ABSTRACT

Psoriasis (PsO) is a chronic, immune-mediated, inflammatory and polygenic dermatosis associated with both physical and psychological burden that can be triggered by injury, trauma, infections and medications. The etiology of PsO is not fully elucidated but genetic, epigenetic and environmental factors are all likely to play a role. A case-control study was carried out to evaluate the frequency of the IL36G C>T (rs13392494) and the IL36G A>G (rs7584409) variants and their association with susceptibility, joint involvement and severity of PsO. The study included 154 patients with PsO and 154 controls from Brazilian population. The severity of PsO was assessed by the Psoriasis Area and Severity Index (PASI). The IL36G (rs13392494 and rs7584409) variants were genotyped by allelic discrimination assay using the real-time polymerase chain reaction. The association between the IL36G genetic variants and the study variables was analyzed in allelic, dominant, codominant, overdominant, recessive, and haplotype models. The main results were that PsO patients were older (p < 0.001) and had higher body mass index (p < 0.001) than controls; 95.8% of the patients had plaque PsO, 16.1% had psoriatic arthritis (PsA), and 27.9% had PASI > 10. The IL36G rs1339294 variant showed no association with PsO in all genetic models while the IL36G rs7584409 variant showed a protective effect in PsO. However, the G allele of the IL36G rs7584409 in the dominant model was positively associated with PASI > 10 (p = 0.031). Moreover, patients with the GG genotype of the IL36G rs7584409 variant had about 5.0 times more chance of PsA than those with the AA genotype (p = 0.014). Regarding the haplotypes, the C/A in a recessive model (CACA versus C/G and T/A carriers) was associated with PsO (p = 0.035) while the C/G haplotype in a dominant model (C/A carriers versus C/G and T/A carriers) showed a protective effect for PsO (p = 0.041). In conclusion, the G allele of the IL36G rs7584409 variant was associated with protection to PsO; however, in patients with PsO, this same allele was associated with moderate to severe disease and PsA. These results suggest that the IL36G rs7584409 variant may be used as a possible genetic biomarker to predict severity and joint involvement of PsO.

Resumo: Introdução: O curso de graduação em Medicina da Universidade Estadual de Londrina foi o segundo do Brasil a adotar currículo integrado e Aprendizagem Baseada em Problemas (PBL). Apesar de seu currículo inovador ter servido de referência a outras escolas, avaliações recentes mostraram a necessidade de reforma. Relato de experiência: As avaliações sistemáticas do curso indicaram os seguintes problemas: dificuldade de adaptação dos ingressantes à primeira série; desorganização da sequência de conteúdos ao longo do curso; falta de motivação docente para as atividades da primeira à quarta série; necessidade de incluir tópicos obrigatórios e novas tendências; e desgaste da metodologia (PBL) a partir da terceira série. Um amplo trabalho de reforma curricular foi iniciado, baseado na construção coletiva, culminando em mudanças, como: o desenho de uma primeira série mais acolhedora por meio da inclusão de nivelamento de ciências básicas e mentoria; a reorganização cronológica dos conteúdos; o redesenho dos módulos, agora organizados ao redor de grandes áreas ou especialidades afins; a adoção de metodologias ativas mais motivadoras; e a inclusão de novos conteúdos. Discussão: A adoção de novas metodologias ativas em substituição à PBL em alguns momentos apresenta vantagens estratégicas. A Aprendizagem Baseada em Equipes (TBL), mais estruturada que a PBL, pode ajudar na adaptação dos ingressantes à primeira série e facilitar a realização de metodologias ativas num contexto de escassez de docentes. A Aprendizagem Baseada em Casos (CBL) é mais motivadora e pode ser mais efetiva para desenvolver habilidades de raciocínio clínico nas séries pré-internato. Conclusão: O novo currículo, que incorpora as mudanças mencionadas, foi implantado em 2022. Novas avaliações mostrarão se as mudanças trarão melhorias ao curso em termos de adaptação, motivação e resultados de aprendizagem.


