ABSTRACT

Unilateral pharmacologic mydriasis is one of the differential diagnoses of anisocoria. This is a clinical case of a 37-year-old male patient admitted to the ophthalmology emergency department with unilateral mydriasis, an infrequent side effect of the antihistaminic drug azelastine. A comprehensive medical history including ocular medication was essential to avoid the need for additional tests and to exclude life-threatening conditions responsible for a similar presentation.

ABSTRACT

Objectives: This study aims to analyze the occurrence of delirium in critically ill older patients and to identify predictors of delirium. Methods: This prospective study included critically ill older patients admitted into level II units of Intensive Care Medicine Department of a University Hospital. Patients with Glasgow Coma Scale score ≤11, traumatic brain injury, terminal disease, history of psychosis, blindness/deafness, or inability to understanding/speaking Portuguese were excluded. The Confusion Assessment Method-Short Form (CAM-4) was used to assess the presence of delirium. Results: The final sample (n = 105) had a median age of 80 years, most being female (56.2%), widowed (49.5%), and with complete primary education (53%). Through CAM-4, 36.2% of the patients had delirium. The delirium group was more likely to have previous cognitive decline (48.6% vs 19.6%, P = .04) and severe dependency in instrumental activities of daily living (34.3% vs 14.8%, P = .032), comparing with patients without delirium. The final multiple logistic regression model explained that patients with previous cognitive decline presented a higher risk for delirium (odds ratio: 4.663, 95% confidence Interval: 1.055-20.599, P = .042). Conclusions: These findings corroborate previous studies, showing that cognitive decline is an independent predictor for delirium in older patients. This study is an important contribution for the knowledge regarding the predictors of delirium. The recognition of these factors will help to identify patients who are at high risk for this syndrome and implement early screening and prevention strategies. However, further studies with larger samples, recruited from other clinical settings as well as analyzing other potential factors for delirium, will be needed.

ABSTRACT

This study aimed to identify the teleost fish species sold in Bragança, a major fishing hub on the north coast of Brazil. The COI gene analysis was performed for the identification of fish species. The local market uses common names that are not accurate and do not reflect the diversity of the species. 204 sequences were obtained, with 119 haplotypes. 83 species were identified by comparing with public databases and constructing phylogenetic trees, with Carangidae being the most prevalent family. The study also found Haemulon atlanticus, Menticirrhus cuiaranensis and Hoplias misioneira, a newly described species from the Amazon basin, among the samples. Additionally, 73 commercial names were recorded, including 10 categories, and the illegal trade of Epinephelus itajara was detected. The DNA Barcode method proved to be effective for discriminating the species. The study highlights that common and commercial names are vague and underestimate the fish diversity, and that Brazil needs to revise its regulations for commercial and scientific names.

Methods Cervical CT scans of 50 adults were measured according to predefined screw trajectories of C1-C2 transarticular (C1C2TA), C1 lateral mass (C1LM), C2 pedicle (C2P), C2 pars and C2 laminar (C2L) screws. For each of these trajectories, screw length and angles were measured and compared between males and females. Results For the C1C2TA screw trajectory, the mean length, medial, and cranial angles were 34.12 ± 3.19 mm, 6.24° ± 3.06, and 59.25° ± 5.68, respectively, and for the C1LM screw trajectory, they were 27.12 ± 2.15 mm, 15.82° ± 5.07, and 13.53° ± 4.80, respectively. The mean length, medial, and cranial angles for the C2P screw trajectory were 23.44 ± 2.49 mm, 27.40° ± 4.88, and 30.41° ± 7.27, respectively; and for the C2 pars screw trajectory, they were 16.84 ± 2.08 mm, 20.09° ± 6.83, and 47.53° ± 6,97. The mean length, lateral, and cranial angles for the C2L screw trajectory were 29.10 ± 2.48 mm, 49.80° ± 4.71, and 21.56° ± 7.76, respectively. There were no gender differences except for the lengths of the C1C2TA ( p = 0,020) and C2L ( p = 0,001) screws, which were greater in males than in females. Conclusion The present study provides anatomical references for the posterior atlantoaxial fixation in a Portuguese population. These detailed data are essential to aid spine surgeons to achieve safe and effective screw placement.

