ABSTRACT

A meta-analytic approach deciphered the taxonomic profile of the zebrafish gut microbiota at different developmental stages. Data (16S rDNA) were systematically searched in databases, selecting those with intestine samples of fish not exposed to a particular treatment or challenge (e.g., pathogens, dietetic tests, xenobiotics, etc.) and obtaining 340 samples to be processed. Results revealed marked differences between the developmental phases. Proteobacteria was the dominant phylum in the larval phase, with a relative abundance of 90%, while the rest of the phyla did not exceed 2%. Vibrio, Aeromonas, Plesiomonas, Pseudomonas, Shewanella, and Acinetobacter were the dominant genera in this phase. Transitional changes were observed after the larvae stage. Proteobacteria still registered high abundance (48%) in the juvenile phase, but Fusobacteria (40%) and Bacteriodota (5.9%) registered considerable increases. Genera, including Cetobacterium, Plesiomonas, Aeromonas, Vibrio, and Flavobacterium, dominated this stage. The phyla Proteobacteria (48%) and Fusobacteria (35%) were strongly established in the adult phase. Cetobacterium was registered as the most abundant genus, followed by Aeromonas, Acinetobacter, Plesiomonas, Vibrio, and ZOR0006 (Firmicutes; 6%). In conclusion, the composition of the intestinal microbiota of zebrafish is consistently determined by two primary phyla, Proteobacteria and Fusobacteria; however, this composition varies depending on the developmental stage. Cetobacterium and Aeromonas are the most relevant genera in juveniles and adults. Finally, these results reveal a consistent pattern of certain bacterial groups in the zebrafish microbiota that could help shape gnotobiotic models (colonized with a specific known bacterial community) or synthetic microbiota (in vitro assembly of microbes), among other approaches.

ABSTRACT

Nocardiosis has caused high mortalities among fish cultures; however, the effects of Nocardia infections in the fish gastrointestinal microbiota are unknown. In this research, tilapia was infected with Nocardia sp., to analyze the effect of infection on the gastrointestinal microbiota. Tilapia infected with Nocardia sp. reported a 46 % survival (100 % in non-infected). Moreover, the infection caused severe damage to the stomach microbiota, with a loss of diversity and a significant increase of Proteobacteria (94.8 %), resulting in a negative correlation network between Proteobacteria and other important phyla. Nocardia sp. is an emerging pathogen capable of inducing dysbiosis and causing significant mortalities.

ABSTRACT

Tilapia species are among the most cultivated fish worldwide due to their biological advantages but face several challenges, including environmental impact and disease outbreaks. Feed additives, such as probiotics, prebiotics, and other microorganisms, have emerged as strategies to protect against pathogens and promote immune system activation and other host responses, with consequent reductions in antibiotic use. Because these additives also influence tilapia's gut microbiota and positively affect the tilapia culture, we assume it is a flexible annex organ capable of being subject to significant modifications without affecting the biological performance of the host. Therefore, we evaluated the effect of probiotics and other additives ingested by tilapia on its gut microbiota through a meta-analysis of several bioprojects studying the tilapia gut microbiota exposed to feed additives (probiotic, prebiotic, biofloc). A total of 221 tilapia gut microbiota samples from 14 bioprojects were evaluated. Alpha and beta diversity metrics showed no differentiation patterns in relation to the control group, either comparing additives as a group or individually. Results also revealed a control group with a wide dispersion pattern even when these fish did not receive additives. After concatenating the information, the tilapia gut core microbiota was represented by four enriched phyla including Proteobacteria (31%), Fusobacteria (23%), Actinobacteria (19%), and Firmicutes (16%), and seven minor phyla Planctomycetes (1%), Chlamydiae (1%), Chloroflexi (1%), Cyanobacteria (1%), Spirochaetes (1%), Deinococcus Thermus (1%), and Verrucomicrobia (1%). Finally, results suggest that the tilapia gut microbiota is a dynamic microbial community that can plastically respond to feed additives exposure with the potential to influence its taxonomic profile allowing a considerable optimal range of variation, probably guaranteeing its physiological function under different circumstances.