Abstract: Introduction: The undergraduate medical course of the State University of Londrina was the second in Brazil to adopt an integrated curriculum and Problem-Based Learning (PBL). Despite its innovative curriculum, which became a reference for other schools, new assessments showed the need to reform it. Experience Report: Systematic course evaluations showed some issues: difficulties in adaptation of new students attending the first year; disorganized sequence of contents throughout the course; teachers' lack of motivation for activities from first to the fourth years; need to include new contents; and deterioration of the methodology (PBL) in third and fourth years. A wide collective effort for curricular reform was initiated, which led to important changes, such as: a more welcoming first year, by including mentoring and activities for the leveling of basic knowledge; chronological reorganization of contents; redesign of modules around great areas of knowledge or related specialties; adoption of new and more motivating active learning and teaching methodologies, and the inclusion of new topics/trends. Discussion: The adoption of other active learning and teaching methodologies present strategic advantages in replacement for PBL. Team-Based Learning (TBL) is a more structured method than PBL, so it can help newcomers to adapt to the first year and make it easier to implement active methodologies in a context of teacher shortage. Case-Based Learning (CBL) generates higher motivation and can be more effective to foster the development of clinical reasoning skills in the preclinical years. Conclusion: The new curriculum, incorporating the changes described above, started in 2022. Further evaluations will show whether the changes will improve the course in terms of adaptability, motivation and learning outcomes.

O melanoma amelanótico acral é raro e pode mimetizar muitas entidades, como poroma écrino, carcinoma de células escamosas, verruga plantar e úlceras crônicas. Devido a esta variedade de possíveis diagnósticos diferenciais, é um diagnóstico difícil e, muitas vezes, tardio. As características da dermatoscopia podem ajudar no diagnóstico precoce. O clínico deve ter esse diagnóstico em mente ao se deparar com uma lesão rosada, de crescimento progressivo e formato irregular, principalmente se localizada nas mãos e nos pés.


Acral amelanotic melanoma is rare and can mimic many entities, such as eccrine poroma, squamous cell carcinoma, plantar wart, and chronic ulcers. Due to the variety of possible differential diagnoses, it is a challenging and frequently late diagnosis. Dermoscopy features can help in early diagnosis. The dermatologist should keep this diagnosis in mind when faced with a pink, progressively growing, irregularly shaped lesion, mainly if located on the hands and feet.

ABSTRACT

Sarcomas são neoplasias mesenquimais malignas, raras, que acometem, principalmente, crianças e adolescentes. O rabdomiossarcoma, subtipo oriundo da musculatura esquelética, é condição incomum em adultos, acometendo sítios de localização não habitual, crescimento rápido e de difícil tratamento. Apresenta-se caso de adulto jovem com nodulação em lóbulo auricular esquerdo, cuja análise histopatológica e imuno-histoquímica confirmou tratar-se de rabdomiossarcoma alveolar, o qual foi conduzido em conjunto com a Oncologia.


Sarcomas are rare and malignant mesenchymal neoplasms that mainly affect children and adolescents. Rhabdomyosarcoma, a subtype originating from skeletal muscle, is an uncommon condition in adults. It affects sites of unusual location, presents fast growth, and is challenging to treat. We report a case of a young adult with nodules in the left auricular lobe. The histopathological and immunohistochemical analysis confirmed the alveolar rhabdomyosarcoma, and treatment was conducted in association with Oncology.

