ABSTRACT

Drug resistance in infectious diseases developed by bacteria and fungi is an important issue since it is necessary to further develop novel compounds with biological activity that counteract this problem. In addition, new pharmaceutical compounds with lower secondary effects to treat cancer are needed. Coordination compounds appear to be accessible and promising alternatives aiming to overcome these problems. In this review, we summarize the recent literature on coordination compounds based on nitrobenzoic acid (NBA) as a ligand, its derivatives, and other nitro-containing ligands, which are widely employed owing to their versatility. Additionally, an analysis of crystallographic data is presented, unraveling the coordination preferences and the most effective crystallization methods to grow crystals of good quality. This underscores the significance of elucidating crystalline structures and utilizing computational calculations to deepen the comprehension of the electronic properties of coordination complexes.

ABSTRACT

The title compound, C8H7NO2, crystallizes as prismatic colourless crystals in space group P , with one mol-ecule in the asymmetric unit. The pyridine ring is fused to acrylic acid, forming an almost planar structure with an E-configuration about the double bond with a torsion angle of -6.1 (2)°. In the crystal, strong O-H⋯N inter-actions link the mol-ecules, forming chains along the [101] direction. Weak C-H⋯O inter-actions link adjacent chains along the [100] direction, generating an R 2 2(14) homosynthon. Finally, π-π stacking inter-actions lead to the formation of the three-dimensional structure. The supra-molecular analysis was supported by Hirshfeld surface and two-dimensional fingerprint plot analysis, indicating that the most abundant contacts are associated with H⋯H, O⋯H/H⋯O, N⋯H/H⋯N and C⋯H/H⋯C inter-actions.

ABSTRACT

Cobalt(III) and chromium(III) salophen chloride complexes were synthesized and tested for the cycloaddition of carbon dioxide (CO2) with epoxides to obtain cyclic carbonates. The cat1, cat2, cat4, and cat5 complexes presented high catalytic activity without cocatalysts and are solvent-free at 100 °C, 8 bar, and 9 h. At these conditions, the terminal epoxides (1a-1k) were successfully converted into the corresponding cyclic carbonates with a maximum conversion of ∼99%. Moreover, cat5 was highlighted due to its capability of opening internal epoxides such as limonene oxide (1l) with a 36% conversion to limonene carbonate (2l), and from cyclohexene oxide (1m), cyclic trans-cyclohexene carbonate (2m) and poly(cyclohexene carbonate) were obtained with 15% and 85% selectivity, respectively. A study of the coupling reaction mechanism was proposed with the aid of electrospray ionization mass spectrometry (ESI-MS) analysis, confirming the single-component behavior of the complexes through their ionization due to epoxide coordination. In addition, crystallographic analysis of cat1 single crystals grown in a saturated solution of pyridine helped to demonstrate that the substitution of chloride ion by pyridine ligands to form an octahedral coordination occurs (Py-cat1), supporting the proposed mechanism. Also, a recyclability study was performed for cat5, and a total turnover number of 952 was obtained with only minor losses in catalytic activity after five cycles.

ABSTRACT

Antimicrobial resistance is a global public health threat. Metallo-ß-lactamases (MBLs) inactivate ß-lactam antibiotics, including carbapenems, are disseminating among Gram-negative bacteria, and lack clinically useful inhibitors. The evolving bisthiazolidine (BTZ) scaffold inhibits all three MBL subclasses (B1-B3). We report design, synthesis, and evaluation of BTZ analogues. Structure-activity relationships identified the BTZ thiol as essential, while carboxylate is replaceable, with its removal enhancing potency by facilitating hydrophobic interactions within the MBL active site. While the introduction of a flexible aromatic ring is neutral or detrimental for inhibition, a rigid (fused) ring generated nM benzobisheterocycle (BBH) inhibitors that potentiated carbapenems against MBL-producing strains. Crystallography of BBH:MBL complexes identified hydrophobic interactions as the basis of potency toward B1 MBLs. These data underscore BTZs as versatile, potent broad-spectrum MBL inhibitors (with activity extending to enzymes refractory to other inhibitors) and provide a rational approach to further improve the tricyclic BBH scaffold.

