Basing on ECG findings, the authors compare hemodynamic effects of a single dose of isadrin, obsidan and spesicor in patients with coronary heart disease complicated by bundle-branch block. Isadrin improves left ventricular contractility, reduces pulmonary hypertension and right heart overload. Obsidan and spesicor aggravate the existing hemodynamic disorders in the above patients.

Doppler echocardiography was used to quantify pulmonary hypertension and phase pattern of right heart systole in 140 patients with nonspecific pulmonary diseases to study hemodynamic characteristics of pulmonary circulation and right heart as well as external respiration. There appeared a relationship between the severity of pulmonary hypertension, bronchial obstruction and arterial hypoxemia. It seems that in nonspecific pulmonary diseases hemodynamic changes arise prior to functional disorders of external respiration.

[Study of the right heart and external respiratory functions in patients with liver cirrhosis by Doppler echocardiography]. / Sostoianie pravykh otdelov serdtsa i funktsii vneshnego dykhaniia u bol'nykh tsirrozom pecheni po dannym dopplerékhokardiografi- cheskogo issledovaniia.

The paper presents clinical and instrumental evidence on 47 patients with hepatic cirrhosis. The revealed ventilatory disorders and worse bronchial permeability closely correlated with the degree of pulmonary hypertension which in its turn depended on portal hypertension degree. Pulmonary hypertension decreased with a decline in the activity of the pathological process in the liver due to treatment. External respiration insufficiency observed in hepatic cirrhosis contributes to the onset of arterial hypoxemia and hypoxic impairment of the liver activating in it a pathological process. It is recommended that such patients should receive a combined treatment incorporating oxygen therapy, calcium antagonists, antioxidants.

[Difficulties in the diagnosis of rheumatic heart defects in elderly patients]. / Trudnosti raspoznavaniia revmaticheskikh porokov serdtsa u bol'nykh pozhilogo vozrasta.

Clinical and autopsy data presented in this paper illustrate difficulties arising in diagnosis of rheumatic heart disease (RHD) in elderly patients. RHD misdiagnosis is explained by unclear clinical symptoms, undone laboratory tests, concomitant diseases, misinterpretation of some objective indications (auscultatory symptoms, ECG). To diagnose acquired RHD earlier the physicians are recommended to refer their patients to ultrasound investigations of the heart.

[Status of capillary blood flow in the lungs in various outcomes of acute pneumonia]. / Sostoianie kapilliarnogo krovotoka v legkikh pri razlichnykh iskhodakh ostroi pnevmonii.

The method of perfusion scanning of the lungs with albumin microaggregate 131I was used for studying the state of pulmonary blood flow in patients with acute pneumonia (AP). The outcome of the disease was chronic non-specific lung disease (CNLD). The results of study demonstrated the improvement of microcirculation by the time of discharge from a hospital, but in 54.2 per cent of the patients blood flow was reduced. It was proved that reduced blood flow, by the time of discharge, together with impaired external respiration function, X-ray changes and biochemical blood shifts are the unfavourable factors with respect to prognosis which contribute to the transition of AP into CNLD. Combined therapy of AP should include measures directed to the improvement of pulmonary microcirculation.
