ABSTRACT

Osseointegrated dental implants represent a part of oral rehabilitation, being an increasingly used alternative in Dentistry in order to replace lost teeth. Similar to periodontal diseases, the etiological factor of peri-implant diseases is the accumulation of biofilm around dental implants. This pathology is also classified according to the tissues affected by it, in mucositis and peri-implantitis. For a correct treatment and success in peri-implant therapy, the diagnosis must be based on its etiology, and its treatment continues to vary according to each case and stage of the disease. The present work aims to report the treatment of a case of peri-implantitis through the decontamination of the implant surface through an access surgery. Caucasian female patient, 56 years old, sought care at the graduation course in Dentistry at Centro Universitário da Serra Gaúcha FSG, complaining of bleeding/suppuration, pain and edema in the region of teeth 15 and 16, rehabilitated with implants, and exposure of prosthetic components. The patient was diagnosed with peri-implantitis. The proposed treatment plan was to promote decontamination of the implant surface through surgical access. Based on the presented clinical case, it was possible to conclude that the treatment technique used was effective for the resolution of periimplantitis, in the follow-up period of the study (90 days), demonstrating improvement in clinical and radiographic parameters(AU)

ABSTRACT

This study evaluated peripheral intravenous catheter insertion by licensed practical nurses in a simulated scenario. This cross-sectional and descriptive study was performed in a general hospital's simulation center located in Brazil, from January to October 2020. The authors developed a tool from the leading guidelines to measure peripheral intravenous catheter insertion performance in 60 licensed practical nurses. Data were analyzed using descriptive statistics, with proportions for categorical variables and analytical statistics to denote significance, using SPSS version 22.0. Ethical aspects were respected. Despite the sample comprising professionals who declared an average experience with peripheral intravenous catheter insertion of 6.4 years, at least 50% failed in some tool element. This low score on peripheral intravenous catheter insertion performance is because automation is possible without efforts to improve it. Adding years of practice does not generate better performance. The licensed practical nurses' performance was lower than expected, considering their reported experience, which poses a risk to patient safety. The training of these professionals needs to be discussed in countries where the practice is legal. Skills training should include simulation to approximate clinical practice; nurses must exercise their role in supervising the team.

ABSTRACT

AIM: to compare the effect of rapid cycle deliberate practice simulation training with skill-training simulation on peripheral intravenous catheter insertion for Licensed Practical Nurses. BACKGROUND: The use of peripheral intravenous catheters is associated with high rates of complications, although it is widely used in clinical practice. Training strategies to ensure good performance can minimize the risks inherent to this procedure. DESIGN: A randomized simulation experimental pre-post interventional study. METHODS: Sixty participants were allocated to intervention (n = 30) or control (n = 30) groups. Participants allocated to the intervention group were trained through the Rapid cycle deliberate practice simulation strategy, while participants in the control group were trained through the skill-training simulation strategy. A pre-test was applied before any intervention and a post-test after intervention. The primary outcome was the performance in the peripheral intravenous catheter insertion skill. The comparison of correct performance in the tests was analyzed intergroup and intragroup. The effect size of the interventions was also analyzed. The t-Student and Mann-Whitney tests compared the difference between the groups. The training effect was calculated by Cohen's dm and Glass's Δ measures. RESULTS: Performance between the pre-post-test increased from 59.4% to 96% (p < 0.001) in the intervention group and from 57.8% to 93.5% in the control group (p < 0001). There was no statistical difference between the groups after intervention (p = 0225). Cohen's dm measurement was 2.95 and 3.59 in the control and intervention groups, respectively. CONCLUSIONS: The rapid cycle deliberate practice simulation strategy resulted in Licensed Practical Nurses' performance improvements in peripheral intravenous catheter insertion, evidenced by the increase of correct performance actions in the post-test compared to the pre-test. However, with no statistical difference compared to the skill-training simulation strategy.

