ABSTRACT

Objetivo. Revisar una de las posibles opciones terapéuticas de la insuficiencia venosa crónica (IVC), con puesta al día en cuanto a las indicaciones de ésta y sus resultados. Desarrollo. La cirugía hemodinámica de las varices es una técnica mínimamente invasiva que preserva la vena safena y representa un concepto fisiológico en el tratamiento de la insuficiencia venosa basado en el conocimiento de la hemodinámica venosa y su aplicación terapéutica, gracias a la aportación del examen con eco-Doppler. Se describen los aspectos anatómicos y hemodinámicos en que se basa dicha estrategia. Se establecen las indicaciones y la selección de los pacientes así como la técnica quirúrgica y los controles postoperatorios. Se presentan los resultados de varios grupos y los propios. Conclusiones. Aunque no hay evidencia definitiva a favor de la cirugía hemodinámica, los trabajos hasta el momento apoyan esta alternativa frente a la fleboextracción de la safena en el tratamiento de la IVC. Las ventajas de esta técnica son que se realiza de forma ambulatoria, minimiza los riesgos quirúrgicos y permite una rápida integración a la actividad normal


Aim. To review the possible options available for the treatment of chronic venous insufficiency (CVI), while at the same time providing an update on its indications and outcomes. Development. Haemodynamic surgery of varicose veins is a minimally invasive technique that preserves the saphenous vein and represents a physiological concept in the treatment of venous insufficiency based on knowledge of vein haemodynamics and its application to therapy provided by Doppler ultrasound studies. The anatomical and haemodynamic aspects on which this strategy is based are also described. The indications and the selection of patients, as well as the surgical technique and post-operative controls, are established. The results obtained by several groups and our own are presented. Conclusions. Although there is no decisive evidence in favour of haemodynamic surgery, studies conducted to date lend support to the use of this alternative rather than vein stripping of the saphenous in the treatment of CVI. The advantages of this technique are that it is performed as outpatient treatment, minimises the surgical risks and allows a swift return to normal activity