ABSTRACT

INTRODUCTION: The sequelae of COVID-19 have been described as a multisystemic condition, with a great impact on the cardiovascular and pulmonary systems with abnormalities in pulmonary function tests, such as lower diffusing capacity of the lung for carbon monoxide (DLco) levels and pathological patterns in spirometry; persistence of radiological lesions; cardiac involvement such as myocarditis and pericarditis; and an increase in mental disorders such as anxiety and depression. Several factors, such as infection severity during the acute phase as well as vaccination status, have shown some variable effects on these post-COVID-19 conditions, mainly at a clinical level such as symptoms persistence. Longitudinal assessments and reversibility of changes across the spectrum of disease severity are required to understand the long-term impact of COVID-19. METHODS AND ANALYSIS: A prospective cohort study aims to assess the impact of SARS-CoV-2 infection on cardiopulmonary function and quality of life after the acute phase of the disease over a 6-month follow-up period. Sample size was calculated to recruit 200 participants with confirmatory COVID-19 tests who will be subsequently classified according to infection severity. Four follow-up visits at baseline, month 1, month 3 and month 6 after discharge from the acute phase of the infection will be scheduled as well as procedures such as spirometry, DLco test, 6-minute walk test, chest CT scan, echocardiogram, ECG, N-terminal pro-B-type natriuretic peptide measurement and RAND-36 scale. Primary outcomes are defined as abnormal pulmonary function test considered as DLco <80%, abnormal cardiovascular function considered as left ventricular ejection fraction <50% and abnormal quality of life considered as a <40 score for each sphere in the RAND-36-Item Short Form Health Survey. ETHICS AND DISSEMINATION: The study protocol was approved by the Institutional Ethics Committee of the Universidad Peruana Cayetano Heredia (SIDISI 203725) and the Ethics Committee of the Hospital Cayetano Heredia (042-2021). Protocol details were uploaded in ClinicalTrials.gov. Findings will be disseminated through peer-reviewed journals, scientific conferences and open-access social media platforms. ABSTRACT

Seasonal availability of resources can influence the timing and success of reproduction in primates. This study examines the annual pattern of conceptions, mid-term (13 weeks) and full-term (26 weeks) pregnancy losses, births, and infant survivorship of the Critically Endangered cotton-top tamarin (Saguinus oedipus) at Proyecto Tití's long-term field site in Santa Catalina, Colombia. Using 18 years of behavior, physical exam, and fecal steroid data (N = 51 females, 168 conceptions, 121 liters), fecal estrone conjugate (E1 C) and pregnanediol-3-glucuronide (PdG) or progesterone (P4) profiles were examined to investigate the relationships between female reproductive patterns and reproductive success with feeding and climatic data. Cotton-top tamarins demonstrated seasonal patterns of reproduction and pregnancy losses. We found positive correlations between peak conception and nectarivory, births and rainfall, and mid-lactation and frugivory. High body mass mothers reproducing during the peak season were more likely to have infants survive. Overall, these results suggest that tamarins use a "relaxed income-breeder" strategy. Cotton-top tamarins appear to maximize reproductive output at the time of ovulation, and can reduce their investment in the current reproductive effort at many stages. Changes in food availability due to habitat loss and climate change will require the Critically Endangered cotton-top tamarins to alter their reproductive strategies to survive.

ABSTRACT

We performed a comprehensive review of recent publications about type 2 diabetes mellitus (T2DM) in Peru, including studies among people living at high altitude above the sea level. An increase in the prevalence of T2DM in Peru has been reported, the reasons are multifactorial and coinciding with the strong economic growth that our country has experienced over the last 20 years along with migration from the Andean regions to the coast and the adoption of a lifestyle that is a known to be a risk factor for obesity and insulin resistance. Scarce information is available in Peru about the prevalence of chronic complications of T2DM such as retinopathy, neuropathy, and nephropathy. There is a need for a health care plan based on early diagnosis of T2DM to reduce social and economic problems, as recommended by the WHO and the United Nations.

