ABSTRACT

Background: Cystic fibrosis (CF) is a serious autosomal recessive disorder. Early diagnosis, comorbidity prevention, and control are cornerstones for a quality life and for improving life expectancy. In Colombian Caribbean, where there is a genetically admixed population, CF is an orphan disease affecting children and adults, and it remains a challenging issue to be addressed carefully. This work describes the genetic, clinical, and paraclinical profiles of CF patients from Cartagena de Indias, Colombia. Methods: Thirty-six patients were included in the study. The subjects were identified and evaluated through the Regional Program for CF patients. CFTR gene mutations, anthropometric parameters, microbiological infections, and pulmonary function were analyzed. Data on demographic parameters, pharmacological treatments, and comorbidities were reported. Frequency and percentages were established for the categorical variables and mean or median for the quantitative variables. In addition, comparisons were made by sex. Results: The average age of the patients was 11.9 ± 5.3 years and the median age at diagnosis was 14 months. 55.5% were women and 44.5% were men. The mean values for weight, height, and body mass index were 35 ± 17.6 kg, 139.9 ± 28 cm, and 16.5 ± 2.9 kg/m2, respectively. The clinical manifestations that occurred more frequently were steatorrhea (65.4%) and recurrent pneumonia (46.2%). Chronic airway infection with Pseudomonas aeruginosa was identified in 71.4% of the cases and the p.F508del mutation was found in 47.2% of the subjects. Conclusion: The current profile of CF patients from the Colombian Caribbean showed some concerning features, such as nutritional status; however, progress in early diagnosis and clinical follow-up could contribute to improve the general conditions of patients. It is necessary to continue efforts to increase the life expectancy and quality of life of the patients.

BACKGROUND: The influence of climate on the epidemiology of dengue has scarcely been studied in Cartagena. METHODS: The relationship between dengue cases and climatic and macroclimatic factors was explored using an ecological design and bivariate and time-series analyses during lag and non-lag months. Data from 2008-2017 was obtained from the national surveillance system and meteorological stations. RESULTS: Cases correlated only with climatic variables during lag and non-lag months. Decreases in precipitation and humidity and increases in temperature were correlated with an increase in cases. CONCLUSIONS: Our findings provide useful information for establishing and strengthening dengue prevention and control strategies.

ABSTRACT

Introducción: la disostosis cleidocraneal (CCD) es una enfermedad genética rara que compromete el desarrollo óseo normal, causada por la alteración en el gen RUNX2 del cromosoma 6p (brazo corto). Sus consecuencias incluyen alteraciones óseas por anomalías en la osificación intramembranosa que, a su vez, conllevan a modificaciones en el desarrollo de huesos craneales, claviculares, a múltiples efectos sobre el número, erupción y recambio dental, y a dificultades funcionales, además de cambios en la conducta psicosocial por el deterioro en la calidad de vida. Objetivo: describir el manejo integral de una paciente con disostosis cleidocraneal, a través de la revisión de caso clínico y el reporte de hallazgos en su mejoramiento, como consecuencia de tratamientos quirúrgicos, rehabilitación y el apoyo interdisciplinario, aspecto de gran importancia para este tipo de pacientes. Caso clínico: paciente femenina de 31 años con diagnóstico de CCD que asistió a la Unidad Estomatológica de la Universidad de Cartagena ­ Colombia, y fue remitida desde Genética por presentar inconformidad funcional y dolor leve durante la masticación de los alimentos por movilidad dental severa en dientes antero-inferiores; además, manifestó permanencia de órganos dentarios deciduos, la cual fue tratada en fases. Al tratamiento se le dio un enfoque multidisciplinar, lo que mejoró, de forma sustancial, la autopercepción e interrelación de la paciente en la sociedad.


Background: Cleidocranial dysostosis (CCD) is a rare genetic disease that compromises normal bone development, caused by the alteration in the RUNX2 gene of chromosome 6p (short arm), which causes bone alterations due to abnormalities in intramembranous ossification that leads to alterations in the development of cranial and clavicular bones and multiple efects on the number, eruption and dental turnover, which leads to functional difculties, in addition to behavior and alterations in psychosocial behavior and the deterioration of their quality of life, Objective: To describe the comprehensive management of a patient with cranial Cleido dysostosis, through a clinical case review, reporting among the fndings the improvement of the patient through surgical treatments, rehabilitation and interdisciplinary support, of great importance for this type of patient. Clinical case: A 31-year-old female patient with a diagnosis of CCD, who attended the Stomatology Unit of the University of Cartagena - Colombia, referred by the treating geneticist, due to functional discomfort and mild pain during chewing food due to severe dental mobility. in anterior-inferior teeth, also showing permanence of deciduous dental organs, which was treated in phases in which a multidisciplinary approach was given to its management, which substantially improved its self-perception and its interrelation in society.

