ABSTRACT

Introduction: Since the beginning of the COVID-19 pandemic, medical specialties have adapted their routine, postponing non-essential procedures, which resulted in a reduction in the number of surgical visits, directly impacting teaching hospitals. This work aims to evaluate the impact of the COVID-19 pandemic on the activities of the surgical specialties at the University Hospital Professor Edgard Santos (C-HUPES), located in Salvador-BA. Method: Observational, cross-sectional, retrospective study, which compares the year 2020 to the year 2019, in which the impacts of the pandemic on the surgical activities (especially plastic surgery) of the hospital are analyzed through the total number of procedures, hospitalizations, and outpatient consultations of surgical specialties. Results: A reduction of 45,45% of the total number of surgical procedures was observed, with 220 surgeries in 2019 and 120 surgeries in 2020. Reduction of 37.29% in hospitalizations, totaling 236 in 2019 and 148 in 2020. There was a decrease of 40.90% in outpatient consultations, with 2941 consultations performed in 2019 and 1738 consultations in 2020. It is noticed that the worst periods of reduction in 2020 were from March to June, in addition to a new drop in November and December. Conclusion: The pandemic impacted the activities of the surgical specialties at C-HUPES due to the total reduction in the number of surgeries, consultations, and hospitalizations in 2020, impairing the care of plastic surgery patients in absolute numbers. It is inferred that the COVID-19 pandemic hampered the training of plastic surgery residents.

ABSTRACT

Introduction: The high cost of negative pressure wound therapy (NPWT) makes the procedure less accessible in institutions with limited resources. To solve the problem, streamlined vacuum dressings have been proposed, but the usefulness of these devices has been poorly studied. The objective of this work is to evaluate the feasibility (operational and financial) of a simplified vacuum dressing system model (SVDM). Methods: Operational viability was assessed by studying application time and quantity of dressings performed; financial viability, by analyzing the economic costs of dressing changes. Results: Fifty wounds were treated (25 in each group: SVDM x silver hydrofiber). For SVDM, the number of dressings per patient was lower, while the application time was higher. The SVDM showed higher costs. The increase in the expenses associated with the SVDM was related to the average selling price of the product and the number of dressing changes; treatment time and application time of the SVDM did not interfere with costs. In contrast, SVDM costs proved to be below the announced expenses for conventional NPWT. Conclusion: SVDM was considered viable as long as qualified teams perform it and results in few dressing changes (< 3).

ABSTRACT

PURPOSE: Negative pressure wound therapy (NPWT) has revolutionized wound care, but its high cost reduces the procedure's availability. To solve the problem, streamlined vacuum dressings systems have been proposed, but the utility of these devices has been poorly studied. The objective of this study was to evaluate a simplified vacuum dressing system model (SVDM). METHODS: Randomized clinical trial in which wounds were treated with SVDM compared to a complex occlusive dressing (silver hydrofiber, SHF). The analyzed outcomes were cleaning, presence of granulation tissue, clinical appearance, and indication for surgical closure of wounds. RESULTS: Fifty injuries were treated (25 in each group), most located on lower limbs. SVDM proved to be more effective than SHF in the evaluated outcomes. Wound recalcitrance reduced the effectiveness of the equipment used. Despite its efficacy, complications occurred, the most frequent related to dressing changes: minor bleeding, foam adherence to a wound bed, and pain. Only for bleeding no favorable risk-benefit ratio was found. There were no severe complications, worsening conditions of injuries, or deaths. CONCLUSIONS: SVDM proved to be an effective and acceptably safe device for managing studied wounds.