Resumen OBJETIVO: Identificar los principales hallazgos histopatológicos benignos y determinar la tasa de falsos positivos que suelen causar conflicto al categorizar las mastografías en el sistema BI-RADS por su aspecto, que puede simular un proceso maligno. MATERIALES Y MÉTODOS: Estudio de cohorte, retrospectivo, efectuado en pacientes atendidas en la Unidad Médica de Alta Especialidad 4 Luis Castelazo Ayala (2019-2023) con reporte mastográfico alterado o sospecha clínica de malignidad. Para el análisis estadístico se utilizó el programa JASP 2.0 y χ2 para la diferencia de proporciones entre grupos. RESULTADOS: De un grupo de 11,481 pacientes, se reportaron 1643 mastografías alteradas: 444 con reportes falsos positivos, 23 pacientes con sospecha clínica y exclusión de 16 que no cumplieron con los criterios de inclusión establecidos. La muestra poblacional estudiada fue de 451 pacientes. La mayoría permaneció asintomática al momento del estudio (42.1%). El hallazgo histopatológico benigno con mayor prevalencia fue el fibroadenoma y su síntoma más relevante el nódulo palpable. La tasa de falsos positivos fue de 4.3%. CONCLUSIONES: En la actualidad, gracias a la implementación de programas de tamizaje es posible establecer diagnósticos de cáncer de mama en etapas tempranas, aunque con la desventaja que el reporte puede resultar falso positivo y ello dar lugar a incremento de la morbilidad y sobretratamiento. Los estándares internacionales indican que estos no deben sobrepasar el 10%.


Abstract OBJECTIVE: To identify the main benign histopathological findings that often cause conflict when categorizing mastographies in the BI-RADS system due to their appearance, which may simulate a malignant process and false positive rate. MATERIALS AND METHODS: Retrospective cohort study carried out in patients attended at the Unidad Médica de Alta Especialidad 4 Luis Castelazo Ayala (2019-2023) with an altered mastographic report or clinical suspicion of malignancy. For statistical analysis we used the JASP 2.0 programme and χ2 for the difference in proportions between groups. RESULTS: From a group of 11,481 patients, 1,643 altered mastograms were reported: 444 with false positive reports, 23 patients with clinical suspicion and exclusion of 16 who did not meet the established inclusion criteria. The population sample studied was 451 patients. The majority remained asymptomatic at the time of the study (42.1%). The most prevalent benign histopathological finding was fibroadenoma and the most relevant symptom was a palpable nodule. The false positive rate was 4.3%. CONCLUSIONS: Currently, thanks to the implementation of screening programmes it is possible to establish breast cancer diagnoses in early stages, although with the disadvantage that the report may be false positive and this may lead to increased morbidity and overtreatment. International standards indicate that these should not exceed 10%.

ABSTRACT

The discharge of synthetic dyes from different industrial sources has become a global issue of concern. Enormous amounts are released into wastewater each year, causing concerns due to the high toxic consequences. Photocatalytic semiconductors appear as a green and sustainable form of remediation. Among them, graphitic carbon nitride (g-C3N4) has been widely studied due to its low cost and ease of fabrication. In this work, the synthesis, characterization, and photocatalytic study over methylene blue of undoped, B/S-doped, and exfoliated heterojunctions of g-C3N4 are presented. The evaluation of the photocatalytic performance showed that exfoliated undoped/S-doped heterojunctions with 25, 50, and 75 mass % of S-doped (g-C3N4) present enhanced activity with an apparent reaction rate constant (kapp) of 1.92 × 10-2 min-1 for the 75% sample. These results are supported by photoluminescence (PL) experiments showing that this heterojunction presents the less probable electron-hole recombination. UV-vis diffuse reflectance and valence band-X-ray photoelectron spectroscopy (VB-XPS) allowed the calculation of the band-gap and the valence band positions, suggesting a band structure diagram describing a type I heterojunction. The photocatalytic activities calculated demonstrate that this property is related to the surface area and porosity of the samples, the semiconductor nature of the g-C3N4 structure, and, in this case, the heterojunction that modifies the band structure. These results are of great importance considering that scarce reports are found concerning exfoliated B/S-doped heterojunctions.

ABSTRACT

The new title pyrrole-pyrazole derivative, C16H16BrN3, was synthesized through a citric acid-catalyzed Paal-Knorr reaction between acetonylacetone and 1-(4-bromophenyl)-3-methyl-1H-pyrazol-5-amine under mild reaction conditions. This synthetic protocol is noteworthy for its utilization of stoichiometric amounts of the reactants, an ecofriendly solvent and a cost-effective, non-toxic and biodegradable organocatalyst. A comprehensive understanding of the molecular structure was gained through spectroscopic, thermal and X-ray crystallographic analyses. The crystal structure is characterized by weak interactions, where only C-H...π connections contribute to the hydrogen-bond contacts. The supramolecular assembly is controlled by dispersion forces. However, the energy frameworks demonstrate that these forces act in three dimensions, providing enough stability, as observed in TGA-DSC (thermogravimetric analysis-differential scanning calorimetry) studies.

