ABSTRACT

Se presenta el caso de una persona adulta de 24 años con genitales externos ambiguos, nunca estudiada ni tratada y que se había etiquetado de hermafroditismo en su país de origen. Estos casos son evaluados en su mayoría al nacimiento, siendo el diagnóstico más frecuente el de hiperplasia suprarrenal congénita y uno de los más infrecuentes el de la disgenesia gonadal parcial 46XY que presentamos (AU)


We report the case of a 24-year-old adult with ambiguous external genitalia, who had never been studied or treated, but who was labelled as a hermaphrodite in the country of origin. These cases are usually evaluated at birth, the most frequent diagnosis being congenital adrenal hyperplasia. We present a case of one of the least common of these disorders, 46XY partial gonadal dysgenesis (AU)

ABSTRACT

Objetivos: Analizar si el programa de cribado del cáncer de mama en la provincia de Segovia ha supuesto una mejora en la supervivencia en la población afecta por este tumor. Objetivo secundario: analizar la magnitud de esta mejora a los 5, 10 y 15 años de seguimiento. Material y métodos: Estudio retrospectivo descriptivo, analiza la supervivencia de 253 pacientes diagnosticadas de cáncer de mama por el programa de cribado (desde 1992-2007) y la supervivencia de 312 diagnosticadas a través del marco asistencial habitual, todas ellas tratadas en el Hospital General de Segovia. Resultados: La supervivencia en el grupo de cribado es netamente superior a la del grupo de diagnóstico clínico, siendo la diferencia ma ́s marcada a medida que aumenta el seguimiento: 93% vs 78% a los 5 años; 89% vs 68% a los 10 años; 86% vs 56% a los 15 años. Conclusiones: Se ha conseguido mejorar la supervivencia desde la puesta en marcha del programa de cribado, especialmente se observa a los 15 años de seguimiento, una diferencia del 30% (AU)


Objectives: To analyze whether the program for breast cancer screening in the Spanish province of Segovia has increased the survival rate among the population with this disease. Secondary objective: to analyze the magnitude of this improvement at 5, 10 and 15 years of follow-up. Materials and methods: We performed a descriptive, retrospective study that analyzed the survival rates in 253 patients diagnosed with breast cancer by the breast cancer screening program (1992-2007), as well as survival in 312 patients, diagnosed in routine clinical practice. All patients were treated in the General Hospital of Segovia. Results: The survival rate in the screening group was clearly higher than that in the group diagnosed in routine clinical practice. This difference was more marked with increasing follow- up: 93% vs 78% at 5 years, 89% vs 68% at 10 years and 86% vs 56% at 15 years. Conclusions: Survival has improved since the start of the screening program. This increase was especially marked in the 15 years of follow-up, representing a difference of 30% (AU)

The traumatic diastasis of pubic symphysis during the gestation is a rare although potentially serious complication for the mother and the fetus and that forces us to a careful monitoring of the hemodinámic stability of both and that it needs a joint performance by obstetricians and orthopaedic surgeons sometimes emergent (AU)

ABSTRACT

Objetivo: Determinar en qué modo influye el uso sistemático de la resonancia magnética (RM) mamaria preoperatoria en el enfoque quirúrgico del carcinoma de mama debido al diagnóstico de lesiones multifocales, multicéntricas y bilaterales sincrónicas, sólo vistas mediante RM. Sujetos y método: Estudio descriptivo retrospectivo de un total de 125 pacientes diagnosticadas de carcinoma de mama en el Hospital de Fuenlabrada, entre junio de 2004 y diciembre de 2006. Resultados: Se diagnostican lesiones malignas, sólo vistas mediante RM, en 25 pacientes (20% de los casos), que suponen un cambio en el enfoque quirúrgico de 18 pacientes (14,4% de los casos). Conclusiones: Se debe incluir la RM mamaria preoperatoria, de manera sistemática, en el estudio preoperatorio de todas las pacientes diagnosticadas de carcinoma de mama, para un mejor y más preciso tratamiento quirúrgico


Objective: To determine how magnetic resonance imaging (MRI) influences the surgical treatment of breast cancer due to identification of multifocal, multicentric and bilateral lesions, which can only be diagnosed by this technique. Subjects and method: We performed a descriptive retrospective study of 125 women with breast cancer diagnosed in Fuenlabrada Hospital (Madrid, Spain) between June 2004 and December 2006. Results: Malignant lesions, only visualized by MRI, were diagnosed in 25 patients (20% of cases), leading to a change in the surgical treatment in 18 patients (14.4% of cases). Conclusions: Preoperative MRI should be systematically included in the preoperative study of all patients diagnosed with breast cancer in order to provide improved and more accurate surgical treatment