ABSTRACT

Introduction: studies that relate quality of life with abdominoplasty demonstrate positive results in patients' quality of life. The abdomen is one of the most requested surgeries after major weight loss. Propouse: the aim of this study was to evaluate the profile of patients who underwent post-bariatric abdominoplasty from C-HUPES in the period 2008-2015 and to compare these data from C-HUPES with data provided by DATASUS. Methodology: the cross-sectional, observational study performed at a university hospital in the state of Bahia. After the selection of the patients the data were collected from the medical records and the standardized evaluation instruments were applied during the outpatient evaluation. We also performed data collection and time series study with aggregated and secondary data from the DATASUS system and the Hospital Information System / SIH. All bariatric and post-bariatric surgeries performed in the state of Bahia without period of collection at C-HUPES Hospital from 2008 to 2015 were analyzed. Results: weight without period of dermolipectomy ranged from 55.4 to 111.8 kg ( Average of 86.361 kg) presenting about 50 kg less than before gastroplasty. In the postoperative evaluation without ambulatory, an average age of 49 years was observed. The height of the scar with respect to the pubis was of 9.1 cm and a small asymmetry was observed between the distance between the ends of the scar and the central axis of the scar. 0 to 6 cm. The mean horizontal scar length was 57.8 cm. Conclusion: a very large need was identified for patients requiring treatment after gastroplasties. As patients are operated at age and different from expected by the age of gastroplasty, there was some weight gain without late postoperative even when operating with patients with high imc. It was identified as the use of non-esthetic dermolipectomy codes where the postoperative surgery could be used in the hospital

ABSTRACT

Recurrent chronic leg ulcers are among the most severe vasculopathic complications of sickle cell disease (SCD). Their treatment remains a challenge. Stem cell therapy with bone marrow mononuclear cells (BMMC) is a promising new therapeutic option for other forms of chronic ulcers. This prospective pilot study was performed to evaluate safety and feasibility of BMMC implantation in patients with SCD and chronic leg ulcers (SCLU). Ulcer closure, recurrence and local pain were evaluated. BMMC were successfully administered to 23 SCLU patients and no serious adverse events occurred. During the 6-month follow-up period, 91·3% of patients had improved ulcer pain compared with baseline and 29·2% of the treated ulcers achieved total healing. The frequency of progenitor stem cells (CD34CD45low and fibroblast colony-forming units) in BMMC was found to be significantly reduced in SCLU patients and compared to SCD patients without ulcers (P < 0·004 and P < 0·01, respectively). No relationship was observed between treatment outcome and the number of implanted BM progenitor stem cells. In conclusion, BMMC implantation is a feasible and safe procedure, showing favourable outcomes for the treatment of SCLU, and encouraging further controlled clinical trials.

ABSTRACT

Objective: To examine whether the consumption of sugar-sweetened beverages is related to disturbances in the lipid-metabolic profile and markers of adiposity in a sample of university students from Colombia. Methods: A cross-sectional study was undertaken in 280 volunteers from 3 universities in Colombia. Data for sugar-sweetened beverages consumption were collected using a frequency-intake questionnaire in the last week (BEVQ-15) including carbonated beverages and juices. We also estimated data for quantity and frequency of consumption (never, one time, 2-3 times and > 4 times per week). Data for biomarkers included total cholesterol, triglycerides, c-HDL, c-LDL, arterial index and Castelli index. Lipid-metabolic index was calculated through serum concentrations of triglycerides, c-HDL, c-LDL and glucose. Waist circumference, body mass index and fat mass percentage were evaluated via bioelectrical impedance and used as markers of adiposity. Results: Male reported higher sugar-sweetened beverages consumption (> 4 times/wk) and elicited higher values of fat mass, waist circumference, total cholesterol, triglycerides, c-LDL and Castelli and arterial indices (P = NS). These relationships were also observed among women for body mass index, waist circumference, fat mass, triglycerides and c-LDL (P < .05). Furthermore, higher sugar-sweetened beverages consumption categories were negatively associated with lipid-metabolic index (P < .05) after adjustments by sex, age and body mass index. Conclusion: A higher sugar-sweetened beverages consumption was associated with a major lipid-metabolic profile and also with higher concentrations of adiposity markers among university students from Colombia.

