ABSTRACT

En el presente trabajo se exponen las medidas estructurales y logísticas, así como la práctica clínica planificada, para poder responder a la pandemia producida por el virus SARS-CoV-2 en el Departamento de Psiquiatría y Salud Mental del Hospital Central de la Defensa «Gómez Ulla». La planificación de la función asistencial se dividió en cinco grupos: los pacientes psiquiátricos ingresados en la Unidad de Hospitalización Breve; los pacientes con patología psiquiátrica ingresados en otros Servicios diferentes de Psiquiatría; los pacientes ambulatorios atendidos en Consultas Externas; los familiares de los pacientes ingresados por la COVID-19; el personal sanitario del Hospital Central de la Defensa «Gómez Ulla». En función de las necesidades de estos grupos asistenciales se realizó una planificación integral de la atención a los mismos. Durante el periodo 14 de marzo al 30 de mayo el 13% del grupo de profesionales del departamento de psiquiatría y salud mental, presentó síntomas moderados- graves de COVID-19; el 19% de los pacientes psiquiátricos ingresados en la unidad de hospitalización fueron COVID19 positivos, no falleciendo ninguno. El 74% de las interconsultas realizadas fueron sobre pacientes ingresados por COVID-19 que presentaron mayoritariamente cuadros confusionales de diversa intensidad o psicosis secundarias al uso de fármacos en el tratamiento activo del COVID-19. Se hicieron 4.185 llamadas a familiares, de las cuales el 14% (n=575) fueron a demanda de los propios familiares. Se hicieron más de 200 videollamadas y se mantuvo de forma telemática el 100% de las consultas externas


In this work the structural and logistical measures are exposed, as well as the planned clinical practice, to be able to respond to the pandemic caused by the virus SARS-CoV-2 in the Department of Psychiatry and Mental Health of the Central Defense Hospital «Gómez Ulla». The planning of the care function was divided into five groups: psychiatric patients admitted to the Brief Hospitalization Unit; patients with psychiatric pathology admitted to other different Psychiatric Services; outpatients treated in Outpatient Consultations; the relatives of the patients admitted by COVID-19; the health personnel of the Central Defense Hospital «Gómez Ulla». Based on the needs of these care groups, comprehensive care planning was carried out. During the period March 14 to May 30, 13% of the staff presented moderate-severe symptoms of COVID-19; 19% of the psychiatric patients admitted to the hospitalization unit were COVID19 positive, none of whom died. 74% of the inter-consultations carried out were on patients admitted for COVID-19 who presented mostly confusional symptoms of varying intensity or psychosis secondary to the use of drugs in the active treatment of COVID-19. 4.185 calls were made to family members, of which 14% (n = 575) were at the request of the family members themselves. More than 220 video calls were made and 100% of the external consultations were kept online

ABSTRACT

INTRODUCCIÓN: Análisis sobre la importancia de realizar una óptima selección de los aspirantes a Ofciales y Subofciales de las Fuerzas Armadas. OBJETIVOS: Revisar la frecuencia de las evaluaciones psicológicas realizadas a los aspirantes a ofciales y subofciales de las Fuerzas Armadas y las variables implicadas, así como describir el contexto normativo en el que se enmarca y las difcultades que se plantean. MATERIAL Y MÉTODO: Estudio observacional, transversal y retrospectivo de los aspirantes a ofciales y subofciales en julio y agosto de 2016, considerados "no aptos provisionales" en el Inventario de Personalidad de las Fuerzas Armadas (IPFAS). Estos aspirantes fueron evaluados por psicólogos en la Unidad de Reconocimientos del HCD Gómez Ulla para determinar si se encontraban dentro de los criterios de exclusión médica de la normativa vigente. RESULTADOS: Se identifcaron 114 aspirantes (7.39%) que obtuvieron puntuaciones que indicaban desajustes en la personalidad, de un total de 1542 que realizaron su reconocimiento médico. La valoración para éstos consistió en entrevista clínica, Cuestionario de Salud e Inventario Clínico Multiaxial de Millon (MCMI-III). Resultaron "no aptos" 32 aspirantes (2,07% del total). CONCLUSIONES: Sería benefcioso aprovechar las medidas de personalidad aportadas por el IPFAS para seleccionar a los futuros mandos en base a la posesión de atributos relacionados con el éxito profesional independientemente de la valoración médica que explora patología mental, así como encontrar una solución para poder decidir sobre la aptitud de los aspirantes que distorsionan e invalidan las pruebas como resultado de una deseabilidad social anormalmente elevada


