ABSTRACT

Diagnosis recrudescent pulmonary tuberculosis shows different flaws: on the one hand, the progression of early untreated tuberculosis can be mistaken for a recrudescence, on the other, changes are interpreted as a recrudescence if there is no clinical and X-ray evidence for the reactivation of the process. All cases of recrudescent tuberculosis should be reexamined by a medical commission, which allows the above errors to be avoided. If a patient with chronic alcoholism and mental disease has recrudescent tuberculosis, a narcologist and psychiatrist must participate in working out a comprehensive treatment policy.

ABSTRACT

Program complexes that are being developed at the laboratory of the Research Institute of Tuberculosis (USSR Ministry of Public Health) are directed to the employment of the IBM-compatible personal computers of a diverse configuration in antituberculosis institutions of a various level. In this respect the routine work of a doctor of an out- and in-patient department supplied by the appropriate modules can be replaced by work on computers which will enable the attending doctor to fill in medical records according to the scheme of the out- and in-patient departments. A program complex was also developed for the employment of computer technology in control over the antituberculosis service. These systems operate in a dialogue regimen. All the data can be put on a display and (or) printers in the form of texts, tables, diagrams and curves.

ABSTRACT

The results of a many years' clinico-dispensary followup of 1597 patients with focal, infiltrative and disseminated tuberculosis and pulmonary tuberculomas were analysed. Current complex therapy of patients made it possible to distinguish 3 basic variants of the process involution: I--rapid resolution, II--at first rapid and then slow resolution and III-slow resolution. The main factor responsible for one or the other involution variant was a morphologic substrate of the process at the moment of its detection and primarily its recent character and the presence of extensive old specific background in the zone of a repeated outburst. The quality of treatment, including use of rifampicin, was of the decisive importance only in the presence of a recent process and the absence of such a background. Other factors including the patients' age, clinical form, phase and dissemination of the process were not crucial. Recent tuberculosis chemotherapy promoted the acceleration of reparative processes in the lung tissue in the presence of fresh changes.

ABSTRACT

According to the data given by the authors, the tuberculosis morbidity rate in the populations under surveillance of the Ministry of Home Affairs was over that in the general population. High morbidity of such persons is often contributed to their antisocial way of life, and alcohol and drug addiction. Full coverage of these groups by means of X-ray screening, when they are held prisoners during the investigation period, makes it possible to detect all cases with active tuberculosis, to prevent the admission of undetected patients to the reformatory schools and thereby to stop the transmission of tuberculous infection. The observations should be conducted twice a year since these imprisoned groups may be recognized as being at high risk for tuberculosis. The clinical pattern of pulmonary tuberculosis is unfavorable in the convicted persons. Appropriate three-stage chemotherapy with the use of surgical interventions, if indicated permits one to achieve in the reformatory institutions of the Ministry of Home Affairs a high efficacy of treatment of ++newly-diagnosed pulmonary tuberculosis patients, which concurrently significantly decreases their epidemic danger for the general population when they are let free.

ABSTRACT

The data on retrospective observation of 6800 persons registered in the dispensary group VII in Lvov in 1976-1984 are presented. Examination of the persons registered in group VII for concomitant chronic nonspecific diseases of the respiratory organs (CNDRO) revealed their high incidence. Tuberculosis relapses in the persons of group VII A suffering from concomitant CNDRO amounted to 50 per cent of all the relapses. Still, their diagnosis was late which defined their severity and unfavourable prognosis in relation to the relapses. Along with high frequency of tuberculosis relapses there was aggravation of the nonspecific processes in the persons of group VII A with CNDRO. The data indicated the necessity of changing the case follow-up policy with respect to persons of group VIII with concomitant CNDRO.