ABSTRACT

O condiloma acuminado é causado pelo DNA-vírus, que pertence ao grupo papovavírus. Em menores de três anos, a transmissão tende a não ser por abuso sexual, mas, sim, de maneira vertical. Não existe consenso para o tratamento das verrugas anogenitais em crianças. O método escolhido é individualizado para o paciente, sendo a preferência por tratamentos menos traumáticos, com menor sequela. Embora ainda existam algumas restrições ao uso do imiquimode em menores de 12 anos, estudos já mostram o sucesso desta terapêutica nesta faixa etária. Será descrito um caso tratado com imiquimode creme a 5%, com resolução das lesões preexistentes


DNA-virus, which belongs to the papillomavirus group, causes Condylomata acuminata. In children under 3 years of age, transmission tends not to be due to sexual abuse but rather vertically. There is no consensus for the treatment of anogenital warts in children. The chosen method is individualized for each patient, preferring less traumatic treatments, with fewer sequelae. Although there are still some restrictions on the imiquimod use in children under 12, studies have already shown this therapy's success in this age group. We report a case treated with 5% imiquimod cream with the resolution of pre-existing lesions.

ABSTRACT

The objective of this study is to delineate the cellular adhesion molecule (CAM) profile and plasminogen activator inhibitor type 1 (PAI-1), and their association with metabolic syndrome (MetS) and carbohydrate metabolism biomarkers in psoriasis patients with mild and moderate severity. Sixty-seven patients with psoriasis as well as 102 healthy subjects were recruited. Insulin and Homeostatic Model Assessment of Insulin Resistance (HOMA-IR), but not glucose, were significantly higher in psoriasis than in controls. Psoriasis was characterized by increased plasma levels of platelet endothelial cell adhesion molecule 1 (PECAM-1), vascular cell adhesion molecule 1 (VCAM-1), intercellular adhesion molecule 1 (ICAM-1), E-selectin, and PAI-1 as compared with controls. Psoriasis diagnosis could explain 59.0% of CAM and PAI-1 variance, with a particularly strong impact on E-selectin (45.6%), VCAM-1 (32.7%), and PAI-1 (24.8%). Subjects with MetS showed significantly higher E-selectin and PAI-1 than those without MetS. Using VCAM-1, E-selectin, PAI-1 (all positively), and P-selectin (inversely) in a binary regression equation, it was found that 87.6% of all patients were correctly classified with a sensitivity of 92.5% and a specificity of 84.3%. CAM and PAI-1 were correlated with carbohydrate metabolism biomarkers (glucose, insulin, and HOMA-IR). In conclusion, CAM levels are associated with psoriasis diagnosis and MetS may influence E-selectin and PAI-1 concentrations. More studies are needed to verify the causality among these factors, as well as their relation to the different degrees of disease severity.

Objetivo: Explorar la evolución del proceso de afrontamiento y adaptación de 11 casos de adultos que experimentaron un trauma músculo esquelético, por accidentes de tránsito, durante la estancia hospitalaria en la Clínica Universidad de la Sabana en 2015. Método: Estudio de casos que permitió describir y comparar el proceso de afrontamiento y adaptación en 11 personas. Se aplicó la Escala de medición del proceso de afrontamiento y adaptación versión abreviada en español, al ingreso y al egreso hospitalario. En esta investigación se tuvo en cuenta los principios éticos correspondientes, se obtuvo el consentimiento informado de los participantes y las autorizaciones de la Subcomisión de Investigación de la Facultad de Enfermería y Rehabilitación de la Universidad de la Sabana y de la Clínica donde se llevó a cabo el estudio. Resultado: Los hallazgos de este estudio muestran para ocho de los once casos una tendencia a mantenerse en nivel alto de afrontamiento o a pasar de nivel medio a alto desde el ingreso hasta el egreso hospitalario. Conclusión: La mayoría de los casos tuvo un proceso de afrontamiento y adaptación dirigido al problema, donde se encontró interés por resolver dudas relacionadas con la situación antes de actuar, utilización de experiencias pasadas consideradas como útiles, adopción de nuevas habilidades para afrontar las situaciones difíciles y el uso de soluciones alternativas y recursos (AU)


