ABSTRACT

INTRODUCTION: There are inflammatory bowel disease (IBD) patients avoid lacteal products without evidence of lactose malabsorption, probably because of incorrect patient perceptions and arbitrary advice from physicians and diet books. AIM: To evaluate the prevalence of lactose malabsorption in patients with IBP. METHOD: In 18 patients with Crohns disease and 24 with ulcerative colitis and 25 controls the prevalence of lactose intolerance, as measured by lactose breath hydrogen tests. RESULTS: Observed deficiencies of absorption of the lactose in 7 (16.6%) patients with IBP and 5 (20%) subjects control. CONCLUSIONS: Not all the patients with IBD are intolerant to the lactose by the suppression of the lacteal produts must not be generalized in the diet of these patients.

ABSTRACT

Introducción: Muchos pacientes con enfermedad inflamatoria intestinal (EII) sin evidencia de malabsorción de lactosa, evitan tomar productos lácteos, basándose en percepciones erróneas y consejos equivocados de médicos y libros de dietética. Objetivo: Evaluar la frecuencia de malabsorción de lactosa en pacientes EII. Método: Se estudia la frecuencia de intolerancia a la lactosa en 18 pacientes con enfermedad de Crohn, 24 pacientes con colitis ulcerosa y 25 controles sanos, realizando un test de hidrógeno espirado. Resultados: Se observaron deficiencias de absorción de la lactosa en 7 (16,6 por ciento) pacientes con EII y 5 (20 por ciento) sujetos control. Conclusiones: No todos los enfermos con EII son intolerantes a lactosa, por lo que no debe generalizarse la supresión de los lácteos en la dieta de estos enfermos (AU)

ABSTRACT

Thromboembolic episode is a well known extraintestinal manifestation of inflammatory bowel disease, but it is a clinical rare complication. Histological and hematological studies suggest that a hypercoagulable state is involved in the pathogenesis of inflammatory bowel disease. However, the exact mechanism of hypercoagulability is still unknown. During the acute recurrences there is an increase of factor VIII, fibrinogen, platelet, factor V and decrease of antithrombin III. Hematologic disorders seem markedly correlated with the activity of the disease. We report on two patients with Inflammatory bowel disease and hypercoagulable state. We review the literature and discuss about the pathogenic mechanisms of such complication.

ABSTRACT

Los fenómenos tromboembólicos son una manifestación extraintestinal bien conocida de la enfermedad inflamatoria intestinal, pero rara en la practica clínica. Estudios histológicos y hematológicos sugieren que el síndrome de hipercoagulabilidad presente en esta enfermedad está involucrado en la patogénesis de la enfermedad inflamatoria intestinal. De todas maneras el mecanismo exacto de hipercoagulabilidad aún no es bien conocido. Durante la fase de actividad de la enfermedad hay un aumento de los niveles de factor VIII, fibrinógeno, plaquetas, Factor V y un descenso en los niveles de antitrombina III. Hay una correlación clara entre estas alteraciones hematológicas y la actividad de la enfermedad. Presentamos dos pacientes con enfermedad inflamatoria intestinal y síndrome de hipercoagulabilidad. Se revisa la literatura y se comentan los mecanismos patogénicos (AU)

ABSTRACT

Enteropathy associated T cell lymphoma is the most serious complication of celiac sprue. The incidence of malignancy, in general, is approximately twofold greater in celiac disease than in the general population, but the risk of specific gastrointestinal malignancies is markedly increased. Lymphoma of the small intestine, comprises approximately 50% of the malignancies, and the most are of T-cell origin. We report a case of celiac disease associated T-cell Lymphoma of the jejunum in a woman. At the age of 33 the diagnosis of Celiac sprue was made, after institution of a gluten free diet the patient improved, but, fifteen year later, the patient begin with fever and abdominal pain. Laparotomy showed a perforation of the intestine by a tumour.

ABSTRACT

El linfoma de células T es la complicación más grave asociada a la enfermedad celíaca. La incidencia de cáncer es aproximadamente el doble en pacientes celíacos en comparación con la población general, siendo el riesgo de cánceres gastrointestinales marcadamente mayor. El Linfoma de intestino delgado supone el 50 por ciento y la mayoría derivan de los linfocitos T intraepiteliales. Presentamos el caso de una enferma celíaca con linfoma T de yeyuno. A la edad de 33 años fue diagnosticada de celiaquía, después de la introducción de una dieta sin gluten la paciente mejora, pero quince años más tarde presenta un cuadro de fiebre y dolor abdominal. La laparotomía demuestra la perforación del intestino por una tumoración (AU)

ABSTRACT

Between February 1992 and July 1995, we performed 222 ultrasound-guided fine-needle aspiration biopsies (FNAB) in abdominal space-occupying lesions. The sensitivity was 90% and the specificity, 100%, with an overall diagnostic precision of 93%. We also determined the cost of each procedure (2,550 pesetas) and compared it with the cost of computerized tomography (CT)-guided biopsy (22,500 pesetas). These results and those reported in the literature indicate that there is no significant difference with respect to diagnostic yield, but that the difference in terms of expense is considerable. We believe that, in the assessment of abdominal lesions. CT-guided FNAB should be reserved for those lesions that prove inaccessible to ultrasound.

ABSTRACT

The effect on micronuclei (MN) frequency of in vivo exposure to different dose rates of gamma rays in murine polychromatic erythrocytes (PCE) was studied. Groups of animals were irradiated with 1.0 Gy of gamma rays administered in 10, 100, 1000 or 10,000 min; the micronucleated polychromatic-erythrocytes (MN-PCE) frequency was scored in blood samples obtained from the tail of mice at various times before, during or after irradiation. The time-response curves for the 10, 100, and 1000 min exposure were similar; however, the two first curves showed a peak at 1800 min and the last at 2400 min after exposure. The curve obtained from the 10,000 min exposure showed a plateau for a long period during and after the exposure. However, the integration of the area under the curves indicates that the damage caused by the different radiation protocols was the same, suggesting that throughout time, the lesions were not repaired but rather diluted by cell division.

ABSTRACT

The effect of alpha-asarone, a chemical with hypocholesterolemic properties extracted from Guatteria gaumeri, on SCE induction was studied both in human lymphocytes in vitro and in murine bone marrow cells in vivo. A slight but consistent increase in SCE was observed in both biological systems.

ABSTRACT

Seventy adult patients out of 446 with surgical stomas were investigated to ascertain the degree of sexual disfunction caused by the operation. A questionnaire of 74 items was given to all patients. Statistical analysis was done for crossing variables using Chi Square calculations. Sexual activity decreased much more in males than in females, independently of age but in relation to the type of operation.
