ABSTRACT

Biliary complications are one of the main complications after transplantation, especially after hepatic transplantation from living donor. Of 147 patients, in whom hepatic transplantation was conducted, in 40 biliary complications have had occur, including in 29 (72.5%) - biliary leak, and in 11 (27.5%) - biliary stricture. Twelve patients were reoperated: for biliary leak - 8, and biliary stricture - 4. In 9 patients biliary leak stopped spontaneously, in 7 - a drainage under ultrasonographic control was conducted, and in 5 - endoscopic retrograde cholangiopancreaticography. In 8 patients cholangiopancreaticography was conducted for biliary stricture in 3 - transcutaneous transhepatic cholangiography.

ABSTRACT

In 48 patients, suffering gastroduodenal ulcer disease, complicated by hemorrhage, cytological and histological investigations were conducted to reveal helicobacter infection. There was established, that sensitivity, specificity and accuracy of smears--the gastric mucosa imprints--cytological investigation exceeds such of histological investigation of biopsies. In more than 80% patients, in whom pyloroduodenal ulcer, complicated by hemorrhage, was diagnosed, Helicobacter pylori (H. pylori) was revealed in gastric mucosa.

ABSTRACT

The results of experimental and clinical studies for the comparative evaluation of implants used in herniology of polytetrafluoroethylene (PTFE) and polyvinylidene fluoride (PVDF) are presented. Experimental investigations conducted on 200 mice, which in the anterior abdominal wall mesh prosthesis implanted. All animals, regardless of the type of implanted prosthesis, deduced from the experiment at 10, 30 th and 60 th day. Histological investigation of the submissions received. The clinical part is based on an analysis of surgical treatment results of postoperative abdominal wall hernia (POGBS) in 121 patients during the period from 2003 to 2011 patients were divided into 2 groups. The control group included 73 patients who were operated during the period 2005-2007, used PTFE graft "Ecoflon", in the main - 48 patients who from 2008 to 2011 the prosthesis was implanted PVDF - "Uniflex" firm "Linteks". In each group, selected patients whom have examined the profile of cytokines. The results of the studies found that both the material cause marked inflammatory reaction with a characteristic stepwise increased levels of proinflammatory and antiinflammatory cytokines, the dynamics of cytokines indicative of a less pronounced local inflammatory reaction to the prosthesis from the PVDF. In the application of the implant "Uniflex" a lower incidence of exudative complications in comparison with that when using the prosthesis "Ecoflon" it is preferable in clinical use.

ABSTRACT

Comparative analysis of the treatment results in 120 patients, suffering destructive form of biliary calculous disease (DFBCD) was performed. Depending on operative access and postoperative conservative therapy applied the patients were divided into two groups. In the main group in 58 patients, suffering DFBCD, a minilaparotomy access was applied, using "mini-assistant" apparatus named after M. I. Prudkov and postoperatively--low-intensive laser irradiation and regional lymphotropic therapy. In a comparison group in 62 patients various laparotomic accesses were used, and a standard postoperative therapy was performed. Normalization of clinical, laboratory and immune indices in the main group were observed in twice earlier than in a control one, and economical together with moral-psychological effect was measured in reduction of the medicines quantity used as well as the patients stationary stay.

ABSTRACT

The study is based on the examination of 12 patients with perforative duodenal ulcer and 24 patients operated on recurrent bleeding duodenal ulcer. Some component of the immune system, such as T- and B-lymphocytes rates, immunoglobulin rate and macrophagal activity, were decreased prior the beginning of the complex treatment. Normalisation of humoral and cell immunity was registered on 10-12 days after the beginning of the ozone and low-intensive laser irradiation.

ABSTRACT

Clinico-experimental studying the immunomorphological reactions variants in the prosthesis of anterior abdominal wall using synthetic polypropylene mesh was conducted. In 70 patients, operated on for postoperative hernia of abdominal wall using "on lay" method and the immune state were studied up by means of investigation of proinflammatory and anti-inflammatory cytokines content, in part, of tumor necrosis factor-alpha; (TNF-alpha), IL-8, IL-4. Morphological investigations were conducted in experiment on 100 the Vistar line mices, there were studied reactions of fibroblasts, macrophages, lymphocytes and leucocytes. The results of the immunological and morphological reactions on polypropylene obtained trust the necessity of differentiated approach application for the choice of the synthetic prosthesis kind, taking into account its biocompatibility, physical properties, the hernial defect size etc.

ABSTRACT

Morphohistochemical investigations of the duodenum in patients with ulcer disease complicated by hemorrhage have revealed two times higher number of mast cells and correspondingly increased amount of histamine and heparin in the periulcerous area. It accounts for the focal obstacle of blood coagulation and arrest of hemorrhage in patients with ulcer disease.

The visual picture of the ulcerous defect was studied in 222 patients by endoscopic and intraoperative methods. The prognostic assessment of the development of complications of ulcer disease of the stomach and duodenum is given on the basis of the characteristics of the ulcer process (diameter, depth, character of the ulcer fundus and margins).

ABSTRACT

Problems of morphofunctional condition of endocrine cells of gastric mucosa and their importance in prognosis of ulcer disease complications are discussed. Sixty patients with various complications of ulcer disease were examined. Investigation of quantity and functional activity of 4 most frequent types of gastric mucosa endocrine cells in different zones of affected area permitted to reveal strong correlation between quantity and functional activity of these cells. Changes of endocrine cells are reliable and have high prognostic significance in development of ulcer disease complications and must be taken into account by physicians to select right policy for prevention of these complications.

ABSTRACT

The work has shown that patients with ulcer disease of the stomach and duodenum have immunogenetic features. In particular, genetic markers were detected for ulcer disease of the stomach and duodenum whose presence allows to predict with certain probability the development of ulcer disease of the stomach and duodenum. The presence of antigen HA-At and haploids--AxB18 and HLA--A10B27 is prognostically unfavorable factors for the development of complications of this disease.
