ABSTRACT

Methicillin-resistant Staphylococcus (MRS) has been associated with neonatal infections, with colonization of the anovaginal tract being the main source of vertical transmission. The COVID-19 pandemic has altered the frequency of antibiotic usage, potentially contributing to changes in the dynamics of bacterial agents colonizing humans. Here we determined MRS colonization rates among pregnant individuals attending a single maternity in Rio de Janeiro, Brazil before (January 2019-March 2020) and during (May 2020-March 2021) the COVID-19 pandemic. Anovaginal samples (n = 806 [521 samples before and 285 during the pandemic]) were streaked onto chromogenic media. Colonies were identified by MALDI-TOF MS. Detection of mecA gene and SCCmec typing were assessed by PCR and antimicrobial susceptibility testing was done according to CLSI guidelines. After the onset of the pandemic, MRS colonization rates increased significantly (p < 0.05) from 8.6% (45) to 54.7% (156). Overall, 215 (26.6%) MRS isolates were detected, of which S. haemolyticus was the most prevalent species (MRSH, 84.2%; 181 isolates). SCCmec type V was the most frequent among MRS (63.3%; 136), and 31.6% (68) of MRS strains had a non-typeable SCCmec, due to new combinations of ccr and mecA complexes. Among MRS strains, 41.9% (90) were resistant to at least 3 different classes of antimicrobial agents, and 60% (54) of them were S. haemolyticus harboring SCCmec V. MRS colonization rates and the emergence of multidrug-resistant variants detected in this study indicate the need for continuing surveillance of this important pathogen within maternal and child populations.

ABSTRACT

BACKGROUND: C1-C2 injury represents 25-40% of cervical injuries and predominantly occurs in the geriatric population. METHODS: A prospective multicentre study was conducted under the aegis of the french spine surgery society (SFCR) investigating the impact of age, comorbidities, lesion type, and treatment option on mortality, complications, and fusion rates. RESULTS: A total of 417 patients were recruited from 11 participating centres. The mean ± SD age was 66.6 ± 22 years, and there were 228 men (55%); 5.4% presented a neurological deficit at initial presentation. The most frequent traumatic lesion was C2 fracture (n = 308). Overall mortality was 8.4%; it was 2.3% among those aged ≤ 60 years, 5.0% 61-80 years, and 16.0% > 80 years (p < 0.001). Regarding complications, 17.8% of patients ≤ 70 years of age presented with ≥ 1 complication versus 32.3% > 70 years (p = 0.0009). The type of fracture did not condition the onset of complications and/or mortality (p > 0.05). The presence of a comorbidity was associated with a risk factor for both death (p = 0.0001) and general complication (p = 0.008). Age and comorbidities were found to be independently associated with death (p < 0.005). The frequency of pseudoarthrosis ranged from 0 to 12.5% up to 70 years of age and then constantly and progressively increased to reach 58.6% after 90 years of age. CONCLUSIONS: C1-C2 injury represents a serious concern, possibly life-threatening, especially in the elderly. We found a major impact of age and comorbidities on mortality, complications, and pseudarthrosis; injury pattern or treatment option seem to have a minimal effect.

ABSTRACT

Traumatic events such as a motor vehicle accident or falling from heights are very common in veterinary medicine and often lead to vertebral fracture-luxation with concomitant spinal cord injuries, mostly in the thoracolumbar spine. The purpose of this cadaveric biomechanical study was to determine the feasibility of the three-column concept in canine thoracolumbar segments with induced fractures. Eighteen Functional Spinal Units (FSU) of the thoracolumbar segments (T12-L2) were collected from 18 medium-sized adult dog cadavers and were subjected to flexion-extension and lateral bending tests so that range of motion (ROM) was recorded with a goniometer. Fractures were induced by compressive loads applied by a universal testing machine (EMIC®). After this, specimens were screened using computed tomography (CT) and the fractures were graded as affecting one, two or three columns, and divided into groups A, B, and C, respectively. Post-fracture range of motion (ROM) was compared with the previous results. Groups B and C (with fractures in two or three columns) had instability in the two axes evaluated (P<0.05). The outcomes of this study support the applicability of the three-column theory to thoracolumbar spines of dogs, as the FSUs that suffered fractures in two or more columns showed axial instability.(AU)


