ABSTRACT

Different compression traumas and various acute disturbances of the main blood circulation, chiefly of extremities, result in similar ischemic and reperfusion lesions leading to the development of life-threatening endotoxicosis. In the literature there are diverse interpretations of these phenomena. These notions are not similar in medical Encyclopedia either. The authors consider that its pathogenetical essence must be called "postischemic endotoxicosis" or as an alternative "a reperfusion syndrome". For such interpretations to be complete in different clinical situations, it is necessary to mention the specific (or similar but different) causes of ischemia and its localization, and in such a diversity they can not have a common designation. So the interpretation of this pathology in such cases should consist of two parts--a pathogenetical basis common for all, and a diverse etiological characteristic.

ABSTRACT

In order to generalize the indications for drainage of the abdominal cavity the authors have systematized and subdivided them into one-type groups depending on the aims, (medical, prophylactic) and tasks, which on their opinion can allow the surgeons to better realize the reasons of using this method of treatment and to critically assess it. An analysis of clinical material was made in order to find out the frequency and localization of intraabdominal abscesses and to assess the measures taken for their prevention. It was found that in spite of the abdominal cavity drainage after 1568 operations on emergency indications, abscesses developed in 43 patients. An analysis of the investigation has shown that the causes, frequency and localization of postoperative abscesses are not predictable, prophylactic measures have low efficiency due to using passive glove-tube drainage and to too frequent using this not safe method without special need.

ABSTRACT

The authors dealt with treatment of 112 patients aged 27-70 years with the Mallory-Weiss syndrome. The diagnosis was confirmed by esophagogastroduodenoscopy. Diathermocoagulation was used in order to arrest bleeding. In profuse bleeding the margins of the mucosa fissures were first infiltrated with a solution of adrenaline. The Blakemore [correction of Bleikmorr] probe compression method was also used. Organ-saving operations were performed for continuing and recurrent bleedings. Two elderly patients with severe coexistent disease died. The authors consider that patients with the Mallory-Weiss syndrome must be treated by conservative methods. Operations for disruptions of the esophagus mucosa and acute blood loss will entail great risk.

ABSTRACT

An analysis of the diagnosis and operative treatment of 1246 patients with acute appendicitis was used by the authors for considering the causes of groundless appendectomies. The possibilities to reduce the amount of diagnostic errors and improve the surgical strategy for acute simple appendicitis are discussed.

ABSTRACT

A clear definition of the phenomenon of appendicular infiltrate is still lacking in the literature. The author believes that only such a form of appendicitis should be referred to as appendicular infiltrate when the vermiform process is surrounded by extensive and dense formation which completely restricts the zone and prevents spread of infection into the free abdominal cavity.
