ABSTRACT

The medical personnel of mobile teams of emergency medical care are exposed to factors of working environment that affect health state and results in development of occupational diseases. The purpose of study is to analyze morbidity of medical personnel of mobile teams of emergency medical care according data of temporary disability and results of medical check-ups. The analysis was made according information from certificates of incapacity for work and the final acts of periodic medical examinations for five-year period. The study established significant increase of rate and duration of primary and general morbidity with increase of age and length of service of workers, higher susceptibility to diseases of physicians as compared with paramedics and employees of pediatric teams and specialists of other clinical profiles. In the structure of morbidity according to results of medical examinations, diseases of the eye and its accessory apparatus, the circulatory system and the musculo-skeletal system prevailed.

ABSTRACT

The problem of disability being a cause of decreasing of healthy life-span is actual especially nowadays when during last decade rate of increasing of primary disability of able-bodied population significantly advances mortality harming economics. The article considers dynamics of primary disability of population of the Ryazan oblast. The analysis of losses of healthy life-span of able-bodied population of the region caused by disability is implemented.

ABSTRACT

The article presents results of analysis of normative legal documents and basic laws relating to social protection of the disabled of able-bodied age and also to development ofpublic policy concerning the given category ofpopulation. The purpose of actual study is to analyze main normative legal documents in the field of investigation of social protection of the disabled of able-bodied age as applied to experience of the Riazanskaia oblast. The actuality of study is determined by problem of social legal defense of the disabled as one of the most complicated and requiring from society both understanding and participation in this process of many specialized institutions and structures. The analysis of study results permits to conclude that the regional legislation in its total with standards of the Federal legislation contains regulations reflecting fundamental principles and norms of the Convention of the rights of the disabled. On application of currently in force legislation and in the process of legislative activity the state and public institutions of the Riazanskaia oblast seek the possibly most broad implementation of the rights of the disabled and securing access to social, political, civil, cultural and other integration into society.

ABSTRACT

The paid medical services are provided by public and municipal health care institutions. At that, legal relationships occurring in the process of paid provision of medical services are at the stage of development and their further progress depends on considerable amount of components of economical and legal forms. ABSTRACT

The article emphasizes that, the protection of health of mother and child is actual especially in the present conditions considering demographic characteristics of particular territory. Hence, the development of optimal strategy in system of rendering of obstetrics and perinatal care and organization of operation of obstetrics institutions are the most important issues of modern obstetrics. ABSTRACT

The article emphasizes that the major purposes of priority national project in health care field are increase of availability and quality of primary and high-tech medical care, including period during pregnancy and delivery. The prevention and decrease of maternal, perinatal morbidity and mortality are among these priorities too. The reorganization of obstetrics service will promote rationale using of bed stock of obstetrics institutions including the optimal supply rationale structure, proper selection and regulation of flow for hospitalization and stages of medical care delivery.

ABSTRACT

AIM: To estimate the prevalence of dyspepsia and to study its clinical manifestations and risk factors in patients with type 2 diabetes mellitus. SUBJECTS AND METHODS: One hundred and seven patients with type 2 DM and 33 with functional dyspepsia were examined. A clinical and laboratory study and testing were made to identify the symptoms of dyspepsia; dyspepsia-associated factors were studied. RESULTS: Dyspepsia was observed in 71.0% of the examined patients with type 2 DM. It may be attributed to organic gastrointestinal tract (GIT) diseases only in 42.3% of cases. In the type 2 DM patients, dyspepsia that could not be explained by organic GIT diseases was mainly manifested by a dyskinetic type while an ulcer-like type was prevalent in those with organic GIT diseases. In the patients with type 2 DM, dyspepsia that could not be accounted for GIT diseases was associated with the duration of carbohydrate metabolism disturbance, the presence of diabetic complications, Helicobacter pylori infection, and patient age. Some symptoms of dyspepsia (repletion and epigastric discomfort), which could not be explained by organic GIT diseases in patients with type 2 DM were associated with diabetic complications and carbohydrate metabolic parameters. CONCLUSION: Dyspepsia in type 2 DM was observed in 71% of cases; it can be due to organic GIT diseases in 42.3% and its association with digestive organ pathology was not revealed in 57.7%.

ABSTRACT

The article deals with medical social and medical demographic characteristics of childless families. The analysis of causes the families decided to postpone the child birth is presented. The birth reserve is determined and the means to increase the effectiveness of management of reproductive health support and directions of family policy on different hierarchical levels are discussed.

ABSTRACT

In the Ryazan oblast there are 26 thousand families in which both spouses are under 30 years of age, and every third young family is childless. The overwhelming majority (84.5%) young married couples with children have one child, 14.4%--two children, 1.0--three children and more. According to the results of the study, a comparative socio-hygienic characteristic is presented of families which contracted a legal marriage and live in a consensual alliance. An analysis was made of the main reasons which forced a significant number of families to limit births.

ABSTRACT

The frequency of dyspeptic symptoms in patients with diabetes mellitus is higher than in non-diabetic subjects. The origin of dyspepsia in diabetics is debatable. The development of this condition appears to depend on sex, duration of the disease, diabetic complications, correction of hyperglycemia, and infection by different strains of Helicobacterpylori. In patients without organic gastrointestinal diseases, dyspeptic symptoms may be regarded as manifestations of autonomous diabetic neuropathy or of functional dyspepsia syndrome in the absence of neuropathy.

ABSTRACT

A participation of nonopiate peptide brain systems in the mechanism of individual pain sensation was studied. The experiments were carried out on rats of two inbred strains, Fischer-344 and WAG/G, which exhibit different threshold of thermal pain sensation. The action of nonopiate oligopeptides (substance P, bradykinin, and thiroliberin) on the latent period of tail immersion from hot water was studied. It was found that significant analgesic action of substance P, bradykinin, and thiroliberin is more pronounced in rats more sensitive to thermal stimuli (WAG/G). The rats with reduced thermal sensitivity (Fischer-344) are shown to be insensitive to analgesic action of substance P, bradykinin, and thiroliberin.