Eventos traumáticos, como acidentes automobilísticos ou quedas, são muito comuns na medicina veterinária e, frequentemente, levam a fraturas ou luxações vertebrais, associadas a lesões medulares concomitantes, mais frequentemente na coluna toracolombar. O propósito deste estudo biomecânico em cadáveres foi determinar a viabilidade da teoria dos três compartimentos em fraturas induzidas em segmentos toracolombares de cães. Dezoito unidades espinhais funcionais (UEF) de segmentos toracolombares (T12-L2) foram coletadas de 18 cadáveres de cães adultos de médio porte e submetidas a testes de flexão-extensão e curvamento lateral, de modo que a amplitude de movimento (ADM) foi registrada com um goniômetro. Fraturas foram induzidas por meio de cargas compressivas aplicadas por uma máquina universal de ensaios (EMIC®). As amostras foram submetidas à tomografia computadorizada (TC), e as fraturas foram classificadas como afetando um, dois ou três compartimentos e divididas nos grupos A, B e C, respectivamente. A ADM pós-fratura foi comparada com os resultados prévios. Os grupos B e C (com fraturas em dois ou três compartimentos) apresentaram instabilidade nos dois eixos avaliados (P<0,05). Os resultados deste estudo corroboram a aplicabilidade da teoria dos três compartimentos para segmentos de coluna toracolombar em cães, uma vez que as UEF que sofreram as fraturas em dois ou mais compartimentos apresentaram instabilidade axial.(AU)

ABSTRACT

A captive adult male bush dog (Speothos venaticus) was referred to our Veterinary Medical Teaching Hospital from a local zoo due to a two-week history of progressive hind limb gait impairment and ataxia, non-responsive to clinical management. Computed tomography revealed decreased disc space at L3 - L4 level, with probable disc extrusion narrowing the right side of the spinal canal, compressing the spinal cord. We opted to surgically remove the disc material using both fenestration and right lateral pediculectomy (mini-hemilaminectomy) techniques. Twelve days after surgery there was mild residual proprioceptive ataxia. Gait was fully regained with remission of the neurological deficits around the 30th postoperative day. This is - to the best of our knowledge - the first successful report of a lumbar intervertebral disc extrusion in a bush dog (Speothos venaticus) surgically treated by pediculectomy and disc fenestration.(AU)


Um cachorro-vinagre (Speothos venaticus), macho, adulto, mantido em cativeiro, foi encaminhado ao Hospital Veterinário Universitário pelo zoológico local com histórico de duas semanas de comprometimento progressivo da marcha dos membros posteriores e ataxia, que não responderam ao tratamento clínico. Tomografia computadorizada revelou diminuição do espaço em disco no nível L3 - L4, com provável extrusão de disco estreitando o lado direito do canal vertebral, comprimindo a medula espinhal. Optamos por remover cirurgicamente esse material do disco usando técnicas de fenestração e pediculectomia lateral direita (mini-hemilaminectomia). Doze dias após a cirurgia, houve melhora na deambulação, com ataxia proprioceptiva residual leve. A marcha foi totalmente recuperada com remissão dos déficits neurológicos por volta do trigésimo dia de pós-operatório. Este é - até onde sabemos - o primeiro relato bem-sucedido de uma extrusão de disco intervertebral lombar em um cachorro-vinagre (Speothos venaticus) tratado cirurgicamente por pediculectomia e fenestração de disco.(AU)

