ABSTRACT

BACKGROUND: Approximately 30 % of cardiac surgeries are conducted on women by median sternotomy, which often causes discomfort such as pain, affects quality of life, and delayed recovery compared with men. Breast size is related to operative wound complications, such as incisional pain, sternum dehiscence, and infection, which may affect hospital costs due to prolonged hospital stays. OBJECTIVE: To evaluate breast size and operative wound complications and the effect of breast support on the incidence of pain, infection, and quality of life in women after coronary artery bypass grafting. METHOD: Women were randomly assigned to one of three groups: group A (surgical breast support), group B (ordinary breast support), and group C (no-support). Observations were taken daily between the second and seventh postoperative days and at 30, 60, and 180 days. Pain was assessed using the Short-Form 36 Health Survey (SF36) for quality of life and a verbal numerical scale. The authors used the nonparametric Kruskal-Wallis and Friedman tests to examine variance. The authors used the Pearson correlation coefficient or the Spearman correlation for correlations between variables. A multivariate study was conducted to evaluate the occurrence of infection, and the logistic regression model with "stepwise" variable selection was used. A linear regression model with the "stepwise" variable selection was also used for hospitalization. The authors used SPSS 17.0 software for Windows, with a significance level of p < 0.05. RESULTS: There was no difference in pain evaluation between the groups in 190 women (p > 0.05). When comparing quality of life, there was a statistically significant difference in the functional capacity domain at 30 and 60 days, with group A having the best functional capacity (p < 0.05). The larger the breast size, the longer the hospital stay (p < 0.001) and the higher the probability of infection (p = 0.032). Patients with a history of stroke had a 3.8 higher incidence of infection (p = 0.040). CONCLUSION: The use of surgical support did not affect acute pain or sternal infection rate in the 6-month follow-up. However, it was effective in the functional capacity domain 30 days after surgery and maintained at 60 days.

Objetivos: conhecer a representação do Testamento Vital para os enfermeiros que atuam na assistência à pacientes em situação de terminalidade. Métodos:estudo de abordagem qualitativa, realizado em hospital público universitário de alta complexidade localizado na cidade de São Paulo, São Paulo, Brasil, com 15 enfermeiros que atuam na assistência à pacientes terminais, por meio de entrevista norteada pela questão "Fale a respeito do Testamento Vital". A análise dos dados foi desenvolvida pelo Discurso do Sujeito Coletivo. Resultados: identificou-se três categorias que compõem o Discurso do Sujeito Coletivo dos enfermeiros em relação ao testamento vital: "o enfermeiro frente às diretivas antecipadas de vontade"; "o enfermeiro frente à família do paciente em terminalidade" e "o enfermeiro frente ao médico do paciente em terminalidade". Conclusão: o testamento vital representa, na perspectiva dos enfermeiros, a autonomia e o direito do paciente pelas decisões nas situações de terminalidade que devem ser compartilhadas com seus familiares e profissionais de saúde.


Objectives: to know the representation of the living will for nurses working in the care of terminally ill patients. Methods: a qualitative study conducted in a high complexity public university hospital located in the city of São Paulo, São Paulo, Brazil. An interview guided by the question "Talk about the living will" was performed with 15 nurses working in the care of terminally ill patients. Data analysis was performed using the Discourse of the Collective Subject. Results: three categories that make up nurses' Collective Subject Discourse in relation to living wills were identified: "the nurse before advance directives"; "the nurse before the terminally ill patient's family" and "the nurse before the terminally ill patient's physician". Conclusion: from the perspective of nurses, the living will represent the patient's autonomy and right to make decisions in terminally ill situations that must be shared with their family members and health professionals


Objetivos: conocer la representación del Testamento Vital para enfermeros que actúan en el cuidado de enfermos terminales. Métodos: estudio cualitativo realizado en un hospital universitario público de alta complejidad ubicado en la ciudad de São Paulo, São Paulo, Brasil. Se realizó una entrevista guiada por la pregunta "Hablemos del Testamento Vital" con 15 enfermeros que actúan en el cuidado de pacientes terminales. El análisis de los datos se realizó utilizando el Discurso del Sujeto Colectivo. Resultados: se identificaron tres categorías que componen el Discurso del Sujeto Colectivo de los enfermeros en relación a los testamentos vitales: "el enfermero frente a las directivas anticipadas"; "la enfermera frente a la familia del enfermo terminal" y "la enfermera frente al médico del enfermo terminal". Conclusión: en la perspectiva de los enfermeros, el testamento vital representa la autonomía y el derecho del paciente a tomar decisiones en situaciones terminales que deben ser compartidas con sus familiares y profesionales de la salud.