ABSTRACT

BACKGROUND: Support surfaces variables, such as size, material, and density, can determine chest compression depth in cardiopulmonary resuscitation. OBJECTIVE: to analyze the force required to do a high-quality chest compression concerning different surfaces in CPR. METHOD: This experimental study was developed using a Little Anne manikin and a mechanical device to perform chest compressions. Nine sets of surfaces were tested and compared to a control. RESULTS: 230 experimental tests were done in sets of bed or stretcher + mattress and presence or absence of different backboards. In the control condition, the average force to reach 5 cm of depth was 42.14±0.97 (kgf). Set 9, compatible with a narrow stretcher with a thin mattress, had the best surfaces to reach recommended depth, with or without a backboard. All other sets required significantly more force for high-quality chest compression. Regression analysis confirms that backboard size is not significant for the force for high-quality chest compression. CONCLUSION: There is an association of dimensions and types of beds or stretchers and mattresses with a force increase. Type and dimensions of the backboard are not relevant for the force required, regardless of the characteristics of the set of the bed or stretcher and mattress.

ABSTRACT

Introduction: Simulation is a learning strategy that has been increasingly applied in nursing education. The use of this tool generates the need to use reliable and valid instruments to guide the activities developed. Objective: evaluate the satisfaction and self-confidence of nursing students during learning when they are submitted to praticed skills and simulated scenarios. Methodology: observational research with quantitative approach conducted on Skills and Simulation Laboratory of a University located in the city of Rio de Janeiro. Performed with nursing students (enrolled in the 3rd and 6th period) using the Student Satisfaction and Self-Confidence in Learning (SSSCL). The scale is similar a 5 points Likert Scale. It has 13 items and it is categorized in two dimensions: Satisfaction (05 items) and Self-confidence in learning (08 items). Results: 49 students were included. The research participant is typically female (89.8%), brown and black (57.1%), 21-23 years old (55.1%) and with no experience in simulation (93.9%). The analysis of each of the 13 points showed the effectiveness of the teaching method used (98%), with a positive impact on the way the teacher taught (97.9%) and was aligned with the way the student learns (95, 9), reinforcing that teacher performance is extremely relevant to satisfaction. Conclusion: this study achieved the goals and demonstrated that simulation can be a feasible option in nursing students learning for clinical practice. High levels in both dimensions were reach, being those of satisfaction more significant than those of self-confidence. The evaluation of these dimensions may not directly reflect learning results, but they are essential for the improvement of simulation as a teaching technique by institutions and teachers.

ABSTRACT

The aim of the present study was to evaluate the efficiency of visual ELISA-PAG for early pregnancy diagnosis based on the presence of PAG (pregnancy-associated glycoprotein) using sheep blood serum. Experiment 1: 140 ewes were divided into three groups with different stages of pregnancy. In the first group, 41 pregnant ewes were sampled at 26, 28, 30, and 32 days of pregnancy; in the second group, 65 ewes (49 pregnant with 30 days and 16 non-pregnant) were sampled; in the third group, 34 non-pregnant ewes were sampled. Experiment 2: 10 pregnant ewes were sampled weekly from day 35 of gestation until day 70 post-partum to verify the total period in which PAG can be detected in the blood serum by the test. Transrectal ultrasound was used as a gold standard. The detection or non-detection of PAG was analyzed by the logistic model PROC GENMOD of SAS; differences were detected by the chi-squared test. In group 1, there were no differences between the results from 28, 30, and 32 days of gestation, but samples from 30 days were easier to interpret in comparison to 28 days, with a sensitivity of 97.56%. In the second group, using 65 sheep, visual ELISA-PAG showed 100% sensitivity and 93.75% specificity, which indicates the diagnosis of an animal as a false positive. In the third group, 97.06% of the sheep were confirmed as negative and 2.94% as positive, again indicating the presence of a false positive. In 100% of the sheep, the PAG remained in the blood circulation throughout the antepartum period until birth and seven days post-partum, declining thereafter. Based on our results, the visual ELISA-PAG is an effective method for the early diagnosis of pregnancy in sheep and can be performed from day 30 of gestation.

