ABSTRACT

Potassium carbonate sesquihydrate has previously been identified as a promising material for thermochemical energy storage. The hydration and cyclic behavior have been extensively studied in the literature, but detailed investigation into the different processes occurring during dehydration is lacking. In this work, a systematic investigation into the different dehydration steps is conducted. It is found that at higher temperatures, dehydration of pristine material occurs as a single process since water removal from the pristine crystals is difficult. After a single cycle, due to morphological changes, dehydration now occurs as two processes, starting at lower temperatures. The morphological changes open new pathways for water removal at the newly generated edges, corners, and steps of the crystal surface. The observations from this work may contribute to material design as they elucidate the relation between material structure and behavior.

ABSTRACT

INTRODUCTION: The development of new highly accurate, inexpensive and accessible methods for the detection of lower-extremity peripheral artery disease (LE-PAD) in diabetic patients is required. The aim of this study was to evaluate the accuracy of a new incoherent optical fluctuation flowmetry (IOFF) method in detecting legs with hemodynamically significant stenoses compared to ankle brachial index (ABI) and transcutaneous oximetry (TcPO2) in patients with diabetes mellitus (DM). MATERIALS AND METHODS: Patients were recruited into 2 groups. Group 1 included patients with DM without LE-PAD and/or diabetic foot syndrome; Group 2 included patients with DM and LE-PAD. All patients underwent the following measurements: ultrasound (reference method), ABI, TcPO2, and the new IOFF method. RESULTS: The new IOFF method showed a sensitivity of 79.5% and a specificity of 89.8% in detecting limbs with hemodynamically significant stenosis (AUC 0.890, CI 0.822-0.957). TcpO2 allows the diagnosis of LE-PAD with 69.2% sensitivity and 86.2% specificity (AUC 0.817, CI 0.723-0.911). Using a standard ABI cut-off of less than 0.9, the sensitivity and specificity for this parameter were 34.5% and 89.7%, respectively. Increasing the diagnostic cut-off of the ABI on the study group to 0.99 improved sensitivity to 84.6% and specificity to 78% (AUC,0.824 CI 0.732-0.915). CONCLUSIONS: The new IOFF technique has demonstrated high sensitivity and specificity in the detection of LE-PAD in patients with DM. The high accuracy, rapid measurement, and potential availability suggest that the new IOFF method has a high potential for clinical application in the detection of PAD.

ABSTRACT

OBJECTIVES: Hemoptysis in pregnancy is a very rare finding causing diagnostic and therapeutic difficulties. The case report of hemoptysis by a 29 years old patient in the 31st week of pregnancy is presented and discussed along with the diagnostic process and treatment provided.Upon pharyngeal cancer occurrence in a pregnant patient a multidisciplinary medical team performed appropriate treatment along with delivery of a healthy newborn at term. Patients and fetal conditions and outcomes were analyzed and compared to available literature in this newly created literature review. MATERIAL AND METHODS: After MEDLINE database analysis using formula "hemoptysis" AND "pregnancy" more than 125 results were found published during the period 2002-2022. Almost 30 papers about hemoptysis were found and included for full analysis. CONCLUSIONS: The literature review offers a detailed description of previously reported incidents of hemoptysis in pregnancy to gain understanding of the etiology, differential diagnosis, available treatment and predicted future outcomes for both patient and fetus.

ABSTRACT

This work investigates the reactions occurring in K2CO3-H2O-CO2 under ambient CO2 pressures in temperature and vapor pressure ranges applicable for domestic thermochemical heat storage. The investigation shows that depending on reaction conditions, the primary product of a reaction is K2CO3·1.5H2O, K2CO3·2KHCO3·1.5H2O, or a mixture of both. The formation of K2CO3·1.5H2O is preferred far above the equilibrium conditions for the hydration reaction. On the other hand, the formation of double salt is preferred at conditions where hydration reaction is inhibited or impossible, as the thermogravimetric measurements identified a new phase transition line below the hydration equilibrium line. The combined X-ray diffraction, thermogravimetric analysis, and Fourier-transform infrared spectroscopy study indicates that this transition line corresponds to the formation of K2CO3·2KHCO3, which was not observed in any earlier study. In view of thermochemical heat storage, the formation of K2CO3·2KHCO3·(1.5H2O) increases the minimum charging temperature by approximately 40 °C. Nevertheless, the energy density and cyclability of the storage material can be preserved if the double salt is decomposed after each cycle.

ABSTRACT

(1) Background: To date, there are no studies evaluating the ability of the incoherent optical fluctuation flowmetry (IOFF) method to assess foot tissue perfusion. The aim of this study was to evaluate the correlation between perfusion values measured by IOFF and TcPO2 in patients with diabetes-related lower-extremity complications. (2) Methods: This was an observational, cross-sectional, two-center study. Diabetic patients with peripheral artery disease and/or diabetic foot ulcers were studied (n = 27, examinations were carried out on 54 legs). Perfusion in the foot tissues was assessed using TcPO2 (reference standard for this study) and the IOFF method. (3) Results: High correlation coefficients of all perfusion parameters measured by IOFF with TcPO2 (Rs 0.7 to 0.76) were shown. The study demonstrated that the IOFF method allows, with a sensitivity of 85.7% and a specificity of 90.0%, the identification of patients with a critical decrease in TcPO2 < 20 mmHg. (4) Conclusions: The high correlation of IOFF parameters with TcPO2 and the moderately high sensitivity and specificity in detecting patients with severe ischemia of foot tissues shows the promise of the method for assessing a tissue perfusion in patients with diabetes-related lower-extremity complications.

ABSTRACT

Fluorescence imaging in colorectal surgery is considered a novel predominantly intraoperative method of ensuring a greater surgical success. The use of fluorescence is linked to advanced tumor visualization and projection of its lymphatics, both vessels and nodes, which results in a higher chance of achieving a total excision. Additionally, iatrogenic complications prove to be reduced using fluorescence during the surgical excision. The combination of fluorescence and artificial intelligence to better facilitate oncological surgery will soon become an established approach in operating rooms worldwide.

ABSTRACT

Reconstruction of composite defects of the posterior thigh and knee is challenging. Pedicled medial gastrocnemius flaps are the traditional reconstructive approach, but late contractures related to skin grafted muscle may affect knee function. More recently, the medial sural artery perforator (MSAP) flap has been described for such defects, although may necessitate skin grafting of the donor site. To minimise the drawbacks with these options, we describe a combination of a median superficial sural artery perforator (MSSAP) flap and MSAP flap. This allows both tension free closure of the donor site without skin grafting, and facilitates coverage of a round defect by splitting the flap into two adjacent triangular flaps. This is the first report of posterior thigh defect reconstruction with a combination of MSSAP and MSAP flaps.
