ABSTRACT

This field report describes the accessibility and perceived effectiveness of a free acupuncture program among a group of predominantly low-income Hispanic/Latino adults. Surveys, developed based on the Levesque Conceptual Framework of Access to Health Care, were administered to clients. Baseline (n = 245) and 6-week follow-up (n = 79) surveys were analyzed to document early program findings. A majority of clients were Hispanic/Latino (72.7%) and female (73.1%). Most reported their original pain complaint was treated very well/well (98.7%). Clients reported an average 1.2 points pain level decrease (scale 1-10) at follow-up (p < 0.0001). Early program results suggest this acupuncture program was accessible and well received by low-income Hispanics/Latinos.

ABSTRACT

In the critical context of COVID-19 pandemic, healthcare workers are on the front line, participating directly in the care, diagnosis, and treatment of patients with COVID-19. This exposes them to a higher risk of developing chronic stress, psychological distress, and any other mental health symptoms. OBJECTIVE: to evaluate stress and burnout in a health workers population and, in addition, to measure hair cortisol concentration as a current biomarker of stress. MATERIALS AND METHODS: 234 health workers from Hospital de Clínicas "José de San Martín", Buenos Aires University, were included in this study. In this population hair samples were obtained from the posterior vertex as close to the scalp as possible and the individuals completed the following surveys: perceived stress, social support, burnout scale, life event scale, and sociodemographic data. Hair cortisol was measured by an automated chemiluminescent method. The studied population was divided into three groups considering those individuals below the healthy reference sample range (< 40 pg/mg hair), within the healthy reference range (40-128 pg/mg hair) and above the reference range (> 128 pg/mg hair). This study used a transversal and observational design. RESULTS: Our results show that 40% of the studied population presented hair cortisol values outside of the healthy reference range. In the whole studied population, a direct correlation was found between hair cortisol concentration and perceived stress as well as between hair cortisol concentration and the emotional exhaustion component of burnout (r = 0.142, p = 0.030; r = 0.143, p = 0.029, respectively). 12% of the studied population showed Burnout (52% doctors and residents, 19% nurses, 19% administrative personnel). Higher values in hair cortisol levels were found in the group with burnout versus individuals without burnout (p = 0.034). Finally, a mediation analysis was performed, finding that depersonalization is a mediating variable in the relationship between self-perceived stress and hair cortisol level (F = 4.86, p = 0.0086; indirect effect IC: 0.0987-1.8840). CONCLUSION: This is the first study in which a stress biomarker such as hair cortisol is evaluated in this population and in this context. Healthcare workers are subjected to increased levels of stress and burnout. High depersonalization, emotional exhaustion, and decreased personal sense of accomplishment characterize this population. It is the responsibility of the health authorities to implement strategies to manage this psychological emergency.

Los pacientes que se encuentran hospitalizados en la Unidad de Cuidados Intensivos (UCI), requieren de múltiples procedimientos, muchos de ellos no pueden ser realizados en la cama del paciente y por tanto se hace necesario el transporte de pacientes en estado crítico hasta el lugar de su realización, durante el traslado es posible la aparición de complicaciones que pueden empeorar el estado inicial del paciente. El presente estudio titulado "Validación de un registro de verificación para el traslado intrahospitalario del paciente crítico, según opinión de las enfermeras de UCI - Clínica Internacional Sede Lima" tiene como objetivo determinar la Validez de este registro, el cual es de nivel aplicativo, tipo cuantitativo, método descriptivo simple de corte transversal .La población estará conformada por todas las enfermeras que laboran en la UCI .La técnica que se utilizo fue la encuesta y el instrumento un registro de verificación y un opinionario. Los resultados fueron que del 100 por ciento (15) de enfermeros que participaron, 93 por ciento (14), tiene una opinión favorable y 7 por ciento (1) desfavorable. La conclusión fue que el instrumento tiene Validez.


Patients who are hospitalized in the Intensive Care Unit (ICU), require multiple procedures, many of them cannot be made in the patient's bed and thus the transport of critically ill patients is necessary to the point of its implementation during the move is possible the appearance of complications that can worsen the initial status of the patient. This-study entitled "Validation of a record check for intra-hospital transfer of critically ill patients, in the opinion of ICU nurses - International Clinic Lima Officeõ aims to determine the validity of this record, which is level application, type quantitative, simple descriptive method of cross section. The population will consist of all nurses working in ICU. The technique used was the survey instrument and a record check and opinionario. The results were that 100 per cent (15) of nurses who participated, 93 per cent (14) have a favorable opinion and 7 per cent (1) unfavorable. The conclusion was that the instrument is valid.