ABSTRACT

Reversan, a multidrug resistance-associated protein (MRP1) inhibitor described more than a decade ago, is a commercial drug (CAS: 313397-13-6) that has a high price and is six to eight times more potent than known drug transporter inhibitors. However, to date, a complete route for synthesizing pyrazolo[1,5-a]pyrimidine-based Reversan is yet to be published. Herein, the silica gel-mediated synthesis of Reversan and a novel family of its structural analogues (amides) via the microwave-assisted amidation reaction of 3-carboethoxy-5,7-diphenylpyrazolo[1,5-a]pyrimidine (ester) with primary amines is reported. Moreover, a set of this ester-type precursor was obtained using the NaF/alumina-mediated reaction of 5-amino-3-carboethoxy-1H-pyrazole with chalcones, implying a final removal of H2 using Na2S2O8. Both esters and amides were obtained in high yields using heterogeneous catalyst and solvent-free, highly efficient, and scalable synthetic protocols.

OBJECTIVE: To analyze, in a cohort of acromegalic patients, the results of the efficiency and safety of radiosurgery (CyberKnife), as well as the prognostic factors associated with disease remission. MATERIAL AND METHODS: Observational, retrospective, longitudinal, and analytical study that included acromegalic patients with persistent biochemical activity after initial medical-surgical treatment, who received treatment with CyberKnife radiosurgery. GH and IGF-1 levels at baseline after one year and at the end of follow-up were evaluated. RESULTS: 57 patients were included, with a median follow-up of four years (IQR, 2-7.2 years). The biochemical remission rate was 45.6%, 33.33% achieved biochemical control, and 12.28% attained biochemical cure at the end of follow-up. A progressive and statistically significant decrease was observed in the comparison of the concentrations of IGF-1, IFG-1 x ULN, and baseline GH at one year and at the end of follow-up. Both cavernous sinus invasion and elevated baseline IGF-1 x ULN concentrations were associated with an increased risk of biochemical non-remission. CONCLUSION: Radiosurgery (CyberKnife) is a safe and effective technique in the adjuvant treatment of GH-producing tumors. Elevated levels of IGF x ULN before radiosurgery and invasion of the cavernous sinus by the tumor could be predictors of biochemical non-remission of acromegaly.

Resumen OBJETIVO: Determinar la prevalencia de depresión posparto en una muestra de población mexicana por medio de la Escala de Edimburgo y los factores de riesgo asociados con su inicio MATERIALES Y MÉTODOS: Estudio observacional, transversal, relacional y analítico efectuado en una muestra de población mexicana atendida entre los meses de marzo a julio del 2022 en cuatro hospitales de segundo y tercer nivel de cuatro entidades de la República Mexicana. Se aplicó la escala de Edimburgo a pacientes en el puerperio. Los datos obtenidos se procesaron con una técnica de regresión logística binaria ajustada para identificar los factores de riesgo más importantes de depresión posparto. RESULTADOS: De una muestra de 717 pacientes a quienes se aplicó la Escala de Edimburgo, 106 resultaron positivas a depresión posparto, lo que da una prevalencia del 14.9%. La edad promedio de las pacientes fue de 26 años (límites de 12 y 46). El estado civil soltera resultó un factor protector de depresión posparto y, en su contraparte, quienes estaban casadas tuvieron cierta predisposición a la depresión posparto. A mayor grado de escolaridad menor predisposición a la depresión posparto. CONCLUSIONES: Los principales factores de riesgo de depresión posparto fueron: el antecedente de trastornos psiquiátricos en la familia, depresión previa, dificultades económicas y ser soltera. Las pacientes pueden cursar con diversos factores de riesgo simultáneos, circunstancia que potencia el riesgo de depresión. Es primordial que el obstetra identifique los factores de riesgo desde el control prenatal, a fin de prevenir que el estado depresivo se agudice durante el puerperio.