ABSTRACT

O presente estudo objetivou comparar, biomecanicamente, por meio do ensaio de compressão excêntrica, a resistência dos implantes Clamp and Rod Internal Fixation (CRIF) 5,0mm e placa de reconstrução 3,5mm na fixação de fraturas distais de fêmur de cão. Foram utilizados 22 fêmures de 11 cadáveres de cão entre 2 e 7 anos de idade e peso corporal entre 20 e 40kg e subdivididos em dois grupos, denominados grupo CRIF (GC - 11 fêmures) e grupo placa (GP - 11 fêmures). Para realização dos testes, foi simulada falha que mimetizasse uma fratura distal nos corpos de prova, por meio de uma osteotomia de até 0,5cm, realizada com auxílio de serra oscilatória, imediatamente proximal ao início da tróclea. Os implantes foram fixados segundo os padrões AOSIF, lateralmente ao fêmur, sendo utilizados três parafusos distais e cinco proximais ao foco de fratura. Foi empregado o programa de computador PMI para calcular o ponto máximo de resistência antes da falha e avaliaram-se as variáveis força máxima, deformação máxima real, rigidez, força intermediária e deformação intermediária real. Não foram encontradas diferenças estatisticamente significativas entre os GC e GP quanto às variáveis avaliadas, sugerindo que ambos os implantes são boas opções de fixações para tais fraturas.(AU)


This study aims to compare the biomechanical properties, through compression eccentric test, of the resistance of the 5.0mm Clamp and Rod Internal Fixation Implants (CRIF) and 3.5mm reconstruction plate in fixing dog femur distal fracture. It was used 22 femurs from 11 dog cadavers with age between 2 and 7 years old and body weight between 20 and 40kg, subdivided into two groups, called CRIF Group (CG - 11 femurs) and plate group (PG - 11 femurs). A distal fracture in the specimens with a gap of 0.5cm osteotomy, was simulated to perform the test, performed by the oscillating saw, just proximal to the beginning of the trochlea. The implants were fixed by the standards AO / SIF, laterally to the femur, using three distal screws and five proximal to the fracture site. PMI computer program was used to calculate the maximum point of resistance before failure and evaluated the variables maximum strength, real maximum deformation, stiffness, intermediate strength and real intermediate deformation. No statistically significant differences were found between the GC and GP as the variables evaluated, suggesting that both implants are good fixation options for such fractures.(AU)

ABSTRACT

Flaviviruses are agents of a major emerging human public health issue and members of this genus have been associated with central nervous system (CNS) infections. In Brazil, a country endemic for some arboviruses, the most clinically relevant neurotropic flaviviruses include Dengue virus and Zika virus. Flaviviruses cause diseases ranging from mild or sub-clinical infections to severe cases as CNS infections. There is a lack of data about the incidence of flaviviruses in the CNS of children in Brazil. In this review, we provide a general overview of several flaviviruses that cause CNS infections in Brazilian children and explore the importance of epidemiological surveillance of CNS infections in cases of flavivirus infections.

Resumen Introducción: Las mujeres que padecen cáncer de mama requieren de acciones que busquen mejorar su calidad de vida, a pesar del tratamiento. La espiritualidad parece ser un mecanismo de afrontamiento a la enfermedad. Objetivo: Identificar las evidencias disponibles sobre espiritualidad, para la mejora de la calidad de vida de mujeres con cáncer de mama, por medio de una revisión integrativa como metodología. Métodos: La búsqueda fue realizada en las bases de datos: PubMed, LILACS y Scopus, en un intervalo de 10 años. Se incluyeron artículos en los idiomas: inglés, portugués y español; con una temática relacionada a la espiritualidad, calidad de vida y cáncer de mama. Resultados: La muestra final fue de 23 artículos, mismos que se analizaron y agruparon en tres categorías: Calidad de vida y espiritualidad, la espiritualidad como estrategia de afrontamiento y las intervenciones que apoyan la espiritualidad. Discusión: Mujeres con cáncer de mama de diferentes culturas y prácticas espirituales, infieren que la espiritualidad les ayudó a reorganizarse psicológicamente, por lo tanto, es importante reconocer las necesidades espirituales de estas mujeres, proporcionar un cuidado holístico y humanizado, por ende mejorar su calidad de vida. Conclusiones: Las instituciones de salud deben poner énfasis en la incorporación de prácticas espirituales y religiosas, como parte integral en el tratamiento, una vez que, en su mayoría no requieren de recursos financieros, sino de los recursos espirituales propios de cada una de estas mujeres.