ABSTRACT

Studies of cooperative breeding species have suggested that helpers are needed for infant survival and that helpers gain skills to successfully raise their own offspring. Studies of callitrichids in managed care and early field studies suggested that group size correlated with infant survival and that helpers needed to learn parental skills to be successful breeders. We present infant survival data from a 20-year field study of cotton-top tamarins (Saguinus oedipus) in Colombia involving 126 litters born to 41 females. There was no difference in the survival of male and female offspring to 6 months of age. However, litter size impacted survival, with triplet litters having lower survival and male triplets having lower survival than females. Loss of infants was highest in the first week of life and, of the animals remaining in the group at 6 months of age, 90% of them survived to 1 year. The mean length of time in the natal group was 2.3 years but some young migrated to other groups after 4 months of age. Group size and number of male helpers (>1 year) was not related to infant survival. However, groups with only one male had fewer surviving infants. Primiparous females had lower infant survival, but previous infant care experience by mothers as helpers had no effect on survival. A major cause of infant loss was group disruption due to deaths/evictions/loss of the reproductively active animals, or immigration of pregnant females. Thus, factors that cause a change in the stability of the reproductively active animals can disrupt group cohesion. Capture of individuals for the illegal pet trade, as well as habitat fragmentation, may increase competition between groups for access to territories and breeding opportunities. This may have long-term impacts to infant survival in this critically endangered species.

ABSTRACT

Hypoxic hepatitis is an uncommon cause of hepatic damage characterized by a centrolobular necrosis. Its pathophysiology remains unclear. Aortic dissection is a rare but frequently catastrophic event. It is caused by an aortic intimal tear with propagation of a false channel in the media. Depending on the site and extension, it can cause hypoperfusion of any organ leading to cellular ischemia and necrosis. We are presenting a case of hypoxic hepatitis in a patient with an extensive aortic dissection who present to the emergency department.

La hepatitis hipóxica es una causa poco frecuente de daño hepático caracterizada por una necrosis centrolobular. Su fisiopatología sigue siendo poco clara. La disección aórtica es un evento raro pero con frecuencia catastrófico. Dependiendo del sitio y la extensión, puede causar hipoperfusión de cualquier órgano lo que conduce a una isquemia celular y necrosis. Nosotros presentamos un caso de hepatitis hipóxica en un paciente con disección aórtica extensa que se presenta al servicio de emergencia.

ABSTRACT

Background Advanced cardiac imaging permits optimal targeting of cardiac treatment but needs to be faster, cheaper, and easier for global delivery. We aimed to pilot rapid cardiac magnetic resonance ( CMR ) with contrast in a developing nation, embedding it within clinical care along with training and mentoring. Methods and Results A cross-sectional study of CMR delivery and clinical impact assessment performed 2016-2017 in an upper middle-income country. An International partnership (clinicians in Peru and collaborators from the United Kingdom, United States, Brazil, and Colombia) developed and tested a 15-minute CMR protocol in the United Kingdom, for cardiac volumes, function and scar, and delivered it with reporting combined with training, education and mentoring in 2 centers in the capital city, Lima, Peru, 100 patients referred by local doctors from 6 centers. Management changes related to the CMR were reviewed at 12 months. One-hundred scans were conducted in 98 patients with no complications. Final diagnoses were cardiomyopathy (hypertrophic, 26%; dilated, 22%; ischemic, 15%) and 12 other pathologies including tumors, congenital heart disease, iron overload, amyloidosis, genetic syndromes, vasculitis, thrombi, and valve disease. Scan cost was $150 USD, and the average scan duration was 18±7 minutes. Findings impacted management in 56% of patients, including previously unsuspected diagnoses in 19% and therapeutic management changes in 37%. Conclusions Advanced cardiac diagnostics, here CMR with contrast, is possible using existing infrastructure in the developing world in 18 minutes for $150, resulting in important changes in patient care.

