ABSTRACT

Metastases to the patella are rarely described in the literature. A 51-year-old man with stage IIIB adenocarcinoma presented to our clinic. One year after radiation and chemotherapy treatment, it was found that he had a metastasis in the right patella. Six months after incisional biopsy with curettage of the patellar lesion and placement of methylmethacrylate bone cement into the remaining patellar defect, he has no pain and is ambulatory. This case has been presented as a basis for palliative surgery in a patient with a rare metastatic tumor of the patella.

ABSTRACT

Forty-seven hips in 38 patients with femoral head osteonecrosis were reviewed for a mean follow-up period of 2.6 years. The average preoperative Harris hip score was 24.7; postoperatively, the average score was 84.5. Cineroentgenographic motion studies were carried out on 14 hips and revealed motion at both the inner and outer bearing up to seven years postoperatively. In the unloaded mode, inner- and outer-bearing motion were equal. In the weight-bearing position, inner-bearing motion increased significantly while outer-bearing motion decreased. Only one case of acetabular wear or protrusio was noted.

ABSTRACT

Mechanical failures occurred in three Bateman bipolar hemiarthroplasties in two patients. In all three occurrences, the disassembly of the bipolar prosthesis was caused by fracture of the polyethylene liner of the bipolar acetabular component. This caused the 22-mm head of the femoral component to separate and unlock from the acetabular component. Both patients have functional hips after replacement of the acetabular component with a new "self-centering" type. The mechanical design may predispose this bipolar hemiarthroplasty to disassemble.

ABSTRACT

Metastatic tumors to the foot are a rare phenomenon which are often initially misdiagnosed. This article reports two cases of head and neck carcinoma metastases to the foot which were initially diagnosed as infection. These tumors have not been reported previously as metastasizing to the feet. In addition, one of the cases had metastasis to the hand and foot. Only five previously reported cases of such a combination could be found in the literature.

ABSTRACT

In a 15-month period, 26 patients with 31 anterior dislocations of the shoulder were seen. A modified Kocher maneuver was first attempted without the use of analgesics or muscle relaxants. This was successful 19 of 31 times (61%). With use of analgesics or muscle relaxants, 30 of 31 (97%) were successfully reduced without complications or fractures with use of this maneuver. In those dislocations seen within 24 hours, without fracture of the greater tuberosity or without failed prior surgery, the modified Kocher maneuver was successful without premedication in 19 of 24 (79%). One patient required general anesthesia.