ABSTRACT

Paracoccidioidomycosis is an infectious disease whose etiological agent belongs to the Paracoccidioides genus. Although it affects primarily the lungs, it can spread to other tissues, including the skin and mucous membranes. Despite the clinical treatment for this disease, scarring can produce sequelae, manifesting as anatomical and functional deformities of the face. We present a case of extensive, nasal unaesthetic and functional sequelae resulting from paracoccidioidomycosis, reconstructed using the paramedian forehead flap in three stages, through the regional unit principles.

ABSTRACT

INTRODUCTION Besides the diverse situations in which plastic surgery is performed, the procedure has an aesthetic character associated with societal issues, related to the ideals of beauty and to the search for body perfection. Currently, specific body measurements are increasingly being given importance, and shown by the media as being associated with success at diverse social levels. The objective of this study was to identify the concept of plastic surgery, as reported by the print media in Brazil and available in the virtual environment, by analyzing and describing the type of information that is reported to the general public. METHOD: For the data collection, a systematic search was performed on all news articles published by the print media in Brazil and available in the virtual environment. For the search, the descriptor used was "cirurgia plástica" ("plastic surgery" in English) in Portal Interjornal®. All news articles containing the term "plastic surgery" published between January 2008 and December 2011 were included in the study. The selected news articles were read and subjected to a previously established protocol. RESULTS: A total of 1983 news articles were analyzed. From this total, 1367 (68.94%) were classified as aesthetic related, 423 (21.33%) as being associated with reparative surgery, 47 (2.37%) as aesthetic and reparative, and 146 (7.36%) as undetermined. CONCLUSION: The concept of plastic surgery reported by the media was generally associated with aesthetic surgery. Both the content and the quality of information provided by the media are not enough to adequately clarify to the public the real role of the specialty and the care that should be taken when undergoing any type of surgical procedure.

ABSTRACT

Reconstruction of nasal defects is challenging because it requires covering skin, supporting framework, and lining. Traditionally, the forehead flaps are transferred in 2 stages; however, it can be accomplished in a single stage or in 3 stages. Few published studies are available about the paramedian forehead flap using the intermediate stage (3-stage) and the aesthetic subunits principle. The purpose of this study is to evaluate the use of the paramedian forehead flap in 2 and 3 stages for nasal reconstructions, highlighting the indications, complications, and technical details and evaluating the patient's satisfaction through a questionnaire about the quality of life (Derriford Appearance Scale 24). A retrospective review was performed between 2011 and 2013 for a consecutive series of 11 patients who underwent nasal reconstructions using the paramedian forehead flap in 2 or 3 stages. All preoperative and postoperative data were collected, and outcomes were also assessed through a questionnaire about the patients' postoperative quality of life. The causes of nasal lesions varied among skin cancer, trauma, and infection. Two-stage paramedian forehead flap reconstruction was performed upon 4 patients, whereas the 3-stage reconstruction was performed for 7 patients. Of the 10 survey respondents, 6 were highly satisfied (score of 11-27), and 4 were moderately satisfied (score of 28-44), whereas no one was dissatisfied after his/her surgical nasal reconstruction procedure. Whether the approach is accomplished in 2 or 3 stages, all areas of the reconstructed nose must be firmly supported. Applying the nasal subunits principle seems to contribute to an overall satisfied population in our study, according to the score obtained by the questionnaire about quality of life.

ABSTRACT

ABSTRACT

BACKGROUND: The Internet has become one of the most important sources of health information. However, few studies have assessed the characteristics of Internet usage by patients or how the information obtained affects decisions to undergo surgery. This study describes the characteristics of Internet usage by patients who intended to undergo plastic surgery at the Unified Health System (SUS) and private networks and how it affected their confidence. METHODS: A cross-sectional observational study was performed using data collected by 200 surveys filled out by patients before the operation. One hundred surveys were sent to patients attending the outpatient clinic of Plastic Surgery of Professor Edgard Santos University Hospital (SUS), and the remaining 100 to patients treated in private health clinics in Salvador, BA. RESULTS: The frequency of Internet use to seek health information on plastic surgery differed significantly between the SUS (n = 24; 24%) and private health services (n = 64; 64%). Most of the patients (n = 48; 54.5%) visited 1 to 3 sites. The websites of plastic surgery centers were the most sought (n = 74; 84.1%), followed by the Brazilian Society of Plastic Surgery website. Half of the patients reported the existence of contradictory information. Moreover, 71 (80.7%) of the respondents reported feeling that undergoing surgery was safe. CONCLUSIONS: The Internet was widely used to seek information on plastic surgery. Although visited sites contained contradictory information, most patients felt safe before undergoing their desired surgery.