Entre las bacterias poco comunes que causan Endocarditis infecciosa (EI), se encuentran el Streptococcus gordonii, conocido por su habilidad de colonizar y dañar las válvulas cardiacas. Asimismo, se conoce que el hallazgo de aneurisma complicado con fístula intracardiaca es infrecuente en EI, sólo se presenta en el 1,6% de pacientes. Se reporta el caso de un varón de 58 años con EI por Streptococcus gordonii complicada con aneurisma y fístula en la válvula mitral. (AU)


Streptococcus gordonii is a rare cause of infective endocarditis (IE) known for its ability to colonize and damage cardiac valves. Moreover, it is known that complicated aneurism with intracardiac fistula is an infrequent finding in IE, reported in1.6% of patients. We report the case of a 58 year old male patient with IE infected with Streptococcus gordonii and complicated with an aneurism and mitral valve fistula. (AU)

Numerous animals have declining populations due to habitat loss, illegal wildlife trade, and climate change. The cotton-top tamarin (Saguinus oedipus) is a Critically Endangered primate species, endemic to northwest Colombia, threatened by deforestation and illegal trade. In order to assess the current state of this species, we analyzed changes in the population of cotton-top tamarins and its habitat from 2005 to 2012. We used a tailor-made "lure strip transect" method to survey 43 accessible forest parcels that represent 30% of the species' range. Estimated population size in the surveyed region was approximately 2,050 in 2005 and 1,900 in 2012, with a coefficient of variation of approximately 10%. The estimated population change between surveys was -7% (a decline of approximately 1.3% per year) suggesting a relatively stable population. If densities of inaccessible forest parcels are similar to those of surveyed samples, the estimated population of cotton-top tamarins in the wild in 2012 was 6,946 individuals. We also recorded little change in the amount of suitable habitat for cotton-top tamarins between sample periods: in 2005, 18% of surveyed forest was preferred habitat for cotton-top tamarins, while in 2012, 17% percent was preferred. We attribute the relatively stable population of this Critically Endangered species to increased conservation efforts of Proyecto Tití, conservation NGOs, and the Colombian government. Due to continued threats to cotton-top tamarins and their habitat such as agriculture and urban expansion, ongoing conservation efforts are needed to ensure the long-term survival of cotton-top tamarins in Colombia.

ABSTRACT

La diferencia elevada de presión arterial interbraquial (DEPAI) podría predecir eventos cardiovasculares adversos. Objetivos: Conocer la frecuencia de DEPAI y describir factores clínicos y demográficos relacionados. Material y métodos: Estudio descriptivo transversal. Muestra aleatoria de 211 pacientes de un hospital público de Lima. Se tomó la presión arterial tres veces, de forma simultánea en ambos brazos, con dos tensiómetros automáticos OMROM HEM-705CP propiamente calibrados, estando el paciente en posición sentada, con un reposo previo de cinco minutos. Se recolectaron datos demográficos y clínicos. Se estableció la DEPAI sistólica (≥10 mm Hg y ≥ 20 mm Hg) y diastólica (≥ 10 mm Hg). Se consideró las tres tomas y sólo las dos últimas. Se empleó las pruebas de Chi Cuadrado o Exacta de Fisher y el coeficiente de correlación puntual biserial para variables cualitativas y cuantitativas, respectivamente. Resultados: La edad promedio fue 48,92 ± 16,75 años. El 67,3% fue de sexo femenino. 23,08% de los participantes fueron obesos; 19,23%, hipertensos y 13,42%, fumadores. Según los punto de corte y cantidad de medidas consideradas, las frecuencias de DEPAI sistólica fueron 40,38%, 30,29%, 15,88% y12,98%; y las de DEPAI diastólica 13% y 10,1%. Hubo correlación entre DEPAI y HTA e IMC (p<0,05). Conclusiones: La frecuencia de DEPAI en un hospital de Lima es similar a la encontrada en otros medios. Este estudio encontró una asociación estadística de DEPAI con el IMC y HTA, pero se necesitan más estudios con poblaciones más grandes para conocer la relación de esta variable con predictores conocidos de morbilidad y mortalidad. High difference of inter-arm blood pressure (HDIABP) measurements could predict adverse cardiovascular events. Objectives: To determine the frequency of HDIABP and to describe clinical and demographic factors related to it. Methods: Cross sectional study. A random sample of 211 patients from a public hospital in Lima was taken. Blood pressure was measured three times, simultaneously in both arms with two automated sphygmomanometers OMROM HEM705CP properly calibrated, with the patient seated with at least 5 minutes of previous resting. Clinical and demographic data were collected. Systolic HDIABP was defined as ≥ 10 mmHg and ≥ 20 mmHg, and diastolic HDIABP as ≥ 10 mmHg. Chi square test and Fisher´s exact test were used, as well as the correlation coefficient for qualitative and quantitative data. Results: Mean age was 48.92 ± 16.75 years; 67.3% were females; 23.08% were obese; 19.23% had blood hypertension and 13.42% were smokers. The frequencies of systolic and diastolic HDIABP were 40.38%, 30.29%, 15.88%, 12.98% and 13%, 10.1%. There was correlation between HDIABP and body mass index (p<0.05). Conclusions: The frequency of HDIABP found is similar than that reported elsewhere. We found and association between HDIABP and body mass index, but more studies are needed to determine a true association between these two variables. (AU)

