ABSTRACT

OBJECTIVE: The aim of the study was to determine changes in the oral mucosa in patients with bruxism using the method of autofluorescence stomatoscopy. MATERIAL AND METHODS: 50 patients with bruxism aged 35-65 years were examined at the Department of Prosthetic Dentistry, Faculty of Dental Medicine, Medical University - Sofia, Bulgaria. Using the digital diagnostic system OccluSense (Bausch, Germany), deviations in static and dynamic occlusion were determined. For the diagnosis of precancerous diseases and early stages of malignant neoplasms of the oral mucosa, we used the method of autofluorescent stomatoscopy using a LED stomatoscope «AFS¼ made in Russia with radiation in the spectral range of 400 nm. RESULTS: The normal mucous membrane of the mouth at this frequency of the spectrum has a green glow. Metabolic and/or structural changes occurring at the cellular and/or tissue level of the oral mucosa lead to a change in its optical properties.Analysis of occlusion in 50 patients with bruxism showed uneven distribution of the chewing load. In 60% of patients, the presence of supercontacts was revealed, and in 76% of cases, occlusion disorders were detected, in 88% of patients, hyperkeratosis of the buccal mucosa was noted, and in 77.3% they were localized along the line of closing of the teeth. CONCLUSION: Examination of the oral mucosa using the autofluorescent stomatoscopy method allows visualizing and, accordingly, objectifying the presence of hyperkeratotic changes in the buccal mucosa in patients with bruxism. The APS apparatus allows for a reliable and effective assessment of non-inflammatory and inflammatory changes, precancerous and cancerous lesions, which makes it indispensable for the manifestation of oncological alertness in the daily clinical practice of dentists.

ABSTRACT

This article analyzes the results obtained by national research groups in the implementation of the Russian State Cancer Program. From 2015 to the first half of 2019, specialists studied the epidemiology and structure of oncological and precancerous diseases of the oral mucosa and the vermillion border in 486,059 persons aged 18-92 years living in Tula, the Tula region, Moscow and Noyabrsk as a result of screening and medical aid appealability. In this period, the productivity of the research group from Tula grew by the factor of 18.92. The results of the research confirm low cancer alertness of practicing dentists. An individual patient registration card has been developed to systematize data and create a diagnostic register for the screening of cancer and precancerous diseases of the oral mucosa and the vermillion border.

ABSTRACT

The aim of the study is to implement the national project created within the framework of the state task of the Ministry of Education and Science of the Russian Federation 'Development of the program of screening of cancer and precancerous diseases of the oral mucosa and red border of the lips'. For its approbation and implementation, national research groups have been created at the bases of the Department of Therapeutic Dentistry of the Moscow State Medical and Dental University, the Tula regional dental clinic, City dental clinic in Noyabrsk City (Yamal-Nenets autonomous district), which in the period from 2015 to the first half of 2018 studied the epidemiology of cancer and precancer of the oral mucosa and the vermilion among 217 806 people aged 18-92 years. Over three and a half years, the effectiveness of the research team increased by 8.45 times. The results of the study confirmed the low oncological alertness of dentists. Educational working program for dentists training 'Screening of diagnostics of precancerous diseases and cancer of the oral mucosa - AFS-D Set' was developed by coordinators of the project and implemented for training of 574 dentists within continuous medical education programs.

ABSTRACT

Research ebjective was to study assessment of quality organization of work treatment recurrent herpetic stomatitis in patients of industrial enterprises from employees.172 patients on the industrial enterprises from 25 to 54 year were treated recurrent herpetic stomatitis. Clinical efficiency of immunomodulator quality therapy of recurrent herpetic stomatitis for assessment organization of work in employees industrial is proved by estimation of oral cavity local immunity (SIgA, lisozyme). After treatment of recurrent herpetic stomatitis in employees signifie and (p<0,005) inerease of lisozyme and SIgA. The obtaiquality of treatment of recurrent herpetic stomatitis et employees of industrial enterprises.

