ABSTRACT

In 27 beagle dogs, acute necrotizing pancreatitis was induced by retrograde injection of autologous bile and trypsin into the main pancreatic duct. Animals were randomly assigned to the following treatments: group 1--(9 dogs) aprotinin 600,000 KIU/d by i.v. route; group 2--(9 dogs) peritoneal dialysis for 6 d plus 500,000 KIU/L of aprotinin in the dialysate fluid; group 3--(9 dogs) peritoneal dialysis without aprotinin in the dialysate fluid. All dogs of the group 1 died within 16 h following the induction of pancreatitis and extensive necrotizing and hemorrhagic changes were seen in the pancreatic and peripancreatic areas. Six dogs of the group 2 survived and no necrotizing changes were observed 30 or 50 d after the induction of pancreatitis. Three dogs of the group 3 survived, but slight necrotizing lesions were found at the autopsy. The survival rate was higher in dogs with peritoneal lavage (p = 0.0129) or with peritoneal lavage plus aprotinin (p less than 0.0001) than in those receiving i.v. aprotinin, indicating that the latter treatment has no beneficiary effect on the course of acute pancreatitis.

ABSTRACT

The Authors report on the data obtained and their evaluation by means of analysis of seven variables and four indices, in 49 cirrhotic patients with portal hypertension and in 10 control patients. Haemodynamic analysis offers the possibility of identifying the dynamic transition stages of a response to stress showing varying degrees of compensation and establishing categories of patients on the basis of cardiovascular and metabolic parameters in relation to liver damage. The Authors, according to Siegel studies, retain that this is a valid method which enables us to obtain useful information on the haemodynamic status of the cirrhotic patient with portal hypertension.

ABSTRACT

Thirty-six patients with neoplastic disease suffering from chronic bilateral pain were subjected to bilateral percutaneous cervical cordotomy. The technique and precautions to be taken in bilateral percutaneous cervical cordotomy performed either in one or two stages are described using a traditional or Levin's thermocouple-monitored electrode. The sequelae, complications and immediate and long-term results are reported.

ABSTRACT

In two minipigs chronic pancreatic and duodenal fistulas, which allowed the diversion and the intestinal replacement of pancreatic secretion, were prepared. In a third minipig a chronic biliary fistula was also prepared so the bile secretion could be fed back into the intestine through a duodenal catheter. In these animals a chronic gastric fistula was made so gastric secretion could also be collected. Diversion-replacement of pure pancreatic juice and of bile were carried out in the fasting state. This study confirms the presence of a feedback regulatory mechanism in exocrine pancreatic secretion in the pig. Moreover, it suggests that bile can interfere with this phenomenon.

ABSTRACT

An improvement in Corring's experimental model in the minipig is described. The model requires: -- a complete diversion of the biliopancreatic secretion and the possibility of its reintroduction into the duodenum; -- a particular anaesthesiological, technical and management problems taken up during the course of the research. Data are also given about basal pancreatic and biliary secretions in this particular experimental model.

ABSTRACT

The authors describe one case of relapsing postoperative atelectasis occurring with considerable severity after an upper left lobectomy for malignancy in a man of 68. Said complication recurred no fewer than six times during a hospital stay of 44 days and constituted a major threat to the good outcome of surgery. The patient was finally discharged in good conditions, and a later recheck revealed no important alterations in the surviving lower left lobe.
