ABSTRACT

AIM: To study the effectiveness of system for prevention of community-acquired pneumonia (CAP) in military teams and determine the main ways for its improvement. MATERIALS AND METHODS: Data on incidence of CAP in military staff during 1993 - 2008 were used. Analysis of morbidity was performed using classical methods of epidemiological analysis. RESULTS: Registered incidence of CAP in military staff was 5 - 7 times higher compared with average incidence in adult population of the country. Main factors determining realization of CAP epidemic process mechanisms in military teams were revealed. Two-fold and more rise in incidence of CAP and acute respiratory infection in military recruits is forecasted for 2010. Effect of use of Pneumo 23 vaccine for freshmen recruits is grounded. CONCLUSION: Immunization of military staff with Pneumo 23 vaccine considered effective and needed component in complex of prophylactic measures performed in troops.

ABSTRACT

The article presents actual questions of epidemiology of outhospital pneumonia (OP), peculiarities of epidemic process of these infections in troops in now-day conditions and updates of main directions of their prophylaxis. It was marked that alongside with clearly defined tendency of lowing, the morbidity by OP in the Armed Forces of RF is still rather high--in 5-7 folds higher than among adult population of the country. Whereby it is predicted further complication of sanitarium-epidemiological situation in connection with increase of call-up. The article uncovers the main factors, supplying the realization of mechanism of epidemic process of OP in military collectives. Vaccination of staff by registered in RF multipartial pneumococcal vaccine Pnevmo-23 is considered as an effective and necessary medical measure in complex of prophylactic measures, effectuating in troops.

ABSTRACT

Nowadays, taking into account new tasks, that should be resolved by the medical service, could be marked enlargement of informational cooperation at the expense of perspective program-technical solutions, put in the complex of program-technical means of automatization of administration of state disease control CPTMADC during it's creating. It's planed to elaborate an additional programmatic module for the purpose of using in CPTMADC the methodology of valuation of risk for resolving of tasks of socially-hygienic monitoring. Before it's elaborating, there was examined a capacity to conversion of patterns of weekly reports about the sanitarian condition of objects and it's following using for informational transporting using electronic means of connection of the complex CPTMADC by the way of including in patterns the data about the staff under the influence of adverse factor, and following rangering the objects by the level of risk for staff.

ABSTRACT

Start with the 1990s on the background of declining of social-economic situation in the country, it was marked the upgrade of level of tuberculosis morbidity among population of RF. In RF also was marked the upgrade of tuberculosis morbidity among military-service-men by contract, as well as among military-service-men by call-duty. According to the results of analyze, in conditions of existing organization of work, in conditions of reorganization of the Armed Forces, it's forecasted further upgrade of levels. For the purpose of stabilization and degrade of morbidity, were formed new methods of prophylaxis of tuberculosis in RF, which were used in plan of main measures of lowering of tuberculosis morbidity in the Armed Forces of RF. In new plan main attention is paid to questions of prophylaxis and, particularly, non-admission of up-calling persons with tuberculosis, as well as to rising of discipline of functionaries of medical service. Realization of planed measures in Chief bureau and on the seats began.

ABSTRACT

Sanitarium-epidemiological well-being of the Army and Navy--it's a condition of health of service-men and their habitat, when noci-influence on the staff is excluded, and favorable environments of their life are ensured. Indicator exponents are exponents of soldier health. For the present time the most problematical and significant diseases are Outhospital pneumonia, dermal and underdermal cellulose diseases and tuberculosis. In comparisment with 2006, in 2007 morbidity by outhospital pneumonias, among service-men by appeal, declined on 10,4%. In conditions of general positive tendency of morbidity by outhospital pneumonias, correlation between all registrated cases of diseases and cases of acute respirator infections among the Armed Forces in 2007 was 1:8,5. Correlation of prior years was from 1:10 to 1:12. Actuality of dermal and underdermal cellulose diseases is still high. Among service-men on appeal the degree of these diseases is about 200%. Realizing of planed measures will decline the morbidity by tuberculosis more, then for 20-25% and stabilize it on the degree of 0.60-0.65% among service-men on appeal. Realization of final cause--assurance of sanitarium-epidemiological well-being of the Army and declining of morbidity in now-day's conditions--could be achieved in condition of cooperation between command, chief medical officers and medicines of military units of medical service.

ABSTRACT

A new information system (IS) - the software and hardware complex for controlling the state sanitary-and-epidemiological inspection (SSEI) was created. The system represents the aggregate of automated working places of RF MD chief state sanitary physician arid specialists from the department of state sanitary-and-epidemiological inspection of the Main Military Medical Headquarters. They interact through communications with working places of specialists from SSEI Main Center, chief state sanitary physicians from the Armed Forces, military districts (fleets) and RFAF CSSEI. The special software provides automation of the following technological processes: operative sanitary-and epidemiological and epidemiological monitoring; the epidemiological analysis of infectious diseases; the evaluation of quality and efficiency of sanitary-and epidemiological work. At present the complex works in the regime of experimental exploitation when the adjustment of communications and special software is performed.

ABSTRACT

Influenza polymer-subunit vaccine "Grippol", made up of the sterile conjugate of surface proteins of influenza viruses, groups A and B, with polyoxidonium (copolymer), was obtained at the State Scientific Centre of Research Institute of Immunology. As the result of our investigations, the coefficient of prophylactic effectiveness of vaccine "Grippol" was found to be 71%; in the presence of the 50% immune stratum in the group the coefficient of antiepidemic protection was found to be 66.4%. At the same time, cases of influenza and acute respiratory diseases were shown to take a milder form in persons immunized with vaccine "Grippol" than in the group of nonimmunized subjects. The results of this investigation did not show any side effects caused by the vaccine.