INTRODUCTION: Analysis on the importance of performing optimum selective process of candidates Armed Forces offcers and NCO's. Objetive: To assess the frequency of the psychological evaluations performed to candidates offcers and NCO's and the variables involved, as well as to describe the normative context and the diffculties that may arise. MATERIAL AND METHOD: Observational, transversal and retrospective study of candidates offcers and NCO's declared as "provisionally not suitable" according to the Armed Forces Personality Inventory (IPFAS) carried out during July and August 2016. These people were assessed at the Reconnaissance Unit located in HCD Gomez Ulla in order to determine if they met the medical exclusion criteria according to existing legislation. RESULTS: 114 candidates out of a total of 1542 (7.39%) were identifed whose scores indicated personality misadjustments. For these individuals, the assessments consisted of clinical interview and Millon Clinical Multiaxial Inventory. 32 candidates were found "not suitable" (2.07% of the total). Conclussions: It would be convenient to take beneft from personality measurements provided by IPFAS in order to select future commanders based on possession of personal qualities or attributes related to professional success, regardless of the medical assessment exploring mental pathology, as well as coming up with a solution allowing to take a decision on the suitability of the candidates that distort and invalidate the tests as a consequence of a social desirability abnormally elevated

ABSTRACT

El herpes zóster está producido por la reactivación del virus de la varicela zóster tras una primoinfección varicelosa. Es infrecuente en pediatría. En la mayoría de las ocasiones que aparece en niños, tiene unas manifestaciones benignas en comparación con los adultos. El diagnóstico es clínico y los estudios de laboratorio no suelen ser necesarios. El tratamiento debería ser sintomático en la mayoría de los pacientes, reservándose el aciclovir oral o intravenoso para situaciones de riesgo. Respecto a otros antivirales empleados en adultos, debido a la falta de estudios al respecto, no existe consenso sobre su uso en niños. Aprovechamos la comunicación de un caso clínico para hacer una revisión y actualización del tema, incidiendo sobre todo en los aspectos diagnósticos y terapéuticos (AU)


Herpes zoster or shingles is caused by reactivation of varicella-zoster virus long time after the primary infection, varicella. It is unusual in pediatrics. Contrary to the way it can affect adults zoster manifestation is mild if it appears in children. Diagnosis is based on distinctive clinical appearance and laboratory tests are not usually required. Symptomatic treatment of skin lesions should be the best choice to take while systemic oral or intravenous acyclovir should be restricted only to special cases. Regarding other antiviral drugs of proved efficacy in adults, no evidence is gathered for their use in children. To sum up, our main aim is to review and update this subject stressing diagnosis and treatment (AU)

The authors describe the school children health activities developed by the Primary Health Care Area 1 in Madrid Teams from the 1996-97 academic year until the 2004-05 year During this time period, 559 EPS activities were carried out, 56% belonged to the Program to Promote Health in School in Area 1. The other 44% are projects developed by sanitary professionals. These activities were developed in 70 educational centers, focusing on different groups: 91% on students, 6% on parents and 3% on teachers. This analysis demonstrates the leadership of nursing professionals have in the EPS school program. These activities worked mainly with students.

Se describen las actividades de salud escolar desarrolladas por los Equipos de AP del Área 1 de Madrid desde el curso 1996-97 hasta el 2004-05. Durante el periodo analizado se han realizado 559 actividades de EpS, 56% pertenece al Programa de Promoción de la Salud en la Escuela del Área 1. El 44% restante son proyectos elaborados por los profesionales sanitarios. Estas actividades se desarrollaron en 70 centros educativos, enfocadas a diferentes grupos: alumnos (91%), padres (6%) y profesorado (3%). Se demuestra el liderazgo de los profesionales de Enfermería en EPS en la escuela. Se trabajó, mayoritariamente, con alumnos(AU)