Resumo Objetivo O presente estudo tem como objetivo avaliar o comprimento e os ângulos de trajetória do parafuso para fixação atlantoaxial posterior em uma população portuguesa por meio do estudo de tomografia computadorizada (TC) cervical. Métodos Tomografias computadorizadas cervicais de 50 adultos foram analisadas quanto às trajetórias pré-definidas dos parafusos transarticulares C1-C2 (C1C2TA), na massa lateral de C1 (C1LM), no pedículo de C2 (C2P) e na pars de C2 e C2 laminar (C2L). O comprimento e os ângulos dos parafusos em cada uma destas trajetórias foram medidos e comparados entre homens e mulheres. Resultados O comprimento médio e ângulos medial e cranial da trajetória do parafuso C1C2TA foram de 34,12 ± 3,19 mm, 6,24° ± 3,06 e 59,25° ± 5,68, respectivamente; as medidas da trajetória do parafuso C1LM foram 27,12 ± 2,15 mm, 15,82° ± 5,07 e 13,53° ± 4,80. Estes dados detalhados são essenciais para ajudar os cirurgiões de coluna a colocar os parafusos de maneira segura e eficaz.

ABSTRACT

The Latarjet procedure is the most popular surgical procedure to treat anterior glenohumeral (GH) instability in the presence of large anterior glenoid bone defects. Even though the placement of the bone graft has a considerable influence on its efficacy, no clear indications exist for the best graft position. The aim of this study was to investigate the influence of the medial-lateral positioning of the bone graft on the contact mechanics and GH stability due to the bone block effect. Four finite element (FE) models of a GH joint, with a 20% glenoid bone defect, treated by the Latarjet procedure were developed. The FE models differed in the medial-lateral positioning of the bone graft, ranging from a flush position to a 4.5 mm lateral position with respect to the flush position. All graft placement options were evaluated for two separate shoulder positions. Anterior GH instability was simulated by translating the humeral head in the anterior direction, under a permanent compressive force, until the peak translation force was reached. Joint stability was computed as the ratio between the shear and the compressive components of the force. The lateralization of the bone graft increased GH stability due to the bone block effect after a 3 mm lateralization with respect to the flush position. The increase in GH stability was associated with a concerning increase in peak contact pressure due to the incongruous contact between the articulating surfaces. The sensitivity of the contact pressures to the medial-lateral positioning of the bone graft suggests a trade-off between GH stability due to the bone block effect and the risk of osteoarthritis, especially considering that an accurate and consistent placement of the bone graft is difficult in vivo.

ABSTRACT

Acorn is the fruit of holm oak (Quercus rotundifolia), being mainly used nowadays to feed animals, however a substantial part remains in the fields without any valorization. Underexploited crops are gaining new interest, driven by food security concerns and health benefits potential as well. In the present work, it was studied the physicochemical characteristics and functional perspective of acorn flour, as an ingredient for human diet. The study included nutritional composition analysis, phenolic compounds profile through HPLC, starch content and its microstructure, fibre, and pasting properties assessment. Acorn flour presented a high content in fat, particularly monounsaturated and polyunsaturated (oleic and linoleic acids), and high minerals content in particular K. Concerning phenolic profile, rutin, catechin, ellagic acid, gallic acid, and syringic acid were identified. In regards to technological profile, fibre was mainly insoluble, with around 11%, and starch content was 50%. Its pasting behaviour revealed a high gelatinization temperature (85 °C), with low breakdown, and higher retrogradation consistency. These results show acorn flour potential as a valuable and sustainable multipurpose food ingredient.

ABSTRACT

Sustainability is increasingly important in the wastewater treatment sector, and benchmarking studies are needed to identify the best practices in using resources efficiently. This work addresses this need by characterizing and benchmarking this sector in Portugal. Data from around 120 service providers, from 2015 to 2019, regarding inputs consumption, desirable and undesirable outputs, are used in a data envelopment analysis to benchmark these operators and to determine improvement potentials. A productivity change analysis revealed an increase from 2015 to 2019, essentially due to an increased technical efficiency. On the other hand, no real technological change seems to have occurred. A positive effect of the service providers dimension and certifications (quality, environmental and energy) on efficiency was found. Service providers with concessionary governance models and urban typology presented the highest efficiencies, whereas treatment plants underuse contributed negatively to efficiency. These results can inform the definition of strategic policies for the water sector, namely concerning the aggregation of small providers. The proposed methodology, based on the use of a comprehensive new set of studied variables and explanatory factors, allowed to determine the best characteristics regarding the techno-economic efficiency of Portuguese service providers, and can potentially be applied to identify role models in other countries.