Abstract Introduction: Women with breast cancer require actions aimed at improving their quality of life. Spirituality is a way to address this situation. Objective: Through an integrative review, to identify available evidence related to the use of spirituality to improve the quality of life of women with breast cancer. Methods: An integrative search was conducted on the databases of PubMed, LILACS and Scopus, considering an interval of 10 years. Articles written in English, Portuguese, and Spanish, addressing spirituality, quality of life, and breast cancer, were included. Results: The final simple contained 23 articles which were analyzed and clustered into 3 categories: quality of life and spirituality, spirituality as an addressing strategy and spirituality supporting interventions. Discussion: Women from diverse cultures and spiritual practices referred that spirituality had helped them reorganize their minds, suggesting that, it is important to consider spirituality as part of the holistic care aimed at improving the quality of life of women with breast cancer. Conclusion: Health institutions should consider incorporating spiritual and religious practices into their integral treatments, echoing the spiritual needs of patients, including women with breast cancer.

Discussão: Mulheres com câncer de mama de diferentes culturas e práticas espirituais, inferem que a espiritualidade lhes ajudou a reorganizar-se psicologicamente, portanto, é importante reconhecer as necessidades espirituais destas mulheres, proporcionar um cuidado holístico e humanizado, por conseguinte melhorar sua qualidade de vida. Conclusões: As instituições de saúde devem dar especial atenção na incorporação de práticas espirituais e religiosas, como parte integral no tratamento, uma vez que, em sua maioria não requeiram de recursos financeiros, senão dos recursos espirituais próprios de cada uma destas mulheres.

ABSTRACT

At the end of 2016, Brazil experienced an unprecedented yellow fever (YF) outbreak. Clinical, molecular and ecological aspects of human and non-human primate (NHP) samples collected at the beginning of the outbreak are described in this study. Spatial distribution analyses demonstrated a strong overlap between human and NHP cases. Through molecular analyses, we showed that the outbreak had a sylvatic origin, caused by the South American genotype 1 YFV, which has already been shown to circulate in Brazil. As expected, the clusters of cases were identified in regions with a low vaccination coverage. Our findings highlight the importance of the synchronization of animal surveillance and health services to identify emerging YF cases, thereby promoting a better response to the vulnerable population.

ABSTRACT

Objetivou-se relatar um caso de protrusão do disco intervertebral com localização atípica e descrever a técnica cirúrgica de corpectomia lateral parcial no tratamento da afecção em um cão. Um cão de sete anos de idade, macho, raça Pug, foi atendido com ataxia proprioceptiva dos membros pélvicos havia três meses, associada à doença do disco intervertebral entre a nona e 10ª vértebras torácicas. Após um mês de tratamento conservativo sem melhora, o paciente foi encaminhado para o tratamento cirúrgico, quando foi realizada corpectomia lateral parcial esquerda para descompressão da medula espinhal. Após a cirurgia, o paciente apresentou melhora considerável do quadro, sendo a técnica eficaz na remoção da compressão extradural, sem manipulação da medula espinhal ou outras complicações.(AU)


The aim of this study is to report a case of intervertebral disc protrusion with atypical location and describe the surgical technique of partial lateral corpectomy in the treatment of this disease in a dog. A dog, seven -years-old, male, Pug breed was attended with proprioceptive ataxia of the hind limbs for three months correlated with intervertebral disc disease between the 9th and 10th thoracic vertebrae. After a month of conservative treatment without improvements, the patient was referred for surgical treatment, which was performed as a partial left lateral corpectomy for decompression of the spinal cord. After surgery, the patient showed considerable improvement, this technique being shown to be effective in the removal of extradural compression, without manipulation of the spinal cord or other complications.(AU)