ABSTRACT

The role of Escherichia coli in healthy microbiota of psittacine is controversial, and the presence of Salmonella sp. indicates possible disease. Therefore, this study aimed to identify the presence of E. coli and Salmonella spp. in a psittacine pet that died in Fortaleza, Brazil, correlating pathogenicity aspects of the isolates through the evaluation of lesions and antimicrobial susceptibility. Psittacine pets sent to the Laboratory of Ornithological Studies, State University of Ceará, that died in 2014 and 2015 were necropsied. Fragments of liver, kidneys, intestine, lung, heart, spleen and brain were collected for microbiological and histopathological analyses. Scores were attributed to lesions and isolated strains submitted to antimicrobial susceptibility test. From the seventy necropsied birds, nineteen were positive for E. coli and one for Salmonella Typhimurium. Congestive lesions and lymphoplasmocitic inflammatory infiltrate were observed varying from light to moderate and were the main findings. In the analyzed strains, multidrug resistance against different groups of antibiotics was observed. In conclusion, according to the results, E. coli strains and the Salmonella Typhimurium isolate produced significant lesions in the psittacine pets, and multidrug resistance may hinder treatments with antibiotics used in avian pet medicine.(AU)

ABSTRACT

The illegal wildlife trade is widespread among several countries, and Brazil is one of the most frequently involved due to its great territorial extension and large diversity of animal species. Birds are the main target, and among them the Psittaciformes order, which are removed indiscriminately from nature, is one of the most frequently traded due to their intelligence and great communicating skills. However, the conditions in which these birds are captured and maintained in captivity favor several diseases, among them the ones caused by some Enterobacteriaceae. In this family, the most frequently associated bacteria to illnesses and deaths in psittacine are Salmonella and Escherichia coli , which cause salmonellosis and colibacillosis, respectively. Therefore, this study presents a review of the literature concerning these enterobacteria in Psittaciformes, emphasizing the zoonotic potential of these bacteria.(AU)

ABSTRACT

The Enterobacteriaceae family contains potentially zoonotic bacteria, and their presence in canaries is often reported, though the current status of these in bird flocks is unknown. Therefore, this study aimed to identify the most common genera of enterobacteria from canaries (Serinus canaria) and their antimicrobial resistance profiles. From February to June of 2013, a total of 387 cloacal swab samples from eight domiciliary breeding locations of Fortaleza city, Brazil, were collected and 58 necropsies were performed in canaries, which belonged to the Laboratory of Ornithological Studies. The samples were submitted to microbiological procedure using buffered peptone water and MacConkey agar. Colonies were selected according to their morphological characteristics on selective agar and submitted for biochemical identification and antimicrobial susceptibility. A total of 61 isolates were obtained, of which 42 were from cloacal swabs and 19 from necropsies. The most isolated bacteria was Escherichia coli with twenty five strains, followed by fourteen Klebsiellaspp., twelve Enterobacterspp., seven Pantoea agglomerans, two Serratiaspp. and one Proteus mirabilis. The antimicrobial to which the strains presented most resistance was sulfonamides with 55.7%, followed by ampicillin with 54.1% and tetracycline with 39.3%. The total of multidrug-resistant bacteria (MDR) was 34 (55.7%). In conclusion, canaries harbor members of the Enterobacteriaceae family and common strains present a high antimicrobial resistance rate, with a high frequency of MDR bacteria.

ABSTRACT

BACKGROUND: Alloimmunisation is a major complication in patients with sickle cell disease (SCD) receiving red blood cell (RBC) transfusions and despite provision of Rh phenotyped RBC units, Rh antibodies still occur. These antibodies in patients positive for the corresponding Rh antigen are considered autoantibodies in many cases but variant RH alleles found in SCD patients can also contribute to Rh alloimmunisation. In this study, we characterised variant RH alleles in 31 SCD patients who made antibodies to Rh antigens despite antigen-positive status and evaluated the clinical significance of the antibodies produced. MATERIALS AND METHODS: RHD and RHCE BeadChip™ from BioArray Solutions and/or amplification and sequencing of exons were used to identify the RH variants. The serological features of all Rh antibodies in antigen-positive patients were analysed and the clinical significance of the antibodies was evaluated by retrospective analysis of the haemoglobin (Hb) levels before and after transfusion; the change from baseline pre-transfusion Hb and the percentage of HbS were also determined. RESULTS: We identified variant RH alleles in 31/48 (65%) of SCD patients with Rh antibodies. Molecular analyses revealed the presence of partial RHD alleles and variant RHCE alleles associated with altered C and e antigens. Five patients were compound heterozygotes for RHD and RHCE variants. Retrospective analysis showed that 42% of antibodies produced by the patients with RH variants were involved in delayed haemolytic transfusion reactions or decreased survival of transfused RBC. DISCUSSION: In this study, we found that Rh antibodies in SCD patients with RH variants can be clinically significant and, therefore, matching patients based on RH variants should be considered.