Abstract OBJECTIVE: To determine the prevalence of postpartum depression in a Mexican population sample by means of the Edinburgh Scale and the risk factors associated with its onset. MATERIALS AND METHODS: Observational, cross-sectional, relational and analytical study carried out in a sample of Mexican population attended from March to July 2022 in four second and third level hospitals in four Mexican states. The Edinburgh scale was applied to postpartum patients. The data obtained were processed with a binary logistic regression technique adjusted to identify the most important risk factors for postpartum depression. RESULTS: From a sample of 717 patients to whom the Edinburgh Scale was applied, 106 were positive for postpartum depression, giving a prevalence of 14.9%. The average age of the patients was 26 years (range 12 and 46). Unmarried marital status was a protective factor for postpartum depression and, on the other hand, those who were married had a certain predisposition to postpartum depression. The higher the level of schooling the lower the predisposition to postpartum depression. CONCLUSIONS: The main risk factors for postpartum depression were: history of psychiatric disorders in the family, previous depression, economic difficulties and being single. Patients may have several simultaneous risk factors, a circumstance that increases the risk of depression. It is essential for the obstetrician to identify the risk factors from the prenatal check-up, to prevent the depressive state from worsening during the puerperium.

BACKGROUND: Pregnancy-induced hypertension (PIH) has been related to impaired fetal growth, possibly by affecting hematopoiesis. This study aimed to analyze the most frequent hematological alterations in preterm infants born to mothers with PIH. METHODS: We conducted a cross-sectional study in newborns born to mothers with PIH. We reviewed 130 hemograms of preterm infants: 45 from mothers with PIH, 71 with preeclampsia, and 14 with HELLP syndrome (hemolysis, elevated liver enzymes, and low platelet count). Normality, cytosis conditions, or cytopenia values were adjusted for gestational ages. Differences between groups were analyzed with classical and Bayesian statistics (BF01 = null/alternative hypothesis ratio). RESULTS: Anemia was found in only 1.2% of newborns. In the white blood cell count, the most frequent finding was lymphopenia (56.2%) and monocytosis (38.5%) (p = 0.6, FB01 = 249 y p = 0.81, FB01 = 19.9). Thrombocytopenia was found in 12.5% (p = 0.56, FB01 = 67). No significant differences were observed among PIH groups. CONCLUSIONS: Hematological alterations of newborns born to mothers with PIH are frequent and do not show a distinct pattern related to the severity of the affection in the mother. We recommend a full hematological evaluation in these preterm neonates.

El objetivo de este estudio fue analizar las alteraciones hematológicas más frecuentes en los recién nacidos prematuros hijos de madres con EHE. MÉTODOS: Se llevó a cabo un estudio transversal en recién nacidos de madres con EHE. Se revisaron los hemogramas de 130 neonatos prematuros: 45 madres con hipertensión gestacional, 71 con pre-eclampsia y 14 con síndrome de HELLP (hemólisis, enzimas hepáticas elevadas y bajo recuento de plaquetas). Las cifras de normalidad, condiciones de citosis o citopenia fueron ajustadas a las edades gestacionales. Las diferencias entre los grupos se analizaron con estadística clásica y bayesiana (FB01= relación hipótesis nula/alterna). RESULTADOS: Se encontró anemia en solo el 1.2% de los recién nacidos. En la serie blanca el hallazgo más frecuente fue la linfopenia (56.2%) y monocitosis (38.5%) (p = 0.6, FB01 = 249 y p = 0.81, FB01 = 19.9). La plaquetopenia se encontró en el 12.5% (p = 0.56, FB01 = 67). No se observaron diferencias significativas entre los grupos de EHE. CONCLUSIONES: Las alteraciones hematológicas en recién nacidos de madres con EHE son frecuentes sin mostrar un patrón distinto con relación a la gravedad del padecimiento de la madre. Aun así, es recomendable la valoración hematológica en estos neonatos.

ABSTRACT

Structural analysis and docking studies of three adamantane-linked 1,2,4-triazole N-Mannich bases (1-3) are presented. Compounds 1, 2 and 3 crystallized in the monoclinic P21/c, P21 and P21/n space groups, respectively. Crystal packing of 1 was stabilized by intermolecular C-H⋯O interactions, whereas compounds 2 and 3 were stabilized through intermolecular C-H⋯N, C-H⋯S and C-H⋯π interactions. The energy frameworks for crystal structures of 1-3 were described. The substituent effect on the intermolecular interactions and their contributions were described on the basis of Hirshfeld surface analyses. The 11ß-hydroxysteroid dehydrogenase type 1 (11ß-HSD1) inhibition potential, pharmacokinetic and toxicity profiles of compounds 1-3 were determined using in silico techniques. Molecular docking of the compounds into the 11ß-HSD1 active site showed comparable binding affinity scores (-7.50 to -8.92 kcal/mol) to the 11ß-HSD1 co-crystallized ligand 4YQ (-8.48 kcal/mol, 11ß-HSD1 IC50 = 9.9 nM). The compounds interacted with key active site residues, namely Ser170 and Tyr183, via strong hydrogen bond interactions. The predicted pharmacokinetic and toxicity profiles of the compounds were assessed, and were found to exhibit excellent ADMET potential.

