Menopausa, ninho vazio e subjetividade feminina: relato de um atendimento numa enfermaria / Menopause, empty nest and female subjectivity: an attendance report in a ward

Este artigo é o relato de um atendimento de psicologia hospitalar durante a internação de uma paciente com Lúpus Eritematoso Sistêmico, no Hospital Universitário Clementino Fraga Filho da UFRJ. Além da doença, a paciente passava por dois acontecimentos, que para ela significavam perdas importantes: a saída de casa do filho único e a cessação da menstruação com a chegada da menopausa. Em sua narrativa ficou marcado o sofrimento face àqueles acontecimentos, que a faziam reviver uma perda antiga, ou seja, atualizavam o abandono do ex-marido, realidade que ela nunca aceitara. Desde que se separou, ocupava-se apenas da maternidade e do trabalho. Assim, naquele atendimento, ela refletiu sua dificuldade em se desprender de antigos vínculos e dos papéis femininos de esposa e mãe, e pôde construir novos projetos de vida, se permitindo viver outras experiências(AU)

This article reports an attendance of health psychology with a patient with Systemic Lupus Erythematosus (SLE) during her hospitalization at the University Hospital Clementino Fraga Filho in Rio de Janeiro. Besides the disease, the patient went through two tough losses to herself: the growth of her only son with 22 years old, who was leaving home, and the cessation of her menstruation with the arrival of her menopause. These losses have updated an old loss in the patient: the abandonment of ex-husband, which she still did not accept. As she was making motherhood the only option for her life, she was suffering those losses. In the attendance of psychology, the patient reflected the difficulty of separating from her marriage and family ties, and lively, and with expectations, she built a new life project, which was mainly her conquest of a new network of relations(AU)

Cultura e empowerment: promoção à saúde e prevenção da Aids entre prostitutas no Rio de Janeiro / Culture and empowerment: health promotion and Aids prevention among prostitutes in Rio de Janeiro

Neste artigo, discutimos as dificuldades encontradas na realização de projetos de promoção à saúde entre grupos marginalizados, a partir de questionários e grupos de discussão realizados no trabalho de prevenção da aids entre prostitutas, no Mangue, no Rio de Janeiro (RJ), em 1989, passando pelas histórias de vida das prostitutas da Praça São João, em Niterói (RJ), e pelo movimento de prostitutas do Rio de Janeiro, durante a década de noventa. Na análise dos dados, observamos que, apesar de a saúde pública ver as prostitutas como um "grupo", estas raramente se viam dessa forma. Ou seja, enquanto a meta das agências de promoção à saúde e do movimento de prostitutas era a de construir uma comunidade de prostitutas que pudesse se organizar, lutando por seus direitos e cidadania, a maioria das prostitutas estudadas tinham uma representação negativa da sua atividade e, consequentemente, criavam narrativas que negavam o seu pertencimento ao grupo das prostitutas. Diante desse impasse, fica evidente a necessidade de se pensar o processo de empowerment e de organização comunitária, sempre levando em conta o contexto cultural do grupo em que se quer promover a saúde.

This paper discusses the difficulties that can arise when health promotion projects are developed within marginalized groups. This could be documented using the example of aids prevention among prostitutes. We applied questionnaires and focus group interviews were performed with prostitutes in Mangue, Rio de Janeiro in 1989. Later, during the decade of 1990, we accomplished open interviews with prostitutes who frequented São João Square in Niterói and with the leaders of the prostitutes' movement of Rio de Janeiro. During the analysis of the interviews we observed that although, from a public health point of view, prostitutes are considered as a group, they seldom represent themselves in this way. In other words, while the goal of health promotion agencies and the prostitute' movement is to build a prostitutes' grassroots movement able to organize and fight for prostitutes' rights and citizenship, most of the subjects studied believed that prostitution was an evil activity and consequently created narratives which denied their belonging to the prostitutes' community.

Visando a introduçäo rápida de novas drogas no mercado, tais estudos recusam os laços entre pesquisa e clínica, fazendo crer que é possível agir de acordo com o tempo do capital - e ignorando que o tempo da clínica näo pode ser reduzido a este - sem ferir a ética. No Brasil essa tendência da pesquisa psiquiátrica ainda permanece obscura. Enquanto que nos EUA a questäo já ganhou amplo espaço nos tribunais, aqui pouco se discute sobre esse assunto

A survey, based on interviews with prostitutes from a poor region of Rio de Janeiro - Brazil, was performed with the intention of gaining a better understanting on the perception and representation of health and mental illness by this group. It was observed that the population under study has a clear organized system of representing mental disease being the psychopathology divided basically into two great groups: the one of the "mad" and the one of the "nervous", corresponding respectively to the medical terms "neurosis" and "psychosis". From that basic division the group develops a whole spectrum of atittudes that go from how to deal with the mentally ill, up to trying to reason the causative factors leading to the existence of mental disorders. It was also noticed that, in this group, the medical or religious explanations for the psychic phenomena are not exclusive, i.e. it is not felt as a conflict that a person considered "mad" should seek the help of a priest or a spiritual healer, as well as a psychiatrist

Buscamos correlacionar o alto ìndice de tentativas de suicìdio encontrados entre as prostitutas (4//) com os mecanismos de defesa utilizados pelas mesmas para se protegerem do estigma que sua profissäo carrega; o alto ìndice de uso abusivo de álcool e outras drogas que atuam no sistema nervoso central; além da falta de atitudes preventivas em relaçäo à SIDA (Sìndrome de Imunodeficiência Adquirida). Um fator comum a todas essas caracterìsticas encontradas é um padräo de comportamento autodestrutivo, que culminaria com o próprio suicìdio
