ABSTRACT

BACKGROUND: Gait is the manner or style of walking, involving motor control and coordination to adapt to the surrounding environment. Knowing the kinesthetic markers of normal gait is essential for the diagnosis of certain pathologies or the generation of intelligent ortho-prostheses for the treatment or prevention of gait disorders. The aim of the present study was to identify the key features of normal human gait using inertial unit (IMU) recordings in a walking test. METHODS: Gait analysis was conducted on 32 healthy participants (age range 19-29 years) at speeds of 2 km/h and 4 km/h using a treadmill. Dynamic data were obtained using a microcontroller (Arduino Nano 33 BLE Sense Rev2) with IMU sensors (BMI270). The collected data were processed and analyzed using a custom script (MATLAB 2022b), including the labeling of the four relevant gait phases and events (Stance, Toe-Off, Swing, and Heel Strike), computation of statistical features (64 features), and application of machine learning techniques for classification (8 classifiers). RESULTS: Spider plot analysis revealed significant differences in the four events created by the most relevant statistical features. Among the different classifiers tested, the Support Vector Machine (SVM) model using a Cubic kernel achieved an accuracy rate of 92.4% when differentiating between gait events using the computed statistical features. CONCLUSIONS: This study identifies the optimal features of acceleration and gyroscope data during normal gait. The findings suggest potential applications for injury prevention and performance optimization in individuals engaged in activities involving normal gait. The creation of spider plots is proposed to obtain a personalised fingerprint of each patient's gait fingerprint that could be used as a diagnostic tool. A deviation from a normal gait pattern can be used to identify human gait disorders. Moving forward, this information has potential for use in clinical applications in the diagnosis of gait-related disorders and developing novel orthoses and prosthetics to prevent falls and ankle sprains.

ABSTRACT

Background: During the COVID-19 pandemic, guideline documents on the management of anticoagulation were rapidly published. However, these documents did not follow a structured methodology, and significant differences existed between the guidelines. The aim of this expert consensus was to provide recommendations on the clinical management of oral anticoagulation in patients in the context of the COVID-19 pandemic. Methods: A two-round Delphi study was conducted using an online survey. In the first round, panellists expressed their level of agreement with the items on a 9-point Likert scale. Items were selected if they received approval from ≥66.6% of panellists and if they were agreed by the scientific committee. In the second round, panellists revaluated those items that did not meet consensus in the first round. Results: A total of 147 panellists completed the first round, and 144 of them completed the second round. Consensus was reached on 161 items included in five dimensions. These dimensions addressed: (I) management of anticoagulation in patients with atrial fibrillation (AF) without mechanical valves or moderate/severe mitral stenosis during COVID-19 infection; (II) thromboprophylaxis in patients hospitalised for COVID-19; (III) management of anticoagulation at hospital discharge/after COVID-19; (IV) anticoagulation monitoring in the COVID-19 pandemic setting; and (V) role of telemedicine in the management and follow-up of patients with AF in the COVID-19 pandemic setting. Conclusions: These areas of collective agreement could specially guide clinicians in making decisions regarding anticoagulation in patients with COVID-19 during hospitalisation and at discharge, where results from clinical trials are still limited and, in some cases, conflicting.

ABSTRACT

Type 2 diabetes (T2DM) is one of the main public health care problems worldwide. It is associated with a marked increased risk of developing atherosclerotic vascular disease, heart failure, chronic kidney disease and death. It is essential to act during the early phases of the disease, through the intensification of lifestyle changes and the prescription of those drugs that have been shown to reduce these complications, with the aim not only of achieving an adequate metabolic control, but also a comprehensive vascular risk control. In this consensus document, developed by the different specialists that treat these patients (endocrinologists, primary care physicians, internists, nephrologists and cardiologists), a more appropriate approach in the management of patients with T2DM or its complications is provided. A particular focus is given to the global control of cardiovascular risk factors, the inclusion of weight within the therapeutic objectives, the education of patients, the deprescription of those drugs without cardiovascular benefit, and the inclusion of GLP-1 receptor agonists and SGLT2 inhibitors as cardiovascular protective drugs, at the same level as statins, acetylsalicylic acid, or renin angiotensin system inhibitors.