ABSTRACT

The epidemiology of the coronavirus disease (COVID-19) has been scarcely described in individuals under 18 years old, particularly during the first months of the pandemic. The study aimed to describe the COVID-19 epidemiology in the Colombian department of Bolívar from March 2020 to April 2021 among individuals under 18 years. Furthermore, we explored whether the use of data generated by a Bolívar reference laboratory captures the departmental epidemiology. Two information sources were used; the national COVID-19 surveillance system and the Bolívar COVID-19 reference laboratory. In using a population-based ecological approach and information from confirmed symptomatic cases, epidemic curves and heat maps were constructed to assess the COVID-19 dynamics and patterns by sex, age, and residence (Cartagena vs. 45 other municipalities). The COVID-19 incidence was comparable between males and females but varied by age group, being higher in children aged 10 years and older. Cartagena had a significantly higher number of cases and experienced early epidemic peaks. Our analyses suggest that information generated by the reference laboratory does not capture the COVID-19 departmental epidemiology, despite conducting population-based surveillance across Bolívar. The study provides a retrospective characterization of the COVID-19 epidemiology in an understudied population and information that may be useful for future evidence-based responses.

Introduction: Cleidocranial dysplasia (CCD) is a rare disease characterized by craniofacial, skeletal, and oral anomalies. The disease prevalence is estimated to be 1 per million inhabitants; thus, only a few studies have described large cohorts of CCD patients. This study reviewed the clinical-radiological and demographic characteristics of patients with CCD in South America. Methods: We conducted a systematic review of all cases of CCD reported in South America following the PRISMA guidelines. Demographic information (sex, age at diagnosis, origin, reason for consultation, and family history) was also recorded. CCD signs were divided into "craniofacial" and "skeletal" categories. Results: A total of 72 cases were included. We found that oral anomalies were the most common reason for consultation leading to a diagnosis in patients, with a median age at diagnosis of 14 years. Fifty percent of the patients were women. Open fontanels or cranial sutures, the presence of at least one of the typical CCD facies (frontal bossing, brachycephaly, hypertelorism, or depression of the nasal bridge), and supernumerary teeth were reported in 92%, 85%, and 88% of cases, respectively. Clavicular dysplasia was present in 98.6% of cases, and other skeletal abnormalities such as scoliosis, pubic symphysis diastasis, and flat feet were found; short stature was present in 71% of cases, and one case presented cognitive deficits. Conclusion: Although the phenotypic spectrum of CCD is variable, clavicular dysplasia, open fontanels or cranial sutures, dental anomalies, and at least one of the typical CCD facies are present in at least 80% of cases.

ABSTRACT

Leptospirosis is an acute febrile disease that mainly affects developing countries with tropical climates. The complexity and magnitude of this disease is attributed to socioeconomic, climatic, and environmental conditions. In this study, in a 10-year period from 2008 to 2017, the relationship between human leptospirosis cases and climatic factors in Cartagena de Indias, Colombia were evaluated. Monthly leptospirosis cases, climatic variables, and macroclimatic phenomena (El Niño and La Niña) were obtained from public datasets. Local climatic factors included temperature (maximum, average, and minimum), relative humidity, precipitation, and the number of precipitation days. Time series graphs were drawn and correlations between cases of leptospirosis and climatic variables considering lags from 0 to 10 months were examined. A total of 360 cases of leptospirosis were reported in Cartagena during the study period, of which 192 (53.3%) were systematically notified between October and December. Several correlations were detected between the number of cases, local climatic variables, and macroclimatic phenomena. Mainly, the increase of cases correlated with increased precipitation and humidity during the La Niña periods. Herein, seasonal patterns and correlations suggest that the climate in Cartagena could favor the incidence of leptospirosis. Our findings suggest that prevention and control of human leptospirosis in Cartagena should be promoted and strengthened, especially in the last quarter of the year.

RESUMEN Objetivo Estimar la frecuencia de enfermedades huérfanas (EH) o raras (ER) de origen genético en Cartagena de Indias, Colombia. Metodología Se realizó un estudio descriptivo con datos de los Registros Individuales de Prestación de Servicios (RIPS), del periodo 2003-2010, conservados por el Departamento Administrativo Distrital de Salud-DADIS. Fueron escogidas las EH de origen genético a partir de la clasificación del Ministerio de Salud Colombiano, y que se identifican con la referencia Q en los Códigos Internacionales de Enfermedad (CIE10). Resultados En total se registraron 522 181 personas que consultaron al sistema de salud. El 0,8 % (n=16 527) tuvieron algún diagnóstico Q. Finalmente, 391 personas (0,075 %) tuvieron impresión diagnóstica de EH de origen genético. En esta población se encontraron 75 impresiones diagnósticas distintas. Las Malformaciones y Deformidades Congénitas del Sistema Osteomuscular (Q65-Q79) fueron las más frecuentes (38,6 %), seguidas por Otras Malformaciones Congénitas (Q080-089) con 16,6 %. Conclusiones Se obtuvo el primer reporte de la frecuencia de las EH de origen genético en la segunda ciudad más poblada sobre el litoral del Caribe Colombiano. Es posible que en los RIPS haya un subregistro de las ER que dificulta conocer la prevalencia real.(AU)

