ABSTRACT

In this report, we present a 65-year-old man who presented with signs and symptoms consistent with impending brain herniation. Emergent imaging revealed a hyperdense mass in the suprasellar region. Urgent surgery was performed and final pathology eventuated a pilocytic astrocytoma. Although rare cases of suprasellar pilocytic astrocytoma in children and adults have been reported, we report an interesting case of a hemorrhagic suprasellar pilocytic astrocytoma in an elderly adult (without prior anticoagulant use) causing impending brain herniation secondary to obstructive hydrocephalus.

ABSTRACT

Delineating the genetic variability of loci coding for complex diseases helps to understand the individual variation in disease susceptibility and drug response. We present the allelic architecture of the F7 gene. This gene is the major determinant of FVII plasma levels, and these plasma levels constitute an important intermediate risk factor for cardiovascular disease. As part of the Genetic Analysis of Idiopathic Thrombophila Project, we completely re-sequenced the F7 locus (promoter, exons, introns, and 3'-untranslated region) in 40 unrelated individuals. We found 49 polymorphisms with only two amino acid changes suggesting that regulatory non-coding and intronic variants are responsible for the FVII variability. These results are important for mapping susceptibility alleles of complex diseases, because differences in pair-wise linkage disequilibrium patterns between DNA variants and haplotype frequency distributions may help to detect disease-associated alleles. In addition, we present the results of an in silico search that established genomic comparisons among different species. In conclusion, our study of the F7 DNA sequence variations is an example of a strategy for analyzing the genetic architecture of a quantitative trait locus. Furthermore, it provides a model for future analyses of genetic factors that contribute to the susceptibility of complex diseases in humans.

ABSTRACT

To determine the functional status in elderly patients after a hospitalisation in an Internal Medicine unit. We prospectively studied patients aged 80 or above hospitalised in the Hospital Provincial de Ciudad Real in an Internal Medicine unit, between February and July, 2003. The functional status was determined by Barthel Index. We examined 206 patients (77.4%). They showed a previous Barthel Index of 70.9; one of 48.9 in the hospitalisation stage and one of 58.6 when discharged (p<0.001). We noticed a Barthel Index when discharged which was lower than the previous Barthel one in 73.8% patients. Hospitalisation implies a great functional impairment in the functional status elderly patients. It would be convenient, therefore, to identify the risk factors to be able to set some guidelines for a preventive model.

Valoramos la repercusión en la capacidad funcional de los pacientes ancianos tras un ingreso hospitalario en un servicio de Medicina Interna. Se estudió de forma prospectiva los pacientes con 80 años o más ingresados en el Servicio de Medicina Interna del Hospital Provincial de Ciudad Real, entre los meses de febrero a julio de 2003. La valoración de la capacidad funcional se realizó mediante el índice de Barthel. Se incluyeron 206 (77,4%) con un índice de Barthel previo de 70,9, al ingreso de 48,9 y al alta de 58,6 (p<0,001). Se recogió un índice de Barthel al alta menor al previo en el 73,8% de los pacientes. La hospitalización conlleva un deterioro importante en la capacidad funcional de los pacientes ancianos. Es necesario identificar los factores de riesgo responsables de esta pérdida funcional y poder establecer pautas preventivas de actuación

ABSTRACT

The dose dependent antiproliferative effect of an alkaloidal substance extracted from the sponge Amphimedon viridis was tested on Leishmania mexicana promastigotes. Sponges were collected in Isla Larga, Venezuela (10 degrees 20' 20" - 10 degrees 24" N, 64 degrees 19' - 64 degrees 22' W), cut and dipped in methanol for vacum filtering extraction every 24 hr. The aqueous extract was separated by chromatography over silica gel. The parasites were from the Venezuelan NR strain. Their growth rate was reduced by 50% with a dose of 10 microg/ml in 48 hr, whilst concentrations of 30 and 40 microg/ml induce leishmanicidal action after 110 and 20 min, respectively. Lysis is preceded by an immediate increase in cellular volume associated with progressive damage of cellular content and the destruction of organelles. These findings suggest that one important factor associated with the antiproliferative effect of this alkaloidal substance on L. mexicana promastigotes is the loss of the plasma membrane selective permeability.

ABSTRACT

The characteristics are reported of five myopathies initially considered toxic in nature caused by the administration of blood lipid lowering agents. In three cases the final diagnosis was hypothyroidism, thus demonstrating the need to rule out the potential causes of secondary dyslipidemia before initiating therapy. Likewise, and because of the rare true muscle involvement from blood lipid lowering agents, it is necessary to investigate those diseases or factors enhancing muscle toxicity when it appears.