ABSTRACT

Ascher syndrome is defined by the association between double lip, blepharochalasis, and nontoxic goiter. Because it is a rare disease, it is most often misdiagnosed, despite its implications for quality of life. We report a variation of an incomplete type of Ascher syndrome affecting the upper lip, upper eyelids, and lateral canthi of a young male patient. The surgical management, follow-up, and a brief overview of the syndrome are described. The results presented show an aesthetic and functional improvement of the facial deformities.

ABSTRACT

This retrospective epidemiological study focused on scorpion stings in Belo Horizonte, Minas Gerais State, Brazil, from 2005 to 2009. Data on reported scorpion stings were taken from the National System for Notifiable Diseases (SINAN) and the Epidemiological Surveillance System (SISVE) and patient records from the João XXIII Hospital. There were 2,769 cases of scorpion stings in Belo Horizonte (114.7 cases per 100,000 inhabitants), with a downward trend over time and the highest incidence from August to January. Tityus serrulatus was the species involved in most of the stings. There was no statistical difference in the incidence between male and female victims, and the most frequently affected age bracket was 55 to 64 years. Ninety-six percent of cases evolved to cure, and there were two deaths. It is necessary to improve the reporting process for scorpion stings and to consider the determinants of scorpion stings in order to plan and implement effective public health interventions.

ABSTRACT

The number of lung transplantations that are performed in Spain continues to grow, with 235 transplant recipients 2010. Non-heart-beating donations have contributed to this upward progression. Our Lung Transplant Unit began its activity in October 2008 and during these last three years 97% of the transplant interventions performed have been successful. In order to increase the number of donations, we have developed a non-heart-beating donor program as part of the existing organs program in our hospital. In doing so, the development of a multi-organic preservation method (lung, liver and kidney), which we call «Bithermia Preservation», was necessary. This paper presents this methodology as well as the first year of clinical application experience. During this time, 3 patients have been transplanted using such non-heart-beating donations. None of them developed primary graft dysfunction; all the patients have been discharged and lead active lives without any evidence of bronchiolitis obliterans syndrome (AU)

ABSTRACT

Introduction: Objective evaluation of nasal function is a constant challenge for plastic surgeons, otolaryngologists, and allergists. The modified Glatzel mirror can evaluate nasal expiratory flow; however, there is little information on this method and its use in the measurement of nasal patency after surgical procedures. Objective: To compare, in a prospective study, the functional results before and after cosmetic rhinoplasty and evaluate the use of the Glatzel mirror as an objective method to assess nasal patency. Methods: To achieve this objective, we analyzed the functional results of surgery through a subjective questionnaire and objective evaluation through a modified Glatzel mirror, and evaluated the correlation between the 2 methods. Twenty patients (14 women and 6 men) underwent aesthetic rhinoplasty using spreader grafts. Pre- and postoperative evaluation (90-120 days) included a respiratory quality score (subjective) and modified Glatzel mirror test (objective). Subsequently, the Spearman test was used to compare the pre- and postoperative subjective and objective data. Results: The subjective evaluation demonstrated a statistical difference between pre- and postoperative scores (8 ± 2 and 9.4 ± 0.7, P< 0.001). There was no statistical difference in mean nasal patency by modified Glatzel mirror. No statistically significant correlation was observed when comparing the modified Glatzel mirror values with the subjective scores reported by patients pre- or postoperatively. Conclusion: The Glatzel method lacks sensitivity in detecting patient-reported improvements in breathing following rhinoplasty. This suggests that the method is a poor assessment tool to detect small, post-surgical changes in the nasal airways...