BACKGROUND: In handball, the kinematics of the frontal plane seem to be one of the most important factors for the development of lower limb injuries. The knee valgus angle is a fundamental axis for injury prevention and is usually measured with 2D systems such as Kinovea software (Version 0.9.4.). Technological advances such as computer vision have the potential to revolutionize sports medicine. However, the validity and reliability of computer vision must be evaluated before using it in clinical practice. The aim of this study was to analyze the test-retest and inter-rater reliability and the concurrent validity of a beta version app based on computer vision for the measurement of knee valgus angle in elite handball athletes. METHODS: The knee valgus angle of 42 elite handball athletes was measured. A frontal photo during a single-leg squat was taken, and two examiners measured the angle by the beta application based on computer vision at baseline and at one-week follow-up to calculate the test-retest and inter-rater reliability. A third examiner assessed the knee valgus angle using 2D Kinovea software to calculate the concurrent validity. RESULTS: The knee valgus angle in the elite handball athletes was 158.54 ± 5.22°. The test-retest reliability for both examiners was excellent, showing an Intraclass Correlation Coefficient (ICC) of 0.859-0.933. The inter-rater reliability showed a moderate ICC: 0.658 (0.354-0.819). The standard error of the measurement with the app was stated between 1.69° and 3.50°, and the minimum detectable change was stated between 4.68° and 9.70°. The concurrent validity was strong r = 0.931; p < 0.001. CONCLUSIONS: The computer-based smartphone app showed an excellent test-retest and inter-rater reliability and a strong concurrent validity compared to Kinovea software for the measurement of the knee valgus angle.

INTRODUCTION: Neck pain is one of the most frequent reasons for consultation in primary care. Clinicians evaluate different variables, including movement and cervical strength, to determine the prognosis of patients. Usually, the tools employed for this purpose are expensive and bulky, or more than one is needed. This study aims to describe a novel device designed to assess the cervical spine and describe its test-retest reliability. METHODS: The Spinetrack device was designed to measure the strength of deep cervical flexor muscles and the chin-in and chin-out movement of the upper cervical spine. A test-retest reliability study was developed. The flexion, extension and strength needed to move the Spinetrack device were registered. Two measurements were developed, with one week between each assessment. RESULTS: Twenty healthy subjects were evaluated. The strength of the deep cervical flexor muscles in the first measurement was 21.18 ± 3.15 Newtons, the displacement movement during chin-in movement was 12.79 mm ± 3.46 and the displacement during chin-out movement was 35.99 mm ± 4.44. The test-retest reliability of strength was ICC 0.97 (95% CI (0.91-0.99)). CONCLUSION: The Spinetrack device has shown excellent test-retest reliability values for the measurement of the strength of the cervical flexor muscles and for the chin-in and chin-out movements.

ABSTRACT

This study aimed to find the optimum mechanical characteristics of the restorative materials for the manufacture of implant crowns subjected to impact loading when different combinations of materials are used for the inner and outer crown. Several combinations of external-internal crown restorative materials were analyzed. The dynamic stresses at eight different zones of a dental implant subjected to an impact load and the influence of several mechanical properties, such as the Young's modulus, Poisson's ratio, density, and initial velocity, were analyzed and compared. A detailed 3D model was created, including the crown, the retention screw, the implant, and a mandible section. The model was then built by importing the 3D geometries from CAD software. The whole 3D model was carefully created in order to guarantee a finite element mesh that produced results adjusted to physical reality. Then, we conducted a numerical simulation using the finite element method (FEM). The results of the FEM analysis allowed for evaluating the effect that different combinations of restorative materials and mechanical properties had on the stress distribution in various regions of the implant. The choice of restorative material is a factor to be considered in order to preserve the integrity of osseointegration. Restorative materials transfer more or less stress to the dental implant and surrounding bone, depending on their stiffness. Therefore, an inadequate Young's modulus of the rehabilitation material can affect the survival of the implant over time. Eight interactive graphics were provided on a web-based surface platform to help clinical dentists, researchers, and manufacturers to select the best restorative materials combination for the crown.

