ABSTRACT

INTRODUCTION: More than half of all fatalities on U.S. highways occur due to roadway departure (RwD) each year. Previous research has explored various risk factors that contribute to RwD crashes, however, a comprehensive investigation considering the effect of lighting conditions has been insufficiently addressed. DATA: Using the Louisiana Department of Transportation and Development crash database, fatal and injury RwD crashes occurring on rural two-lane (R2L) highways between 2008-2017 were analyzed based on daylight and dark (with and without streetlight). METHOD: This research employed a safe system approach to explore meaningful complex interactions among multidimensional crash risk factors. To accomplish this, an unsupervised data mining algorithm association rules mining (ARM) was utilized. RESULTS AND CONCLUSIONS: Based on the generated rules, the findings reveal several interesting crash patterns in the daylight, dark-with-streetlight, and dark-no-streetlight, emphasizing the importance of investigating RwD crash patterns depending on the lighting conditions. In daylight condition, fatal RwD crashes are associated with cloudy weather conditions, distracted drivers, standing water on the roadway, no seat belt use, and construction zones. In dark lighting condition (with and without streetlight), the majority of the RwD crashes are associated with alcohol/drug involvement, young drivers (15-24 years), driver condition (e.g., inattentive, distracted, illness/fatigued/asleep), and colliding with animal(s). PRACTICAL APPLICATIONS: The findings also reveal how certain driver behavior patterns are connected to RwD crashes, such as a strong association between alcohol/drug intoxication and no seat belt usage in the dark-no-streetlight condition. Based on the identified crash patterns and behavioral characteristics under different lighting conditions, the findings could aid researchers and safety specialists in developing the most effective RwD crash mitigation strategies.

ABSTRACT

BACKGROUND: Immunization acts as a key intervention to reduce under-five mortality and morbidity. Despite global progress on vaccination, difficulties in the utilization of this service in developing countries have been observed. According to Nepal Demographic and Health Survey (NDHS) 2016, only 78% of children received a complete dose of vaccine among which the first-dose receiver of DPT is 98%, whereas only 83% received a third dose. This study aims to explore the influencing factors of DPT vaccination dropout in Nepal. METHODS: The explorative study was done through secondary data analysis of NDHS 2016. The KR file was used for the analysis of information for 2883 children. Factors influencing dropout of DPT vaccination were explored against the independent variables such as external environment, predisposing factors, and enabling resources. All the analyses were weighted before the analysis. The descriptive, bivariate, and multivariate analyses were performed. The variables showing collinearity have been removed in the final model. RESULTS: A higher dropout was reported in Terai (18.9%) and province 2 (22.0%), among uneducated mothers (18.1%) and uneducated fathers (19.4%), less than once a week internet users (22.2%), the nonradio listener (17.4%), who had <4 ANC visits (22.7%), home delivery (19.2%), no advised SBA (19.1%), long distance to health facility (17.9%), no iron supplementation in pregnancy (24.3%), and PNC by TBA/others (21.1%). All these tested relationships were found statistically significant (P value <0.05). The aOR for dropout was found to be 7.94 (4.07-15.51) for mothers with less than 4 or no ANC visit, long distance to health facility 4.68 (1.98-10.67), province 2 3.53 (1.13-11.03), and mother without formal employment 2.33 (1.52-3.55). CONCLUSION: Factors related to health services, distance, provinces, and socioeconomic status of the family were influencers for vaccine dropout. Targeted intervention towards disadvantaged regions, counseling the mother during ANC, improving the education status of parents, access to the health facility, and use of mass media for advocacy are hereby recommended.