ABSTRACT

The aim of the study was to assess direct restorations mechanical properties (both in vitro and in vivo) to improve dental restorations quality after root canal treatment. Laboratory tests showed that using nanocomposite materials of dual curing with the fiberglass reinforced posts improves restoration strength in endodontically treated teeth: by 3.9±5.8% in class II Peroz restorations, 12.6±5.9 and 24.2±4.2% in class III and IV, correspondently. Using fiberglass reinforced posts (LuxaPost) for the restoration of the tooth crown after endodontic treatment significantly decreases the number of complications associated with marginal leakage of the restoration during first 2 years after treatment (p<005).

ABSTRACT

Cluster analysis of the results of studies of the functional status of the dentition postural and autonomic nervous systems in 251 patients (129 men and 122 women) aged 20-60 years was carried out. European stabilometry variant was used. The patients could be distributed into two groups by the results of cluster analysis. By functional parameters, groups 1 and 2 could be defined as patients with decompensated and compensated functional status, respectively. The characteristics of the functional status of patients in the two groups are presented.

ABSTRACT

We performed cluster analysis of stabilometry data of 129 men and 122 women at the age of 20-60 years using the following 4 methods: European stabilometry variant, Romberg test (European variant), Romberg test (American variant), and Romberg test (American Universal variant). Results of factor analysis of primary data matrix obtained during the test "eye open-teeth clenched" attest to higher clinical significance of absolute stabilometry parameters in comparison with relative ones. The results of factor analysis revealed peculiarities of functional status of the stomatognathic system as the postural sensor and its effect on postural balance. The contribution of the functional status of the stomatognathic system into the postural balance is about 2%.

ABSTRACT

We present the results of comparative enzyme-linked immunosorbent assay of matrix metalloproteinases MMP-2, MMP-8, MMP-9; IL-1ß and IL-6; tissue inhibitors of MMP (TIMP-1, TIMP-2), and TNF-α. The object of study was oral fluid of practically healthy subjects with intact periodontium and patients with chronic generalized periodontitis with various structural materials of dental restorations. It was found that MMP-9 in oral fluid could be considered as a marker of chronic generalized periodontitis irrespective of the presence or absence of metal dental restorations. The level of MMP-8 surpassed the normal only in oral fluid of patients with chronic generalized periodontitis with metal restorations. The correlations between the studied parameters in patients attest to relatively similar regulation of MMP, IL and TIMP secretion in oral fluid in practically healthy subjects with intact periodontium. In patients with inflammatory destructive periodontal diseases with and without metal dental restorations, the correlation coefficients indicate initiation of biochemical cascade accompanied by activation of cytokine production in response to etiological factors. Patients with periodontitis and metal orthodontic structures showed more pronounced reaction. Orthodontic structures made from chromium-cobalt, or chromium-nickel alloys in the oral cavity of these patients increased the levels of MMP-2, IL-1ß and IL-6 in oral fluid.

ABSTRACT

The content of MMP-8 and MMP-9 in oral fluid of 145 patients (95 women and 50 men, 18-52 years) was measured by enzyme immunoassay. We examined 63 subjects with the intact periodontium and 82 patients with chronic generalized periodontitis (25 patients with mild disease, 45 patients with moderate disease, and 12 patients with severe disease). All patients were examined during the remission of chronic periodontitis and did not have clinical signs of associated somatic diseases. No significant differences were found in the content of MMP-8 and MMP-9 in oral fluid from periodontitis patients and subjects with the intact periodontium. The content of MMP-8 an MMP-9 in oral fluid of patients with severe periodontium was slightly higher than that in other patients and subjects with the intact periodontium. The depth of the periodontal pocket was shown to be the most reliable clinical sign for the state of periodontal tissues. A strong correlation was revealed between this criterion and MMP-9 content in oral fluid.

ABSTRACT

Cluster analysis of stabilometry results in 251 subjects (129 men and 122 women, age 20-60 years) was carried out. Four methods were used in the study: European stabilometry variant, Romberg test (European variant), Romberg's test (American variant), and Romberg's test (American Universal variant). Cluster analysis has shown virtually the same diagnostic informative value of all 4 methods. The absolute and relative stabilometry parameters are the most objective, this validating their clinical usefulness. The frequency and amplitude parameters possess no diagnostic significance. The European stabilometry variant is the best fit for practical use and provides information about the function of the ocular, maxillofacial, and other components of the postural system.