ABSTRACT

Statement of problem. Previous peri-implantitis, peri-implant bone regeneration, or immediate implant placement postextraction may be responsible for the absence of cortical bone. Single crown materials are then relevant when dynamic forces are transferred into bone tissue and, therefore, the presence (or absence) of cortical bone can affect the long-term survival of the implant. Purpose: the purpose of this study is to assess the biomechanical response of dental rehabilitation when selecting different crown materials in models with and without cortical bone. Methods: several crown materials were considered for modeling six types of crown rehabilitation: full metal (MET), metal-ceramic (MCER), metal-composite (MCOM), peek-composite (PKCOM), carbon fiber-composite (FCOM), and carbon fiber-ceramic (FCCER). An impact-load dynamic finite-element analysis was carried out on all the 3D models of crowns mentioned above to assess their mechanical behavior against dynamic excitation. Implant-crown rehabilitation models with and without cortical bone were analyzed to compare how the load-impact actions affect both type of models. Results: numerical simulation results showed important differences in bone tissue stresses. The results show that flexible restorative materials reduce the stress on the bone and would be especially recommendable in the absence of cortical bone. Conclusions: this study demonstrated that more stress is transferred to the bone when stiffer materials (metal and/or ceramic) are used in implant supported rehabilitations; conversely, more flexible materials transfer less stress to the implant connection. Also, in implant-supported rehabilitations, more stress is transferred to the bone by dynamic forces when cortical bone is absent.

ABSTRACT

In the literature, many researchers investigated static loading effects on an implant. However, dynamic loading under impact loading has not been investigated formally using numerical methods. This study aims to evaluate, with 3D finite element analysis (3D FEA), the stress transferred (maximum peak and variation in time) from a dynamic impact force applied to a single implant-supported prosthesis made from different materials. A 3D implant-supported prosthesis model was created on a digital model of a mandible section using CAD and reverse engineering. By setting different mechanical properties, six implant-supported prostheses made from different materials were simulated: metal (MET), metal-ceramic (MCER), metal-composite (MCOM), carbon fiber-composite (FCOM), PEEK-composite (PKCOM), and carbon fiber-ceramic (FCCER). Three-dimensional FEA was conducted to simulate the collision of 8.62 g implant-supported prosthesis models with a rigid plate at a speed of 1 m/s after a displacement of 0.01 mm. The stress peak transferred to the crown, titanium abutment, and cortical bone, and the stress variation in time, were assessed.

ABSTRACT

The aim of this study is to evaluate the stresses transferred to peri-implant areas from single implants restored with different restorative materials and subjected to a static vertical load with low eccentricity. A total of 12 crowns were made with four types of materials: carbon fiber-composite, metal-ceramic, metal-composite, and full-metal, all of them cemented over a titanium abutment. Three different ways of approaching the problem have been used independently to verify the robustness of the conclusions. The experimental results of stress distribution around the implant were obtained by two image processing techniques: Digital Photoelasticity and Digital Image Correlation (DIC). The tests have been modelled by 3D Finite Element Method (FEM). The FEM models have also been used to study the sensitivity of the results to slight changes in geometry or loads, so that the robustness of the experimental techniques can be analyzed. In addition, the realistic bone morphology of the mandible has also been modelled by FEM, including the cortical and trabecular bone property distinctions.

El tratamiento hipolipemiante es uno de los pilares de la prevención cardiovascular; en la prevención secundaria de pacientes con cardiopatía isquémica, es una de las estrategias de mayor eficacia, pero el tratamiento hipolipemiante actual, junto con cambios en el estilo de vida, en una importante proporción de pacientes no consigue alcanzar los objetivos recomendados por las guías de práctica clínica. Los inhibidores PCSK9 han mostrado eficacia y seguridad en el tratamiento de la dislipemia y se han incorporado recientemente en España para empleo clínico con el objetivo de reducir el riesgo cardiovascular debido a su efecto en la reducción del colesterol unido a lipoproteínas de baja densidad (AU)

Nevertheless, the current treatment of lipid disorders, together with lifestyle changes, fails to achieve the targets recommended in clinical guidelines in a substantial proportion of patients. PCSK9 inhibitors have demonstrated safety and efficacy in the treatment of dyslipidemia. Due to their ability to reduce low-density lipoprotein cholesterol levels, these drugs have recently been approved for clinical use by Spanish regulatory agencies, with the aim of reducing cardiovascular risk in selected patient groups.