ABSTRACT

Studies have shown that children and adolescents with low back pain are also similarly affected when they reach adulthood, thus highlighting the importance of investigating causes of low back pain in school-age children. The study examined low back pain and associated factors in 770 schoolchildren 7 to 17 years of age in a private school in Porto Alegre, Rio Grande do Sul State, Brazil. Low back pain was defined as pain or discomfort in the lumbar region in the previous month, assessed by a questionnaire. Low back pain was found in 31.6% of the subjects and was more prevalent in girls (41.9%) than boys (21.4%). Factors associated with lumbar pain were female gender, age 9 to 17 years, hyperactivity (borderline and abnormal categories), and emotional symptoms (abnormal category). Mapping the occurrence of low back pain and associated factors is important for identifying children and adolescents at risk and for developing effective programs for primary prevention.

ABSTRACT

This study is a report of an experience of a training period of the Specialist Course in Family Therapy in DOMUS – Center Therapy of Couple and Family. In courses like that, the change of therapeutics is a usual and necessary practice. The aim of this study was to associate the change of therapeutics with the repercussion around the process. Two procedures with the same family were done: first, planning and adaption to a new therapeutics, and second, without such management. The results of this study show that the interruption of bonds between the previous therapists and the family triggered strong feelings in all. It is concluded that there is a possibility of absorption of the change by the family due to the therapist's personal characteristics, including establishing a bonding quality.(AU)

ABSTRACT

The goals of this study was a) to verify the possible relations between the sit and reach test with chair (SR) and without chair (VSR) in children and adolescents, and b) to porpoise a regression equation to predict the SR from VSR. The sample was composed for 698 children and adolescents (357 boys and 341 girls) between 7 to 17 years old from the PROESP-BR dates? bank. The dates analyze was made with the Pearson correlation and with multiple linear regression. The results suggest a positive and high (r=0,885) correlation between SR and VSR. When the sex and age group (AG1 - 7 to 10 years old; do AG2 - 11 to 13 years old; and AG3 - 14 to 17 years old) variables are consider, correlation coefficients get lower at male sex and at AG1 for both sex, and get high AG2 and AG3 for both sex. Before this, we chose to add the sex and AG variable in SR test model estimate from VSR. The regression analyze results released a equation with 78,8% determination capacity. Therefore, considering the relation degree between flexibility tests, we can predict one using the results of the other. Thus, the VSR can be used as an alternative instrument for the children and adolescents flexibility measured.

ABSTRACT

ABSTRACT

An attempt was made to evaluate microbiological quality of horticultural crops grown organically. Three species of vegetables were used, lettuce (Lactuva sativa), radish (Raphanus sativus) and spinach (Tetragonia expansa), grown organically, in fertile soil. Six different treatments were applied: mineral fertilizer, chicken, cow, and pig manure, chicken litter and cow manure, in association with a liquid foliar biofertilizer. These crops were handled according to correct agronomic practices for growing crops organically for commercial purposes. Samples were examined for the Most Probable Number (MPN/g/mL) of total and fecal coliforms and to detect the presence of Escherichia coli and Salmonella spp. All analyzed samples were considered acceptable for consumption, as Salmonella spp. was not detected. However, 63.3 percent, 50.0 percent, and 23.3 percent of the samples of lettuce, radish and spinach, respectively, contained >10² total coliforms/g of product. None of the samples of spinach or radish presented >10² fecal coliforms/g, and only 6.6 percent of lettuce samples contained >10² fecal coliforms/g. The presence of E. coli was confirmed in one sample of spinach, cultivated with cow manure.
