ABSTRACT

BACKGROUND: The aim of this study is to investigate the correlation of clinical and ultrasound parameters with characters of vulnerable atherosclerotic carotid plaque, as evaluated at preoperative magnetic resonance angiography (MRA), in patients submitted to carotid endarterectomy (CEA), in order to develop a clinical risk score for plaque vulnerability. METHODS: Preoperative data of patients submitted to CEA for significant carotid stenosis from January 1, 2012 to December 31, 2016 were retrospectively collected. The available case series was randomly divided into 2 groups, including a training (60%) and a validation series (40%). Data of plaque vulnerability were assessed at preoperative MRA scans. Univariate analysis was used on the training series to correlate the preoperative covariates available to the features of plaque vulnerability. Therefore, a backward selection procedure was performed again on the training series and on the validation series to assess if the same variables were associated to data of plaque vulnerability, in order to obtain a prediction model for the risk of plaque vulnerability. Odds ratios (ORs) with 95% confidence intervals were reported. P values <0.05 were considered statistically significant. RESULTS: The training case series consisted of 352 patients, while the validation case series of 248 patients. After univariate analysis and logistic regression, on the training and the validation series respectively, 6 variables were significantly associated to features of vulnerable plaque at preoperative MRA. These included male sex (OR 2.05), diabetes mellitus (OR 3.06), coronary artery disease (OR 1.95), neutrophil/lymphocyte ratio (OR 17.99), platelet counts (OR 1.03), and gray-scale median value (OR 0.84). A nomogram was then obtained from the final logistic model, in order to predict the probability of the presence of vulnerable carotid plaque, using a weighted points system. This risk score was then applied to the validation series. The validation data were found to have a C-index of 0.934. CONCLUSIONS: Sex, diabetes mellitus, coronary artery disease, neutrophil/lymphocyte ratio, platelet counts, and gray-scale median value were significantly associated to the features of vulnerable plaque at preoperative MRA in patients undergoing CEA. In particular, when combined together in a "risk score," these variables provided an accurate probability of the presence of a vulnerable plaque at MRA scans.

ABSTRACT

BACKGROUND: We report our "real-world" experience of endovascular repair of thoracic/thoraco-abdominal aortic lesions in patients treated from May 2002 to May 2017. METHODS: Data of all consecutive treated patients were retrospectively collected in a database and analyzed. Patients were divided into 4 groups: atherosclerotic thoracic/thoraco-abdominal aneurysms (TAA/TAAA) and floating thrombus (group A); acute complicated type B dissection (TBD), penetrating aortic ulcers (PAU) and intra-mural hematomas (IMH) in group B; chronic TBD evolving in TAA (group C); traumatic injuries (group D). Mortality, reinterventions and occurrence of neurological complications, both at 30 days and in the long term, were analyzed as primary outcomes for each group. RESULTS: Ninety-four patients were treated complessively, most for a TAA (55.3%). Thirty-days deaths and neurological complications were observed in group A only (5 cases each, 5.3%). A reintervention was necessary in 6 patients (6.4%) of group A. At 5 years, in group A survival was 62.8%±6.3% and freedom from neurological complication was 88.3%±4.2%. Neither deaths nor neurological complications were recorded in the other groups. No late aortic ruptures were recorded. Freedom from reintervention in group A was 54.7%±7.6% at 5 years and a reintervention was needed in all patients of group D. Overall, the main cause for reintervention was a type I endoleak. CONCLUSIONS: The endovascular repair of thoracic/thoraco-abdominal aortic lesions had acceptable mortality and neurological complication rates, both at 30 days and in the long term. Reinterventions in the long term occurred more frequently after TAA/TAAA and traumatic injuries, and were mainly required for a type I endoleak. ABSTRACT

AIMS: Genetic and environmental factors all interact in the risk of progression of valvular dysfunctions. Previous studies reported a relation between valve diseases and epicardial adipose tissue (EAT) thickness. The aim of this study was to verify the possible relationship between the molecular pattern of EAT related to IL-13 fibrogenic cytokine expression and valve dysfunction. METHODS AND RESULTS: A valvular heart disease (VHD) population was stratified according to their median EAT thickness (7 mm). The molecular expression of IL-13 in EAT is directly related to the molecular expression of genes associated with extracellular matrix (ECM) turnover, macrophage infiltration and promotion of the formation of ectopic calcific nodules involved in aorta coarctation and calcification. CONCLUSION: IL-13 gene expression in altered EAT is directly related to the expression of genes involved in ECM turnover and the formation of ectopic calcific nodules, suggesting measurements of EAT as a stratification risk factor for valve instability in the VHD patients.