Background: Mandibular movements (MM) can offer important information regarding temporomandibular joint health. Objective: Evaluate MM and the ratio between maximum jaw opening and mandibular lateral excursion in Brazilians with and without temporomandibular disorder (TMD). Methods: A cross-sectional study was conducted with 801 individuals between five and 80 years. All individuals answered a screening questionnaire; and MM were measured using digital calipers. The Mann-Whitney test was used to compare the differences between the sexes and groups with and without TMD. The Kruskal-Wallis test was used to determine differences in MM among age groups. Results: MM were smaller in individuals with TMD than in those without TMD. The ratio between jaw opening and lateral excursion was 5.23 in women without TMD and 5.59 in women with TMD. In males, the ratio was 4.75 and 5.52 in individuals without and with TMD, respectively. Conclusion: MM are smaller in Brazilians with TMD, whereas the ratio between jaw opening and lateral excursion is larger.


Introdução: Os movimentos mandibulares (MM) podem oferecer informações importantes sobre a articulação temporomandibular. Objetivo: Avaliar os MM e a relação entre abertura máxima e excursão lateral mandibular em brasileiros com e sem disfunção temporomandibular (DTM). Métodos: Estudo transversal com 801 brasileiros entre cinco e 80 anos. Os indivíduos responderam um questionário de triagem; e os MM foram medidos com paquímetro digital. O teste de Mann-Whitney foi utilizado para comparar diferenças entre sexos e grupos com e sem DTM. O teste de Kruskal-Wallis foi aplicado para determinar diferenças nos MM entre faixas etárias. Resultados: Os MM foram menores nos indivíduos com DTM. A razão entre abertura mandibular e excursão lateral foi de 5,23 em mulheres sem DTM e 5,59 em mulheres com DTM. Nos homens, a proporção foi de 4,75 e 5,52 em indivíduos sem e com DTM, respectivamente. Conclusão: Os MM são menores em brasileiros com DTM, enquanto que a relação entre abertura mandibular e excursão lateral é maior.

ABSTRACT

Re-using and adding value to by-products is one of the current focuses of the agri-food industry, following the Sustainable Development Goals of United Nations. In this work, the by-products of four plants, namely chestnut burr, acorn peel, olive leaf, and grape stem were used as coagulants to treat elderberry wastewater (EW), a problematic liquid effluent. EW pre-treatment using these natural coagulants showed promising results after pH and coagulant dosage optimization. However, the decrease in total organic carbon (TOC) was not significant, due to the addition of the plant-based natural coagulants which contain carbon content. After this pre-treatment, the photo-Fenton advanced oxidation process was selected, after preliminary assays, to improve the global performance of the EW treatment. Photo-Fenton was also optimized for the parameters of pH, H2O2, Fe2+, and irradiance power, and the best conditions were applied to the EW treatment. Under the best operational conditions defined in the parametric study, the combined results of coagulation-flocculation-decantation (CFD) and photo-Fenton for chestnut burr, acorn peel, olive leaf, and grape stem were, respectively, 90.2, 89.5, 91.5, and 88.7% for TOC removal; 88.7, 82.0, 90.2 and 93.1%, respectively, for turbidity removal; and finally, 40.6, 42.2, 45.3, and 39.1%, respectively, for TSS removal. As a final remark, it is possible to suggest that plant-based coagulants, combined with photo-Fenton, can be a promising strategy for EW treatment that simultaneously enables valorization by adding value back to food by-products.

ABSTRACT

Spontaneous hemoperitoneum in pregnancy is a rare complication resulting in high maternal and fetal morbidity and mortality. The authors describe the case of a pregnant woman presenting at 32 weeks of gestation with abdominal pain and free abdominal fluid on ultrasound. Laparotomy revealed a hemoperitoneum resulting from a suspected ruptured varices on the uterine posterior surface. A live newborn was delivered by cesarean-section, and hemorrhage was controlled with sutures and compression. Clinicians should be aware of this diagnosis when a pregnant woman presents with abdominal pain, anemia or hypovolemic shock. Early intervention will avoid poor outcomes for both the mother and the fetus.