ABSTRACT

After the World Health Organization declared COVID-19 to be a pandemic, the elaboration of comprehensive and preventive public policies became important in order to stop the spread of the disease. However, insufficient or ineffective measures may have placed health professionals and services in the position of having to allocate mechanical ventilators. This study aimed to identify instruments, analyze their structures, and present the main criteria used in the screening protocols, in order to help the development of guidelines and policies for the allocation of mechanical ventilators in the COVID-19 pandemic. The instruments have a low level of scientific evidence, and, in general, are structured by various clinical, non-clinical, and tiebreaker criteria that contain ethical aspects. Few instruments included public participation in their construction or validation. We believe that the elaboration of these guidelines cannot be restricted to specialists as this question involves ethical considerations which make the participation of the population necessary. Finally, we propose seventeen elements that can support the construction of screening protocols in the COVID-19 pandemic.

Introdução As Diretivas Antecipadas de Vontade, também denominadas Testamento Vital constituem um documento com o registro expresso do desejo de um cidadão de recusar tratamento, caso venha a sofrer de alguma enfermidade terminal. Dessa forma, busca desconstruir uma cultura centrada no paternalismo que reduz o indivíduo doente a um paciente que deve aguardar, resignado e submissamente, que deliberações acerca de sua vida sejam tomadas por outros, sem que possa se manifestar ou decidir, autonomamente, como quer ser tratado ou que tipo de práticas de intervenção está disposto a aceitar. Objetivo - O presente trabalho objetivou conhecer a representação do Testamento Vital para os enfermeiros que atuam na assistência à pacientes em situação de terminalidade. Método - Trata-se de pesquisa de natureza qualitativa realizada com quinze enfermeiros que atuam na assistência à pacientes terminais, por meio de entrevistas norteadas pela seguinte questão Fale a respeito do Testamento Vital. Após a aprovação do Comitê de Ética em Pesquisa, houve a obtenção dos depoimentos que foram analisados de acordo com o método do Discurso do Sujeito Coletivo com fundamentação na Teoria das Representações Sociais de Serge Moscovici. Resultados - A pesquisa identificou três categorias que compõem o Discurso do Sujeito Coletivo: o enfermeiro frente às diretivas antecipadas de vontade; o enfermeiro frente à família contrária à vontade do paciente e o enfermeiro frente ao médico contrário à vontade do paciente. Conclusão - O testamento vital representa, na perspectiva dos enfermeiros, a autonomia e o direito do paciente pelas decisões nas situações de terminalidade que devem ser compartilhadas com seus familiares e profissionais de saúde. Não obstante, o enfermeiro pode vivenciar conflitos éticos nas situações em que o desejo manifestado por meio desse documento não é respeitado por familiares e/ou médicos. Diante dessa situação, o enfermeiro pode perceber a própria ação como limitada e sentir-se frustrado com a impossibidade de atender o desejo do paciente.


Introduction: Advance Directives also known as the Living Will Declaration constitute a manifestation of will by means of a document written by a person fully healthy of his mental faculties in order to dispose of the care, treatment and procedures that whether or not he or she wants to be submitted in case of a disease that is beyond therapeutic possibilities and incapable of manifesting his or her will freely. Objective - To know the representation of Living Will Declaration for nurses who work with terminally ill patients. Method - This is a qualitative research carried out with fifteen nurses who work in the care of terminal patients, through interviews guided by the following question "Talk about the Living Will Declaration". After the approval of the Committee of Ethics in Research, the interviews were obtained, which were analyzed according to the Collective Subject Discourse method based on Serge Moscovici's Theory of Social Representations. Results - The research identified three categories that make up the Discourse of the Collective Subject: "the nurse in front of the advance directives "; "The nurse facing the family contrary to the will of the patient" and "the nurse facing the doctor contrary to the will of the patient". Conclusion - The living will declaration represents, from the perspective of nurses, the autonomy and the right of the patient for decisions in terminal situations that should be shared with family members and health professionals. Nonetheless, nurses may experience ethical conflicts in situations where the desire expressed through this document is not respected by family members and / or doctors. Faced with this situation, the nurse can perceive the action as limited and feel frustrated with the impossibility of fulfilling the patient's desire.