Objetivos: Conocer las estrategias de reperfusión en pacientes con síndrome coronario agudo ST elevado (SCASTE) atendidos en el departamento de emergencia de un hospital general, y determinar el tiempo para realizar dichas estrategias a través de los tiempos puerta-aguja (P-A), puerta-balón (P-B), electrocardiograma (ECG) y total de isquemia. Material y métodos: Estudio retrospectivo transversal en pacientes con diagnóstico de SCASTE en el Hospital Nacional Cayetano Heredia (HNCH) desde el 1° de enero de 2011 al 31 de enero de 2013. Se registraron las características demográficas y clínicas, tiempos de reperfusión miocárdica, de síntomas, para toma de ECG. Los factores asociados fueron analizados por análisis bivariado. Resultados: Cuarenta y cinco pacientes fueron elegibles. La edad media fue 60,5 ± 10,98 años, 88,8% fueron varones. El tiempo medio de hospitalización fue 9,79 ± 10,36 días. Se encontró obesidad (IMC ≥ 30) en 61,3%; hipertensión en 40,9%: tabaquismo en 43,1%; y diabetes mellitus II (DM II) en 25%. El tiempo medio P-A de 57 ± 54,1 minutos; en 28,1% se administró el agente trombolítico antes de los 30 minutos. Angioplastia fue realizada en un paciente con un tiempo P-B de 440 minutos. El tiempo total de isquemia (TTI) medio fue 244 ± 143,9 minutos. Conclusiones: Las estrategias de reperfusión fueron usadas en 71,1% de pacientes, siendo trombólisis la más usada; sin embargo, el tiempo P-A se encontró fuera del rango óptimo. (AU)


Objectives: To get insight into the reperfusion strategies used in a general hospital among patients with acute coronary syndrome and persistent ST-segment elevation (ACSSTE), and to determine times to door to needle (TDN) and time to door to balloon (TDB), time to EKG and total ischemic time (TIT). Methods: Cross-sectional study in patients with ACSSTE attended at Hospital Nacional Cayetano Heredia from January 1st, 2011 to January 31st, 2013. Variables were analyzed in a bivariate analysis. Results: Forty-five patients were included; mean age was 60,5 ± 10,98 years, 88,8% were males. Mean hospitalization time was 9,79 ± 10,36 days. Obesity was found in 61,3%; blood hypertension in 40,9%; smoking in 43,1%; and type II diabetes mellitus in 25%. Mean TDN was 57 ± 54,1 minutes; in 28,1% of cases a thrombolytic agent was used before 30 minutes. Angioplasty was performed with a TDB of 440 minutes. Mean TIT was 244 ± 143,9 minutes. Conclusions: Reperfusion strategies were used in 71,1% of patients, being thrombolysis the most frequently used. However, TDN was out of optimal accepted ranges. (AU)