Background: Episodic angina-like retrosternal pain is a prevalent symptom for achalasia patients pre- and post-treatment. The cause of postoperative chest pain remains poorly understood. Moreover, there are no reports on their predictive value for chest pain in the long-term post-treatment. The effect of laparoscopic Heller myotomy (LHM) and fundoplication techniques (Dor vs. Toupet) is unclear. Methods: We analyzed a cohort of 129 achalasia cases treated with LHM and randomly assigned fundoplication technique. All the patients were diagnosed with achalasia by high-resolution manometry (HRM). Patients were followed up at 1-, 6-, 12-, and 24-month post-treatment. We implemented unadjusted and adjusted logistic regression analyses to evaluate the predictive significance of pre- and post-operative clinical factors. Results: Preoperative chest pain with every meal was associated with an increased risk of occasional postoperative chest pain [unadjusted model: odds ratio (OR) = 12, 95% CI: 2.2-63.9, P = 0.006; adjusted model: OR = 26, 95% CI: 2.6-259.1, P = 0.005]. In type II achalasia, hypercontraction was also associated with an increased risk of chest pain (unadjusted model: OR = 2.6 e9 in all the patients). No significant differences were associated with age, type of achalasia, dysphagia, esophageal shape, and integrated relaxation pressure (IRP) with an increased risk of occasional postoperative chest pain. Also, there was no significant difference between fundoplication techniques or surgical approaches (e.g., length of myotomy). Conclusion: Preoperative chest pain with every meal was associated with a higher risk of occasionally postoperative chest pain.

ABSTRACT

The presence of cardio-metabolic and respiratory comorbidities, immunosuppression, and chronic kidney disease have been associated with an increase in mortality from COVID-19. The objective of this study is to establish the risk factors associated with 30-day mortality in a cohort of hospitalized patients with COVID-19. This paper conducts a retrospective and analytical study of patients hospitalized for COVID-19 in a tertiary care center. A Cox proportional hazard analysis was performed to estimate the association of comorbidities with 30-day mortality. A total of 1215 patients with a median age of 59 years were included. In the adjusted Cox proportional hazards regression model, hypothyroidism, D-dimer ≥ 0.8 µg/mL, LHD ≥ 430 IU/L, CRP ≥ 4.83 ng/mL, and triglycerides ≥ 214 mg/dL were associated with an increased risk of death. The presence of a history of hypothyroidism and biomarkers (D-dimer, lactic dehydrogenase, CRP, and triglycerides) were associated with an increase in mortality in the studied cohort.

BACKGROUND: Fetal growth restriction (FGR) may be related to ethnicity. Additionally, ethnic groups experience adverse socioeconomic circumstances that increase FGR risk. However, the dearth of evidence of the interaction between socioeconomic factors and FGR highlights the need for additional research. OBJECTIVE: To analyze the association between socioeconomic factors and FGR in Maya and non-Maya populations in Yucatan, Mexico. METHODS: A total of 21,320 singleton births in 2017 in Yucatan were analyzed. The student's t-test and the chi-square test were used to compare the means and proportions of maternal and perinatal variables between the FGR group and the birthweight appropriate for gestational age (AGA) group. Path analysis was performed to identify the direct and indirect effects of socioeconomic factors on FGR and mediators between predictors and FGR. RESULTS: The prevalence of FGR at birth was 9.06%; this rate was higher in the Maya population (12.4, 95% CI 11.3-13.5), without differences between socioeconomic levels. Path analysis revealed sociostructural variables (ethnicity and poverty) are reliable predictors of FGR at birth mediated by maternal education (ß = -.152, p < .001) and teenage pregnancy (ß = .065, p = .037). The proposal path model had a good fit index CFI = .968, TLI = .920, RMSEA = .046. CONCLUSION: The prevalence of FGR was higher among Maya women than non-Maya women The socioeconomic conditions associated with FGR at birth were ethnicity, poverty, maternal education, and teenage pregnancy. Maternal education and teenage pregnancy act as mediators between sociostructural variables and FGR at birth.