ABSTRACT

IntroductionMass media plays a crucial role in creating awareness and knowledge sharing in this Corona virus disease 2019 (COVID-19) pandemic. However, the risk of exposure and extent of COVID-19 infection among media professional are less elucidated yet. Therefore, this study was intended to investigate the workplace-related risk of COVID-19 exposure and the association between exposure to COVID-19 and participants characteristics, including various forms of respiratory protection for mass-media professionals. MethodsThis closed web-based cross-sectional survey was conducted among 199 mass-media professionals in Bangladesh by snowball sampling approach. A multivariate logistic regression model was used for the analytical exploration. Adjusted and Unadjusted Odds Ratio (OR) with 95% confidence intervals (95% CI) were calculated for the specified exposures. Chi-square test was used to observe the association. Ethical issues were maintained according to the guidance of the declaration of the Helsinki. ResultsOf all, 39.2% of mass-media professionals were tested positive for COVID-19, whereas 6% of symptomatic or suspected participants did not do the test. Mass media professionals who worked in electronic media reported more COVID-19 infection (adjusted odds ratio, AOR= 6.25; 95% Confidence interval: Lower limit 1.43, upper limit 27.43; P =0.02). However, no significant relationship was found between the type of job role and COVID-19 infection. Furthermore, infected colleagues (OR/P=1.92/0.04) were identified as significant contact of acquiring infection. However, the study result showed that reused/new medical mask, homemade/cloth-made mask (vs. use of respirator mask) was not significantly (p=0.82) associated with mass media professionals infection. ConclusionsProfessionals working in electronic media were at higher risk of being infected by COVID-19 and mostly acquired from infected colleagues. Using a respirator mask was not associated with a lower risk of test positive infection in mass media professionals. This study will aid the policy maker and public health authorities during the COVID-19 pandemic to make proper implementation strategies.

ABSTRACT

Telomere length (TL) influences the development of lifestyle-related diseases, and neonatal TL may influence their prevalence. Various factors have been reported to affect neonatal TL. Although the fetus is exposed to multiple conditions in utero, the main factors affecting the shortening of neonatal TL are still not known. In this study, we sought to identify factors that influence fetal TL. A total of 578 mother-newborn pairs were included for TL analysis. TL was measured in genomic DNA extracted from cord blood samples using quantitative PCR. The clinical factors examined at enrollment included the following intrauterine environmental factors: maternal age, assisted reproductive technology (ART) used, body mass index (BMI), gestational diabetes mellitus (GDM), maternal stress, smoking, alcohol consumption, preterm delivery, small-for-gestational-age, neonatal sex, and placental weight. Univariate and multivariate regression analyses were used to verify the relationship between neonatal TL and these clinical factors. The median neonatal TL to single-copy gene ratio was 1.0. Pregnancy with ART was among the 11 factors associated with shorter neonatal TL. From multiple regression analysis, we determined that neonatal TL was significantly shorter for pregnancies in the ART group than in the other groups. We conclude that pregnancy with ART is associated with shorter neonatal TL.

ABSTRACT

Background: Lymphocyte telomere length is strongly correlated with patient prognosis in several malignant tumor types and is thought to be related to tumor immunity. However, this correlation has not been studied in gynecological cancers. We determined the prognostic significance of peripheral blood lymphocyte telomere length in gynecologic cancers. Methods: Telomere length of lymphocytes from patients with gynecological malignant tumors (ovarian cancer (OC), N = 72; cervical cancer (CC), N = 63; endometrial cancer (EC), N = 87) was examined by quantitative reverse-transcription PCR of isolated mononuclear cells. Kaplan-Meier and Cox proportional hazard analyses were used to determine the association between lymphocyte telomere length and clinicopathological factors. Results: The overall survival (OS) and progression-free survival (PFS) of patients were based on the dichotomized lymphocyte telomere length using the median as a threshold (OC: 0.75, CC: 1.94, and EC: 1.09). A short telomere length was significantly correlated with residual tumors (≥1 cm) in OC and with advanced stage (III and IV) of CC. In OC and CC, patients with shorter relative lymphocyte telomere length (RLT) had significantly poorer OS and PFS than patients with longer RLT (p = 0.002, p = 0.003, and p = 0.001, p = 0.001, respectively). However, in EC, RLT was not significantly associated with OS or PFS (p = 0.567 and p = 0.304, log-rank test). Multivariate analysis showed that shorter RLT was a significant independent prognostic factor of PFS and OS for OC (p = 0.03 and p = 0.04, respectively) and CC (p = 0.02 and p = 0.03, respectively). Conclusions: Patients with OC and CC with shorter lymphocyte telomeres have significantly reduced survival; therefore, the peripheral blood lymphocyte telomere length is a prognostic biomarker in OC and CC.

ABSTRACT

Designing controllers for compliant, underactuated robots is challenging and usually requires a learning procedure. Learning robotic control in simulated environments can speed up the process whilst lowering risk of physical damage. Since perfect simulations are unfeasible, several techniques are used to improve transfer to the real world. Here, we investigate the impact of randomizing body parameters during learning of CPG controllers in simulation. The controllers are evaluated on our physical quadruped robot. We find that body randomization in simulation increases chances of finding gaits that function well on the real robot.
