ABSTRACT

Desmoid tumors (DTs) are a rare and biologically heterogeneous group of locally aggressive fibroblastic neoplasm: their biological behavior spectrum ranges from indolent to aggressive tumors. DTs are classified as intra-abdominal, extra-abdominal, and within the abdominal wall lesions.It is well known that abdominal and extra-abdominal DTs are associated with familial adenomatous polyposis (FAP) and Gardner syndrome. Possible risk factors are prior trauma/surgery, pregnancy, and oral contraceptives.There was a real revolution in the management of DT: from aggressive first-line approach (surgery and radiation therapy) to a more conservative one (systemic treatment and "wait-and-see policy").In these clinical settings, radiologists play an important role for assessing lesion resectability, evaluating recurrence, monitoring the biological behavior if an expectant management is chosen, and assessing response to systemic treatment as well as to radiation therapy.Awareness of common locations, risk factors, and imaging features is fundamental for a correct diagnosis and an adequate patient management.

RESUMEN Latungiasisesuna ectoparasitosistransitoria en el ser humano, que se produce a causa de la penetración en la piel de la hembra grávida de la "pulga de arena", Tunga penetrans. También es conocida como "nigua", "bichodopé", "sandflea",entre otras denominaciones. Es una enfermedad endémica en áreas rurales y playas de Centro y Sudamérica, Caribe y África Subsahariana. En Argentina se la encuentra en zonas del Noreste y Mesopotamia. Debido al incremento de los destinos vacacionales a dichas regiones tropicales, comenzaron a observarse casos de tungiasis fuera de las áreas endémicas (casos importados). Comunicamos el caso de una niña argentina, que vacacionó en las playas caribeñas y adquirió la infestación por Tunga penetrans, la cual evolucionó de manera llamativa, manifestando un fenómeno isotópico.


SUMMARY Tungiasis is a transitory ectoparasitosis caused by penetration into the skin of the female flea Tungapenetrans, also known as "nigua", "bichodo pé", or "sand flea". It's an endemic disease that can be found at rural zones, Caribbean beaches and Sub-Saharan Africa. In Argentina, it's distributed in the northeastand Mesopotamia. Because of the increasement of holiday destinations, in these tropical regions, cases of tungiasis were reported outside the endemic areas. People who went to those zones, carried the disease to their birthplaces (imported Infestations). We report the case of a little Argentinian girl who went to a Caribbean beach and acquired the infestation by tungapenetrans, which made its develop in a striking way; showing an isotopic phenomenon.

ABSTRACT

We describe two cases in which delayed rotation of the cerebellar vermis simulated a Dandy-Walker malformation (DWM) on early second-trimester magnetic resonance imaging (MRI). Two pregnant women with suspected fetal posterior fossa anomaly on ultrasound examination underwent fetal MRI at 21 (Case 1) and 19 (Case 2) weeks' gestation. In both cases, upward rotation of the cerebellar vermis was noted; on midsagittal imaging, the brainstem-vermis angle was 28° and 43°, respectively, while cerebellar morphometry showed a reduced vermian anteroposterior diameter compared to reference data. The posterior fossa appeared to be mildly enlarged, while all other findings were normal. Follow-up MRI at 28 + 3 weeks' gestation (Case 1) and at 1 postnatal year (Case 2) showed completely normal findings. Both children had normal psychomotor development and neurological examinations at 1 year of age. Incomplete rotation of the cerebellar vermis can be a physiological finding on early second-trimester fetal MRI examination and can simulate DWM or other forms of cerebellar hypoplasia. Embryologically, delayed permeabilization of Blake's pouch could account for the delayed vermian rotation. Follow-up imaging at a later gestational age is crucial to ensure that this condition is not over-reported and to avoid the potential risk of unnecessary pregnancy interruption. ABSTRACT

Las dermatosis plasmocitarias comprenden un conjunto de enfermedades inflamatorias de causa desconocida y evolución incierta. Se han descrito para ellas múltiples tratamientos con resultados dispares y éxitos terapéuticos anecdóticos, con recaídas posteriores en la mayoría de los casos. Presentamos en este trabajo, además de una actualización y revisión del tema, cuatro dermatosis plasmocitarias que tuvimos oportunidad de ver en nuestro Servicio: la conocida balanitis de Zoon, las poco descritas vulvitis de Zoon y queilitis plasmocitaria y la infrecuente plasmocitosis cutánea de la que hay muy pocos reportes a nivel mundial. Queremos con este trabajo integrar didácticamente las dermatosis benignas, producidas directamente por la infiltración de plasmocitos, sin causa subyacente demostrable.


Cutaneous plasmocitosys is a group of skin's inflammatories disorders with unknown cause and uncertain evolution. In all treatments cases they describe different and uneven results with a few therapeutic success, most of them with reopen symptoms. In the following work, besides an updates and reviews, we present four cases that we received in our Service. The known balanitis of Zoon, the almost unknown vulvitis of Zoon, plasma cell cheilitis and finally the very unfrequently cutaneous plasmocytosis, which is very worldwide unreported. A purpose of this work have a didactic will to include whole skin disorders directly caused for the plasma cells infiltration with no causes in background.

ABSTRACT

A statistical study was done on two groups of patients. The first group included 128 subjects of less than 65 years of age. The second group comprised 75 subjects aged 65 years or older. The purpose of the study was to identify the most relevant clinical aspects of myocardial infarction in older aged patients. The following variables were taken into consideration: sexual distribution, predisposing factors, causative factors, initial symptoms, site of infarction, physical and instrumental indications, cardiac and extracardiac complications, immobilization time, recovery time, residual aspects at patient discharge, mortality and tyme and type of death. The analysis was performed using both non parametric X2 test and correlating some variables with age independently of the subdivision of cases in groups, according to the method of multiple step-wise and simple regression. In the older age group the most significant statistical data was: the major incidence of infarcts was in women; the prevalence of predisposing factors such as hypertension and diabetes; the most frequent presentation of initial atypical symptoms; the most elevated incidence of hemodynamic complications and the highest mortality.
