation between the dynamics of the fatty acid components of plasma triglycerides and indicators of ischemia, ectopic activity in patients with progressive angina.

ABSTRACT

59 patients with bronchial asthma (BA) combined with hypertension have been observed. The authors have studied the level changes of cortisol and sexual hormones. Considerable decrease in level of progesterone and cortisol, and also increase in the level of estradiol have been revealed in the patient's blood. It enhances maintenance and progression of bronchoobstructive a syndrome. The authors have also assessed the dynamics of sexual hormones against antihypertensive therapy of patients with BA. Increase in the level of progesterone against administration of amlodipin has been observed in women and increase in the level of testosterone against administration of of enalapril in men. Obtained data allows to optimize the choice of hypotensive medication in patients with BA combined with hypertension.

ABSTRACT

Ischemic heart desease (IHD) remains actual issue of the current clinical medicine, because, IHD takes a leading position among reasons of death rate of population. Purpose of the study was to find diagnostic features of fat-acid composition of lipid of thrombocytes in patients with stable and unstable course of ischemic heart disease (IHD). 52 patients with an unstable angina pectoris aged 42-65 years and 31 patients with stable angina aged 41-66 years have been observed. Reliable changes in relation of the sum of saturated, unsaturated and poli-unsaturated of fat acids of phosphotides of thrombocytes in patients with unstable course of ischemic heart disease (IHD) have been detected. It improves knowledge about the mechanisms of destabilization of coronal circulation of blood.

ABSTRACT

The purpose of this study was to establish phospholipid composition of the myocardium in patients with ischemic heart disease, and to estimate possible correlation of biochemical parameters with myocardium extrasystolic activity. The patients (n = 28) including 15 patients with ischemic heart disease and 13 patients with secondary atrium septum defect (control group) were studied. During surgical intervention the right atrium myocardium bioptates were taken. Phospholipid metabolism was studied in the myocardium samples. At the eve of surgical intervention a holter monitoring was performed. Deep changes in the myocardium lipid metabolism were found, including accumulation of free and estherified cholesterol, lysophospholipids, and sphingomyeline. An increase of free cholesterol content was accompanied by accumulation of sphingomyeline. This can be an evidence of changes in the constitution of lipid rafts. Extrasystoles, particularly ventricular ones, in patients with ischemic heart disease might depend on accumulation of lysophospholipids as they took place simultaneously with it.

ABSTRACT

The authors presents in the article information on influence of endothelial dysfunction and level of blood leukocytes with clinical course of stable angina pectoris in patients with ischemic heart disease (IHD). 104 male patients aged from 36 to 69 (on average 54.9+/-2.1 years) with IHD and stage I-III stable angina pectoris have been observed. The level of leukocytes, ECG daily monitoring, endotelial function with the help of Celermaera-Sorensena test and plasma concentration of endothelin I have been studied in patients. It was established that there is no significant different (P>0.05) in the level of leukocytes between health persons and patients with IHD independently of functional class of angina pectoris. Patients with IHD had higher level of endothelin I in their blood serum than those in a control group. Patients with IHD have a complicated endothelial-non-dependent vasodilation and an increased thickness of the complex intima-media, there is a negative correlation bond between these characteristics. Patients III functional class of angina pectoris has also reliably decreased endothelial-dependent vasodilation by 37%. Frequency of ventricular extrasystoles in patients with IHD and endothelial dysfunction increases by 12 times.

ABSTRACT

The article presents results of the analysis of indicators immunogenotypic tests of 104 patients with stable stenocardia I-III functional classes. It was established that patients with stable stenocardia have lower level of CD3+ T- lymphocytes, decreased rate of T- lymphocytes helpers/ suppressors (CD4+/CD8+) and increased concentration of circulating immune complexes. These changes did not depend from Functional class of stenocardia and old myocardial infarction. It was established a close interrelation between immunological indicators on the one hand and cholesterol and leukocytosis from another. This correlation has allowed to offer the new classification approach for patients with stenocardia, which considers level of cholesterol and leukocytes.

