ABSTRACT

Background: There is a growing preference for the use of marijuana for medical purposes, despite limited evidence regarding its benefits and potential safety risks. Legalization status may play a role in the attitudes and preferences toward medical marijuana (MM). Objectives: The attitudes and beliefs of cancer patients in a legalized (Arizona) versus nonlegalized state (Texas) regarding medical and recreational legalization and medical usefulness of marijuana were compared. Settings/Subjects: Two hundred adult cancer patients were enrolled from outpatient Palliative Care centers at Banner MD Anderson Cancer Center in Gilbert, AZ (n = 100) and The University of Texas MD Anderson Cancer Center in Houston, TX (n = 100). Design and Measurements: Adult cancer patients seen by the Palliative Care teams in the outpatient centers were evaluated. Various physical and psychosocial assessments were conducted, including a survey of attitudes and beliefs toward marijuana. Results: The majority of individuals support legalization of marijuana for medical use (Arizona 92% [85-97%] vs. Texas 90% [82-95%]; p = 0.81) and belief in its medical usefulness (Arizona 97% [92-99%] vs. Texas 93% [86-97%]; p = 0.33) in both states. Overall, 181 (91%) patients supported legalization for medical purposes whereas 80 (40%) supported it for recreational purposes (p < 0.0001). Patients preferred marijuana over current standard treatments for anxiety (60% [51-68%]; p = 0.003). Patients found to favor legalizing MM were younger (p = 0.027), had worse fatigue (p = 0.015), appetite (p = 0.004), anxiety (p = 0.017), and were Cut Down, Annoyed, Guilty, and Eye Opener-Adapted to Include Drugs (CAGE-AID) positive for alcohol/drugs (p < 0.0001). Conclusion: Cancer patients from both legalized and nonlegalized states supported legalization of marijuana for medical purposes and believed in its medical use. The support for legalization for medical use was significantly higher than for recreational use in both states.

ABSTRACT

BACKGROUND: Since 2002, priority Model for End-stage Liver Disease (MELD) exception status has been given to patients with hepatocellular carcinoma (HCC) who meet the Milan criteria. Since then, the number of liver transplantations performed in patients with HCC has increased, but to the authors' knowledge, few studies to date have examined the effect of MELD exception recommendations on therapy use and survival rates in a nationwide sample. The current study examines therapy use and long-term survival rates among patients with HCC tumors meeting the Milan criteria in the post-MELD exception era. METHODS: The current study is a retrospective cohort study of 2179 patients with localized HCC meeting the Milan criteria who were registered between 2004 and 2007 in the Surveillance, Epidemiology, and End Results database. RESULTS: A total of 43% of patients did not receive any specific therapy. Overall, the 5-year relative survival rate for patients receiving only supportive care was dismal at 24% (95% confidence interval [95% CI], 21%-27%), whereas that for patients undergoing liver transplantation was 77% (95% CI, 71%-82%). Long-term survival was found to be dependent on age, race/ethnicity, and type of therapy received. A multivariate Cox proportional hazards model adjusted for age, race/ethnicity, and type of therapy received demonstrated that, compared with white patients, black patients had significantly poorer survival outcomes (hazards ratio, 1.23; 95% CI, 1.03-1.47 [P = .02]), whereas Asian/Pacific Islander patients had significantly better survival rates when compared with white patients (HR, 0.66; 95% CI, 0.57-0.77 [P < .001]). CONCLUSIONS: Despite having localized disease that met transplantation criteria, nearly 50% of the large nationwide cohort of patients with HCC in the current study received only supportive care and had dismal 5-year relative survival rates, especially among black patients.

ABSTRACT

BACKGROUND AND AIM OF THE STUDY: Discrepancies in mean transvalvular gradient have been observed between Doppler echocardiography and catheter-based techniques in the assessment of aortic stenosis (AS). The Reynolds number (RE) has been shown to influence Doppler-derived gradients, and may be useful in resolving Doppler- and catheter-based gradient discrepancies in AS. The study aim was to assess the influence of the RE on such discrepancies. METHODS: A pulsatile in-vitro heart model using a bioprosthetic aortic valve with leaflets sutured together was used to simulate AS. Simultaneous gradients were measured using Doppler echocardiography and high-fidelity catheters while the RE was varied, by testing solutions of different density and viscosity across a range of cardiac outputs. RESULTS: The echocardiographic and catheter-derived mean gradient (MG) values were correlated (r = 0.89; p < 0.0001); however, significant differences in the MG were observed across hemodynamic states. A direct linear relationship was identified between RE and the absolute difference in MG measured using the two techniques (r = 0.94, p < 0.0001). Relative to catheter-based measurements, the MG was underestimated by Doppler (range: 13-16 mmHg) at low RE (median 6,999) and overestimated (7-33 mmHg) at high RE (median 34,268). However, agreement between catheter- and Doppler-derived gradients was within 5 mmHg at intermediate RE (median 17,284) (p < 0.0001). CONCLUSION: The underestimation of Doppler-derived MGs at low RE relative to catheter-based measurements may be due to an exclusion of viscous friction from the simplified Bernoulli equation, while the overestimation of Doppler-derived MGs at high RE may be due to a pressure recovery effect. However, within an intermediate range of RE, where the effects of viscous and inertial forces are balanced, the agreement between catheter- and Doppler-derived gradients was excellent.

ABSTRACT

Carcinoids are rare neuroendocrine tumours that occur primarily in the gastrointestinal tract. Carcinoid heart disease is characterized by fibrous plaque deposition on the endocardial surface of the cardiac valves and chambers. It affects the right heart valves in 85% of cases and the left heart valves in 15%. We present an unusual case of a patient with metastatic carcinoid heart disease in whom typical carcinoid aortic and mitral valve lesions developed 2 years prior to the development of severe right-sided carcinoid valvular heart disease.
