ABSTRACT

The clinical significance of 201Tl myocardial perfusion scintigraphy was studied in 7 patients with abnormal branch of the left coronary artery from the pulmonary trunk and in 1 patient with that of the anterior interventricular artery from the pulmonary trunk. Radiological criteria for the defect were established on the basis of comparative analysis of the data of myocardial scintigraphy with those of electrocardiographic, coronary angiography and anatomic examinations. It was shown that it was feasible to make a differential diagnosis between abnormal left coronary artery branch and dilated cardiomyopathy and that the technique was of value for comparative assessment of the efficacy of various types of surgical treatment for abnormal left coronary artery ramus from the pulmonary trunk.

ABSTRACT

Experiments on 9 dogs were staged for a study of a method of early diagnosis of infectivity of vascular prostheses of the abdominal aorta with the help of 99m-Tc-labelled leukocytes. A sterile synthetic vascular prosthesis was used for prosthetics of the abdominal aorta in the 1st control group of animals, a prosthesis, previously infected with St. aureus, was used in the 2nd group, a sterile prosthesis with its subsequent infecting by i.v. injection of St. aureus was used in the 3rd group. The role of antibiotic therapy in infectivity of vascular prostheses was studied on the animals of the 3rd group. A radionuclide study with autologous 99m-Tc-labelled leukocytes was conducted using the Deina-2 camera (Picker). Visualization of the prostheses was unobserved in the 1st control group; in the 2nd group the accumulation of 99m-Tc-leukocytes in the zone of the prosthesis and its visualization on the 4th-5th min. were noted; in the 3rd group the prosthesis was visualized on the 10th-15th min. After antibiotic therapy in the 3rd group visualization of the prosthesis was undetectable. The proposed method permitted visualization of a zone of infection in the early postoperative period, observation of the time course of infection development, and assessment of the efficacy of antibiotic therapy.

ABSTRACT

Altogether 111 patients with coronary heart disease (CHD) were examined using loading perfusion scintigraphy testing of the myocardium with 201Tl. Quantitative assessment of scintigrams was based on a computer-assisted method proposed by the authors which was used for determination of indices characterizing a degree of perfusion disorder in each segment of the left ventricular myocardium: the M/B ratio (the ratio of a mean level of radioactivity of a myocardial segment to a mean level of background radioactivity) and the TAI (the thallium accumulation index--a quantitative value of segmental perfusion). The method permits the detection of a zone of disturbed perfusion (ischemia, scar changes and left ventricular aneurysm) and quantitative assessment of the depth of scar lesion of the left ventricular myocardium.

ABSTRACT

Systemic, central and intracardiac hemodynamics and left-ventricular contractility were studied radiocardiographically and radioventriculographically in 22 patients with stable pulmonary hypertension, developing in the presence of chronic obstructive pulmonary diseases. A tendency to increased circulating blood volume, significantly elevated end diastolic and end systolic indices, reduced total ejection fraction, and a tendency to decreased segmental ejection fractions were demonstrated. A significant reduction of the speed and percentage of left-ventricular myocardial circular fibre contraction is another evidence of incompetent left-ventricular contractility, in addition to the reduced ejection fraction.

ABSTRACT

Radioisotope angiography demonstrates successfully occlusion of the abdominal aorta, aneurysms of the thoracic and abdominal aorta, and occlusion of the iliac and femoral arteries. In occlusion of the renal artery this method supplements contrast vasography of the kidneys providing a quantitative concept of the functional peculiarities and dynamics of the renal blood flow. The results of radioisotope angiography were in full agreement with the clinical picture of the disease and the findings of contrast X-ray angiography in 81.7% of cases; in 11.3% they coincided partly and only in 7% of cases the results were dissimilar.

ABSTRACT

The article deals with a complex study of parameters of the hemodynamics and volume of the cardiac cavities in comparison with a clinical state of patients suffering from mitral defect before and in different periods following prosthesis of the mitral valve. On the basis of the results obtained during examination of 54 patients there was revealed a complete conformity between their clinical state, data of the hemodynamics, the volume of the heart and left auricle. A reverse development of many signs characteristic of the defect was established. The absence of an absolute improvement of some clinical and hemodynamic indices in remote periods following the operation in a number of patients was connected with a marked increase of the cardiac volumes before the operation, an unsatisfactory contractile capacity of the myocardium, the remaining of cardiac fibrillation after the operation.

ABSTRACT

The clearance of intracoronarily introduced 133Xe was employed in investigating the myocardial blood flow in 105 patients, 10 of whom--controls and 95 suffering from various manifestations of ischemic heart disease. Use was made of the biexponential calculation of the clearance curve by taking account of the fast (circulation in normal tissues) and slow exponent (circulation along the cicatricially-altered sections of the myocardium) contributions to the common "effective" myocardial blood flow. In patients with ischemic heart disease a reduction of the myocardial blood flow depending upon the severity of the clinical picture and the degree of the coronary arteries lesion was revealed. A direct correlational dependence between the initial state of the myocardial circulation at rest and the level of the physical efforts tolerance was educed. A conclusion is drawn that the determination of the myocardial circulation by the method of the 133Xe clearance following intracoronarily administration of the substance should be done by taking account of the slow and fast exponents of the clearance curve, these exponents reflecting the circulation in sections of the myocardium differing in their functional activity.