O hemoperitoneu espontâneo na gravidez é uma complicação rara, associado a morbimortalidade materna e fetal elevadas. Os autores descrevem o caso clínico de uma grávida que, às 32 semanas de gestação, recorreu ao serviço de urgência por dor abdominal, apresentando líquido livre na ecografia abdominal. Na laparotomia identificou-se um hemoperitoneu com aparente origem num vaso varicoso da face posterior do útero. Fez-se cesariana, resultando um nado-vivo. A hemorragia foi controlada com pontos hemostáticos e compressão. Os médicos devem considerar este diagnóstico perante uma grávida com queixas de dor abdominal, anemia aguda ou sinais de choque hipovolémico. Uma intervenção precoce prevenirá maus desfechos maternos e fetais.

ABSTRACT

Baking bread without gluten presents many challenges generally related with poor sensorial and nutritional characteristics, and strategies to overcome this issue are needed. Despite many gluten-free (GF) bread studies, to the best of our knowledge, few are dedicated to sweet GF bread. Sweet breads have traditionally been an important type of food and are still frequently consumed worldwide. Apple flour is naturally GF, and is obtained from apples which do not accomplish market quality requirements and are being wasted. Apple flour was, therefore, characterized in terms of nutritional profile, bioactive compounds, and antioxidant capacity. The aim of this work was to develop a GF bread with incorporation of apple flour, in order to study its effect on nutritional, technological, and sensory characteristics of sweet GF bread. Additionally, in vitro starch hydrolysis and glycemic index (GI) were also analyzed. Results demonstrated the influence of apple flour in dough's viscoelastic behavior, increasing G' and G''. Regarding bread characteristics, apple flour led to better acceptance by the consumer, with firmness increasing (21.01; 26.34; 23.88 N), and consequently specific volume decreasing (1.38; 1.18; 1.13 cm3/g). In addition, an increase of bioactive compounds content and antioxidant capacity of the breads were revealed. As expected, the starch hydrolysis index increased, as well as GI. Nevertheless the values were really close to low eGI (56), which is a relevant result for a sweet bread. Apple flour showed good technological and sensory properties as a sustainable and healthy food ingredient for GF bread.

ABSTRACT

BACKGROUND: Some pancreatic tumors considered resectable on the preoperative staging are unresectable during surgical exploration. Moreover, some patients subjected to tumor resection have an early recurrence. METHODS: Patients with pancreatic or periampullary carcinoma diagnosed between January 2005 and August 2017 at the Prof. Doutor Fernando Fonseca Hospital were retrospectively analyzed. Biochemical and radiological inflammatory biomarkers were compared according to disease staging at diagnosis, intraoperative staging and early recurrence (<6 months). RESULTS: Three hundred ninety-one patients were included. Neutrophil-to-lymphocyte ratio (NLR), lymphocyte-to-monocyte ratio, C-reactive-protein-to-albuminemia ratio (CRP/ALB), Prognostic Nutritional Index, modified Glasgow Prognostic Score and CA19-9 were associated with metastatic disease at diagnosis. NLR, CRP/ALB, mGPS and CA19-9 were independent predictors of disease staging at diagnosis on multivariate analysis. One hundred eight patients underwent surgery, of which 23.8% were found to have unresectable disease at intra-operative staging. 26.9% had early disease recurrence. CRP/ALB and CA19-9 were significantly higher in patients with evidence of disease at 6 months postoperatively. Computed tomography sarcopenia index HUAC was significantly lower in patients with evidence of disease at 6 months postoperatively. When adjusted for histology, none of the biomarkers were independent predictors of unresectable disease or early recurrence. CONCLUSIONS: NLR, CRP/ALB, mGPS and CA19-9 at diagnosis were predictors of disease staging with low performance. Preoperative inflammatory biomarkers were not predictors of unresectable disease or early recurrence.