ABSTRACT

Prognostic studies are investigations to estimate the risk or probability of future outcomes in people with established disease. Ideally based on cohort studies where it is established: the time of onset or analysis of the evolution of a disease, the follow-up period for the outcome of interest, the type of outcome and the prognostic factors or markers to investigate. Ultimately, the decision on how to run and analyze forecast studies will depend on their purpose: exploratory, explanatory, or predictive.


Los estudios pronósticos son investigaciones para estimar el riesgo o probabilidad de desenlaces futuros en las personas con una enfermedad establecida. Idealmente deben sustentarse en estudios de cohortes en los que se establece el momento de inicio o de análisis sobre la evolución de una enfermedad, el periodo de seguimiento al desenlace de interés, el tipo de desenlace y los factores o marcadores pronósticos por investigar. Finalmente, la decisión de la forma de ejecutar y analizar los estudios de pronóstico dependerá de su propósito: exploratorio, explicativo o predictivo.

ABSTRACT

INTRODUCTION: Goiter is very common in patients with acromegaly; its development is correlated to the duration of the disease. Thyroid cells express the IGF-1 receptor and the TSH/IGF-1 interaction has been demonstrated to have a synergistic effect in thyroid cell growth. There is a correlation between IGF-1 levels and the thyroid volume of patients with acromegaly. The aim of this study was to evaluate, in a retrospective case-cohort study of patients with acromegaly, the associated risk factors for thyroid nodules disease in this population. METHODS: This was a case-cohort study matched by age, gender, and growth hormone at diagnosis. Cases consisted of acromegalic patients that developed thyroid nodules during the follow up, and controls consisted in acromegalic patients without thyroid nodules. A Cox proportional hazard estimation was carried out for measure the associated risk factors for thyroid nodules disease in acromegalic patients. A nodular thyroid disease-free survival analysis was estimated using the Kaplan-Meier analysis. RESULTS: We recruited 49 cases and 56 controls. In a multivariate Cox proportional hazard analysis age and IGF-1 ≥ 2.2 x ULN were significantly related with the presence of thyroid nodules [HR of 2.21 (95% CI; 1.15-4.25, p = 0.01)]. Nodularity-free survival rates in patients who had an IGF-1 X ULN ≥ 2.2 was found to be lower in comparison to those who had IGF-1 X ULN < 2.2, according to a Kaplan-Meier survival analysis. CONCLUSIONS: Our findings support that exist more probability to develop thyroid nodular disease in patients with acromegaly that present IGF-1 X ULN ≥ 2.2, suggesting a possible direct effect between the time of exposure to the IGF-1 axis hyperactivity and the genesis of thyroid nodules.

ABSTRACT

Informing in the studies about the effect size of an intervention or the impact of the factor(s) about an outcome, allows better decision-making for the application of the results in clinical practice. This article presents different methods to analyze the effect size, which can be through direct or indirect statistical methods. Within the direct methods, there's the difference in means between groups and the difference of absolute or relative frequencies. Among the indirect methods, Cohen's "d" family (which are based on standard deviation values), the "r and R2" family, measures of association (e.g. OR, RR, HR), and impact measures (e.g. NNT) are shown. The decision to use any of these methods depends on the objectives of the study and the measuring scale that is used to assess the results, as well as the data distribution. In order to enhance the understanding of the methods described in this article, examples are included, and the need to include level of precision (e.g. confidence intervals) is highlighted, along with the clinical decision thresholds for a better interpretation.


Informar en los estudios sobre el tamaño del efecto de una intervención o del impacto de factor(es) sobre un desenlace, permite tomar mejores decisiones para la aplicación de los resultados a la práctica clínica. En este artículo se presenta la manera de analizar el tamaño del efecto, lo cual puede ser mediante métodos estadísticos directos o indirectos. Dentro de los métodos directos, se encuentra la diferencia de promedios entre grupos y la diferencia de frecuencias absolutas o relativas. Dentro de los métodos indirectos se muestran los índices de la familia de "d" de Cohen (que se basan en valores de desviación estándar), la familia de "r y R2", medidas de asociación (RM, RR, HR) e impacto (NNT). La decisión del uso de cualquiera de los métodos descritos, depende de los objetivos del estudio, la escala de medición usada para evaluar los resultados y la distribución de los datos. Para facilitar la comprensión, se incluyen ejemplos y se resalta la necesidad de incluir los diferentes estadísticos con su nivel de precisión (ej. intervalos de confianza), junto con los umbrales clínicos de decisión, a fin de mejorar su interpretación.