Conclusions It was obtained the first report of the frequency of genetic orphan diseases from the second largest on the Colombian Caribbean coast city. It is suspected that misreporting in RIPS database is reducing actual frequency of OD. Further active search could reveal complete prevalence.(AU)

ABSTRACT

La fibrosis pulmonar es una enfermedad crónica, progresiva y letal, cuya etiología se desconoce. El modelo de fibrosis pulmonar inducida por Bleomicina en ratas es útil para ilustrar la patobiología in vivo de la enfermedad, así como para identificar nuevos blancos farmacológicos y estimar la eficiencia de nuevas moléculas o procedimientos Objetivo: El objetivo de este trabajo fue construir un modelo animal de fibrosis pulmonar secundaria a Bleomicina, en ratas Wistar, como herramienta que pueda servir de base para futuros diseños experimentales. Materiales y métodos: Se trabajó con dos grupos de ratas Wistar para la administración del medicamento por vía intratraqueal. El grupo experimental recibió una dosis única (2.0 U/Kg) de Bleomicina, mientras que el grupo control recibió un volumen equivalente de solución salina. A los 14 o 28 días se realizó un lavado broncoalveolar con recuento total y diferencial celular y análisis histopatológico pulmonar. Resultados: La histología de una parte del grupo experimental tratado con Bleomicina y sacrificado a los 14 días reveló daño pulmonar caracterizado por inflamación aguda, hemorragia intraalveolar y proliferación fibroblástica intersticial incipiente; en el resto del grupo experimental la histología a 28 días reveló además alteración de la arquitectura pulmonar debida a fibrosis y aumento en el número de macrófagos intraalveolares e inflamación linfocitaria. Conclusiones: Se implementó satisfactoriamente un modelo de fibrosis pulmonar inducido farmacológicamente por Bleomicina en ratas Wistar.


Pulmonary fibrosis is a chronic, progressive and fatal disease, whose etiology is unknown. The model of Bleomycininduced pulmonary fibrosis in rats is useful to illustrate the pathobiology of the disease in vivo as well as to identify new drug targets and to estimate the efficacy of new promising molecules or procedures. Objective: The aim of this work was to make an animal model of pulmonary fibrosis secondary to bleomycin, in Wistar rats, as a tool that can serve as a basis for future experimental designs. Materials and methods: We worked with two groups of Wistar rats which were anesthetized and intubated for intratracheally drug administration. The experimental group received a single dose (2.0 U / kg) of Bleomycin, while the control group received an equivalent volume of saline. At 14 or 28 days after treatment, a bronchoalveolar lavage with total and differential cellular count were performed. Additionally, the lungs were dissected for histopathogical analysis. Results: In the experimental group treated with Bleomycin and sacrificed at 14 days, histology revealed lung damage characterized by acute inflammation, intra-alveolar hemorrhage and fibroblast proliferation; in sacrificed animals at 28 days, alteration of lung architecture due to fibrosis evidenced by trichrome stain, increase in the alveolar macrophages number and lymphocytic chronic inflammation were observed. Conclusions: In this study, a model of pharmacologically induced pulmonary fibrosis by Bleomycin has been successfully implemented.

ABSTRACT

Objetivo Investigar en una universidad estatal del Caribe colombianoel estado de la salud sexual y reproductiva especialmente conocimientos sobre Enfermedades de Transmisión Sexual (ETS), fertilidad, sexualidad, embarazo y violencia. Métodos Estudio transversal tipo encuesta. Se seleccionaron estudiantes matriculados en el segundo período de 2010 que llenaron voluntariamente la encuesta. Los datos cualitativos fueron tabulados y graficados usando medidas de tendencia central para variables cuantitativas. Resultados La población tenía alrededor de 20 años de edad, provenía de la zona urbana (57,9%; IC95%=54,7-61,1), predominantemente heterosexual (89,7%) con edad de inicio de vida sexual antes de los 18 años, con 11,8 % de promiscuidad que usaba principalmente el preservativo como Método de Planificación Familiar (MPF) (55 %). Aunque poseían información previa sobre salud sexual, MPF y ETS, no se comportaban acorde con ello, por la desinformación sobre formas de transmisión del VIH, bajo nivel de realización de pruebas serológicas para ETS, así como conductas de riesgo (sexo/alcohol/drogas). El 12,3% tuvo antecedente de embarazo, violencia física (21,6%) y sexual (4,6%) con un predominante silencio de las víctimas de abuso sexual (61,8%). Conclusiones La muestra refleja la población universitaria de esta región del país. Se planea organizar un programa con apoyo médico y psicológico que permita disminuir los índices de ETS, embarazos no planificados, preparando al adolescente en esta importante etapa de su vida, sirviendo de modelo para otras universidades latinoamericanas.