O livro, "Narrativas, afetos e saberes coletivos: caminhos do PET-Saúde/Interprofissionalidade do Campus UFRJ ­ Macaé", é fruto do pensamento coletivo do corpo social da Universidade Federal do Rio de Janeiro ­ Campus Macaé e dos trabalhadores de saúde de Macaé, cidade do Norte Fluminense. Todos, co-participantes do Projeto PET-Saúde/Interprofissionalidade. Assim, evidenciamos ser este um marco histórico local, onde a academia, a gestão e os trabalhadores do Sistema Único de Saúde (SUS) se unem para transformar e inovar numa perspectiva ímpar e complementar às práticas formativas e de cuidados, em prol dos usuários e voltadas para a potência de vida em cada encontro. Ainda neste sentido, a edição 2018/2019 do PET-Saúde, trouxe o desafio da implementação curricular da Educação Interprofissional (EIP), este tema, por sua vez, propõe de maneira enfática uma maior aproximação entre a academia, profissionais da saúde e a comunidade, cujos objetivos são ampliar, promover, articular e apoiar ações interprofissionais voltadas, tanto para as mudanças na formação acadêmica em saúde, quanto para a integração da tríade "ensino-serviço-comunidade", articuladas à Educação Permanente em Saúde (EPS). Destarte, nossa obra traduz a confecção de um leque de experiências e relatos multidisciplinares e interprofissionais, a partir de narrativas, saberes e afetos singulares e coletivos, que são desvelados em cada capítulo. O prisma interprofissional do PET-Saúde, reflete uma formação colaborativa e participativa, que é apreendida em todo manuscrito. Pois, "aprendemos juntos, para trabalhar juntos".

ABSTRACT

OBJECTIVES: Evaluate whether there is an association between convergence insufficiency and temporomandibular disorder (TMD) and whether there is an association between pain and range of motion in different degrees of TMD. METHODS: We evaluated 138 individuals with TMD and 46 without TMD using the Research Diagnostic Criteria for temporomandibular disorders, the Fonseca Anamnestic Index (FAI), Numeric Pain Rating Scale, and the measurement of mandibular range of motion (ROM). Convergence insufficiency was diagnosed using the convergence test and Convergence Insufficiency Symptom Survey. Analysis of variance was used to compare age and mandibular ROM. The Kruskal-Wallis was used to compare mandibular ROM and pain between groups. The chi-square test was used to evaluate associations between TMD subgroups and the FAI, sex, and ocular convergence. RESULTS: The majority of individuals without TMD did not exhibit convergence insufficiency. The frequency convergence insufficiency was significantly higher among individuals with severe TMD (p < 0.003). Mean pain severity differed between individuals with and without TMD. Mandibular ROM diminished with the increase in TMD severity. CONCLUSIONS: Convergence insufficiency, age, the increase in pain, and the reduction in mandibular range of motion were associated with the degrees of TMD severity. Despite the significant associations between convergence insufficiency and both pain and TMD severity, these variables cannot be indicated as predictive factors due to the low variability in the linear regression analysis. CLINICAL RELEVANCE: The present findings can assist in decision making regarding the treatment of severe TMD and the evaluation of ocular convergence.

ABSTRACT

OBJECTIVE: Current scientific evidence is insufficient to determine the best vascular access for each patient. It is an unmet clinical need because vascular access dysfunction accounts for 20% to 30% of hospital admissions. Our aim was to evaluate preoperative color flow Doppler ultrasound (CDUS)-derived parameters (vein diameter and brachial artery flow and diameter) and their effect interaction with comorbidities as predictors of brachiocephalic (BC) and brachiobasilic (BB) arteriovenous fistula (AVF) maturation. METHODS: A prospective analysis was performed of patients who underwent BC and BB AVF as primary definitive vascular access between January 2016 and May 2017. Variables included patients' demographics, comorbidities, medication, preoperative blood pressure, and CDUS-derived parameters. Outcomes were patency 48 hours after surgery and fistula maturation at 6 and 12 weeks. Nonparametric descriptive and univariate statistics were used. Logistic regression models and receiver operating characteristic curve analyses were performed. RESULTS: There were 132 patients (91 with BC AVF and 41 with BB AVF) included. The 48-hour patency was 91.7%. AVF maturation at 6 weeks was observed in 71.3%, and AVF maturation at 12 weeks was observed in 66.3%. There were no associations in univariate and multivariate logistic regression analysis between AVF maturation and comorbidities. Systolic blood pressure was an independent predictor of 48-hour patency with an optimized cutoff of 154 mm Hg (area under the curve, 0.73; P = .013; Youden index, 0.40). Vein diameter with tourniquet was an independent predictor of AVF maturation at 6 and 12 weeks with an optimized cutoff of 3.9 mm (area under the curve, 0.74; P < .001; Youden index, 0.38). CONCLUSIONS: AVF maturation was independent of comorbidities. Systolic blood pressure ≥154 mm Hg and vein diameter with tourniquet ≥3.9 mm were the associated conditions that better predicted BC and BB AVF maturation. There were no effect interactions between CDUS-derived parameters and associated comorbidities.

