ABSTRACT

The short term clinical and radiological results of a hydroxyapatite-coated hip prosthesis with bonding osteogenesis on the hydroxyapatite ceramic and bony tissue reaction had to be studied. 233 patients who had hip replacement were followed up for two to five years and assessed radiologically and by the Merle D'Aubigné rating. Primary total hip arthroplasty as well as revision arthroplasty show sufficient clinical results. Tight pain and aseptic loosening of the stem did not occur. Hydroxyapatite-coating and contour for the Furlong hip prosthesis show a reliable short term anchorage.

ABSTRACT

High doses of both calcium antagonists and magnesium lead to disorders in the early phase of calcification. Analogous to known mechanisms of calcification outside the bone, the calcium overload of cellular suborganelles, which may play a role in enchondral ossification, is assumed to be prevented or delayed by calcium antagonists and magnesium. In accordance with the relevant literature, which describes analogous cytoprotective effects of calcium antagonists and magnesium in association with extraskeletal calcification and ossification, the qualitative agreement of histologic findings in the epiphyseal growth plate suggests a homologous, primarily intracellular mechanism of action of both magnesium and organic calcium antagonists. This common mechanism of action of the two substances on the molar Mg-Ca quotient, with reference to intracellular calcium phosphate mineralization, is regarded as the cause of the analogous effects.

ABSTRACT

The mechanical properties of bone are changed by osteoporosis. A collective of 50 hip endoprostheses was examined for loosening of the acetabular cup and stem, the appearance of para-articular ossification, and the reaction of osteoporotic bone to the implant.

ABSTRACT

This is a report about our experience with the replacement of loosened acetabular endosprotheses by the self locking ceramic Endoprosthesis Mittelmeier. That's an analysis of 60 patients concerning age, sex incidence, indications, primary implant type and long term findings. The clinical findings are illustrated according to the schedule of Merle d' Aubigné. The critical evaluation shows, that the self locking ceramic cup allows the additional implantation of homologous or autologous bone chips and grafts avoiding further allogenous implants.

ABSTRACT

A group of 26 out of 32 patients with idiopathic scoliosis treated by lateral external electric stimulation within the last 4 years was examined. In about 80% an arrest of progression or an improvement of the spinal curvature was found within 2 years. The average improvement was 7.6 degrees Cobb. A good effect can be expected in scoliosis with a Cobb angle up to 25 degrees. Analysis showed that good flexibility, low rotation, low Risser sign and Harrington factor seem to be decisive for a successful treatment. Even if the range of electrostimulation can be extended in the future up to higher degrees of spinal curvature it is doubtful that electrostimulation can substitute orthotic treatment in the whole established spectrum.

ABSTRACT

In comparison to the pattern of lesions of the soft tissue of the neck and the CVC in case of suicidal hanging 17 postmortal fracture-dislocations of the neck are investigated. It could be demonstrated that on principle in case of hanging the dens-fracture can occur. With respect to the hangman's fracture (Wood-Jones) a differentiation into 3 types is proposed.