ABSTRACT

A 10-year-old male Rottweiler was evaluated for a 2-month history of recurrent forelimb weakness. Neurologic examination revealed proprioceptive ataxia, tetraparesis and moderate cervical pain. Disk-associated cervical spondylomyelopathy (CSM) with static lesion at C3-4 was diagnosed based on magnetic resonance imaging (MRI). The dog was surgically treated by a ventral slot procedure and distraction-stabilization of the vertebral bodies through insertion of vertebral screws with transverse connective bars. The patient had favorable clinical outcome. Neurologic assessment performed 120 days after surgery showed absence of neurologic defects. Radiographic assessment performed at the same time indicated adequate spinal cord decompression although vertebral fusion was not achieved. To the authors' knowledge, this is the first case of distraction-stabilization with vertebral (pedicle) screws to treat CSM in a dog. The treatment was well tolerated with no complications and excellent outcome and can be a viable option for this condition.(AU)


Um cão, macho, adulto, Rotweiller de 10 anos de idade, foi atendido com histórico de claudicação do membro torácico esquerdo com evolução para fraqueza havia aproximadamente dois meses. Ao exame neurológico, observou-se ataxia proprioceptiva nos quatro membros e dor cervical moderada. Mediante ressonância magnética, espondilomielopatia cervical disco-associada com a característica estática da compressão C3-C4 foi diagnosticada. Realizou-se cirurgia descompressiva por meio de slot ventral e estabilização-distração com o uso de parafusos vertebrais e barras conectoras. O paciente apresentou evolução clínica favorável do quadro. A evolução foi progressiva e, no último retorno, 120 dias após a cirurgia, não apresentou nenhum déficit neurológico ou sinal de falha do implante nas imagens radiográficas; no entanto, não foi evidenciada fusão vertebral. Pelo conhecimento dos autores, esse é o primeiro relato de EMC disco-associada tratada por distração e estabilização com parafusos vertebrais (pediculares) em um cão. A fixação espinhal por meio do uso de parafusos vertebrais foi uma alternativa viável no caso apresentado.(AU)

ABSTRACT

PURPOSE: Anterior or lateral interbody fusion is a treatment option for lumbar disc disease. A segmental change occurs after such surgery. This study was designed to evaluate the changes in the lumbar regional alignment after a single or two-level standalone anterior or lateral interbody fusion (ALIF or LLIF). METHODS: Data from patients referred to our institution between March 2013 and November 2015 for standalone ALIF or LLIF for low-grade isthmic spondylolisthesis or degenerative discopathy were retrospectively included in our analysis. Patients with a history of spinal fusion were excluded. Global and regional alignments were analyzed pre- and postoperatively. Pelvic tilt (PT), sacral slope (SS), sagittal vertical axis (SVA), lumbar lordosis (LL), index segmental lordosis (ISL) and L4S1 lordosis were compared. Three groups according to the pelvic incidence (PI) (low, normal and high) were separately analyzed then compared. RESULTS: Forty-one women and 27 men (mean age was 46 years; range 25-66) were included. The mean follow-up was 10.8 (range 3-34 months). The patients were globally well balanced preoperatively and remained after surgery (SVA stagnated from 16.76±28.42mm to 15.97±28.20mm, P=0.75). PT and LL did not vary. L4S1 lordosis, and ISL were significantly increased respectively from 30.56±8.59 to 34.58±7.47 (P=0.0026) and from 5.94±5.25 to 12.99±5.87 (P<0.0001) at latest follow-up. CONCLUSION: Despite effective changes in the segmental lordosis at the index levels, our findings suggest that one or two-levels standalone ALIF or LLIF had no effect on the global balance and the lumbar lordosis. The three groups behaved similarly, the regional lordosis was redistributed in a better harmony (L4S1/LL ratio went up from 55% to 61%, P=0.01). STUDY TYPE: Retrospective study. Na osteocondrite dissecante (OCD), a articulação do ombro é mais comumente afetada, mas o joelho é ocasionalmente lesionado, o que, muitas vezes, passa desapercebido. O tratamento cirúrgico precoce é indicado para remover cartilagem solta, aliviar a dor e minimizar a artrose. Benefícios putativos da transferência de autoenxerto osteocondral em relação às técnicas convencionais incluem a reconstrução exata do contorno subcondral e articular, recobrimento da superfície com cartilagem hialina e criação de uma barreira imediata entre o líquido sinovial e o osso subcondral. O objetivo deste trabalho é relatar a técnica de transferência de autoenxerto osteocondral para o tratamento da osteocondrite do côndilo femoral. Um cão da raça Bull Terrier foi tratado cirurgicamente por meio da técnica de transferência de autoenxerto osteocondral, após ter sido diagnosticado com OCD do côndilo femoral, apresentando melhora clínica significativa e completa recuperação aos 30 dias de pós-operatório.(AU)


