ABSTRACT

Liver abscess due to Mycobacterium tuberculosis associated with pulmonary tuberculosis is a rare diagnosis, particularly in patients without AIDS. We report the case of a male immigrant with no HIV infection who developed a tuberculous liver abscess and pulmonary tuberculosis simultaneously. Ziehl-Neelsen stain, PCR and Löwenstein-Jensen culture of material sampled from the abscess established the microbiological diagnosis. The patient's course was good, with disappearance of symptoms and the abscess after tuberculostatic treatment and drainage of the abscess with monitoring by computed tomography.

ABSTRACT

Histiocytosis X or Langerhans cell histiocytosis is an infrequent disease, which consists on proliferation of Langerhans cells. The etiology is unknown. Diagnosis is reached by electron microscope study of the biopsy, in which the Birbeck intracytoplasmatic granules of the Langerhans cells are found and/or by immunohistochemistry procedures able to detect S-100 antigen and CD1 cells. Diagnosis can also be reached with a bronchoalveolar lavage in which CD1 cells will appear in a score higher than 5%. We present the case of a 16 year-old girl that first appeared with a pulmonary lesion with a honey comb X-ray pattern and unsyntomatic mandibular bone affectation. Diagnosis was reached by biopsy study using S-100 antigen detection procedures. We consider this case an important one due to the infrequency of this particular disease.

ABSTRACT

El absceso hepático por Mycobacterium tuberculosis asociado a una tuberculosis pulmonar es un diagnóstico poco frecuente, e incluso aún más raro es encontrarlo en pacientes sin el síndrome de la inmunodeficiencia adquirida (sida).Presentamos el caso de un paciente inmigrante con serología negativa para el virus de la inmunodeficiencia humana (VIH) que presentó simultáneamente un absceso hepático tuberculoso y una tuberculosis pulmonar. La tinción de Ziehl-Neelsen, la PCR y el cultivo en Löwenstein-Jensen del material extraído del absceso permitieron establecer el diagnóstico microbiológico del mismo. La evolución fue buena, con desaparición de los síntomas y del absceso, tras tratamiento tuberculostático y drenaje del absceso bajo control mediante tomografía computarizada (TC) (AU)

ABSTRACT

La histiocitosis X o histiocitosis de células de Langerhans es una enfermedad infrecuente, que consiste en la proliferación de células de Langerhans. Su causa es desconocida. Al diagnóstico se llega mediante el estudio de biopsia de la lesión por microscopía electrónica, detectándose los gránulos de Birbeck intracitoplasmáticos de las células de Langerhans, y/o mediante técnicas inmunohistoquímicas, que detecten la proteina S100 y las células CD1. También es diagnóstico un porcentaje mayor del 5 por ciento de células CD1 en el lavado broncoalveolar. Presentamos el caso de una paciente de 16 años, que se manifestó como una lesión pulmonar con un patrón radiológico en panal de miel y una afectación ósea mandibular asintomática. Al diagnóstico se llegó mediante el estudio de la biopsia pulmonar por técnicas de detección de proteína S100. Consideramos de interés este caso por la infrecuencia de esta enfermedad (AU)

ABSTRACT

La aspiración bronquial de cuerpos extraños es más frecuente en niños. La etiología más usual son los cuerpos extraños de origen vegetal, que no se aprecian en una Rx simple de tórax. Las manifestaciones clínicas son variables, incluyendo la sensación de ahogo, tos y sibilancias. La hemoptisis es un síntoma infrecuente, relacionándose su aparición, generalmente, con la existencia de bronquiectasias. Presentamos el caso de un niño que presentó hemoptisis de repetición y neumonitis local por Eikenella corrodens tres años después de la aspiración de una espiga. Se llegó al diagnóstico de la broncoaspiración tras la expulsión del cuerpo extraño, días después de la realización de un lavado broncoalveolar. Lo inusual de este caso es la presentación clínica de hemoptisis de repetición, el largo periodo asintomático tratándose de un cuerpo extraño de origen vegetal y la neumonitis por Eikenella Corrodens asociada a la existencia de un cuerpo extraño intrabronquial (AU)

ABSTRACT

Exogenous lipoid pneumonia is a rare disease which is often underdiagnosed. Exogenous lipoid pneumonia has traditionally been considered a chronic process secondary to continued aspiration or inhalation of fatty substances. In its acute form exogenous lipoid pneumonia sometimes coincides with accidental massive aspiration of lipidic material, as has been traditionally described for fire eaters. We review the literature and report a case of acute lipoid pneumonia secondary to accidental aspiration of vaseline used to place a nasogastric tube, which had been inserted for gastric lavage after a suicide attempt by flurazepam ingestion.

ABSTRACT

La neumonía lipoidea exógena es una entidad poco frecuente y a menudo infradiagnosticada. Clásicamente, se ha considerado un proceso crónico secundario a la aspiración o inhalación continuada de sustancias grasas. En ocasiones, se puede observar una forma aguda coincidiendo con aspiraciones accidentales masivas de material lipídico descrita tradicionalmente en "comedores de fuego"1,2. Revisamos la bibliografía y presentamos un caso de neumonía lipoidea aguda secundaria a la aspiración accidental de vaselina utilizada en la colocación de una sonda nasogástrica para un lavado gástrico, tras intento de autólisis mediante la ingesta de flurazepam. ABSTRACT

Listeriosis is a infectious disease which can affect to immunosuppressed patients. We report a case of pneumonia by Listeria monocytogenes as unusual presentation, because the central nervous system infections are the most common clinical entities. Many cases reported had a underlying disease, most commonly Hodgkin's disease, and had received immunosuppressive therapy. In our case the patient was considered to be immunosuppressed by had received corticosteroids and had been diagnosed of stage IIIa squamous cell carcinoma 3 years ago. The illness began 48 hours before the admission to hospital, he had fever, mental confusion and the location of pneumonia was right lower lobe, 24 hours later the patient die.

ABSTRACT

Listeriosis es una infección que puede presentarse en pacientes inmunodeprimidos. Presentamos un caso de neumonía por Listeria monocytogenes como forma poco habitual de presentación, siendo la afectación del sistema nervioso central la entidad clínica más frecuente. La mayoría de casos publicados presentan una enfermedad subyacente, siendo la más frecuente la enfermedad de Hodgking, y habían recibido tratamiento inmunosupresor. En el caso que nos ocupa el paciente se encontraba inmunodeprimido porque había recibido tratamiento con esteroides y había sido diagnosticado de carcinoma pulmonar epidermoide estadio III a 3 años antes. La enfermedad comenzó 48 horas antes de su ingreso en el hospital, presentando fiebre, cuadro confusional y la neumonía se localizaba en el lóbulo pulmonar inferior derecho, 24 horas después el paciente falleció (AU)

ABSTRACT

Foreign body aspiration requires emergency bronchoscopic examination when the product is chemically active. We describe the case of a female patient who aspirated a ferrous sulfate tablet that, in spite of being extracted immediately, caused lesions that persisted in the bronchial tree for 6 weeks. We then discuss the potential of ferrous sulfate substance to cause damage to the tracheobronchial tree.