ABSTRACT

The objective of the study is to study of the fatty acids spectr of LDL and HDL of blood plasma unstuble stenocardia (US) and increace efficiency of treatment with statines. 21 patients with US were presckribed atorvastatine with the first day of treatment Patients in control group received only basis therapy. The duration of the treatment was was 21 days. The patients were observed behive and after the treatment. The change of the content of LDL and HDL was made with jafic chromotography method. Payly ECG monotoring was used to check ST segment dynamie, and heart rate disordens. It was esheblished That the use of atorvastatine normalises contents of LDL and HDL and decreases episodes of the duratuion of miocardial ischemia and the quantity of ventricular extrasystols.

ABSTRACT

Endothelial dysfunction and endothelial cells activation as it was shown in patients with ischemic heart disease play important role in atherosclerosis progression and the development of cardiovascular events. Relationship between E-selectine and functional/ structural changes of the arterial vessels in patients with metabolic syndrome was not explored. We revealed that both activation of the endothelial cells and structural/functional changes of the arterial wall mostly depend on obesity and dislipedemia and in less extent on carbohydrates metabolism disorders.

ABSTRACT

The objective of the work was the study of particular features of fatty acid (FA) composition of the low density lipoproteids (LDL) in patients with metabolic syndrome (MS) (39,7+/-3,4 years). In patients with MS, compared to the healthy adults of the same age was revealed a significant increase in saturated FAs and significant decrease in unsaturated and polyunsaturated FAs in LDL particles. Significant correlation was revealed between indexes of abdominal adiposity, intima media thickness of arteria carotis communis, and level of some FAs in the LDLs. This changes of FA composition could be a cause of enhancing of LDL atherogenic process in patients with methabolic syndrome.

ABSTRACT

Cardio-vascular diseases and acute coronary syndrome (ACS) in the first place play the first role in the structure of diseases and mortality of world population. Effective method of diagnostics of ACS consists in detecting early and late biomarkers of myocardium necrosis. The article presents characteristics of such biomarkers indices as myoglobin, cardiotroponins, MB-fractions of creatine phosphokinase, cardio-form of protein, lactate dehydrogenase, aspartate aminotransferase. The authors described in the artcile diseases which are accomponied with increase in the level of cardiotroponins.

ABSTRACT

The authors presents in the article based on observations of 89 patients questions related to the changes of intracardial hemodynamics in patients with bronchial asthma and arterial hypertention (AH) observed using echocardiography. Disorders of indices of intracardial hemodynamics in patients with bronchial asthma and arterial hypertention have more evident negative tendency than that in patients with isolated bronchial asthma (BA) or arterial hypertention. It was established that such patients (without taking into consideration the degree of severity of BA) have considerably an increased volume indices of left and right ventricule, increased pressure in the lung artery, right ventricule hypertrophy, left ventricule concentric hypertrophy as well as disorders of diastolic heart function of relaxation type. Comparing indices of intracardial hemodynamics depending on the degree of severity of BA it was found that progression of lung hypertension, disorders of diastolic and systolic heart functions correlates with the degree of severity of BA. It leads to earlier formation of biventricular heart failure.

ABSTRACT

We have studied the influence of Etmosin medication on severity of IHD clinical presentations and comprised biliary motility in 162 patients aged 26 to 60. Dynamic echocholecystography (DECG) has been applied. The obtained data suggest using Etmosin in patients with ischemic heart disease and comprised biliary motility makes for normalizing tonus of the Odi sphincter and decreasing the rate of episodes of angina pectoris, although was not found any influence of Etmosin on gall-bladder motility. The use of Etmosin enables in patients with IHD and comprised biliary motility to decrease reflex angina pectoris.

ABSTRACT

Heart syndrome features associated with pain, the structure of myocardial ischemia and heart rate derangement were studied in patients with ischemic heart disease (IHD) and concomitant biliary dyskinesia (BD). It was established that BD considerably aggravates clinical course of IHD: increase in frequency of pain and painless episodes of myocardial ischemia in patients with stable exertional angina I-II functional classes by NYHA and single supraventricular and ventricular extrasystoles.

ABSTRACT

Corinfar-Retard efficacy in patients with hypertonic-hyperkinetic type of biliary dyskinesia has been studied. 56 patients aged 26 to 30 had been prescribed Corinfar-Retard in 20 mg per day for 7 days. Dynamic echocholecystography has applied. Decrease in contraction of gall bladder and normalization of tonic contraction of sphincter Ody was found in the patients with previously diagnosed hypertonic-hyperkinetic type of biliary dyskinesia after having been treated with Corinfar-Retard.