In osteochondritis dissecans (OCD), the shoulder joint is most commonly involved, but the stifle is occasionally involved and often goes unnoticed. Early surgical treatment is indicated to remove loose cartilage and minimize osteoartrosis. Putative benefits of Osteochondral Autograft Transfer (OAT) over conventional techniques include accurate reconstruction of subchondral and articular contour, resurfacing with hyaline or hyaline-like cartilage, and creation of an immediate barrier between synovial fluid and subchondral bone. The purpose of this work is to report the technique of OATs for the treatment of osteochondritis of the femoral condyle. A canine attended the Laboratory of Comparative Orthopedics and Traumatology USP-FMVZ underwent surgery after being diagnosed with OCD of the femoral condyle, with significant clinical improvement at 30 days postoperatively.(AU)

ABSTRACT

Objetivou-se reportar, pela primeira vez descrita até então, a fixação de uma fratura de sétima vértebra lombar (L7) com luxação de L7-S1, por meio de fixação pedicular, utilizando-se Clamp and Rod Internal Fixation (CRIF) em um gato. Um felino de um ano de idade, macho, SRD, foi atendido com histórico de atropelamento havia sete dias, apresentando paraparesia não ambulatória secundária à fratura da sétima vértebra lombar e compressão da cauda equina. Após avaliação geral do paciente, no dia seguinte, ele foi submetido à cirurgia descompressiva por meio de laminectomia dorsal e posterior estabilização pedicular entre L7 e S1, utilizando-se CRIF bilateralmente. O paciente apresentou evolução favorável com deambulação normal após 15 dias do procedimento cirúrgico.(AU)


The aim was to report the establishment of a seventh lumbar vertebra fracture (L7), the first described so far, with L7 -S1 luxation through pedicle fixation using Clamp and Rod Internal Fixation (CRIF) on a cat. A 1 year old male mongrel feline was attended with a history of being hit by car seven days prior, with no ambulatory paraparesis secondary to fracture of the seventh lumbar vertebrae and compression of the cauda equina. After general evaluation of the patient, the next day, the patient was submitted to decompressive surgery through dorsal laminectomy and posterior pedicle stabilization between L7 and S1 using CRIF bilaterally. The patient presented a favorable outcome with normal ambulation after 15 days of surgery.(AU)

ABSTRACT

Due to their anatomical characteristics and the complexity of the procedures required to obtain their complete occlusion, the treatment of giant intracranial aneurysms is a real challenge. Direct reconstructive strategies, whether by interventional neuroradiology (coils, stents) or microsurgical (clipping) means, are not always applicable and, in patients that would not tolerate parent or collateral artery sacrifice, the adjunction of a revascularization procedure using a bypass technique might be necessary. Cerebral arterial bypasses can be classified according to their function (3 types: flow replacement, flow reversal or protective), the branching mode of the graft used (3 types: pedicled, interpositional or in situ), the sites of anastomosis (2 types: extracranial-intracranial or intracranial-intracranial) and the class of flow they are supposed to provide (3 types: low-, intermediate- or high-flow). In this article, the authors review the different aspects in the management of patients with a giant intracranial aneurysm using a bypass: preoperative work-up, types of bypass and indications, surgical techniques and results.

