ABSTRACT

PURPOSE: To compare the pupil size of patients having migraine with photophobia versus normal subjects and to correlate the pupil size to the severity of headache. METHODS: This was a prospective case-control study. One hundred eyes of 50 patients who had migraine with photophobia were compared with same number of control subjects who had no history of headache. Pupil size was measured using the IOL Master 700 in all cases. Severity of headache was evaluated by the Migraine Disability Assessment (MIDAS) test questionnaire, and MIDAS grades from 1 to 4 ranging from minimal disability to severe disability were obtained in all cases. The pupil sizes in all grades were compared. RESULTS: The mean pupil size was 4.89 ± 0.9 mm (95% confidence interval [CI]) in patients of migraine with photophobia compared to 3.8 ± 0.49 mm (95% CI) in normal subjects, with a P value of <0.0001, which was statistically significant. Among the 50 patients with migraine, according to MIDAS grades, 20 had Grade 4 or severe disability, 15 had Grade 3 or moderate disability, 10 had Grade 2 or mild disability, and five had Grade 1 or minimal disability. Pupil size in Grade 4 category was the largest with a mean value of 5.6 ± 0.79 mm (95% CI). CONCLUSION: The pupil size in patients of migraine with photophobia is larger than in normal subjects and could be a trigger for the migraine attacks. The larger the pupil size, the more severe is the migraine disability.

Introduction and Purpose: The aim of this study is to evaluate binocular vision parameters such as near point of accommodation (NPA), near point of convergence (NPC), fusional vergence ranges in subjects with orthotropic Duane retraction syndrome (DRS) and to compare with normal age-matched controls Patients and Methods: A total of 24 subjects (14 Type 1, 2 Type 2, 8 Type 3 DRS) who were diagnosed to have DRS and 15 normals were included in the study. The following binocular vision parameters were assessed in all subjects: amount of the deviation (for near and distance in all gazes), NPC, NPA, positive and negative fusional vergence (PFV and NFV) reserve and stereopsis. Results: Accommodative amplitude was normal in subject with DRS (median (95% confidence interval (CI):15D (11.9-18.1)) compared to age-matched controls. However, NPC was significantly receded (median [95% CI]:17 cm [14.4-19.6]). PFV reserve and NFV reserve were reduced in subjects with DRS. Subjects with Type 3 DRS have poor PFV reserve and NFV reserve for distance when compared to Type 1 DRS (p = 0.009 and 0.006 respectively). Stereopsis was poor in the subjects with DRS (p < 0.001). Conclusion: Subjects with DRS have good accommodation, but have poor convergence and fusional vergence. Stereopsis was noted to be poor as well.

ABSTRACT

PURPOSE: To compare outcomes of frontalis sling (FS) silicone and levator resection (LR) in ptosis associated with monocular elevation deficiency. METHODS: Retrospective interventional comparative case series of FS and LR in monocular elevation deficiency associated ptosis. Favorable outcome was defined as difference in margin reflex distance 1 of ≤1 mm between the 2 eyes in unilateral cases and margin reflex distance 1 of 4 mm in bilateral cases at last follow-up visit. RESULTS: One hundred four eyes of 95 patients were included. Median age at surgery was 14 years, and the mean follow-up period was 19.75 ± 34.55 months. Ptosis was severe in 91 (87.5%) patients. Associated Marcus Gunn jaw-winking (MGJW) phenomenon was seen in 43 (42%) patients. Frontalis sling was performed in 76 (73%) and LR in 28 (27%). Mean pre- and postoperative margin reflex distance 1 were -1.27 ± 2.17 mm and 2.18 ± 1.49 mm (p < 0.0001). The mean improvement in margin reflex distance 1 was significantly more with FS (4.46 ± 2.19) compared with LR (1.85 + 2.5) (p < 0.0001). There were no cases of exposure keratopathy requiring reversal of surgery in either group. The number of resurgeries required was 42 (55%) in the FS group and 10 (36%) (p = 0.08) in the LR group. Favorable outcome was seen in 54 (71%) in FS group and 16 (57%) (p = 0.17) in LR group. CONCLUSIONS: When compared with levator resection, frontalis suspension with silicone gives a better eyelid elevation but has greater regression requiring more resurgeries. In spite of a poor Bells phenomenon, exposure keratopathy is not a concern.

ABSTRACT

Strabismus in Duane retraction syndrome is frequently associated with significant globe retraction and overshoots. However, there is no method to objectively grade retraction and overshoot. Our purpose is to describe a novel objective grading method. This novel and simple grading method has excellent agreement. It will help standardise measurements and guide the clinician in taking the decision for surgery and predicting its outcome.