Tubulovillous adenomas of the duodenal ampulla are rare neoplasms. The present report describes a case with radiological-endoscopic and pathological correlation in which the patient underwent duodenal pancreatectomy with good postoperative progression. With advanced imaging methods, especially magnetic resonance and endoscopic ultrasound, locoregional aspects and extraluminal, lymphovascular, and metastatic invasion have been increasingly discussed as contributors to therapeutic decision making. This progression improves lesion staging and is especially useful in selecting eligible candidates for endoscopic treatment.

RESUMO Os adenomas túbulo-vilosos da ampola duodenal são neoplasias raras. Neste trabalho apresentamos um caso com correlação radiológico-endoscópica e patológica, tendo a paciente sido submetida à duodenopancreatectomia com boa evolução pós-operatória. Com os avanços dos métodos de imagem, em especial da ressonância magnética e ultrassonografia endoscópica, aspectos locorregionais, além da invasão extraluminal, linfovascular e metastática, têm sido discutidos de maneira crescente como contribuintes na decisão terapêutica. Essa evolução contribui para o melhor estadiamento destas lesões e é especialmente útil para selecionar candidatos elegíveis ao tratamento endoscópico.

Resumo Introdução O Sistema de Vigilância Alimentar e Nutricional (Sisvan) é de abrangência nacional e essencial para monitorar o crescimento e o desenvolvimento da criança na Atenção Primária à Saúde. Objetivo Descrever a cobertura e a tendência temporal do estado nutricional de crianças de 5 a 9 anos cadastradas no Sisvan entre 2008 e 2015. Método Análise de regressão linear simples para verificar a tendência temporal do número de registros, da cobertura e da classificação do estado nutricional de crianças de 5 a 9 anos em quatro territórios (município de Dourados, estado do Mato Grosso do Sul, região Centro-Oeste e Brasil). Resultados Verificou-se tendência de aumento no número de registros, exceto para Dourados, que apresentou padrão irregular (p = 0,123). Magreza e eutrofia apresentaram tendências de queda, enquanto sobrepeso e obesidade aumentaram no período e nos territórios avaliados, com exceção de Dourados, que não apresentou resultados significativos para eutrofia (p = 0,145) e obesidade (p = 0,318). Conclusão O número de registros e, consequentemente, a cobertura do Sisvan aumentaram de forma significativa em três dos quatro territórios avaliados. Os resultados referentes ao estado nutricional retratam e fortalecem o processo de transição nutricional vivenciado no país.


Abstract Background The Food and Nutrition Surveillance System (Sisvan) is a national and essential System to monitor the growth and development of children in Primary Health Care services. Objective To describe the coverage and temporal trend of the nutritional status of children aged 5 to 9 registered in Sisvan between 2008 and 2015. Method Simple linear regression analysis was performed to verify the temporal trend in the number of records, coverage, and nutritional status classification among children from 5 to 9 years old in four territories (Dourados, Mato Grosso do Sul, Midwest region and Brazil). Results There was a rising trend in the number of records, except for Dourados that presented irregular patterns (p=0.123). Underweight and normal weight presented falling trends, while overweight and obesity increased in the period in the territories evaluated, except for Dourados, which did not present significant results for normal weight (p=0.145) and obesity (p=0,318). Conclusion The number of records and, consequently, the coverage of Sisvan increased significantly in three territories evaluated in the period. Results of the nutritional status strengthen the nutritional transition process experienced in Brazil.
