ABSTRACT

In equal groups of patients with duodenal ulcers truncal vagotomy with draining operations were performed in 98 patients, gastric resection--in 196 patients. The observations lasted from 2 to 7 years. Immediate lethal outcomes did not take place after vagotomy. Six patients died after gastric resection (3.05%). Postoperative complications were observed in 8% and 22.7% correspondingly. In remote periods after vagotomy 75% of the patients were referred to the I and II group (by the Wisik scale), 12% of patients--to the III group, 12%--to the IV group, after gastric resection--70%, 20% and 10% correspondingly.

ABSTRACT

Complex examination of 72 patients with obliterating atherosclerosis and 10 patients with obliterating endarteritis was performed. All the patients were given autotransfusion of UV-irradiated blood as conservative treatment, preoperative preparation and postoperative management. The course of AUVIB was followed by a decrease of the lactic acid level, increase of functional properties of hemoglobin, activation of humoral immunity, change of the blood hemostatic potential to the side of hypocoagulation. The AUVIB treatment was most effective in patients at initial stages and those with peripheral forms of the disease.

ABSTRACT

Truncal vagotomy with draining measures was fulfilled in 70 patients with complicated duodenal ulcers. In 50 of them there was an insufficient cardial constrictor (in 26 patients--of light degree, in 20--of mild degree, in 4--of severe degree). Long-term results of the treatment of the disease were good in 68 patients, two patients had recurrences. Truncal vagotomy resulted in the improvement of the functional state of the cardial constrictor.

ABSTRACT

Results of the hospital treatment of severe purulent mastitis in 127 women after childbirth were analyzed. The authors believe that opening of the purulent cavity and dissection of the purulent focus within the limits of intact tissues do not exclude one another. They should be performed according to corresponding indications. The intensive infusion therapy is thought to be necessary for struggle against toxemia, for correction of the acidic-alkaline state and partial parenteral feeding. Feeding from the diseased breast should be permitted under conditions of sterile milk.

ABSTRACT

The authors believe that the main criterion for choice of the method of treatment of diabetic patients with gangrene of lower extremities is the degree of compensation of blood circulation which was estimated by temperature asymmetry of necrosis on the crus. The conclusion is made on the basis of treatment of 62 patients. The amputation at the femur level is indicated in decompensated blood circulation, amputations on the foot associated with intensive pharmacotherapy are indicated in subcompensation. Complex conservative treatment is possible in patients with a starting gangrene.

ABSTRACT

Social rehabilitation was studied in 799 patients with atrial fibrillation. Before the sinus rhythm was restored 26% of the patients had been working. Among the working patients the history of atrial fibrillation was usually of a short duration (9.8 +/- 1 months, on the average), and among the nonworking patients it comprised 28.9 +/- 0.6 months. After the reestablishment of the sinus rhythm the precentage of rehabilitation grew to 40. It greatly depended on the nature of the underlying pathology, sex (among the working patients, 51.8% were men and 25.5% -- women), place of habitation (capacity for work was restored in 45.8% of urban patients, and in 32.0% of rural patients). Upon the restoration of the sinus rhythm the capacity for work increased most markedly among the patients previously subjected to surgical correction of mitral stenosis (by 26.3%), and among those with post-myocarditis cardiosclerosis (by 17.4%). Repeated counterpulsation procedures in cases of atrial fibrillation recurrences provided for a lasting maintenance of the patients' capacity for work. To prolong the rehabilitation results it is rational to recommend such jobs for the patients that would not entail important physical and emotional stress.