No existen descripciones locales de hipotensión ortostática (HO). Los criterios diagnósticos no están uniformizados ni basados en evidencias. La presencia de hipotensión ortostática es un factor de riesgo independiente de mortalidadcardiovascular. Objetivos: Determinar la frecuencia de hipotensión ortostática en el adulto mayor, el tiempo en el que ocurre y describir sus características clínicas y demográficas. Material y métodos: Fueron incluidos 280 adultos mayores sin enfermedad aterosclerótica, metabólica o inmunológica clínicamente evidente. La presión arterial fue medida con un monitor ambulatorio OMRON serie HEM 705-CP con calibración vigente. Tras reposo de 5 minutos en decúbito dorsal, se determinó la presión arterial basal y luego de pararse se midió la presión en cada minuto durante 5 minutos. Se consignaron los síntomas asociados al cambio de postura.Resultados: Cuarenta y siete(16,79%) pacientes presentaron HO. La hipotensión ortostática fue sistólica en 74,7%, diastólica en 8,51% y sisto-diastólica en 17,02% de los casos. No se encontró asociación entre HO y edad, género, índice de masa corporal, ser hipertenso y recibir medicación antihipertensiva. La HO fue más frecuente en los que tenían PAS basal ≥180mmHg (p<0,001). En 61,7% la HO ocurrió en el primer minuto.Conclusiones: La hipotensión ortostática es una condición frecuente en el adulto mayor, es más frecuentecon PAS basal ≥ 180 mmHg y más del 80% ocurre en los primeros3 minutos


There is no local data on orthostatic hypotension (OH). The diagnostic criteria of OH are not standardized and are not based on good evidence. OH is an independent risk factor for cardio-vascular mortality. Objectives: To determine the frequency of OH in the elder, and to describe its clinical and demographic features. Methods: 280-elder patients without clinical evidence of atherosclerotic, metabolic or immunologic diseases were included. Blood pressure measurements were performed with an ambulatory monitor OMRON-HEM 705-CP recently callibrated. Baseline blood pressure was measured after resting the patient in the supine position for five minutes. Then, the blood pressure was measured every minute with the patient in the standing position. Symptoms associated with change in position were recorded. Results: 47 patients (16.79%) presented OH. OH was systolic in 74.7%; diastolic in 8.51%, and systo-diastolic in 17.02%. No association was found between age, gender, body mass index, blood hypertension and receiving medication for blood hypertension and OH. OH was more frequent in patients with baseline systolic pressure greater than or equal to 180mmHg (p<0.001). OH occurred in the first minute measurement in 61.7% of cases. Conclusions: OH is frequent among elder patients, it is more frequent among those with baseline blood pressures greater than or equal to 180 mmHg, and it happens in more than 80% of cases in the first 3 minute measurements.

ABSTRACT

El Lupus Eritematoso Sistémico (LES) es una enfermedad sistémica autoinmune. La verdadera incidencia de miocarditis es difícil de determinar; se encuentra en 1 a 9% de las autopsias de rutina y de 5 a 12% de las muertes súbitas no explicadas. La miocarditis debe ser diagnosticada por la combinación de criterios clínicos y patológicos, por su presentación heterogénea. La miocarditis es una manifestación rara, pero potencialmente fatal; por ese motivo su reconocimiento temprano y el manejo de soporte e inicio temprano de terapia inmunosupresora pueden lograr resultados favorables. Reportamos el primer caso documentado de miopericarditis lúpica en una mujer joven sin evidencia de infección viral asociada, realizándose el diagnóstico por la evolución y la respuesta terapéutica. Destacamos además la secuencia de los eventos; inicialmente como pericarditis efusiva masiva y que rápidamente progresó a disfunción sistólica severa del ventrículo izquierdo. (AU)


Systemic lupus erythematosus (SLE) is an autoimmune disorder. The true incidence of myocarditis is difficult to determine. It is found in 1-9% of necropsies and in 5-12% of unexplained sudden deaths. Myocarditis is a rare but potentially lethal condition, that is diagnosed by a combination of clinical and pathological findings. Early diagnosis and prompt initiation of treatment improves outcomes. We report the first case of SLE associated myopericarditis in a young female patient with no evidence of viral infection. The diagnosis was made by the clinical evolution and response to therapy. We emphasize the initial clinical presentation with massive effusive pericarditis rapidly progressing towards severe left vebtricular systolic disfunction. (AU)
