ABSTRACT

BACKGROUND AND AIM: Delayed introduction of calcineurin inhibitors (CNI) in liver transplantation (OLT) seeks to protect renal function, although the optimal length of the delay is not well established. The aim of this study was to analyze the effects on renal function of CNI initiation on different days after OLT. METHODS: We reviewed the charts of 260 OLT recipients. Group D1-a (n = 36) underwent the standard initial immunosuppression at our center: namely, CNI introduction on day 1 with further daily administration to achieve target levels of 8 to 15 ng/mL for tacrolimus or 150 to 300 ng/mL for cyclosporine. Due to renal concerns, 126 patients (group D1-b) had CNI introduced on day 1 either not daily or at doses to achieve less than the target on at least two occasions. In 43 patients (group D2), CNI were introduced on day 2 in 23 on day 3 (group D3), in 12 on day 4 (group D4), and at least at day 5 in 20 others (group D5). In periods without CNI treatment, patients received mycophenolate mofetil. Steroids were administered to all patients. The study period included the first 3 months post-OLT. Renal function was estimated as creatinine clearance (CrCl) using the Cockcroft-Gault equation. RESULTS: Changes in CrCl from pre-OLT to month 3 were -19% ± 28% in group D1-a; -27% ± 19% in group D1-b; -29% ± 19% in group D2; -23% ± 26% in group D3; -4% ± 38% in group D4, and +4% ± 33% in group D5 (P < .05 vs groups D1-a, D1-b, D2, and D3). On multivariate analysis, CNI introduction at day ≥ 5 was protective for kidneys when adjusted for other variables that potentially influence renal function. CONCLUSION: CNI should be introduced at day 5 after OLT to protect renal function.

ABSTRACT

We studied the effect of an avocado oil-rich diet on (1) the blood pressure response to angiotensin II (AngII) and (2) the fatty acid composition of cardiac and renal membranes on male Wistar rats. The avocado oil-rich diet induced a slightly higher AngII-induced blood pressure response in the rats as compared to the control rats. In cardiac microsomes, avocado oil induced an increase in oleic acid content (13.18+/-0.33% versus 15.46+/-0.59%), while in renal microsomes, the oil decreased alpha-linolenic acid content (0.34+/-0.02% versus 0.16+/-0.12%), but increased the arachidonic acid proportion (24.02+/-0.54% versus 26.25+/-0.54%), compared to control. In conclusion, avocado oil-rich diet modifies the fatty acid content in cardiac and renal membranes in a tissue-specific manner. The rise in renal arachidonic acid suggests that diet content can be a key factor in vascular responses.

ABSTRACT

The process of testicular maturation in relation to intrapuparial development was studied in the sheep nasal bot fly, Oestrus ovis L. (Diptera: Oestridae). After formation of the puparium during larval-pupal apolysis and the cryptocephalic pupal stage (approximately 24-72 h), spermatogonia had undergone mitotic divisions and sperm cysts had been formed. Five days after pupariation, spermatogonia transformed into primary spermatocytes during the phanerocephalic pupal stage, and secondary spermatocytes first appeared during the pupal-adult apolysis. Secondary spermatocytes began undergoing the second meiotic division by day 8 (transparent-eye pharate adult stage). By days 9 and 10, round spermatids were present and began to elongate by day 11. By day 12, the first bundles of tailed spermatozoa had appeared. By day 15 (the yellow-orange eye pharate adult stage), round, elongated, tailed and bundled spermatids were predominant and by day 17 differentiating spermatids occupied nearly 35% of the testicular cavity, and 60% was occupied by free sperm. By day 21 (the red-brown eye pharate adult stage), spermatozoa colonized the seminal vesicle. At emergence (approximately day 22), a complement of free sperm occupied the testis and the seminal vesicle, but groups of developing cells frequently remained in certain zones. Spermatogenesis was carried out after pupariation and spermiogenesis occurred during the pharate adult stage. After emergence, males possessed fully formed spermatozoa ready for ejaculation.

ABSTRACT

A case of spontaneous tubal heterotopic pregnancy in primigest patient with survival of intrauterine conception is presented. She was treated in H.G.Z.M.F. No. 8 IMSS in Ciudad Sahagún Hgo. The frequency antecedents, clinical picture, auxiliary methods of diagnosis and therapy are discussed. It is concluded that the heterotopic pregnancy is a rare illness, but it can occur, and is potentially dangerous to the mother and intrauterine conception. The instant detection improves the prognosis.

ABSTRACT

This study examined the characteristics of couples in whom the male partner was experiencing erectile difficulties and seeking treatment by surgical intervention with the penile prosthesis. The purpose was to examine and describe the psychological characteristics of the individual spouses as well as the nature of their relationship. Additional purposes were to develop an ongoing assessment program and establish normative data on this particular population. Twelve couples in whom the men were considered by their physicians as potential candidates for the penile prosthesis were the subjects. The Minnesota Multiphasic Inventory, the Marital Communication Inventory, and the Multi-Modal Marital Satisfaction Questionnaire were administered to husband and wife. The couples' communication scores were lower than expected from established norms. The scores on the Marital Satisfaction Inventory, however, indicate relatively high levels of satisfaction with the marriage overall. A high positive correlation was found between the measures of communication and marital satisfaction. The MMPI data indicate that there is no one profile configuration that is characteristic of either husbands or wives. As a group, neither the husbands nor the wives evidence any signs of psychopathology. A composite description of the couples is presented along with a description of the individual psychological characteristics of the husbands and wives as a group. The findings are discussed in terms of their implications for treatment, and recommendations for further research are indicated.