Conclusion The sample reflects the student population in this region of Colombia. We plan to organize a health program with medical and psychological support to reduce the rates of STDs and unplanned pregnancies, preparing the adolescent for this important step in their life and serving as a model for other Latin American universities.

OBJECTIVE: To investigate the state of sexual and reproductive health in students at a public university in the Colombian Caribbean, with an emphasis on sexually transmitted diseases (STDs), fertility, sexuality, pregnancy and violence. METHODS: Cross-sectional survey study. University students, enrolled in the second semester of 2010 and who completed a self-administered survey based on the Reproductive Health survey of the Pan American Health Organization, were selected. Qualitative data was tabulated and graphed using measures of central tendency for quantitative variables. RESULTS: The age of population studied was around 20 years old, came from the urban area (57.9 %; IC95 %=54.7-61.1), was predominantly heterosexual (89.7 %), with an age of initiation of sexual activity of less than 18 years old, 11.8 % promiscuity, mainly using the condom as a Family Planning Method (FPM) (55 %). Although they had prior information on sexual health, STDs and FPMs, they did not behave according to this due to low education about HIV transmission routes, low incidence of serological tests for STDs, and high risk behavior (sex/alcohol/drugs). It was observed that 12.3 % had a history of pregnancy, physical violence (21.6 %) and sexual violence (4.6 %) with a predominant silence from the victims of sexual abuse (61.8 %). CONCLUSION: The sample reflects the student population in this region of Colombia. We plan to organize a health program with medical and psychological support to reduce the rates of STDs and unplanned pregnancies, preparing the adolescent for this important step in their life and serving as a model for other Latin American universities.

ABSTRACT

Background. Sinus-pulmonary manifestations are a major concern in CF patients. Pulmonary function shows a strong relationship with nutritional status. In Colombia, malnutrition is a public health issue; however there are no reports that fully analyze nutritional status and lung function in CF children. Materials and Methods. A cross-sectional study was developed conducted with 32 patients registered in the CF Attention Program. Measurements were performed following the NIOSH Spirometry Training Guide. Body Mass Index (Z-Score) was calculated according to Onis.et.al. Correlation was determined by a regression model. Results. 14 children were able to perform the test with criteria for the study, mean for age 12.4 ± 3.4 years. % Forced Expiratory Volume1 and % Forced Vital Capacity means were 66.7 ± 28.5 and 69.5 ± 2.0, respectively. Body Mass Index (Z-score) mean was -1.17. BMI-regression for % Forced Expiratory Volume1, r2=0.31(P<0.01); % Forced Vital Capacity, r2=0.22(P<.01). Conclusions. Results demonstrated a correlation between Body Mass Index and Lung Function. This correlation persists, even in populations with notorious nutritional deficit such as this group. Special nutritional therapies should be implemented for this group and similar populations.


Introducción: Las manifestaciones sinopulmonares son de gran interés en pacientes con fibrosis quística. La función pulmonar se ha correlacionado con el estado nutricional en estos pacientes. En Colombia, la malnutrición es un problema de importancia para la salud pública; sin embargo, no se han publicado reportes que analicen el estado nutricional y la función pulmonar en pacientes con fibrosis quística. Materiales y métodos: Se disenó un estudio transversal con 32 pacientes registrados en el Programa de Atención Integral para Pacientes con Fibrosis Quística y sus familias. Los exámenes se realizaron siguiendo la guía de entrenamiento de la NIOSH. El índice de Masa Corporal (Z-Score) fue calculado siguiendo las recomendaciones de Onis et al. El grado de correlación fue determinado por un modelo de regresión. Resultados: 14 ninos fueron incluidos para realizar las pruebas con los criterios del estudio; la edad media fue 12,4±3,4anos; para %Volumen Expiratorio Forzado y %Capacidad Vital Forzada las medias fueron 66,7±28,5 y 69,5±22,0 respectivamente. La media del índice de Masa Corporal (Z-score) fue -1,17. La regresión del índice de Masa Corporal para %Volumen Expiratorio Forzado1 fue r²=0,31(P<0,01), y para %Capacidad Vital Forzada, r²=0,22(P<0,01). Conclusiones: Los resultados demostraron correlación entre el índice de Masa Corporal y la función pulmonar. Esta correlación persistió incluso en poblaciones con un notorio déficit nutricional como la de este estudio. Una terapia nutricional específica debería ser implementada en este grupo y poblaciones similares.
