ABSTRACT

El carcinoma sarcomatoide es una neoplasia maligna, poco frecuente,de localización extremadamente rara en pulmón, más frecuenteen varones y en clara relación con el hábito tabáquico.Describimos el caso de un varón de 72 años que presentaba clínicade tos con expectoración hemoptoica, síndrome febril y cuadroconstitucional, en el que se objetivó en la radiografía de tóraxduplicación del tamaño de una lesión quística, ya conocida previamentey con sospecha clínica de quiste hidatídico en lóbulo superiorderecho. Ante la falta de diagnóstico con las pruebas no invasivas,se realizó una toracotomía diagnóstico-terapéutica,estableciéndose el diagnóstico anatomopatológico de carcinomasarcomatoide de pulmón tras la extirpación de la pieza quirúrgica (AU)


Sarcomatoid carcinoma is an infrequent malignant neoplasia,extremely rare in the lung, more frequent in men and with a clearrelationship with the tobacco habit.We describe the case of a 72 year old man who presented a clinicalpicture of cough with haemoptoic expectoration, febrile syndromeand constitutional syndrome, in which chest X-ray examinationshowed a duplication in the size of a previously known cystic lesion,with the clinical suspicion of a hydatid cyst in the right upper lobe.Faced with the lack of a diagnosis with non-invasive tests, a diagnostic-therapeutic thoracotomy was made, establishing the anatomopathologicdiagnosis of sarcomatoid carcinoma of the lung afterthe surgical resection of the lesion (AU)

El pseudotumor inflamatorio pleural múltiple es una rara entidad,recientemente descrita en la literatura. Es uno de los tumoresbenignos que se manifiestan en niños, y aparecen en tejidos blandosy serosas, como pleura y peritoneo. Su patogénesis no esta bienestablecida. Existen pocas referencias bibliográficas de esta entidad.Los casos publicados, se suelen diagnosticar en gente joven ytienen buen pronóstico. El tratamiento de elección es la cirugía.Presentamos un caso de un paciente de 23 años, que mostraba enuna radiografía de tórax, realizada en el estudio de una lumbalgiacrónica de meses de evolución, una tumoración para vertebralderecha en mediastino posterior (AU)


The calcifying fibrous multiple pleural pseudotumor is a rarelesion, which has been recently incorporated into the medical literature.Is one of the benign tumours which appears in children,soft tissues and serous locations (peritoneal and pleural). Thepathogenesis origin has not been clearly established yet. There arenot many bibliographic references of this type in literature. Thosecases already published, were discovered in young population andhave good prognostic. The treatment to be selected is surgical. Wehere in below present the case of a 23 years old patient, whichshown- in a thorax radiography taken during the study of a cronicalumbalgia after several months of evolution- a right para-vertebraltumoracion in mediastino posterior (AU)

ABSTRACT

OBJECTIVE: To study the real situation of respiratory medicine specialists and chest surgeons in the Spanish health service. SUBJECTS AND METHOD: Using the database of the secretary of the Spanish Society of Respiratory Medicine and Chest Surgery (SEPAR) for 1999, we gathered data on 326 Spanish hospitals. We were thus able to ascertain the level of staffing of respiratory medicine and chest surgery departments. The results were expressed as simple percentages showing distribution by age, duties and geographic area. The number of specialists per 100,000 inhabitants in each community was also calculated based on population data provided by the National Statistics Institute for 1997. RESULTS: We studied 1,786 physicians or surgeons (1,245 pneumologists, 185 surgeons and 356 from other specialties). The mean age was 41 9 years for pneumologists, 44 10 years for surgeons and 51 8 years for the others. We observed unequal distribution by geographic area, with a high of 6.66 pneumologists and 0.9 surgeons per 100,000 inhabitants in Asturias and Madrid, respectively and a low of 1.4 pneumologists per 100,000 inhabitants in Murcia and 0 surgeons in Extremadura. CONCLUSIONS: Specialist coverage by pneumologists and chest surgeons is inadequate in many provinces and communities in Spain. The professional market is not saturated, indicating that future specialists can look forward to market demand.

ABSTRACT

OBJECTIVE: To demonstrate the validity of diurnal polysomnography for the diagnosis of obstructive sleep apnea syndrome (OSAS). METHODS: Twenty-five patients with suspect of OSAS were studied with two serial polysomnographic recordings: one during nighttime-nocturnal polysomnography (NPLS) and then a second polysomnographic study during the following morning (DPLS). In both recordings the Sensor Medic Polysomnograph 4250 was used. The certainty OSAS diagnosis was considered when an apnea-hypopnea index (AHI) > or = 10 in the nocturnal recording was obtained. The statistical study was performed with the R-Sigma Horus SA software and sensitivity and specificity values following formulas next indicated. RESULTS: The male/female ratio was 72%/28%. All patients were snorers and 64% had diurnal hypersomnia. Fourteen out of the 25 patients were true positive, five true negative, five false negative and one false positive patients. The specificity and sensitivity of DPLS were 83% and 73%, respectively, with a positive predictive value of 93% and a negative predictive value of 50%. CONCLUSIONS: Abbreviated diurnal polysomnographic recordings, performed with conventional polysomnography, have a good sensitivity and specificity for the diagnosis of OSAS. Given the possibility of false negative results in DPLS, a NPLS should be performed in the patient with presumed OSAS and negative diurnal polysomnographic study.

ABSTRACT

To assess the role of some pro-inflammatory cells in inflammatory processes in lung cancer by measuring their respective activation markers in different portions of bronchoalveolar lavage (BAL) fluid. Prospective study in a university hospital. We studied 52 BAL samples, 37 from patients with lung cancer and 15 from a control group, using a radioimmunoassay technique to analyze for tryptase (T), hyaluronic acid (HA) and eosinophil cationic protein (ECP) in separate bronchial and bronchoalveolar samples from BAL fluid. Statistical analysis was performed using the R-SIGMA program. Patients with tumors had significantly higher T and HA levels in BAL fluid than did control patients, in both bronchial and bronchoalveolar portions. Lung cancer patients had higher T and ECP levels in bronchoalveolar portions. Mast cells and fibroblasts, at least, play a part in lung cancer, mainly in the distal portions of the bronchial tree.