ABSTRACT

Fracture nonunions represent important complications in orthopedic surgeries. Nonunion repairs or bone defects are surgically challenging. Our aim was to describe a nonunion case, which was repaired with rapid bone recovery. An 8-month-old male mixed breed dog that has been previously operated was presented to the Veterinary Medical Teaching Hospital of São Paulo State University, with a right radius-ulna nonunion and an amputated contralateral forelimb. A cancellous bone graft was collected from a partially amputated limb, in order to correct the nonunion, and used in association with a locking plate. After four weeks, the bone graft had been incorporated into the original bone. Clinical union with good weight bearing was achieved after eight weeks.


As não uniões ósseas representam uma importante complicação nas cirurgias ortopédicas. Não uniões ósseas ou defeitos ósseos são um desafio na cirurgia. O objetivo do presente trabalho foi descrever um caso de não união óssea, que foi reparada de forma rápida. Um cão SRD de oito meses, macho, que havia sido operado previamente, deu entrada no Hospital Veterinário da Unesp - Jaboticabal com uma não união óssea em radioulna e o membro contralateral amputado. Um enxerto ósseo autógeno foi coletado do coto do membro contralateral amputado e utilizado junto a uma placa óssea bloqueada para o tratamento. Após quatro semanas, o enxerto ósseo estava incorporado ao osso original. A união clínica e o retorno normal à função foram alcançados após oito semanas.

ABSTRACT

INTRODUCTION: Transforaminal lumbar interbody fusion with a minimally invasive approach (MIS TLIF) has become a very popular technique in the treatment of degenerative diseases of the lumbar spine, as it allows a decrease in muscle iatrogenic. However, iterative radiological controls inherent to this technique are responsible for a significant increase in exposure to ionizing radiation for the surgeon. New techniques for radiological guidance (O-arm navigation-assisted) would overcome this drawback, but this remains unproven. OBJECTIVES: To analyze the exposure of the surgeon to intraoperative X-ray during a MIS TLIF under fluoroscopy and under O-arm navigation-assisted. MATERIALS AND METHODS: This prospective study was conducted at the University Hospital of Lille from February to May 2013. Twelve patients underwent a MIS TLIF for the treatment of low-grade spondylolisthesis; six under standard fluoroscopy (group 1) and six under O-arm system (group 2). Passive dosimeters (rings and glasses) and active dosimeters for thorax were used to measure the radiation exposure of the surgeon. RESULTS: For group 1, the average time of fluoroscopy was 3.718 minutes (3.13-4.56) while no radioscopy was perform on group 2. For the first group, the average exposure dose was 12 µSv (5-20 µSv) on the thorax, 1168 µSv (510-2790 µSv) on the main hand and 179 µSv (103-486 µSv) on the lens. The exposure dose was measured zero on the second group. CONCLUSION: The maximum recommended doses can be reached, mainly for the lens. In addition to the radioprotection measures, O-arm navigation systems are safe alternatives to significantly reduce surgeon exposure.

Cardiac migration is a rare complication of the ventriculoperitoneal shunt. We report a case of a late migration of the distal shunt into the pulmonary arteries. The authors underline the radiological features that suggested the presence of a knot. Preoperative planning and the assistance of a vascular surgeon were required due to the presence of a knot.

ABSTRACT

This study aimed to describe in detail the surgical technique of partial lateral corpectomy (PLC) associated with pediculectomy to treat an in vitro extradural chondroma. A 12-year old female Cocker Spaniel was seen in the hospital with proprioceptive ataxia of hind limbs associated with extradural compression between the 1st and 2nd lumbar vertebrae. The owner opted for euthanasia since the patient's condition changed due to simultaneous illnesses that culminated in a poor prognosis.