Objective: Exploring the evolution of coping and adaptation strategies by 11 cases of adults who had suffered traffic accident-related musculoskeletal trauma during their stay in hospital at Clínica Universidad de la Sabana during 2015. Methods: Studying the aforementioned cases enabled describing and comparing the 11 patients' coping and adaptation strategies. The abridged Spanish version of Roy's Coping Adaptation Processing Scale (CAPS) was used for measuring such strategies; it was used on patient admission to and discharge from hospital. This research took the relevant ethical principles into account; the patients gave their written informed consent and the Universidad de la Sabana and its hospital where the study was carried out authorised the study (i.e. the Faculty of Nursing and Rehabilitation's Research Subcommittee). Results: This study's findings showed that eight of the eleven cases maintained a high coping ability, or improved from moderate (on admission to hospital) to high level ability (on discharge). Conclusion: Problem-centred coping and adaptation strategies were involved in most cases. Patients approached this through resolving situation-related questions before acting, using past experiences considered useful, adopting new skills to cope with difficult situations and using alternative solutions and resources (AU)

Objective: To explore the process of coping and adaptation of 11 cases of adults who experienced musculoskeletal trauma by traffic accidents during the hospital stay at the Clinic Universidad de la Sabana in 2015. Method: Case studies allowed to describe and comparing the process of coping and adaptation in 11 people. It was used the Scale measurement of coping and adaptation process abridged version in Spanish, it was applied at admission and hospital discharge. This research took into account the relevant ethical principles, the informed consent process and authorization of the Subcommittee on Research of the School of Nursing and Rehabilitation Sabana University and Clinic where was obtained the study. Result: The findings of this study showed for eight of the eleven cases a tendency to keep high level of coping or move from medium to high level from admission to hospital discharge. Conclusion: Most cases had a process of coping and adaptation addressed to the problem. It was approached by participants through the clarifying questions related to the situation before acting, the use of past experiences considered useful, the adoption of new skills to cope with difficult situations and the use of alternative solutions and resources. ABSTRACT

O pioderma gangrenoso é uma dermatose neutrofílica, rara da pele e do tecido subcutâneo, caracterizada por um processo necrosante progressivo e doloroso. A conduta no Pioderma gangrenoso requer, com frequência, o uso de drogas sistêmicas, tais como: corticoides, sulfonas e imunossupressoras, seja de maneira isolada, seja em combinação. Muitos relatos, na literatura, documentam o tratamento com êxito do Pioderma gangrenoso, com a oxigenoterapia hiperbárica. No nosso caso, uma jovem com lesões extensas e muito dolorosas, o tratamento com oxigenoterapia hiperbárica associado ao corticoide e imunossupressor promoveu cicatrização, com excelente resultado, com fechamento rápido da lesão e diminuição do desconforto.

Pyoderma Gangrenosum is a rare neutrophilic dermatosis of skin and subcutaneous tissue characterized by a painful and progressive necrotizing process. The management of pyoderma gangrenosum often requires systemic drug therapy, such as corticosteroids, sulfones or immunosuppressants, either alone or in combination. Several reports in the literature document the successful treatment of pyoderma gangrenosum with hyperbaric oxygen therapy. In our case, hyperbaric oxygen therapy associated with corticoids and immunosuppressants promoted healing of large and very painful lesions in an adolescent girl with an excellent outcome, including rapid wound closure and decreased discomfort.

Few cases of dermatosis neglecta have been reported in the medical literature, although the diagnosis is well-known to dermatologists. Recognizing this condition avoids unnecessary, aggressive diagnostic and therapeutic procedures. This case report discusses two cases of this condition in female patients in whom the dermatosis developed as a result of deliberate or unconscious neglect of personal hygiene.

Poucos são os casos de dermatitis neglecta relatados na literatura, apesar de o diagnóstico ser conhecido pelos dermatologistas. Reconhecer o quadro evita condutas diagnósticas e terapêuticas agressivas e desnecessárias. Os autores relatam dois casos de pacientes do sexo feminino nos quais a dermatite se desenvolveu como resultado intencional ou não de negligência com o asseio pessoal.
